알코올의존으로 입원한 환자에서 나이와 성별에 따른 퍼센트 탄수화물-결핍트랜스페린과, 감마-글루타밀전이효소 변화 양상의 차이 (The Alteration of % Carbohydrate-Deficient Transferrin and Gamma-Glutamyl Transferase Levels of Alcohol-Dependent Inpatients according to Age and Sex)

  • Objectives This study sought to investigate the relationship between age, sex and alterations in levels of % carbohydrate-deficient transferrin (%CDT) and gamma-glutamyl transferase (GGT) in patients admitted with alcohol dependence. Methods The study retrospectively enrolled 187 patients who were diagnosed with alcohol dependence according to the Diagnostic and Statistical Manual of Mental Disorders-Fourth edition (DSM-IV) and were admitted into a closed ward in Hallym University Hangang Sacred Heart Hospital from 2009 to 2012 and Hallym University Kangnam Sacred Heart Hospital from 2012 to 2017. Demographic factors (age, sex) and biochemical markers [%CDT, GGT, mean corpuscular volume (MCV), aspartate transferase (AST), alanine transferase (ALT)] were collected by reviewing medical records. Alterations in the levels of %CDT and GGT in different groups for each demographic factor were compared after correcting for confounding variables (age, initial %CDT, GGT, MCV, AST, ALT). Results Decreased %CDT and GGT were observed during the period of abstinence after admission. The normalization period for %CDT increased with age, while the normalization period for GGT was longer in female patients. Conclusions These results suggest that alcohol-dependent patients that vary in age have different alterations in %CDT, while different sexes have different alterations in GGT. Age and sex can be potential indicators of treatment response after abstinence in patients with alcohol dependence. Further studies are needed to evaluate the relationship between these factors with regards to physiological and hematological changes in alcohol dependence.

남성 알코올 의존 환자 대뇌의 휴지기 네트워크별 피질 두께 (Cortical Thickness of Resting State Networks in the Brain of Male Patients with Alcohol Dependence)

  • Objectives It is well known that problem drinking is associated with alterations of brain structures and functions. Brain functions related to alcohol consumption can be determined by the resting state functional connectivity in various resting state networks (RSNs). This study aims to ascertain the alcohol effect on the structures forming predetermined RSNs by assessing their cortical thickness. Methods Twenty-six abstinent male patients with alcohol dependence and the same number of age-matched healthy control were recruited from an inpatient mental hospital and community. All participants underwent a 3T MRI scan. Averaged cortical thickness of areas constituting 7 RSNs were determined by using FreeSurfer with Yeo atlas derived from cortical parcellation estimated by intrinsic functional connectivity. Results There were significant group differences of mean cortical thicknesses (Cohen's d, corrected p) in ventral attention (1.01, < 0.01), dorsal attention (0.93, 0.01), somatomotor (0.90, 0.01), and visual (0.88, 0.02) networks. We could not find significant group differences in the default mode network. There were also significant group differences of gray matter volumes corrected by head size across the all networks. However, there were no group differences of surface area in each network. Conclusions There are differences in degree and pattern of structural recovery after abstinence across areas forming RSNs. Considering the previous observation that group differences of functional connectivity were significant only in networks related to task-positive networks such as dorsal attention and cognitive control networks, we can explain recovery pattern of cognition and emotion related to the default mode network and the mechanisms for craving and relapse associated with task-positive networks.

  • Objective : Dopamine transporter is member of family of Na/Cl dependent neurotransmitter transporter, 12 transmembrane domain, that has high substrate specificity, affinity. It is related with dopamine reuptake in presynaptic vesicle. DAT has a VNTR in its 3'-untranslated region(UTR). 3'-UTR VNTR polymorphism is related with modification of dopamine transmission. The association between with VNTR polymorphism and neuropsychiatric disorders such as alcohol dependence, and low activity ALDH has been studied, but their relationship is unclear. We study about association of 3'-UTR VNTR of DAT gene and G2319A and alcohol dependence. Method : Group of Korean subjects were studied with alcohol dependence(n=49 male) compared to mentally healthy controls(n=53 male). The peripheral blood sample was acquired, and Polymerase Chain Reaction(PCR) amplification, MspI procedure was done. Result : There was a significant difference between alcohol dependence group and normal control(genotype frequency p<0.05, allele frequency p<0.05) Allele A frequency and genotype(GG, GA) frequency was a significant difference between alcohol dependence group and normal control(p<0.05). Conclusion : Our study showed that genetic polymorphism of DAT1 G2319A had relation with alcohol dependence.

  • Objectives : Alcohol-induced oxidative stress has been known to injure various tissues or organs. This stress is related with free radicals which are produced as the result of long-term alcohol consumption. Malonyldialdehyde(MDA) is produced by the interaction of free radicals and cell membrane lipids, and indicates the degree of lipid peroxidation indirectly. The purpose of this study was to investigate the relationship between red blood cell(RBC) membrane lipid peroxidation by free radicals, and associated hepatic injuries and hematologic changes. Methods : Thirty-three subjects diagnosed as alcohol dependence according to DSM-IV diagnostic criteria were evaluated within 72 hours after discontinuing alcohol drinking. Clinical characteristics were evaluated by CAGE questionnaire and Korean Michigan Alcoholism Screening Test(MAST). RBC membrane MDA level was measured as the marker of RBC membrane lipid peroxidation. Aspartate aminotransferase(AST), alanine aminotransferase(ALT) and gamma-glutamyltransferase(GGT) were used as the biochemical markers of liver damage due to alcohol ingestion. The alcohol-induced hematologic change was assessed by mean corpuscular volume(MCV). Results : The results were as follows. Clinical characteristics were not different between two groups having normal and abnormal levels of AST, ALT, GGT or MCV. The levels of MDA were not correlated with the clinical characteristics and serum levels of AST, ALT and GGT. However, there was a significant correlation between the levels of MDA and the value of MCV(p=0.017). Conclusions : These findings suggest that oxidative stress in alcohol dependence may not be reflected in liver enzyme markers such as AST, ALT and GGT, but may be reflected in MCV.

알코올 의존 제I형 및 II형 환자에서 검지-약지 길이 비의 차이 (The Difference of the $2^{nd}$ to $4^{th}$ Digit Length Ratio between Type I and Type II Alcoholism)

  • Objectives The second to fourth digit ratio (2D : 4D) is known to reflect testosterone level during intrauterine period. Testosterone is essential for masculinization of brain. Cloninger's type II alcoholism is characterized by early age at onset and largely limited to males. Compared with type I alcoholism, type II alcoholism is thought to be masculine. There is a possibility that the digit ratio of type II alcoholism is low compared with type I. We examined digit length ratio in Cloninger's type I and II alcoholism to understand the effects of intrauterine testosterone on the development of alcoholism. Methods The participants were 87 alcohol dependent patients. We divided two subtypes of patients by Irwin's symptom list, based on Cloninger's concept. We captured the images of both palms by the scanner and measured the lengths of digits by the graphic program. Then we analyzed the digit ratios by independent-sample t-test. Results The mean 2D : 4D of type I was 0.942 (right), 0.952 (left). The mean 2D : 4D of type II was 0.927 (right), 0.940 (left). In both hands, 2D : 4D of type II was significantly lower than type I. Conclusions Type II alcoholism showed masculine type of digit length ratio compared with type I. It suggests the exposure of testosterone during intrauterine period might play an important role in determining the course and feature of alcoholism.

  • Objectives:Recent studies have raised the possibility that nerve growth factor(NGF) is abnormally regulated in the central nervous system(CNS) of animal models with alcohol dependence. The possible alteration of NGF by prolonged alcohol intake may play an important role in alcohol-induced neurotoxicity. Carbohydrate-deficient transferrin(CDT) is regarded as a reliable biological marker of alcohol dependence. The goal of this study was to estimate the changes of %CDT and serum NGF level according to the duration of alcohol abstinence, and to identify whether %CDT level is associated with the serum NGF level in the patients with alcohol dependence. Methods:The subjects were 24 patients with alcohol dependence. We used the Axis-Shield ASA to measure the %CDT level and the enzyme-linked immunosorbent assay(ELISA) to measure the serum NGF level. %CDT and NGF levels were measured immediately after the admission and at 2 weeks after the admission. Results:Decreased %CDT were observed during the period of 2 weeks after the admission. NGF level was not significantly different after 2 weeks. The NGF levels were not correlated with %CDT. The possibility of %CDT as a predictor of alcohol-induced neurotoxicity was not confirmed. Conclusion:Serum NGF levels is not a reliable indicator of abstinence state in the patients with alcohol dependence. Further studies are needed to evaluate the relation between two indicators in regard to hematological and neurological changes in alcohol dependence.

  • Objectives : The Alcohol Urge Questionnaire(AUQ) has been used in alcohol dependence treatment and research. The goal of this study is to develop of the Korean Alcohol Urge Questionnaire(AUQ-K). Methods : To examine the AUQ-K's psychometric properties, responses from 104 patients admitted in alcohol dependence treatment facility were investigated. Results : The internal consistency of the 8-item AUQ-K, measured by coefficient ${\alpha}$, was high(Cronbach's ${\alpha}$=0.78). AUQ-K scores showed significant correlation when the retest interval was 1 day(p<0.01). The AUQ-K's validity was investigated using correlational analyses with two other craving scales[the Obsessive Compulsive Drinking Scale(OCDS) and the Visual Analogue Scale(VAS)]. The high correlations were obtained between total AUQ-K scores and total OCDS scores, and between total AUQ-K scores and the VAS scores(p<0.01, respectively). Conclusion : The AUQ-K is a reliable and valid short scale for measurement of self-reported alcohol craving. This scale may offer significant advantages over existing single-item measures of alcohol craving in the fields of alcohol dependence treatment and research.

Naltrexone과 ondansetron의 병합투여가 C57BL/6형 생쥐의 알코올 섭취량에 미치는 영향 (Effect of Ondansetron Alone and Combination of Naltrexone and Ondansetron on Alcohol Intake in C57BL/6 Mice)

  • 알코올 의존의 음주 행동에 VTA에서 NA로 뻗어 있는 도파민 대뇌 보상 경로가 중요하다. 이러한 경로의 토파민 활성도가 $5-HT_3$계 신경에 의하여 조정되고 있으며, $5-HT_3$ 수용체 길항제인 ondansetron (OND)이 알코올리즘 환자에서 음주량을 감소시키고 금주률을 높인다는 보고가 있다. 이에 생쥐의 섭취량에 대하여 $5-HT_3$ 수용체 길항제인 ondansetron의 투여 효과와, 이러한 ondansetron 및 비 특이적 아편계 수용체 길항제인 naltrexone (NTX)과의 병합 투여 효과를 검증하는데 목적이 있다. 알코올 의존화 된 C57BL/6형 수컷 생쥐를 4군으로 나눈 뒤, 10일간 각 군에 vehicle, OND 0.01 mg/kg 단독, NTX 1.0 mg/kg 단독, 및 OND 0.01 mg/kg과 NTX 1 mg/kg 병합 투여하면서 2시간 알코올의 섭취량, 22시간 물 섭취량, 24시간 사료 섭취량 및 체중을 조사하였다. 본 연구의 결과로 2시간 알코올 섭취량의 10일간 변화에 대해 vehicle 투여 군과 나머지 3군의 약물 투여군과 repeated measure ANOVA를 이용하여 각각 비교하였을 때, vehicle 투여군과 NTX 단독 투여군간에 유의한 교차가 관찰되었으나 (p=0.042), OND 단독 투여군과 NTX과 OND 병합 투여군은 vehicle 투여군과 유의한 차이가 없었다. 일별 2시간 알코올 섭취량에 대하여 NTX 투여군과 vehicle 투여군의 군간 비교시 vehicle 투여군에 비하여 NTX 투여군에서 약물 투여 4일부터 10일까지 유의하게 감소하였다(4일 p=0.010; 6일 p=0.050; 8일 p=0.017; 10일 p=0.005). 그리고 NTX과 OND 병합 투여군과 vehicle 투여군의 양군을 비교하였을 때에는 2시간 알코올 섭취량이 4일과 10일에만 유의하게 감소하였다(4일 p=0.049; 10일 p=0.022). 그러나 22시간 물 섭취량, 24시간 사료 섭취량 및 체중의 10일간 변화에 대해 vehicle 투여군과 나머지 3 군의 약물 투여군과의 repeated measure ANOVA를 이용하여 각각 비교하였을 때, 모두 유의한 교차효과가 없었다. 이상의 결과는 생쥐의 섭취량에 대하여 OND의 투여 효과는 없었으며, OND과 NTX의 병합 투여시에는 NTX의 알코올의 섭취량 억제 효과가 감소되었다. 따라서 앞으로 $5-HT_3$ 신경계와 도파민 및 아편 신경계와의 상관관계에 대한 연구가 필요할 것으로 생각된다.

단가아민 산화제 A CA 반복 유전자 다형성이 한국 알코올의존 남자환자의 행동특성과 임상양상에 미치는 영향 (The Effects of Monoamine Oxidase A CA Repeat Polymorphism on Behavioral Trait and Clinical Characteristics in Korean Male Alcoholics)

  • 이 연구는 MAOA CA 반복 유전자다형성에 따라 알코올의존 환자에서 행동 및 성격특성 그리고 임상양상의 차이를 알아봄으로써 유전자형과 표현형과의 관계를 알아보기 위해 수행되었다. 이 연구 결과 임상양상에서 S군에 비해 L군의 임상양상이 보다 부정적인 양상을 보이고 있었다. 이는 MAOA CA 반복 유전자다형성이 음주양상을 변화시키는 역할을 할 가능성을 시사하고 있다. 그렇지만 이러한 영향이 유전자의 직접적인 인과관계로 설명하기는 어렵고 간접적이고 이차적인 영향에 따른 결과로 생각해 볼 수 있겠다. 또한, 행동특성에서 L군이 S군에 비해 전체적으로 공격성이 많았고 자기주장적 태도가 강했다. 그리고 MAOA CA 반복 유전자다형성의 대립유전자 길이가 자기주장적 태도와 상관관계가 있었으며 이러한 자기주장적 태도를 MAOA CA 반복 유전자다형성의 대립유전자 길이가 부분적으로 설명하고 있었고, 자기주장적 태도가 강한 환자의 경우 길이가 긴 대립유전자를 가질 가능성이 많았다. 공격성도 그 자체가 여러 가지 측면을 가지고 있는 복합적인 행동특성이기 때문에 전체 공격성을 어느 하나의 유전자만으로 설명할 수 있을 것으로 기대하는 것은 비현실적이다. 그렇지만 이 연구에서 자기주장적 태도는 분화된 공격성의 한 측면이어서 MAOA CA 반복 유전자다형성이 일부분이지만 설명을 할 수 있다는 것을 시사하고 있어, 연구자들은 이 연구가 향후 유전자형이 공격성 같은 표현형을 설명하는 연구들에 있어 기초자료가 될 수 있을 것으로 생각된다. 아직까지 MAOA CA 반복 유전자다형성과 MAOA 활성도와의 관계가 명확하지 않고 대사산물과의 관계도 설명이 어려운 실정이어서 향후 이에 대한 연구들이 필요하겠다.

알코올중독전문병원의 서비스 제공 현황에 관한 연구 (A study on the present situation of the treatment services provided by alcohol addiction specialized hospitals)

  • 본 연구에서는 국내 알코올중독전문병원의 서비스 제공형태, 인력현황, 구조화된 알코올 프로그램 등의 현황과 향후의 경향성에 대해 조사하고자 국내 알코올중독전문병원임을 표방하는 총 8곳 중 조사에 동의한 6개 기관을 대상으로 2010년 6월 17일부터 7월 15일까지 구조화한 설문지 조사를 수행하였다. 응답한 알코올 중독전문병원의 운영 주체는 개인(공동)이 66.7%로 가장 많았으며, 소재지 별로는 대도시 지역에 위치한 기관이 50%로 가장 많았다. 알코올 환자를 전문적으로 치료해 온 기간은 5년 미만이 50.0%로 가장 많았으며 조사 대상 기관의 병상 수는 평균 208 병상이었다. 서비스 제공 형태는 모든 기관이 입원 외래치료를 함께 제공하고 있는 반면 주당 최소 2~3시간 정도의 구조화된 외래 치료서비스는 33.3%였다. 신체 개인 심리사정이 모든 기관에서 시행되고 있는 반면 낮 병원을 위한 프로그램은 16.1%였다. 보호병동 병상이 72.0%였으며 밤 병원 및 낮 병원 병상은 각각 7.6%, 2.6% 으로 낮았다. 정신 보건법 시행규칙 기준과 비교하면 정신보건사회복지사와 정신보건간호사는 비교적 높은 것으로 나타났으나 기관별 차이가 높음에 비하여 정신과전문의와 간호사의 인력현황 비율은 전체적으로 높은 것으로 나타났다. 임상전문가들이 느끼는 가장 큰 문제점으로는 알코올 의존증에 대한 환자의 치료 거부로 원활한 치료가 시작되지 않는다는 것과 퇴원 후 연계 가능한 지역사회정신보건 기관의 수가 부족하다는 의견이 높았고 다음으로 낮은 의료수가와 음주폐해에 대한 낮은 사회적 인식 문제가 제기되었다.
