  • 안면신경마비(facial nerve paralysis)는 주로 편측성으로 발생하는 안면신경장애에 의한 안면표정근의 마비를 뜻한다. 이러한 안면신경마비는 중추성 안면신경마비와 말초성 안면신경마비 두 가지로 나뉜다. 안면신경마비의 증상으로는 이환측, 구각부의 처짐 및 침을 흘리는 등 입 주위의 증상이 있어 안면표정의 변화를 일으킨다.[1] 본 논문은 사진을 입력 받아 얼굴영역에서 입 특징점을 추출하여 입력 받은 데이터가 안면신경마비 환자인지 아닌지 판단하고자 한다.

  • 이 연구는 안면마비의 재발환자에 대한 한의학적인 원인 및 발생빈도, 나이분포, 재발기간, 가족력, 당뇨와의 관계를 파악하고자 한 것이다. 안면마비는 한방병원을 찾는 환자의 주요한 질환의 하나로서 침치료에 의해 좋은 효과를 보여왔으며 이에 대한 많은 연구가 있었으나 그 재발율에 대한 구체적인 연구는 없었다. 이에 본 연구에서는 경희의료원 한방병원 침구과에 내원한 안면마비 외래환자를 대상으로 초발, 동측재발, 대측재발로 나누어 단면적 분석연구를 시행하였다. 통계처리는 SPSS 8.0를 이용하였으며 chi-square test를 시행하였다. 544명의 외래환자중 7.4%의 재발율을 나타냈으며 동측재발과 건측재발은 통계상 유의성은 없었다. 또한 재발환자의 남여비에 대한 비율은 통계상 유의성이 없었다. 재발된 환자의 평균나이는 46.8세 이었고 재발기간은 8.74년이었다. 재발된 확률은 40세에서 59세 까지 높게 나타났다. 재발된 원인으로는 과로가 가장 높은 빈도를 보였으며 풍한, 정신적인 원인, 음주순으로 나타났다. 안면마비가 재발된 환자중에 당뇨병의 기왕력이 있는 자는 22.5%였으며 안면마비의 가족력은 42.5%의 높은 비율을 나타내었다.

Therapeutic Trial of Bee Venom Acupuncture for Idiopathic Facial Paralysis in a Dog (특발성 안면마비를 지닌 개에서 봉독 약침을 이용한 치료 1예)

  • A 6-year-old, castrated male Shih-tzu dog was presented due to left side facial paralysis with head tilt. Neurological examination revealed absence of facial sensation, menance response, and palpebral reflex on the left side. On magnetic resonance imaging (MRI), intracranial intra-arachnoid cyst (IIAC) was noted. The dog was poor response to steroid and dieuretic therapy. Based on characteristic historical and clinical findings, and excluding of other causes of acute facial nerve dysfunction, the dog was tentatively diagnosed as idiopathic facial paralysis. The clinical signs were improved gradually after acupuncture therapy with bee venom. Eight weeks after initial acupuncture with bee venom, the patient recovered sensory and neurological facial signs. This case report demonstrates that bee venom acupuncture for an idiopathic facial paralysis could be useful in a dog.

  • Bell's palsy is an acute-onset unilateral peripheral facial neuropathy. For patients with sequelae of facial paresis, the successful rehabilitation of fully edentulous arches is challenging. This case report described the treatment procedures and clinical considerations to fabricate complete dentures of a patient who showed unilateral displacement of mandible, unilateral chewing pattern and parafunctional jaw movement due to sequelae of Bell's palsy. Gothic arch tracing was used to record reproducible centric relation and lingualized occlusion was performed to provide freedom to move between centric relation and the patient's habitual functional area in fabricating satisfactory dentures in terms of function and esthetics.

  • Bilateral sagittal split ramus osteotomy(BSSRO) of the mandible is an essential and commonly used procedure to correct dentofacial deformities and malocclusion. The possible complications associated with BSSRO include inferior alveolar nerve injury, bleeding, temporomandibular disorder, unfavorable fractures, and clinical relapse. The incidence of facial nerve palsy after orthognathic surgery recently reported is 0.1%. The probable etiologies have included facial nerve compression, complete or incomplete nerve transection, nerve traction, and nerve ischemia from anesthetic injection. Postoperative facial palsy is one of the most serious complications because it reduces the quality of life and significantly reduces social interaction. The case of a 24-year-old patient who underwent bilateral sagittal split ramus osteotomy is described. The medical records and postoperative photographs were reviewed in detail to collect information on the clinical course, treatment, and outcomes.

Therapeutic Effect of Oyaksungisan in Dogs with Facial Nerve Paralysis (개 안면신경마비에 대한 오약순기산의 치료효과)

  • Herbal medicines are widely used to treat a variety of human diseases. However, therapeutic effect of herbal medicine on the canine facial nerve paralysis (FNP) has not been investigated. This study examined therapeutic effects of Oyaksungisan on the canine FNP. Ten dogs with the induced canine FNP were equally divided into the control and an herbal medicine-treated groups (5 dogs, Oyaksungisan-treated group), respectively. No treatment was given to the control group. In the Oyaksungisan-treated group, Oyaksungisan was administered orally, twice per day for 2 weeks (50 mg/kg) after inducing FNP. Significant differences in the clinical scores were detected between the Oyaksungisan-treated and control groups on day 14 (p < 0.05). The serum creatine kinase (CK) activities in the Oyaksungisan-treated group showed a tendency to decrease but there was no significant difference compared with those of the control group. In conclusion, Oyaksungisan was effective for the induced canine FNP.

  • Objectives : To report the patient bilateral facial palsy, because it is unusual. Methods : One patient with bilateral facial palsy was selected. Herbal medicine(Cheunggansoyosan, bubidongyeom-2) was administered and acupuncture was applied for twice a day in 73 days. Facial function was evaluated by House-Brackmann scale, Yanagihara's scale and Standardization of muscular paralysis. Results : Cheunggansoyosan, bubidongyeom-2 and acupuncture treatment improved facial function after 73 days of treatment. Adverse effects were not reported. Conclusions : This study shows that the Korean medicine treatment was effective in improving bilateral facial palsy.

Facial palsy as the presenting symptom of acute myeloid leukemia in children: Three cases with stem cell transplantations (안면마비로 초기 발현된 소아 급성골수성백혈병: 조혈모세포이식으로 성공적으로 치료한 3예)

  • Facial palsy as the presenting symptom of leukemia is very rare, especially in acute myeloid leukemia. A review of the medical literature identified reports on 8 children with AML who had facial paralysis as the presenting sign. Whole brain irradiation (WBI) has been applied in most cases. We present the cases of 3 such children. Achieving a remission without WBI, the patients underwent stem cell transplantations (SCTs). Two patients remain event-free 52 months and 62 months after allotransplants. Facial palsy was the harbinger of leukemic relapse in one case after autotransplant. This patient is disease-free 59 months after unrelated SCT rescue. Facial palsy persisted in 2 cases. Allogeneic SCT without WBI may be an effective therapy in patients presenting with facial palsy. A brief review of the literature is presented here.

  • Complete facial nerve palsy is emotionally and physiologically devastating condition. Management of patients with the facial nerve palsy poses significant challenges to achieve the goal of returning patients to their premorbid state. We experienced a case of iatrogenic facial nerve palsy in a 66 year-old patient, who was diagnosed as facial nerve schwannoma with previous incisional biopsy. The author describes the management of this patient and reviews the literature.
