Trough방법에 의한 광범위 기관 협착의 치료

  • 일반적으로 기관협착의 치료는 기관절재후 단단문합술로 가능하나 광범위한 기관협착이 발생하였을 때는 그 치료가 매우 어렵다. 이경우는 기도전벽을 개방하여 경부 피부와 기관연을 봉합하는 Trough를 형성한 뒤 이차적으로 연골이식이나 경부 피부를 이용한 피판을 만들어서 기관을 재건하는 Trough방법이 주로 적용되고 있다. 이 때 이식되는 연골은 늑연골 이나 비중격 연골이며 측벽의 재건에는 늑연골이 좋고 전벽의 재건에는 점막이 붙어 있는 비중격 연골의 복합이식편이 사용될 수 있다. 본 교실에서는 기관 절제 후 단단문합술이 불가능하였던 5명의 환자에서 Trough방법을 이용하여 기관 재건에 성공하였기에 그 결과를 보고하고자 한다.

Tissue Engineered Cartilage Formation on Various PLGA Scaffolds (PLGA 종류와 담체의 형성 방법에 따른 인간의 조직공학적 연골형성)

  • The purpose of this study was to evacuate the effect of different types of Poly(lactic-co-glycolic acid) (PLGA) scaffolds on the formation of human auricular and septal cartilages. All of the scaffolds were formed in a tubular shape for potential application for artificial trachea or esophagus with either 110,000 g/mol PLGA. 220,000 g/mol PLGA. or a combination of both. In order to maintain the tubular shape in vivo, two methods were used. One method was inserting polyethylene tube at the center of scaffolds made of 110,000 g/mol PLGA. The other method involved combination of the two different molecular weight PLGA's. The inner surface of tubular shaped scaffold made with 110,000 g/mol PLGA was coated with 220,000 9/mol PLGA to give more mechanical rigidity. Elastic cartilage was taken from the ear of a patient aged under 20 nears old and hyaline cartilage was taken from the nasal septum. The chondrocytes were then isolated. After second passage, the chondrocytes were seeded on the PLGA scaffolds followed by in vitro culture for one week. The cells-PLGA scaffold complex were implanted subcutaneously on the back of nude mice for 8 weeks. The tissue engineered cartilages were separated from nude mice and examined histologically after staining with the Hematoxylin Eosin. The morphology of the scaffolds were examined by scanning electron microscopy. The pores were well formed and uniformly distributed in the various PLGA scaffolds. After 8 weeks in vivo culture, cartilage was well formed with 110,000 g/mol PLGA. however lumen had collapsed. In contrast. a minimal amount of neocartilage was formed with 220,000 g/mol PLGA, while the architecture of scaffold and lumen were well preserved. Elastic cartilage formed more neocartilage than hyaline. Hyaline and elastic neocartilage were well formed on 110,000 g/mol PLGA with the polyethylene tube, exhibiting mature chondrocytes and preservation of the tubular shape. It was found that 110,000 g/mol PLGA was more appropriate for cartilage formation but higher molecular weight polymer was necessary to maintain the three dimensional shape of the scaffold.

Coculture of Bovine Chondrocytes with Demineralized Bone Matrix in Alginate Bead and Pellet Cultures (알긴산 배양과 펠렛 배양에서 소연골세포와 탈회골기질의 공배양)

  • Bio-integration of cartilage grafts with subchondral bone is a significant clinical challenge. To date, the use of demineralized bone matrix (DBM) has been one of the most effective strategies for bone cell proliferation in vivo. Here, we investigated whether coculture of chondrocytes and DBM could serve as a single-platform system containing all the essential elements for purposive bone and cartilage induction. The aim of this study was to evaluate and compare the phenotype and proliferation of bovine chondrocytes cocultured with DBM in two different culture systems, pellet and alginate bead culture. In alginate bead culture, we observed an increase in chondrocyte number and formation of cell clusters. Typical chondrocytic phenotype was maintained for entire eight weeks. Histological analysis showed that chondrocytes maintained a typical round, plump morphology and there was a gradual increase in lacunae. Both coculture systems yielded an expanded cell population as compared to the controls (chondrocytes alone). The production of glycosaminoglycans was also increased in the coculture systems as compared to controls.

The Possibility of Silk Protein to the Chondrogenesis (연골 재생에 대한 실크 단백질의 가능성)

  • A number of researcher have studied biomaterials for cartilage regeneration and are now proceeding. Silk protein was attempted for use as biomedical materials by many researchers because it is natural polymer with biocompatibility and excellent mechanical strength. In this study, we want to know a possibility of silk protein on the cartilage regeneration. We isolated chondrocytes from nasal cartilage and confirmed optimal culture condition of the cells. To observe the effects of silk fibroin on chondrogenesis, we added silk fibroin solutions to the culture medium of chondrocyte and detected gene expression levels related chondrogenesis such as col2, col10. The chondrocytes showed optimal growth when they were cultured in DMEM medium supplemented with 10% FBS 100 ${\cdot}{\ddot{I}}$M ascorbic acid. The levels of col2 gene expression were increased in non-autoclaved silk fibroin, but decreased in autoclaved one. Also the gene expression levels of col10 were increased in silk fibroin, particulary at 3D culture. Based on the results of this study, we had seen the possibility of silk fibroin for cartilage regeneration. In future studies, we should know more clearly the relationship between cartilage regeneration and the silk protein.

Formation of an Intestine-Cartilage Composite Graft for Tracheal Reconstruction (기관 재건을 위한 장과 연골의 복합 이식판 개발)

  • Background: Tracheal transplantation is necessary in patients with extensive tracheal stenosis, congenital lesions and other oncologic conditions but bears. many critical problems compared to other organ transplantations. The purpose of this study was to develop intestine-cartilage composite grafts for potential application in tracheal reconstruction by free intestinal graft. Material and Method: Hyaline cartilage was harvested from trachea of 2 weeks old New Zealand White Rabbits. Chondrocytes were isolated and cultured for 8 weeks. Cultured chondrocytes were seeded in the PLGA scaffolds and mixed in pluronic gel Chondrocyte bearing scaffolds and gel mixture were embedded in submucosal area of stomach and colon of 3 kg weighted New Zealand White Rabbits under general anesthesia. 10 weeks after implantation, bowels were harvested for evaluation. Result: We identified implantation site by gross examination and palpation. Developed cartilage made a good frame for shape memory. Microscopic examinations included special stain s howed absorption of scaffold and cartilage formation even though it was not fully matured. Conclusion: Intestine-cartilage composite graft could be applicable in the future as tracheal substitute and should be further investigated.

Comparison the Preoperative MRI Findings with Postoperative Arthroscopic Findings on Meniscus Injury with Anterior Cruciate Ligament Rupture (전방십자인대 파열에 동반된 반월상 연골판 손상의 관절경 수술 소견과 수술 전 자기공명영상 검사와의 비교)

  • Purpose: The purpose of this study was to compare the preoperative magnetic resonance image (MRI) findings with postoperative arthroscopic findings on meniscus injury with anterior cruciate ligament (ACL) rupture. Materials and Methods: We reviewed MRI images and arthroscopic findings of 225 patients treated by ACL reconstruction due to rupture, from February 2001 to November 2010. There were 154 cases of meniscus tear in arthroscopic findings. We examined the sensitivity for detecting meniscal tears varied with the presence of a rupture of the ACL, with the location of the tear within the meniscus, and among configurations of meniscal tears. Results: In the presence of a rupture of the ACL, the sensitivity of MRI was 0.88 for medial meniscal tears and 0.69 for lateral meniscal tears. And sensitivity of MRI was lowest in posterior horn and peripheral portion tears in lateral meniscus injury accompanying ACL rupture, sensitivity was low in anterior horn and flap shape tears in medial meniscus injury accompanying ACL rupture. Conclusion: In Meniscus injury with ACL rupture, a special attention shoulder given to the posterior horn and peripheral portion injury in lateral meniscus during arthroscopic surgery due to difficulty in detecting on MRI.

Second Look Arthroscopic Finding after Fibrin Matrix Autologous Chondrocyte Implantation for the Treatment of Articular Cartilage Defect of the Knee - Preliminary Report - (슬관절 연골 결손에 대한 fibrin matrix 자가 연골 세포 이식술 후 이차 관절경 소견 - 예비보고 -)

  • Purpose: The purpose of this preliminary report is to investigate the short term outcome of performing gel type fibrin matrix autologous chondrocyte implantation to patients who have damaged knee joint cartilage using secondary arthroscopy. Material and Methods: Six patients who have damaged knee joint cartilage were involved. The average size of defect was $5.13\;cm^2$. While performing primary arthroscopy, whole layer of cartilage bone was obtained either from the margin of damaged cartilage or the bilateral margin of a trochlea. The cartilaginous cells were obtained for culture for four to six weeks. While performing secondary minimal invasive arthrotomy, gel type fibrin matrix autologous chondrocyte was implanted on the chondral defect site. Results: 4 among 6 patients to be more than good in Modified Cincinnati Knee Scoring system. Lysholm function score was 59.5 preoperatively, and it improved to 76.25. ICRS grading by performing secondary arthroscopy revealed 4 out of 6 patients to be nearly normal. Conclusion: Gel type fibrin matrix autologous chondrocyte implantation is a treatment for cartilage defect, which takes less time to operate than the conventional implantation. In addition, this method minimizes the size of incision and allows arthroscopic surgery. However, long term follow up and more case study is thought to be necessary.

  • Chondrosarcoma is one of the most common types of primary bone sarcoma. With the exception of the mesenchymal subtype, chondrosarcomas are usually low-grade lesions and rarely show multicentricity or distant metastasis. Only rare cases of multicentric chondrosarcomas have been reported in association with Ollier's disease and Maffucci's syndrome. To our knowledge, no report has been issued of a synchronous multicentric chondrosarcoma occurrence across a joint. We experienced a 30-year-old man with a synchronous monomelic juxta-articular multicentric chondrosarcoma across a shoulder joint in the absence of pulmonary and visceral metastases. He was treated by curettage and cement filling with allograft in the acromion and wedge resection with cement filling in the proximal humerus. At the 18-month follow-up, there was no evidence of recurrence, and the patient had full range of motion without pain.

  • Chondroblastoma is a benign chondroid-forming tumor usually originating in the epiphysis. The base of metatarsal bone contains neither an epiphysis nor a secondary ossification center and so is the rare site of chondroblastoma. Here, we present a case of chondroblastoma of the base of fifth metatarsal bone in 34-year-old man. Histologically, the osteoclast-like giant cells were abundant enough to simulate a giant cell tumor. And the chondroid intercellular matrix was intermixed but scanty. However, the background mononuclear cells showed irregular and indented nuclei with longitudinal clefts and positive immunoreactivity for S-100 protein, as the evidence of chondroblasts.

Arthroscopic Technique of Partial Meniscectomy for Bucket Handle Tear of Medial Meniscus using Posteromedial Portal (내측 반월상 연골판 양동이형 파열의 후내측 도달법을 이용한 관절경적 부분 절제술 - 수술 수기 -)

  • Purpose : To introduce arthroscopic partial meniscectomy fur bucket handle tear of medial meniscus using posteromedial portal, which is superior to arthroscopic partial meniscectomy using standard anterior portals commonly used. Method : After arthroscopic examination of the knee, we reduce the torn meniscus, advance the arthroscope into posteromedial compartment under arthroscopic visualization, we make posteromedial portal with reexamination of the compartment and perform arthroscopic partial meniscectomy. Conclusion : With the technique of arthroscopic partial meniscectomy using standard anterior portals, accurate partial meniscectomy can not be done because of inadequate visual field, associated meniscal injuries of posterior horn and cartilage lesion of posterior aspect of the medial femoral condyle can be missed, commonly posterior cruciate ligament can be injured and artificial damage to weight bearing surface of medial femoral condyle is possible. An arthroscopic partial meniscectomy using posteromedial portal is an excellent method fur bucket handle tear of medial meniscus.
