A Survey on the Foodservice Management System of the Child Care Centers in Chungnam Asan Area (영유아 보육시설의 급식 운영 실태 조사 - 충남 아산 지역 중심으로 -)

  • We investigated the foodservice management practices in various operation types of childcare centers in Asan, Chungnam Province, with the intention of improving the quality of foodservice and providing the basic information for establishing more effective and efficient foodservice model system. Self-completed questionnaires were collected from the directors of 174 child care centers. The statistical analysis was completed using SPSS Ver. 12.0 program. The followings are about the results of this study. Approximately 94.8% of the directors were women with the average age of 40.3. All of the investigated facilities executed foodservice; the facilities of 96.2% had been self-operated, 1.9% was contract-managed and the remaining 1.9% served delivered meal from outside. Only 20.0% of the investigated centers employed a dietitian. In most of the centers, meals were prepared in a conventional manner and approximately 85.3% of the centers are serving only snacks twice a day as a supplementary due to financial difficulties. Menu planner of the facilities, which have no dietitian was the director (35.8%) or the cook (25.7%). In most centers, the directors purchased the food materials (67.5%). Material inspection was done by the director (54.9%) or the cook (40.5%). However, home care centers did not inspect the food material. These results indicate that food service management guidelines need to be established by the facility type with the government control and financial support. Especially, dietitian employment and the efficient foodservice model system establishment are questions that confront us.

Second Opinion Diagnoses of Cytologic Specimens on Consultation : Asan Medical Center Experience (세포 병리 자문 재판독의 진단 불일치에 관한 연구: 서울아산병원의 경험)

  • Background : Second opinion diagnosis of outside pathology slides is a common practice for efficient and proper patient management. We analyzed cytology slides from outside hospitals submitted for a second opinion diagnosis to determine whether the second opinion diagnosis had any influence on patient care. Methods : We reviewed 1,153 outside cytology slides referred to Asan Medical Center for second opinions from January, 2007, to December, 2007. All cases were categorized into three groups; no diagnostic discrepancy, minor diagnostic discrepancies (no impact on the management), and major diagnostic discrepancies (significant impact on the management and subsequent follow-up). Results : The thyroid was the most common organ system (933 cases, 80.9%), Forty cases (3.6%) belonged to the major diagnostic discrepancy group and 149 cases (12.8%) to the minor discrepancy group. For validation of second opinion diagnoses in major discrepancy cases, subsequent biopsy or surgical resection specimens and clinical information were reviewed, which were available in 29 cases. The second opinion diagnoses resulted in alteration of clinical management in 21 of 29 cases. Conclusion : For all referred patients, second opinion diagnosis is important and mandatory for appropriate patient care.

Construction and Validation of Hospital-Based Cancer Registry Using Various Health Records to Detect Patients with Newly Diagnosed Cancer: Experience at Asan Medical Center (의무기록의 다각적 활용을 통한 충실도 높은 병원 암등록 체계의 구축: 서울아산병원의 경험)

  • Objectives: An accurate estimation of cancer patients is the basis of epidemiological studies and health services. However in Korea, cancer patients visiting out-patient clinics are usually ruled out of such studies and so these studies are suspected of underestimating the cancer patient population. The purpose of this study is to construct a more complete, hospital-based cancer patient registry using multiple sources of medical information. Methods: We constructed a cancer patient detection algorithm using records from various sources that were obtained from both the in-patients and out-patients seen at Asan Medical Center (AMC) for any reason. The medical data from the potentially incident cancer patients was reviewed four months after first being detected by the algorithm to determine whether these patients actually did or did not have cancer. Results: Besides the traditional practice of reviewing the charts of in-patients upon their discharge, five more sources of information were added for this algorithm, i.e., pathology reports, the national severe disease registry, the reason for treatment, prescriptions of chemotherapeutic agents and radiation therapy reports. The constructed algorithm was observed to have a PPV of 87.04%. Compared to the results of traditional practice, 36.8% of registry failures were avoided using the AMC algorithm. Conclusions: To minimize loss in the cancer registry, various data sources should be utilized, and the AMC algorithm can be a successful model for this. Further research will be required in order to apply novel and innovative technology to the electronic medical records system in order to generate new signals from data that has not been previously used.

Determining Method of Sulfites in Foods by Ion Chromatography

  • Anion chromatographic (lC) method was developed for the determination of sulfites infoods. Sulfites refer to sulfur dioxide that was separated from a food sample by addition of acid and heating, and carried into a trapping solution by distillation. The trapping solution was applied to IC system. Sulfites was separated on an anionic separator column, HPIC-AS4A with 0.75mM $NAHCO_3/2.25mM\;Na_2CO_3$ as the eluent and determined by the use of conductivity detector. The recoveries of sulfites added to water, carrot and apple at level of 1 ppm were 99.8%, 91.6% and 83.5%, respectively. The detection limit was 0.2 ppm in the case of a 10 g sample size. All experiment could be finished within 20 minutes

A Study on Exposure Dose from Injection Work and Elution Work for Radiation Workers and Frequent Workers in Nuclear Medicine (핵의학과 주사와 분배업무 작업종사자 및 수시출입자 피폭선량연구)

  • Compared to other occupations, there is a greater risk of exposure to radiation due to the use of radioisotopes in nuclear medicine for diagnostic evaluations and therapy. To consider ways to reduce exposure dose for those in nuclear medicine involved in injection work and elution work among radiation workers as well as for sanitation workers and trainees among frequent workers an investigation into exposure dose and situational analysis from changes in yearly exposure dose evaluations, changes in work environment and changes in forms of inspection were conducted. Exposure dose measurements were taken by using EPD MK2 worn during working hours for one injection worker, one elution worker, two sanitation workers, and one trainee at a general hospital in the Seoul area for three days from July 18th to 20th 2016. Radiation from radioisotopes which are a part of nuclear medicine can significantly affect not only radiation workers who deal with radioisotopes directly but also frequency works as well. According to this study the annual dose limit for elution workers and injection workers were considered safe as the amount of exposure was not large enough to have a significant effect. The limits of this study consist in the duration of this study and the quantity of participants. Also there was a limitation of the measurement device involving accumulated exposure, where the EPD MK2 cannot check the changes in exposure according to a particular activity.

Estimation of Lens Dose of Radioactive Isotopes Using ED3 (ED3를 이용한 방사성동위원소 의약품의 수정체 피폭선량평가)

  • It is suggested that the dose limit recommended in the Enforcement Decree of Korea's Nuclear Safety Act should not exceed 150 mSv per year for radiation workers. Recently, however, ICRP 118 report has suggested that the threshold dose of the lens should be reduced to 0.2~0.5 Gy and the mean dose should not exceed 50 mSv per year for an average of 20 mSv over 5 years. Based on these contents, $^{123}I$, $^{99m}Tc$, and $^{18}F-FDG$, which are radioisotope drugs that are used directly by radiation workers in the nuclear medicine department in Korea are expected to receive a large dose of radiation in the lens in distribution and injection jobs to administer them to patients. The ED3 Active Extremity Dosimeter was used to measure the dose of the lens in the nuclear medicine and radiation workers and how much of the dose was received per 1 mCi.

Bio-applicable Ti-based Composites with Reduced Image Distortion Under High Magnetic Field (높은 자장하에서 자기공명 영상 왜곡이 완화된 생체용 Ti 복합재료)

  • When viewed using a magnetic resonance imaging (MRI) system, invasive materials inside the human body, in many cases, severely distort the MR image of human tissues. The degree of the MR image distortion increases in proportion not only to the difference in the susceptibility between the invasive material and the human tissue, but also to the intensity of the magnetic field induced by the MRI system. In this study, by blending paramagnetic Ti particles with diamagnetic graphite, we synthesized $Ti_{100-x}C_x$ composites that can reduce the artifact in the MR image under the high-strength magnetic field. Of the developed composites, $Ti_{70}C_{30}$ showed the magnetic susceptibility of ${\chi}=67.6{\times}10^{-6}$, which corresponds to 30% of those of commercially available Ti alloys, the lowest reported in the literature. The level of the MR image distortion in the vicinity of the $Ti_{70}C_{30}$ composite insert was nearly negligible even under the high magnetic field of 4.7 T. In this paper, we reported on a methodology of designing new structural materials for bio-applications, their synthesis, experimental confirmation and measurement of MR images.

Failure Analysis of Ti alloy Screws in Fixing Fractured Spines (척추교정 티타늄 앵커나사 파단 손상원인 분석)

  • Failure analyses of the screws in spinal fixation devices were carried out. The fractured screws were retrieved from a patient who had spinal surgery in the thoracic vertebrae from number 10 to 15. The failure occurred one month after the removal of the braces. Microstructures and fracture surfaces were examined by optical and scanning electron microscopy. The microstructures of the screws corresponded to annealed Ti-6Al-4V bar. However, in the vicinity of the screw surface, there was an insufficient number of fine precipitates. Fracture surfaces showed typical fatigue failure modes. Regarding the fact that no machining defects were detected, fatigue crack initiation might have been caused by the lack of precipitates near the screw surfaces. Only the fourth of five fixed screws was severely stress-concentrated by the action of the spinal bones, while the stress of the 4th screw was decreased to half of its acceptable level when the screw was supplemented by one more, which might have been fixed in the 6th vertebra under the 5th position by the switching of its position. The stress simulation was conducted by ANSYS with 3D CAD of PRO/E in order to understand the stress concentration behavior and to provide an effective spinal surgery guide.

  • In order to identify the major stressors of some college students and their relationship with oral habits, a questionnaire survey was conducted for 299 male and female college students in Sunmoon University located in Asan-si, Chungcheongnam-do. As a result convergence study of analyzing stress by college students, the relationship between health and interpersonal relationship among the causes of stress was statistically significant. Based on the above studies, the incidence of oral habits caused by the stress of college students was very high. Considering that the stress factors that occur during college life cannot be completely excluded, it is necessary to have an education framework on improving stress self-recognition, control, and bad habits.

  • Samples of two species of genus Ratts(black rat, Rattus rasttus Linnaeus; common rat, Rattus norvegicus Berkenhaut) in Korea were trapped and their 31 morphometric charcters were analyzed statistically in order to determine the range of geographic variation within each species and the interspecific differences. In addition, chromosomal G-bands and C-bands were compared and the fragment patterns of mtDNA resulted from the digestion with restriction enzymes were also analyzed. Samples of black rats from six localities in Korea were similar with one another in their morphometric characters: in head and body length, length of tail vertebrae, conventional karyotype and C-bands, they are comparable to Rattus rattus tanezumi in Japan. Specimens of common rats from seven localities in Korea were similar with one another in their morphometric characters: in conventional karyotype, they are comparable to Rattus norvegicus caraco in eastern Asia. Common rats differ from black rats in their morphometric characteris, chromosomal karyotypes and mtDNA. It is confirmed that correct species name of black rat in Korea is Rattus rattus tanezumi Tempminck: species name of common rat in Korea is Rattus norvegicus caraco Pallas: the common rat is a species, which is distinct from the black rat.
