Thoracoscopic Lung Biopsy for Diffuse Interstitial Lung Disease (미만성 간질성 폐질환에서 비디오 흉강경을 이용한 폐생검)

  • Diffuse interstitial lung disease due to a wide variety of conditions are amenable to diagnosis by means of clinical evaluation, bronchoalveolar lavage, transbronchial biopsy and lung biopsy. The recently introduced technique of thoracoscopic wedge biopsy provides the potential advantages of greater selection of biopsy site and reduced postoperative chest pain compared with the standard open lung biopsy Video assisted thoracoscopic lung biopsy was performed in twenty patients for diagnosis of diffuse in- terstitial lung disease during the period from January 1993 to June 1994. The ages of the patients ranged from 18 to 69 years(mean 48.5 years), and the patients consisted of 10 males and 10 females. In all twenty, thoracoscpic lung biopsy samples were obtained; 9 from left lower lobe, 7 from right lower lobe, 2 from right upper lobe, 1 from left upper lobe, and 1 from right middle lobe. All patients were correctly diagnosed. The two common diseases were usual in erstitial pneumonia(UIP)(8120) and mil- liary tuberculo si s(4120) . Mean duration of the chest drainage was 3.2 days. There was no complication.

  • It have been known that the probability of accidental happening in towing-barge is higher than that of other merchant vessels. Because the towing-barge is restricted in thier manoeuvring ability due to its towingline. A report from ministry maritime affairs and fisheries said that the rate of collision accident for towing-barge is $40\%$ high tlnn other transport system in sea. A number of researches have been carried out to improve the safety policy in the towing-barge sea transportation system by the gorvernment. This study examined the safety status in domestic towing-barge sea transportation system The registration status, safety operational policy and post accidental data were also examined A survey research of experts relating to towing-barge operation also was carried out to find the detailed of safety status. This study would be applicable to set up safety policy for towing-barge marine transportation system.

  • Three cases of hyperparathyroidism are presented from our experience at Presbyterian Medical Center in Chonju in the hope that this will alert us all to the variegated patterns of clinical presentation. In the first case the principal symptom was muscular weakness. In the second a bone cyst (which was part of the syndrome of osteitis fibrosa cystica); and in the third case rib cage tenderness, backache, and persistent epigastric pain. All three had adenomas, but in Case 2 the adenomas were multiple. All three responded to surgical resection and remain well.

  • Nam(2008a) suggested that the role of teacher for helping students to learn mathematical structures should be the manifestor of tacit knowledge. But there have been lack of researches on embodying the manifestation of tacit knowledge. This study embodies the manifestation of tacit knowledge by showing that exemplification is one way of manifestation of tacit knowledge in terms of goal, contents, and method. First, the goal of the manifestation of tacit knowledge through exemplification is helping students to learn mathematical structures. Second, the manifestation of tacit knowledge through exemplification intends to teach students mathematical structures in the tacit dimension by perceiving invariance in the midst of change. Third, the manifestation of tacit knowledge through exemplification intends to teach students mathematical structures in the tacit dimension by constructing explicit knowledge creatively, reflection on constructive activity and social interaction. In conclusion, exemplification could be seen one way of embodying the manifestation of tacit knowledge in terms of goal, contents, and method.

Membranous PTEE Monocusp Vlave on the Right Ventricular Outflow Tract (막형 PTEE 단엽판막을 이용한 우심실 유출로 확장수술)

  • 배경: 우심실 유출로 재건술후에는 폐동맥판막부전이 우심실기능에 나쁜 영향을 미친다. 이를 막기 위해 여러 재료로 판막을 제조하여 사용하는데 가피 형성이 적은 막형 ePTEE(expanded polytetrafluoroethylene, Goretex) 단엽판막의 효과를 알아보고자 연구를 시행하였다. 대상 및 방법; 1996년 3월부터 1997년 2월까지 우심실유출로 재건술을 시행받고 수술 1일까지 우심실 압력을 측정할 수 있었던 47명중 막형 Goretex단엽판막을 사용한 환자(비교군) 19명, 사용하지 않은 환자(대조군) 28명에서 단엽판막의 유용성을 조사하였다. 두 군간에는 나이, 체중 그리고 McGoon ratio 등에는 통계학적 차이가 없었다.(p>0.05) 이전의 수술은 대조군에서 19번, 비교군에서 22번을 시행하였다. 우심실 유출로에 대한 수술은 대조군에서 REV수술 2예, 우심실폐동맥 연결수술 8예, 우심실 유출로 재개건술 2예, 우심실 유출로에 대한 수술은 대조군에서 16예였으며, 비교군에서는 REV 수술 5예, Rastelli 씨 수술 6예, 재우심실 유출로 재건술 5예, 그리고 우심실 재건술 3예였다. 결과: 대동맥 차단 시간이나 총심폐순환시간에는 두군간의 차이는 없었다. 또한 수술직후와 수술후 1일째 측정한 우심실 압력 및 좌심방압력의 차이도 두군간에는 차이가 없었으며 수술후 7일째 시행한 심에코도 결과 ejection fraction., 우심실/ 좌심실 압력비 , 그리고 우심실 유출로의 압력차, 강심제 투여일수, 호흡기 사용 기간 등에서도 두군간의 차이는 없었으나 흉관 삽입기간이 비교군에서 의미있게 짧았다. 추적 조사 기간 중 비교군 1예에서 사망을 하였으나. Goretex 단엽판막에 의한 것은 아니였으며 심초음파검사상 우심실 유출로 압력차이나 재수술은 두군간에 차이는 없었으나 폐동맥 폐쇄부전은 비교군에서 의미 있게 낮은 것을 보이고 있다. 결론; 막형 Goretex 단엽판막의 조기성적은 우수하였으며 폐동맥 폐쇄분전의 정도를 적게 하는 효과를 보이나 앞으로 이들이 성장함에 따라 우심실 유출로에 폐쇄를 일으키는지는 계속적인 추적관찰이 요구된다.

Radiotherapy Result of Brain Stem Tumors (뇌간 종양의 방사선 치료 성적)

  • Twenty five patients with tumors of the brain stem were treated with radiotherapy between 1979 and 1987. Histological diagnosis could be obtained in 6 cases, and other 19 patients were diagnosed by neurologic findings and CT or MRI. Eighteen patients were treated by radical radiotherapy and 6 patients received both operation and radiotherapy, while 1 patient received chemotherapy after radiotherapy. Total dose ranged from 50 Gy to 55 Gy. By an clinical scoring scale at 2 months after radiotherapy, no complete response was obtained, but 16 cases achieved partial response, 2 cases were stable, and 4 cases were deteriorated. The overall survival rate at 3 years was $36\%$ Age, performance status at diagnosis, degree of cranial nerve involvement, CT pattern of post-contrast enhancement, and clinical responese by scoring scale were correlated with survival.

  • 우리나라의 경우 집중호우와 돌발홍수로 인한 침수 발생에 대응하기 위해 유역 및 하천관리 사업, 각종 풍수해 예방사업 등을 추진하고 있으며, 관련 분야의 스마트기술 도입을 적극 추진하고 있다. 그러나, 2013년 노량진 상수도관 공사 현장 사고, 2019년 신월 빗물저류 배수시설 현장 사고 등과 같은 건설현장 침수 피해 사고가 지속적으로 발생하고 있다. 또한, 건설현장의 다양한 조건 및 시시각각 변화하는 상황에 따라 구조적 대책 및 대응방안을 수립하는 데 한계가 있으며 지금까지는 법, 제도에 기초한 대응 매뉴얼을 제작·배포하여 현장 근로자 교육을 실시하는 수준에서 진행되어 왔다. 본 연구에서는 건설현장의 자연재해, 특히 수재해에 대응하기 위해 보다 과학적인 방법을 통한 현장 침수 예경보 체계를 수립하였으며, 강우예측-침수예측-침수예경보 생산-현장 상황전파에 이르는 일련의 시스템을 개발하여 공사별, 규모별, 공정별 침수 대응 솔루션을 제공하고자 한다. 건설현장 침수예경보 시스템 개발의 주요 내용은 요소기술 개발이며, 간략하게 정리하면 다음과 같다. ① 강우 예측정보 생산: 현장에서 발생하는 집중호우를 고려하는 실시간 강우측정 자료와 연계한 초단기 강우예측 기술 개발, ② 침수 예측모델 개발: 현장의 시공간적 특성, 수재해 피해의 유형 등을 반영할 수 있는 침수피해 예측 모델 개발, ③ 침수예경보 의사결정 방법론 개발: 침수 피해 예경보를 위한 침수 위험단계 세분화 및 노모그래프 개발과 모델 적용(예측정확도 85% 이상), 이를 통합하여 건설현장 침수예경보 시스템 개발을 수행하게 된다. 연구에서 개발된 침수 예경보 통합 시스템은 향후 수재해로 인한 건설현장의 인명, 재산 피해 최소화에 기여할 것으로 기대된다.

  • This paper is provided to improve the discrimination of the towing line for tug-barge at the night sailing. As tugboat industry develops, marine accidents by tugboats are increasing. The main cause of accidents by tugboats is operators' negligence, and that portion is over 70 percent in total marine casualties by tugboats. The biggest cause of negligence is the passing through the towing line at night time. This paper suggests the ways to hinder operators' transit and improve discrimination of towing line(s) at night time.

Clinical Analysis of Infective Endocarditis (감염성 심내막염의 임상적 고찰)

  • Background: The indications and the optimal time of surgery of infective endocarditis are controversial. We report the surgical results of our hospital during the last 10 years with literature review. Material and Method: Between January 2000 and December 2009, we enrolled 23 infective endocarditis patients who underwent surgery, and analyzed retrospectively. In the preoperative blood culture, 8 cases (34.8%) were positive. The average preoperative antibiotics treatment period was $20.78{\pm}16.00$ days. There were 12 (52.2%) urgent operations. The average follow up period was $49.26{\pm}33.21$ months. Result: 20 mechanical valve replacements were performed, 9 in aortic position, 8 in mitral position and 3 in the both positions. The other procedures were one mitral valvuloplasty, one infected myxoma extirpation, and one infected pacemaker lead removal with debridement. The average period of postoperative intravenous antibiotic treatment was $24.39{\pm}15.98$ days. There were 5 complications, including 2 cases of postoperative bleeding, one postcardiotomy syndrome, one cerebral ischemia, and a low cardiac output syndrome. There were statistically significant postoperative improvement in NYHA class, left ventricle end diastolic/end systolic volume, and left atrium size (p-value < 0.05). Conclusion: We could obtain the satisfactory results without any moftalities by using sufficient preoperative antibiotics in hemodynamically stable patients, and by prompt surgery in unstable patients.
