Analysis of Bone Mineral Density of Ankle Fracture Patients (족관절 골절 환자의 골밀도 분석)

  • Purpose: This study analyzed the bone mineral densities of the lumbar vertebrae and femurs of patients with ankle fractures to determine the correlation between ankle fractures and osteoporosis. Materials and Methods: From April 2002 to July 2014, one hundred consecutive ankle fracture patients with bone mineral density tests performed within post-traumatic one year were enrolled. The patients were divided into three age groups according to their age at the time of injury (group 1: <50, group 2: 50-69, group 3: ≥70). The types of ankle fractures were classified into unimalleolar, bimalleolar and trimalleolar fractures. The bone mineral density was analyzed using the T score, Z score, absolute value (g/cm2) of the lumbar spine (L1-L4), femur neck, femur intertrochanter, and total femur. Results: There were 3.2 times more females with ankle fractures than males, and the prevalence of osteoporosis according to age group was 0% in the group under 50 years, 24.2% in the 50 to 69-year-old group, and 15% in the group over 70 years. Osteoporosis was found in 30% of patients with a trimalleolar fracture in the 50 to 69-year-old group. In all patient groups, a lower age indicated a higher frequency of unimalleolar fractures. The relationship between the bone mineral density and the type of fracture is that the frequency of trimalleolar fracture increased with decreasing T score of the lumbar vertebrae and the absolute value of bone mineral density (g/cm2) and the Z score of the femur neck, but there were no other indicators. Conclusion: Among the 100 patients with ankle fractures, females were more common than males, because osteoporosis was less severe in males. The incidence of unimalleolar fracture was higher than that of trimalleolar fracture. On the other hand, the correlation between the ankle fractures and the bone mineral density of the femur and lumbar spine was not significant.

Analysis of the Effect of Entry-Level 3D Printer Materials on CT Images (보급형 3D프린터 재료가 CT 영상에 미치는 영향 분석)

  • In this study, based on PLA, we analyzed the Hounsfield Unit (HU) of materials containing 20% each of aluminum, wood, copper, carbon, and marble, and tried to analyze how they affect the image. A cylindrical phantom of 5×30×30 ㎣ (thickness×diameter×height) was fabricated using a entry-level 3D printer. The kV was changed to 80, 100 and 120, and the mAs was changed to 100 and 200 mAs, and the phantom in the center of the table was cross-scanned under a total of six conditions. A circular ROI was set using image J program and the quantification value of the material part HU and the quantification value of the peripheral part CNR were obtained. The HU average of the material part increased in the order of [PLA - wood 20%], [PLA - marble 20%], [PLA - carbon 20%], [PLA 100%], [PLA - aluminum 20%], [PLA - copper 20%] (p<0.05) a negative correlation was confirmed with the HU by increasing kV. It was confirmed that the CNR value in the peripheral area increased in the order of [PLA - marble 20%], [PLA - copper 20%], [PLA - carbon 20%], [PLA - wood 20%], [PLA - aluminum 20%], and [PLA - 100%] (p<0.05). Human organs with similar HU values for each material are [PLA - copper 20%] compact bone, [PLA - aluminum 20%] cancellous bone, [PLA 100%] coagulated blood, [PLA - carbon 20%] and [PLA - marble 20%] liver, muscle, spleen and [PLA - wood 20%] had similar values to fat. In addition, we confirmed the blur phenomenon that blurs the image around the filament with all materials, and confirmed that [PLA 100%] especially has the most blur around the filament. Therefore, it is considered desirable to reflect the HU value of the target organ and consider cloudiness around the phantom when selecting materials for medical phantom fabrication, and this research can provide basic data.

Association between physical activity measured using an accelerometer and arterial stiffness based on pulse wave velocity and ankle-brachial index in healthy adults (건강한 성인에서 가속도계로 측정한 신체활동과 맥파전달속도 및 상완-발목 간 혈압비에 기반한 동맥경화지표와의 관계)

  • Purpose: Physical activity (PA) has a beneficial effect on the prevention of arteriosclerosis in healthy adults. The purpose of this study was to analyze the relationship between PA measured using an accelerometer and arterial stiffness in healthy Korean adults. Methods: This study involved 87 subjects (36.8% women) aged 20-64 years. PA was evaluated using an accelerometer (wGT3X-BT, ActiGraph, Florida, USA) for 7 days. Based on the results of the accelerometer measurement, subjects were classified into active and inactive groups according to the World Health Organization (WHO) PA guidelines. The brachial-ankle pulse wave velocity (baPWV) and ankle-brachial index (ABI) to assess arterial stiffness were measured by a non-invasive vascular screening device (VP-1000 Plus, Omron). Results: The average age of the study subjects was 47.7 ± 11.3 years and the WHO PA guideline achievement rate was 29.9%. There was no significant difference in arterial stiffness (baPWV and ABI) between the active and inactive groups. In females, the time spent in light PA were positively correlated with ABI (r = 0.396; p < 0.05) and the number of sedentary bouts over 50 minutes was inversely correlated with ABI (r = -0.402; p < 0.05). However, there was no significant correlation between PA and arterial stiffness in males. Conclusions: The results of this study suggest that light PA and sedentary behavior have a positive correlation with arterial stiffness in females.

Consideration of the Usefulness of 18F-FET Brain PET/CT in Brain Tumor Diagnosis (뇌종양진단에 있어 18F-FET Brain PET/CT의 유용성에 대한 고찰)

  • Purpose: 18F-FET, a radiopharmaceutical based on a Tyrosine amino acid derivative using the Sodium-Potassium Pump-independent Transporter (System L) for non-invasive evaluation of primary, recurrent, and metastatic brain tumors, exhibits distinct characteristics. Unlike the widely absorbed 18F-FDG in both tumor and normal brain tissues, 18F-FET demonstrates specific uptake only in tumor tissue while almost negligible uptake in normal brain tissue. This study aims to compare and evaluate the usefulness of 18F-FDG and 18F-FET Brain PET/CT quantitative analysis in brain tumor diagnosis. Materials and Methods: In 46 patients diagnosed with brain gliomas (High Grade: 34, Low Grade: 12), Brain PET/CT scans were performed at 40 minutes after 18F-FDG injection and at 20 minutes (early) and 80 minutes (delay) after 18F-FET injection. SUVmax and SUVpeak of tumor areas corresponding to MRI images were measured in each scan, and the SUVmax-to-SUVpeak ratio, an indicator of tumor prognosis, was calculated. Differences in SUVmax, SUVpeak, and SUVmax-to-SUVpeak ratio between 18F-FDG and 18F-FET early/delay scans were statistically verified using SPSS (ver.28) package program. Results: SUVmax values were 3.72±1.36 for 18F-FDG, 4.59±1.55 for 18F-FET early, and 4.12±1.36 for 18F-FET delay scans. The highest SUVmax was observed in 18F-FET early scans, particularly in HG tumors (4.85±1.44), showing a slightly more significant difference (P<0.0001). SUVpeak values were 3.33±1.13 for 18F-FDG, 3.04±1.11 for 18F-FET early, and 2.80±0.96 for 18F-FET delay scans. The highest SUVpeak was in 18F-FDG scans, while the lowest was in 18F-FET delay scans, with a more significant difference in HG tumors (P<0.001). SUVmax-to-SUVpeak ratio values were 1.11±0.09 for 18F-FDG, 1.54±0.22 for 18F-FET early, and 1.48±0.17 for 18F-FET delay scans. This ratio was higher in 18F-FET scans for both HG and LG tumors (P<0.0001), but there was no statistically significant difference between 18F-FET early and delay scans. Conclusion: This study confirms the usefulness of early and delay scans in 18F-FET Brain PET/CT examinations, particularly demonstrating the changes in objective quantitative metrics such as SUVmax, SUVpeak, and introducing the SUVmax-to-SUVpeak ratio as a new evaluation metric based on the degree of tumor malignancy. This is expected to further contributions to the quantitative analysis of Brain PET/CT images.

A Comparative Study on Awareness of Middle School Students, School Parents, and Human Resources Directors in Industrial Institutions about Admission into Specialized High Schools and Career after Graduating from Specialized High Schools (특성화고 진학 및 졸업 후 진로에 대한 중학생, 학부모, 산업체 인사 담당자의 인식 비교 연구)

  • This study tried to suggest implications about operation direction of specialized high schools (SHS) by researching awareness of middle school students (MSS), school parents (SP), human resources directors in industrial institutions (HRDII) who will be the main users of SHS education, about entering SHS and career after graduating from SHS. Seniors of middle school, SP and HRDII in Asan, Chungnam were the subject of this survey research. The summary of the result of this study is as follow: First, MSS and SP usually hoped to enter general high schools rather than vocational education schools such as SHS, meister high schools, and MSS considered school records and SP considered aptitude and talent for the factors to choose high school. Second, MSS, SP, and HRDII recognized purposes of SHS as improvement of talent and aptitude, and getting a job. As for positive images of SHS, they recognized it as applying talent and aptitude to life early, getting good jobs easily, fast independence after graduation, and learning excellent technologies, and as for negative images of SHS, they recognized it as social prejudices and discrimination, students with bad school records enter them, disadvantages about promotion and wages, and being unfavorable for entering universities. They also recognized education of SHS as being effective for improvement of basic and executive ability and key competency, development of creative human resources, and improvement of right personality and courteous manners. Third, many MSS and SP showed intention to enter SHS if it is established in Asan. They wished to enter SHS because they would like to apply their aptitude and talent to life early, learn excellent skill, and hope for early employment, on the other hand, they did not wish to enter SHS because it was not suited for their aptitude and talent, awareness about SHS is low, it is unfavorable to enter universities, and there were social prejudices and discrimination. They also similarly hoped for getting jobs and entering universities after graduating from SHS. And the reason they wanted to get a job was usually because they want to be successful by advancing into society early, or because it is still hard to get a job even after graduate from the university, on the other hand, the reason they want to enter university is because is usually in-depth education about major and social discrimination about level of education. The ability to perform duties forms the greatest part of the employment standard that MSS, SP, and HRDII aware. MSS and SP usually hoped for industrial, home economics and housework and commercial majors in SHS, and considered aptitude and talent, the promising future, and being favorable for employment for choosing major. The reason HRDII hire SHS student was to develop student into talent of industrial institution, ability of student, and need for manpower with high school graduation level, and there were also partial answer that they can hire SHS student if they have ability to perform duties. The proposals about operation direction of SHS according to the results above are as follow: SHS should diversify major and curriculum to meet various requirements of student and parents, establish SHS admission system based on career guidance, and improve student's ability to perform duties by establishing work-based learning. The Government should organize work-to-school policy to enable practical career development of students from SHS, and promote relevant policy to reinforcing SHS education rather than quantitative evaluation such as employment rate, and cooperative support from each government departments is required to make manpower with skill related to SHS to get proper evaluation and treatment.

Clinical Implication of Malignant Pleural Lavage Cytology (PLC) in Primary Lung Cancer (원발성 폐암에서 악성 흉강 세척액의 임상적 의미)

  • The purpose of study is to evaluate the clinical implication of malignant Pleural Lavage Cytology (PLC) in primary lung cancer. 315 patients were examined with pleural lavage cytology in Asan Medical Center between November 1998 and August 2002. The patients were chosen from primary lung cancer patients with no pleural effusion according to preoperative radiologic examination; no tumor invasion into the chest wall and no diffuse pleural adhesion in intraoperative findings, The pleural cavity and lung were washed with 100 $m\ell$ of warm normal saline. The 315 patients consisted of 237 men and 78 women. The incidence of malignant PLC was found in 28 patients (8.9%). For patients in early stages (I & II), survival rate was 93.9% in positive malignant PLC and 85.7% in negative malignant PLC. 31 patients (13.6%) had local or distant recurrences; 2-year recurrence-free rate was 90.1% in negative PLC and 87.5% in positive PLC. The survival and recurrence-free rate in each stage were not statistically associated with the result of PLC. Median follow-up was 16.4 months from the surgery. To access implication of malignant PLC in primary lung cancer, a long-term follow-up and further study are required.

A systematic study of the Polygonum amphibium L. complex (Polygonaceae) based on chloroplast DNA sequences (엽록체 DNA 염기서열에 근거한 물여뀌 종집단(마디풀과)의 분류학적 연구)

  • The Polygonum amphibium complex (Poygonaceae) is a highly polymorphic taxon that can grow in aquatic environments as well as in moist terrestrial habitats. Aquatic and terrestrial plants of the P. amphibium complex vary significantly in morphology and exhibit very complicated patterns of morphological variation, resulting in the description of numerous infra-specific taxa. Principal components analysis of 107 individuals of the P. amphibium complex from Asia and North America using 11 morphological characters showed that the aquatic plants can be discerned from the terrestrial plants by leaf size, shape, and petiole length. In contrast, both aquatic and terrestrial plants collected from the same population or locality shared identical sequences in the matK, psbA-trnH IGS, rbcL-accD IGS and trnL-trnF regions of the chloroplast DNA (cpDNA), suggesting that aquatic and terrestrial forms of the P. amphibium complex are not genetically diverged; morphological differences between the two forms are probably due to the differences in environmental conditions of the habitats. In addition, results from the morphological analysis and the maximum parsimony analysis of the cpDNA data set revealed that the plants from Asia including Korea, Japan, China, Mongolia and Russia Far East are diverged from those in North America and Europe, suggesting that the Asian populations should be recognized as a distinct variety, P. amphibium var. amurense Korsh.

Reservoir Disaster Monitoring using Unmanned Aerial Photogrammetry (무인항공사진을 이용한 저수지 방재 모니터링)

  • The Disaster planning for the reservoir should be more quickly and intuitively establish measures by means of the sequential monitoring of change status of the reservoir water level and water surface area. This paper presents an approach using the orthophoto image produced by the periodic unmanned aerial photogrammetry and analyzed the feasibility. Total three time of unmanned aerial survey were conducted to make orthophoto images for the Seongnae reservoir and we analyzed the amount of changes for water level and surface area compare with each images. As the Analysis results, it was possible to effectively observe the increase in the water level rises and the surface area due to the rainfall. The maximum deviations of orthophoto images was 7.5cm in X-direction, 10.8cm in Y-direction and 14.1cm in elevation compare with ground surveying results. Therefore, we conclude that the unmanned aerial photogrammetry could be applied with comprehensive reservoir monitoring works for disaster management for reservoir in the future. And, the orthophoto production takes about two hours to shoot the images, and approximately four hours is considered for the image processing. So, the unmanned aerial photogrammetry is considered to be the best disaster work that requires urgent because analysis is possible in the shooting day.

Analyzing the spectral characteristic and detecting the change of tidal flat area in Seo han Bay, North Korea using satellite images and GIS (위성영상과 GIS를 이용한 북한 서한만 지역의 간석지 분광특성 및 변화 탐지)

  • In this study the tidal area in Seo han bay, North Korea was detected and extracted by using various satellite images (ASTER, KOMPSAT EOC, Landsat TM/ETM+) and GIS spatial analysis. Especially, the micro-landform was classified through the spectral characteristic of each satellite image and the change of tidal flat size was detected on passing year. For this, the spectral characteristics of eight tidal flat area in Korea, which are called as Seo han bay, Gwang ryang bay, Hae iu bay, Gang hwa bay, A san bay, Garorim bay, Jul po bay and Soon chun bay, were analyzed by using multi band of multi spectral satellite images such as Landsat TM/ETM+. Moreover, the micro-landform tidal flat in Seo han bay, North Korea was extracted by using ISODATA clustering based on the result of spectral characteristic. In addition, in order to detect the change of tidal flat size on passing years, the ancient topography map (1918-1920) was constructed as GIS DB. Also, the tidal flat distribution map based on the temporal satellite images were constructed to detect the tidal flat size for recent years. Through this, the efficient band to classify the micro-landform and detect its boundary was clarified and one possibility of KOMPSAT EOC application could be also introduced by extracting the spatial information of tidal flat efficiently.

Dental Management of Pediatric Hydrocephalus (소아 수두증 환자의 치과적 관리)

  • Hydrocephalus is a condition in which the volume of cerebrospinal fluid in the cerebral ventricles is abnormally elevated. Hydrocephalus patients may show macrocephaly and delayed development. Ventriculoperitoneal shunt is the most commonly used treatment but ventriculoatrial shunt is another treatment option. This report discusses the dental management process employed for a 7-year, 6-month old patient with ventriculoperitoneal shunt-treated hydrocephalus. This patient showed dental caries on the deciduous molars, facial swelling, macrocephaly and delayed development. Pulpectomy was performed on the deciduous molars under nitrous oxide sedation and mild protective stabilization. This patient did not appear to have any specific complications until the latest follow-up and was scheduled for the management of overall oral hygiene, development of permanent teeth, and craniofacial asymmetry through periodic follow-up. During dental treatment of shunt-treated patients, care should be taken to avoid applying excessive force to the catheter running along the patient's neck to prevent the displacement of the catheter. A referral to a neurosurgeon is recommended for patients with ventriculoatrial shunts for prophylactic antibiotics. As hydrocephalus patients grow, they may show craniofacial asymmetry or differences in the calcification of the permanent teeth, and require periodic oral and craniofacial assessment.
