Clinical Evaluation of a Rebound Tonometer in Patients Who Underwent Penetrating Keratoplasty (전층각막이식을 시행한 안에서 리바운드 안압계의 임상적 유용성)

  • Purpose: To compare the measurements of the rebound tonometer (RT), Goldmann applanation tonometer (GAT) and noncontact tonometer (NCT) in patients who underwent penetrating keratoplasty (PKP), and to evaluate the reproducibility of the RT measurements. Methods: This study included 19 PKP eyes and 28 normal eyes. We compared the intraocular pressure (IOP) measurements of the GAT and NCT with the RT in both groups using Spearman's correlation analysis and the Wilcoxon signed-rank test. The IOP, as measured with an RT in each group, was assessed with respect to reproducibility using the intraclass correlation coefficient (ICC). Results: In normal eyes, there was no significant difference in the measurements obtained with the RT, GAT, and NCT (p > 0.050). In the patient group, the RT measurements were not significantly different from those of the GAT (p = 0.872), but they were significantly lower than those obtained with the NCT (p = 0.011). However, the RT measurements showed a relatively high correlation with those of the GAT and NCT (r = 0.770 and 0.879, respectively). The ICC of the RT was 0.986 for the PKP eye group and 0.961 for the normal eye group, both of which were highly reproducible. Conclusions: In PKP eyes, the measurements obtained with the RT showed a relatively high correlation with those of the GAT and NCT, and the repeatability of the RT measurements was high and similar to those for normal eyes. The RT can therefore be considered a useful method for measuring the IOP in PKP eyes.

Comparison of Intraocular Pressure Values of Normotensive and Glaucomatous Rats Using Two Types of Tonometers (두 종류의 안압계로 측정한 정상안압과 녹내장 쥐의 안압 값 비교)

  • Purpose : We compared intraocular pressure (IOP) values measured by two types of tonometers in condition of normotensive and glaucomatous rat model. We tried to determine which of tonometer can more easily and accurately measure the IOP of animal model. Methods : Glaucomatous eyes were induced by intracameral injections of hyaluronic acid in right eyes of six-week-old male Spargue-Dawley (SD) rats. Normotensive contralateral eyes were left eyes of the SD rats. IOP was measured using a rebound tonometer (Tonolab) and a immersive tonometer ($Tonopen^{(R)}$ XL) about 3:00 pm. Results : The mean IOP values of normotensive control eyes were $10.80{\pm}1.03mmHg$ by Tonopen, and $15.10{\pm}0.73mmHg$ by Tonolab. They were statistically insignificant (p = .1). The mean IOP values of glaucomatous experimental eyes were $30.20{\pm}2.67mmHg$ by Tonopen, and $37.90{\pm}2.73mmHg$ by Tonolab. They were statistically insignificant (p = .95). High IOP values of glaucomatous eyes by two types of tonometers had strong positive correlation each other (r = .904, p < .01). Conclusion : This is the first study to compare IOP values using two types of tonometers between normotensive and glaucomatous model made by intracameral injection of hyaluronic acid. Tonopen should be used carefully when the IOP is within normal range, and both Tonopen and Tonolab can be used reliably when the IOP is high.

A Study of the Variation Factors of Intraocular Pressure (안압 변동 요인에 관한 연구)

  • Purpose: To measure and quantify the factors of intraocular pressure (IOP) variation for normal Korean youth. Methods: The IOP of normal youth 805 persons (male: 367, female: 438) were measured as a function of time and quantity for fixation, reclining posture, hard blinking, drinking water, drinking hard liquor, tobacco and coffee by using non-contact tonometer (AT555, Reichert Co.). In the case of fixation, reclining posture and hard blinking, the intraocular pressure of right and left eye were measured for 6 males and 6 females every 10 minutes during 30 minute. In the case of water, liquor, tobacco and coffee, the measurement for 4 or 5 groups (6 persons/ group) having similar IOP were done for the IOP variation of right eye. Results: The mean IOP was 15.2 mmHg for males and 15.8 mmHg for females (slightly higher than male's), respectively and the 99% of those were in the range of the normal IOP. The measured IOP was increased by fixation, reclining posture and hard blinking with males and females. The gradient of increasing slope was 0.15/0.13/0.07/0.08, 0.09/0.08/0.08/0.08 and 0.19/0.08/0.26/0.31, respectively. The IOP variation was elevated by drinking water (especially in the case of 1000 ml) and lowered in proportion to the amount of drinking by hard liquor. Cigarette smoking was associated with rise in intra ocular pressure, but was not shown a definite relation. And coffee had virtually no effect. Conclusions: Fixation, reclining posture, hard blinking and Tobacco caused a transient increase in IOP, but alcohol elicited a slight reduction in IOP overall.

A Study of the Intraocular Pressure of Korean Youth (한국인 청년의 안압 측정)

  • The intraocular pressure for Korean youth were measured by using tonometer (AT555-Reichert). The relative frequency distributions of intraocular pressures have been studied for samples of 1,027 persons(475 males, 552 females). The most commonly recorded IOP for both men and women was around 14.5mmHg and 17.5mmHg, respectively. They were in the range of 7 to 23mmHg(males) and 7 to 22mmHg(females). The median pressure(cumulative frequency=0.5%) is 13.0~16.5mmHg for males and 16.5~18.5mmHg for females, so the values for females are slightly higher than males. The mean pressure is 15.2mmHg for males and 15.8mmHg for females, respectively. The 98% of population was in the range of the normal IOP. There are long-term diurnal variation in mean intraocular pressure and the IOP was decreased as a function of time from morning to night. The measured IOP was affected by several factors: exercise made to decrease the IOP and tight collars, dark places and posture of decubitus position got to elevate the IOP.

Intraocular Pressure Changes in the Uneventful Extra-capsular Cataract Extraction and Extra-capsular Cataract Extraction with Vitreous Loss followed by the Anterior Chamber Lens Implantation (계획적 수정체낭외적출술과 합병증이 동반된 수정체낭외적출술후 전방렌즈의 삽입술시 안압의 변화)

  • These days the posterior chamber lens implantation following the extracapsular cataract extraction has become the standard procedure except those patients who can not take it in one reason or another. In past, however, the anterior chamber lens implantation was popular for sometimes. Even now in those patient complicated by vitreous loss, the anterior chamber lens implantation following anterior vitrectomy is indicated in some cases. We have become interested in the difference of the intraocular pressure change in the patients who underwent the uneventful extra-capsular cataract extraction followed by anterior chamber lens implantation in routine manner and in those whose extra-capsular cataract extraction were complicated by vitreous loss and eventually needed the anterior vitrectomy before the anterior chamber lens implantation. We reviewed the medical records of 15 cases of the former group and 11 cases of the latter one in 1986 through 1988. There was some difference in the postoperative pressure in the two groups, but it was not statistically significant.

The Effect of Extracapsular Cataract Extraction and Posterior Chamber Lens Implantation on Intraocular Pressure (백내장적출술 및 인공수정체삽입술이 안압에 미치는 영향)

  • We studied the change in intraocular pressure (IOP) in 15 consecutive cataract patients who underwent extracapsular cataract extraction and posterior chamber lens implantation between Feb. 1993 and Apr. 1993 to evaluate the effect of this surgery on postoperative IOP. To evaluate the clinical usefulness of non-contact tonometer, the intraocular pressures were measured with Kowa non-contact tonometer (TM-2000, Japan) as well as Goldmann applanation tonometer. There was a decrease in IOP of $3.4{\pm}2.9$mmHg (p<0.00l) 3 months after this surgery and the intraocular pressure differences between pseudophakic eyes and contralateral phakic eyes at 3 months postoperatively were $2.4{\pm}3.8$mmHg (p<0.05). The correlation coefficient between non-contact tonometer and Goldmann tonometer was 0.8876 (p=0.001) in the postoperative 76 eyes. Therefore, our results suggest that extracapsular cataract extracion and posterior chamber lens implantation alone can be a useful surgical method in cataract patient with ocular hypertension, and non-contact tonometer was relatively accurate in measuring the postoperative intraocular pressure.

Design of a Wireless Intraocular Pressure Sensor Based on MEMS Technology (안압의 비접촉 검출을 위한 MEMS 기반의 센서 설계)

  • Interaocular pressure (IOP) sensor and external coil to detect the resonance frequency of the IOP sensor are designed and implemented using MEMS technology. The IOP sensor is designed using 3-D electromagnetic (EM) simulation. The resonance frequency of IOP sensor needs to be lower than that of the external coil. Additionally, the resonance frequency of the IOP sensor needs to be located near the resonance frequency of the coil to get the sufficient amplitude of phase variation. The frequency where the phase peak appears must be constant according to the distance between the IOP sensor and the external coil. From the measurement results, we demonstrated that the designed IOP sensor has the same resonance frequency with various distances between the IOP sensor and the coil.

A Study on the clarification of the name of Anapji(雁鴨池) and Anapbupyeong(雁鴨浮萍) - Focusing on Anapji through its Appearance in Poems(題詠詩) ('안압지'의 이름과 '안압부평(雁鴨浮萍)'의 의미 연구 - 안압지(雁鴨池) 제영시(題詠詩)를 중심으로 -)

  • This study aims to clarify the meaning of the Anapji which was one of the scenic places of historical interest during Silla Dynasty and of the Anapbupyeong(雁鴨浮萍) which was one of the greatest seven miracles (Dong-do-chil-gue(東都七怪) through appearance of the Anapji in poems. Anapji lies its original meanings on Anhaji(安夏池) and then it was shortened to Anji(雁池), apji(鴨池) while Dongho(東湖) has also been called since it is located in the South of Gyeongju. As of July 2011, the Ministry of Culture officially renamed Anapji and Imhaejeon Hall Site(臨海殿址) as the palace of Crown Prince-Gyonegju Donggung(慶州 東宮) and Wolji(月池) (Historical Site No. 18) and Wolji became the current name. While pond was made during Munmu of Silla, the name Anapji did not exist. Kim Si-Seup (1465~1471) in the early Chosun period referred to the pond as Anhaji(安夏池) and its name started being called as Anapji in the Geography of Korea (Sinjeung dongguk yeoji seungnam,(新增東國輿地勝覽)(Jungjong 25, 1530). Though modern scholar states that it was being called Anapji since there are always many ducks and wild geese in the pond, they fails to present the literature evidences. The author found the line in the poem -Anapji composed by writer Kim Chul Woo, which means that a wild duck and water bird get in and off the pond on their own and the record saying which means Dong-ho(東湖) from the phrase of Dongho-saw-sar-gi(東湖書社記) of Lee Su In(李樹仁)(1739-1822)-Confucian Scholar of Gyeongju was created due to the fact that many wild geese and ducks are flying over. Based on the facts foresaid, the literature evidences for the name of Anapji are presented in the paper. Anapji is a combined word between the Anapbupyeong(雁鴨浮萍) which is a flooding weed and Dong-do-chil-gue(東都七怪). Dong-do-chil-gue is an ancient form of Modern Eight Scenic Views. Eight Scenic Views -Gyeongju Parlgyong is just well-known as scenic places without grotesque elements Dong-do-chil-gue had before. However, the author also found the passage of Ilseongnok: Records of Daily Reflections saying that Anapji has a clod that is as wide as rock(盤石) and the clod also has moneywort on, which are undulating in the wind and clarified how the Anapbupyeong became one of the one of the greatest-seven miracles Dong-do-chil-gue and why the Anapbupyeong frequently appears in the poems of many writers.
