Usefulness of 18F-Florbetaben in Alzheimer's Disease Diagnosis (알츠하이머병 진단에서 18F-Florbetaben의 유용성)

  • Alzheimer's disease is the most common degenerative brain diseases that causes dementia. ${\beta}$-amyloid neuritic plaque density that accumulates in the brain is difficult to perform daily living, such as memory loss, language ability deterioration. It is used to estimate ${\beta}$-amyloid neuritic plaque density in adult patients with cognitive impairment who are being evaluated for Alzheimer's disease and other causes of cognitive impairment. Using the $^{18}F$-Florbetaben with high sensitivity and specificity for the ${\beta}$-amyloid neuritic plaque density to evaluate the usefulness for the early diagnosis of Alzheimer's disease. In $^{18}F$-FDG Brain imaging shows no specific findings. And it appeared on the MR-Brain imaging without atrophy of the hippocampus. However, the intake of ${\beta}$-amyloid neuritic plaque density in $^{18}F$-Florbetaben informs that it is the progress of Alzheimer's disease. Therefore, $^{18}F$-Florobetaben is very useful for early diagnosis of Alzheimer's disease.

  • Alzheimer's disease (AD), an irreversible degenerative disorder, is associated with accumulation and aggregation of amyloid-β peptides, hyperphosphorylated tau proteins, and high level of metal ions in the brain. Up to date, there is no effective therapeutic agent to stop the progress of the disease and thus early and accurate diagnosis of AD has gained increasing attention in recent years. Among several diagnostic methods, an optical imaging using fluorescent probes is one of the most promising tools to visualize AD biomarkers. In this review, we will introduce fluorescent probes that can be applied to in vivo brain imaging of AD models and also their structure. It is expected that the present review will provide useful information to many scientists in the related research fields.

Gender Differences in Behavioral Psychological Symptoms of Dementia in Patients with Alzheimer's Disease (알츠하이머병 환자에서 행동심리증상의 성별차이)

  • Objectives : Behavioral and psychological symptoms of dementia(BPSD) represent significant clinical problems, resulting in functional decline, caregiver distress, institutionalization and increased mortality. A recent study showed gender differences have important role in the development of BPSD, but relationship between BPSD and gender has never been studied in Korea. This study was designed to examine whether patients with Alzheimer's disease show gender differences in behavioral and psychological symptoms of dementia(BPSD). Methods : Ninety-eight subjects with Alzheimer's disease were included in this study. We carried out history taking and cognitive assessment for the diagnosis of Alzheimer's disease based on DSM-IV. Cognitive impairment and BPSD were measured using the Mini Mental State Examination(MMSE), Global Deterioration Scale (GDS), Clinical Dementia Rating(CDR) and the Korean Neuropsychiatric Inventory(K-NPI). Independent samples t-test was used to examine the differences across gender in BPSD. Correlation analysis between MMSE, CDR, GDS and NPI was performed using Pearson's correlation coefficient. Results : There were no significant gender differences between the gender in BPSD. We found statistically significant negative correlations between MMSE with NPI total score, and with scores of several sub-domains such as hallucination. Conclusions : This study showed that gender differences in BPSD are not significant. Further research is necessary to identify whether BPSD affect gender differences or individual differences.

Association of Hemoglobin Levels and Cognitive Function in Alzheimer's Disease and Mild Cognitive Impairment (알츠하이머병과 경도인지장애에서 헤모글로빈 농도와 인지기능과의 연관성)

  • Objectives : Anemia is a common problem in the management of elderly patients. Recent studies reported that anemia was associated with cognitive impairment and Alzheimer's disease. This study was aimed to analyze the differences of hemoglobin levels in Korean patients with Alzheimer's disease(AD) dementia, mild cognitive impairment(MCI), and healthy controls. Furthermore, the study also examined if any association between hemoglobin levels and cognitive functions existed. Methods : A total of 116 Korean elderly adults were participated in this study(mean age 74.67 years ; 60.3% female). The Korean version of the Mini-Mental State Examination(MMSE-K), Clinical Dementia Rating(CDR) and Global Deterioration Scale(GDS) were applied to all subjects. Hematological and related blood chemistry values were investigated. Results : We found that patients with AD dementia had significantly lower hemoglobin and hematocrit levels than MCI subjects and healthy control group. The hemoglobin levels showed a positive correlation with MMSE-K and negative correlation with CDR, GDS. Conclusions : Among Korean elderly, low hemoglobin level is associated with a cognitive impairment. This study indicates that AD is associated with anemia, and low hemoglobin levels may contribute to potentially useful clinical markers of AD. The risk factor for dementia needs to be confirmed by prospective longitudinal studies in a larger group of patients.

Platelets as a Source of Peripheral Aβ Production and Its Potential as a Blood-based Biomarker for Alzheimer's Disease (말초 아밀로이드 베타 원천으로서의 혈소판과 알츠하이머병의 혈액 바이오마커로서의 가능성)

  • Alzheimer's disease causes progressive neuronal loss that leads to cognitive disturbances. It is not currently curable, and there is no way to stop its progression. However, since medical treatment for Alzheimer's disease is most effective in the early stages, early detection can provide the best chance for symptom management. Biomarkers for the diagnosis of Alzheimer's disease include amyloid β (Aβ) deposition, pathologic tau, and neurodegeneration. Aβ deposition and phosphorylated tau can be detected by cerebrospinal fluid (CSF) analysis or positron emission tomography (PET). However, CSF sampling is quite invasive, and PET analysis needs specialized and expensive equipment. During the last decades, blood-based biomarker analysis has been studied to develop fast and minimally invasive biomarker analysis method. And one of the remarkable findings is the involvement of platelets as a primary source of Aβ in plasma. Aβ can be transported across the blood - brain barrier, creating an equilibrium of Aβ levels between the brain and blood under normal condition. Interestingly, a number of clinical studies have unequivocally demonstrated that plasma Aβ42/Aβ40 ratios are reduced in mild cognitive impairment and Alzheimer's disease. Together, these recent findings may lead to the development of a fast and minimally invasive early diagnostic approach to Alzheimer's disease. In this review, we summarize recent advances in the biomarkers of Alzheimer's disease, especially the involvement of platelets as a source of peripheral Aβ production and its potential as a blood-based biomarker.

  • 알츠하이머병은 뇌에 비이상적으로 베타아밀로이드 단백질의 축적으로 인해 신경세포가 손상되는 질병으로 아직까지 명확한 질병의 메커니즘이 밝혀지지 않고 있다. 새로운 알츠하이머병의 생물학적 모델을 제시하기 위해, KEGG의 알츠하이머병의 신호전달패스웨이와 문헌정보를 기반으로 구축된 신호전달 네트워크를 병합함으로써 새로운 질병의 모델을 생성하였다. 분석결과 로바스타틴하부경로를 포함하는 새로운 알츠하이머 생물학적 경로 모델을 제시하고자 한다. 향후 메디컬 페스웨이의 병합기술을 통해 보다 다양한 질병의 원인 기작을 연구하는데 활용하고자 한다.

  • 알츠하이머병은 치매의 주요 원인 중 하나로 기억, 판단, 언어 등의 지적 기능의 감퇴뿐만 아니라 인격, 행동양상 장애 등을 동반하며 결국은 환자를 죽음에 이르게 하는 위험한 병이다. 본 연구는 뇌 백질의 비등방성을 잘 보여줄 수 있는 확산 텐서 영상 (diffusion tensor imaging; DTI) 데이터로 부터 신경섬유 경로를 생성하고, 알츠하이머병 환자와 정상인의 신경섬유 경로의 특징을 비교 분석 하였다. FSL 프로그램을 사용하여 생성한 신경섬유 경로에서 알츠하이머 환자와 정상인 집단간의 신경섬유 경로는 부피와 밝기에 커다란 차이를 보인다. 특히, 신경섬유 경로의 부피에서 현저한 차이가 있다는 점은 알츠하이머의 조기 진단에 유용하게 사용될 수 있다.

  • 알츠하이머병 치매와 조현병 진단을 위한 2단계 분류 모델을 제안한다. 정상군과 환자군의 발화에 나타난 페어 언어 모델 간의 Perplexity 차이에 기반한 분류와 기존 단일 BERT 모델의 미세조정(fine-tuning)을 이용한 분류의 통합을 시도하였다. Perplexity 기반의 분류 성능이 알츠하이머병, 조현병 모두 우수한 결과를 보임을 확인 하였고, 조현병 분류 모델의 성능이 소폭 증가하였다. 향후 설명 가능한 인공지능 기법을 적용에 따른 성능 향상을 기대할 수 있었다.

  • As the aging society continues in recent years, interest in dementia is increasing. Among them, Alzheimer's disease is a degenerative brain disease that accounts for the largest percentage of all dementia patients, with the medical community currently not offering clear prevention and treatment for Alzheimer's disease, and the importance of early treatment and early prevention is emphasized. In this paper, we intend to find the most efficient activation function by combining various activation functions centering on convolutional neural networks using MRI datasets of normal people and patients with Alzheimer's disease. In addition, it is intended to be used as a dementia classification modeling suitable for the medical field in the future through Alzheimer's dementia classification modeling.

Convergence performance comparison using combination of ML-SVM, PCA, VBM and GMM for detection of AD (알츠하이머 병의 검출을 위한 ML-SVM, PCA, VBM, GMM을 결합한 융합적 성능 비교)

  • Structural MRI(sMRI) imaging is used to extract morphometric features after Grey Matter (GM), White Matter (WM) for several univariate and multivariate method, and Cerebro-spinal Fluid (CSF) segmentation. A new approach is applied for the diagnosis of very mild to mild AD. We propose the classification method of Alzheimer disease patients from normal controls by combining morphometric features and Gaussian Mixture Models parameters along with MMSE (Mini Mental State Examination) score. The combined features are fed into Multi-kernel SVM classifier after getting rid of curse of dimensionality using principal component analysis. The experimenral results of the proposed diagnosis method yield up to 96% stratification accuracy with Multi-kernel SVM along with high sensitivity and specificity above 90%.
