Studies on the Neck and Shoulder Pain (목덜미와 어깨의 통증에 관한 연구)

  • Two hundred sisty five patients who complained of neck pain with stiffness and pain of the suprascapular area were studied. In most cases the anatomical locations of pain were in the levator scapulae muscles or trapezius muscles. Hyperactivity of dorsal scapular nerve or spinal accessory nerve which innervate those muscles was thought to be responsible for these pains. The hyperactivity of the nerves may be due to the spasm of the sternocleidomastoid muscle and the scalenus medius muscle which the nerves meet during their courses to the levator scapulae or trapezius muscles. Therefore, spasmolytic treatment on the scalenus medius provided effective relief for neck or shoulder pain.

The Immediate Effects of Pectoralis Minor Self-stretching Exercise on Muscle Length and Acromio-humeral Distance in Normal Adults (작은가슴근 자가 신장운동이 정상 성인의 작은가슴근 길이와 봉우리-위팔 거리에 미치는 즉각적인 효과)

  • Shoulder joint pain is a very common musculoskeletal disorder, in which about 20 percent of adults experience it more than once in their lives. In this study, an objective assessment was made that subacromial space was increased by pectoralis minor stretching exercise, which has a positive effect on impingement syndrome and shoulder pain. Also, This study examined the effects on pectoralis minor length(PML) and acromio-humeral distance(AHD) following pectoralis minor self-stretching exercise in normal adults. In this study, a total of 30 subjects were recruited and randomly divided into a self-stretching exercise group(n=15) and manual stretching exercise group(n=15) to compare the immediate effects of treatment. Studies have shown that both groups have significantly increased both PML and AHD within the group(p<.05). There was no statistically significant difference in the comparison between the groups(p>.05). The results suggest that self-stretching exercise can be recommended as a treatment method for impingement syndrome and prevention of shoulder disease.

Analysis of Correlation Coefficient between Musculoskeletal Symptoms and Job Stress of Physical Therapists in Twenties: Focusing on Therapeutic Exercise Task (20대 물리치료사의 근육뼈대계 증상과 직무스트레스와의 상관관계 분석: 운동치료 업무를 중심으로)

  • The purpose of this study is to investigate musculoskeletal symptoms and job stress of physical therapists in their 20s who are responsible for therapeutic exercise and to understand the correlation between the two. This study conducted a research on 144 subjects based on the measurement tools provided by national institution. The results are as follows. Many subjects were experiencing musculoskeletal symptoms and disorder in their hands/wrists/fingers, shoulders, and waist with high proportionate of subjects experiencing the symptoms and disorder in their right hand/wrist/finger. Among job stress, job environment, job requirement, and compensation had significantly positive correlation with physical burden(r=.297~.392, p=.000). In particular, shoulder pain intensity has significantly positive correlation with job requirement, duration of pain in hand, wrist, and fingers as well as workplace culture(r=.331~.478, p=.003~.009) which indicates that musculoskeletal symptoms are some what correlated with job stress. Based on this study, it is necessary to show more concern and make an effort to manage musculoskeletal symptoms and job stress.

Effect of Shoulder stabilization exercise and McKenzie Exercise on Forward Head Posture (거북목을 가진 사람에게 메켄지 폄운동과 어깨안정화운동이 앞쪽머리자세에 미치는 영향)

  • The purpose of this study was to investigate the effects of cervical vertebral height and shoulder stabilization on anterior head posture in people with turtle neck. The subjects of this study consisted of normal and healthy people with 30 turtle necks. Subjects were randomly assigned to shoulder stabilization exercise and cervical vertebrae exercise (30 men, 15 men and 15 women, height: $168.67{\pm}6.51$, $22.33{\pm}8.27$, $65.03{\pm}11.98$, and $28.66{\pm}1.91$), In the experimental group, shoulder stabilization exercise and cervical spine exercise were performed for 4 weeks. Deep neck flexor, leavator scapular, upper trapezius, rhomboid, and lower trapezius showed significant differences in shoulder stabilization and cervical vertebrae before and after the experiment. As a result, the CVA value decreased significantly and the muscle activity value increased significantly when exercise and Mckenzie program were applied. These results show that exercise and Mckenzie programs have a positive effect on turtle neck improvement.

The Effects of the Action Observation and Visual Feedback Convergence Exercise on the Alignment, Pain and Function of Forward Head Posture and Round Shoulder Posture (동작관찰과 시각적 되먹임 융합 운동이 머리전방자세와 둥근어깨의 정렬, 통증과 기능에 미치는 영향)

  • The aim of this study was to determine the effects of the action observation and visual feedback on the alignment, pain and function of forward head posture(FHP) and round shoulder(RS). A total of 24 participants with FHP and RSP were randomly assigned to general exercise(GE, n=8), action observation(AO, n=8), and visual feedback(VF, n=8). All subjects were exercised three times a week for four weeks. The groups were assessed for craniovertebral angle(CVA), round shoulder posture(RSP), visual analog scale(VAS), and neck disability index(NDI) before and after exercise, There was a significant difference in CVA in the GE, AO and RSP, VAS and NDI were significantly different in all groups. AO was more effective than GE, VF for VAS. The results of this study suggest that action observation may be effective to improve the FHP and RS.

  • A diagnosis of shoulder pain is varied and difficult to make. The initial onset of liver cancer is difficult to detect and patients typically do not complain of symptoms as most tumors are asymptomatic. If the symptoms of the patients develop, the first symptom is usually pain that extends from the abdomen to the back and shoulder. A suprascapular nerve block is used in the treatment of the referred shoulder pain due to a metastatic hepatoma, but the effectiveness of the treatment has been limited because of its short duration. Recently, the advent of pulsed radiofrequency (PRF) lesioning has proved a successful treatment for chronic refractory pain involving the peripheral nerves. We experienced a case of a 66-year-old male patient complaining of referred right shoulder pain due to metastatic liver cancer, which was relieved after PRF lesioning of the suprascapular nerve.

Effects of Shoulder Strength Exercise and Cervical Mobilization to Neck and Shoulder Chronic Pain Patients Pain and Muscle Tension (목과 어깨 만성 통증 환자에게 어깨 강화 운동과 목뼈 관절 가동술이 통증 및 근긴장도에 미치는 영향)

  • Background: Approximately 30% of people suffer from chronic neck and shoulder pain. Chronic neck and shoulder pain arise due to the exposure to continuous low loading and monotonous work. This is a common musculoskeletal disorder in a society. As physical therapists, we should give appropriate treatment to these people. Methods: A total of 26 patients with chronic neck and shoulder myalgia were randomly allocated into two experimental groups. First, 13 patients received shoulder strength training, while the other 13 patients received cervical mobilization. Outcome measures included bilateral pressure pain threshold by using an algometer at upper trapezius, visual analogue scale (VAS) to express their pain scale, and muscle tension by using myoton pro device pre-intervention and after the final treatment. Results: The VAS and muscle tension in upper trapezius significantly decreased (p<.05) in both groups; however, no differences between two groups were observed (p>.05). The pressure pain threshold in upper trapezius significantly increased (p<.05) in both groups; yet, again, the differences between the groups did not reach statistical significance (p>.05). Conclusion: As methods of treatment, shoulder-specific strength training and cervical mobilization can be appropriate approaches to the treatment of neck and shoulder musculoskeletal disorder.

The Effects of IASTM Using Vibration Stimulation on Shoulder Muscle Activity, Flexibility and Pain of Chronic Shoulder Pain Patients (진동을 이용한 IASTM이 만성 어깨통증 환자의 어깨 근활성도, 유연성, 통증에 미치는 영향)

  • Purpose : Instrument-assisted soft tissue mobilization (IASTM) has been widely used to improve pain and range of motion. However, research on IASTM using vibration stimulation is lacking. Therefore, this study investigates how IASTM using vibration stimulation affects chronic pain on muscle activity and flexibility of the shoulders. Methods : A total of 20 patients with chronic shoulder pain were divided into the experimental group and the control group. The experimental group (n=10) used vibration stimulus for IASTM, while the control group (n=10) used general IASTM. For six weeks, this training lasted for 30 min a day, twice a week. Muscle activity was evaluated using the surface electromyograph. The back and reach test was used to assess flexibility, the visual analogue scale to measure pain, and the paired t-test to compare the groups before and after the experiment. An independent ANCOVA was conducted to assess differences in the degree of transition between the two groups before and after the experiment. Results : Significant differences in muscle activity, flexibility, and pain in both groups before and after the experiment (p<0.05) were observed, as well as in the muscle activity of the pectoralis major and associated pain (p<0.05). however, muscle activity and flexibility of the upper trapezius and infraspinatus were not significantly different between groups (p>0.05). Conclusion : This study confirms the importance of IASTM using vibration. Design and manufacture of IASTM using vibration based on our results would be useful in the management of shoulder pain, Further clinical evidence are needed.

A Study on the Rehabilitation Exercise by Bike Pain and Injury and Fitting

  • The purpose of this study is to improve pain and injury through bicycle fitting and to propose rehabilitation methods. Pain and injuries caused by bicycles can be largely divided into knee and ankle pain, wrist, shoulder, neck, and back pain, and hip pain. The cause of these pains and injuries stems from incorrect bicycle fitting and posture. In order to improve these pain and injuries and prevent recurrence, appropriate bicycle fitting and rehabilitation exercise are needed. Pain and injury are divided into kinetic pain and pain by a fixed posture. In the case of kinetic pain, rehabilitation exercise is required along with inflammatory treatment, and pain by a fixed posture should understand the exact characteristics of the body and learn and practice bicycle posture according to the body's characteristics. Through this method, it can contribute to preventing the recurrence of bicycle pain and injuries and further creating an environment where a safe bicycle culture can be established.

A Study on the Diagnostic Usefulness of Ultrasound and Magnetic Resonance Imaging for the Diagnosis of Shoulder Rotator Cuff Tear (어깨 회전근개 파열 진단을 위한 초음파 검사와 자기공명영상 검사의 진단적 유용성 연구)

  • Rotator cuff tears are a leading cause of shoulder pain in adults. Due to the increase in social activities, the number of patients complaining of shoulder pain is increasing, and interest in shoulder diseases is also increasing. With the development of ultrasound equipment, the sensitivity and specificity of diagnosis are high, and it is used to diagnose rotator cuff tears in musculoskeletal disease. Ultrasound is recognized as a complementary method to MRI examination in rotator cuff tears. Therefore, this study aimed to find out the diagnostic usefulness of ultrasound and MRI examinations in the diagnosis of shoulder rotator cuff tears.A retrospective analysis was performed on 262 patients who were diagnosed with final rotator cuff damage by arthroscopy after completing ultrasound and MRI examinations. Sensitivity, feature, positive predictive value, image predictive value, and touch were disassembled for the test results. In addition, the degree of clavicular tear was scored and recorded in 5 stages. Ultrasound examination was similar to MRI examination results for both full-thickness and partial tears, and there was no statistically significant difference. Partial tear test results showed higher positive predictive value and accuracy than MRI test. In conclusion, ultrasound can be fully utilized as a screening test for rotator cuff disease, and it is thought that it will be selected and used clinically according to the patient's constitution and situation.
