아동기에 주의력결핍 과잉행동증상을 경험한 성인의 정신병리와 기질 성격 요인 (Psychopathology, Temperament, and Character Factors of Adults with Childhood Attention-Deficit Hyperactivity Symptoms)

  • Objectives : The objective of this study is to find out the differences in psychopathology, temperament, and character factors between young adults with and without childhood attention-deficit hyperactivity disorder (ADHD) symptoms. Methods : The subjects consisted of 314 university students (male=72, female=242) in Gongju. They were divided into childhood ADHD group (N=54) and normal group (N=260) to the Wender-Utah Rating Scale. Participants were assessed using Korean Adult ADHD Scale, Beck Depression Inventory-II, Beck Anxiety Inventory, Kimberly Young's Internet Addiction Test, Barratt Impulsiveness Scale, Paranoia Scale, Korean-Schizotypal Ambivalence Scale, Lubben Social Network Scale, and Temperament and Character Inventory-Revised. Descriptive statistics, t-test, chi-square, spearman correlation, and stepwise multiple regression were applied to analyze the data. Results : Participants with childhood ADHD symptoms had high level of adult ADHD symptoms (p<.001), impulsiveness (p=.001), depression (p<.001), anxiety (p<.001), internet addiction (p<.001), paranoia tendency (p<.001) and low level of selfesteem (p<.001) compared to normal group. Self-directedness (p<.001) was lower, Harm avoidance (p=.001) and Self-transcendence (p=.029) were higher in the childhood ADHD group. In correlation and stepwise multiple regression, childhood ADHD symptoms were significantly associated with cooperativeness (r=-0.515 and ${\beta}$=-0.547, p<.001 respectively). Conclusion : This result showed that young adults with childhood ADHD symptoms had various forms of psychopathology and childhood ADHD symptoms were influenced by character such as cooperativeness. Therefore, more thorough evaluation regarding childhood ADHD symptoms is needed.

Pulse Oximeter를 이용한 치수생활력측정 (PULSE OXIMETER AS A DIAGNOSTIC TEST OF PULP VITALITY)

  • 치수 생활력 검사시 전통적인 방법으로 전기치수검사나 온도변화검사 등이 있다. EPT와 ice test 즉, 전기치수검사와 온도변화검사는 치아의 신경학적 반응에 의해 치아의 실활여부를 판단하는 방법으로 환자가 아동일때는 정확한 반응을 얻기가 어렵고, 환자의 주관적 반응을 판단해야 하므로 객관적이지 못하고, 소아 환자에게 좋지 못한 자극을 주어 행동 조절의 문제를 일으키며, 거짓 양성반응과 거짓 음성반응이 나을 수 있다는 등의 한계가 있다. 최근에는 전통적인 방법의 한계를 극복하려는 시도에서 혈관의 보전성을 평가하는 방법인 laser doppler flowmetry와 pulse oximeter를 이용하는 방법이 소개되고 있다. Pulse oximeter의 원리는 두 가지 종류의 파장의 빛을 귀, 손가락등 생체의 말단에 투과시켜 발산된 빛과 감지된 빛간의 두 파장의 흡수비로 산소 포화 정도를 알아내는 것으로서 이에 착안하여 또 하나의 생체 말단인 치아에 이를 적용하여 치아의 실활 여부를 판단하는 것이다. 이 보고에서는 치수 생활력 검사시 pulse oximeter의 사용 가능성에대해 검증하고 이의 임상적용에 대해 기초적 자료를 제공하고자 함을 목적으로 했는데 생활치 에서는 평균 96.3%의 산소 포화도를 실활치에서는 평균 0.0%의 산소 포화도를 얻어냄으로서 pulse oximeter가 치아의 실활여부 판단에 있어 유용한 진단도구로서의 가치가 있다는 것이 밝혀졌다.

취학 전 아동의 감각처리능력에 따른 행동적 특성에 관한 연구 (The Correlation Between Sensory Processing Skills and Behavior Characteristics for Preschoolers)

  • Objectives : The purpose of the study is to understand the sensory processing capacity and behavioral characteristics for preschoolers without disabilities, and to investigate the relationship between sensory processing skills and the behavioral characteristics. Methods : Mothers of preschoolers without disabilities between ages of 4 and 6 who attend Y kindergarten which is located in Busan, H kindergarten in Suwon, S kindergarten in Gimhae was participated. Mothers filled out the questionnaire at home from 4th to 14th of January in 2004. We used SSP(short sensory profile) which was used by Kim, Mi-Sun in 2001 to understand the sensory processing skills. We also used Behavior During Testing Checklist find their places in Miller Assessment for preschoolers to understand behavior characteristics. The results were analyzed with SPSS 10.0. Results : 1. Total incidents in sensory processing were 157 out of 190. Among sub items of the sensory processing, the mark of lower energy/weak was highest with 4.39 point and the mark of taste/smell sensitivity was lowest with 3.60 point. Total incidents in behavioral characteristics were 20 out of 24. Among the area of behavioral characteristics, the mark of sensory responses/threshold area was highest with 2.73 point and the mark of social interaction area was lowest with 2.29 point. 2. Children's total sensory processing capacity correlates with behavioral characteristics, the more sensory processing capacity, the more behavioral characteristics. Sensory processing point correlates with behavioral characteristics points except this item, reaction to separation from caretaker. Conclusions : We hope that the children who have behavioral characteristics with difficulties in sensory processing skills can be distinguished on the basis of this studying. Also, as we find and relieve early stage of the symptoms, following study which can present based to facilitate children's social development and improve the learning ability.

대한감각통합치료학회지의 연구동향: 2003년부터 2014년까지의 연구를 중심으로 (An Analytical Study on Research Trends in Journal of Korean Academy of Sensory Integration : From 2003 to 2014)

  • 목적 : 본 연구는 대한감각통합치료학회지의 연구 분야와 동향을 분석하여 나아갈 방향을 모색하고자 학회지가 발간된 2003년부터의 연구를 분석하였다. 연구 방법 : 2003년부터 2014년까지 학회지에 수록된 97개 연구의 형태 및 동향, 근거수준, 대상자, 연구비 지원 수혜 연구의 비율, 연구영역을 분석하였다. 결과 : 매 5년마다 연구 형태는 실험연구 비율이 가장 높았고, 실험연구와 고찰연구가 꾸준히 증가하는 추세이다. 질적 수준을 살펴보면, 단일 집단 비무작위 연구가 가장 많았고, 다음은 사례 연구이었다. 연구 대상자는 장애아동을 대상으로 한 연구가 가장 많았는데, 그 중 전반적 발달장애와 발달지연, 지적장애 순으로 많았다. 대상자의 연령은 초등학생, 유아 순으로 많았으며, 대상자 수는 10명 이하가 가장 많았다. 2008년 이후 수록된 전체 연구 중 연구비 지원 수혜 대상 연구는 50%이었다. 표준화된 평가 중심으로 연구영역을 살펴보면, 감각조절, 상호작용, 발달 순으로 많았고, 비표준화된 평가 중심으로 분석한 결과 행동관찰과 임상관찰 순으로 많았다. 결론 : 높은 근거 수준의 연구와 다양한 연구 방법을 사용한 연구가 활발히 이루어져야 하며, 국내에서 사용할 수 있는 감각통합을 측정하는 민감도가 높은 체계적인 평가도구의 개발이 필요하다. 이를 통해, 궁극적으로는 작업치료의 근거중심실행을 하는데 이바지 할 수 있을 것이다.

양육행동에 대한 어머니 신념과 아동의 자기능력 지각과의 관계 (A Study on the Relationship with Maternal Beliefs on the Child Rearing and Children's Self-perceptions)

  • The purpose of this study was to investigate the relationships between mother's beliefs on child rearing and children's self-perceptions. The subject of this study were comprised of 167 children aged 8-10 in Iksan & Kunsan city from Dec. 2 to Dec. 8, 2000. The children's self-perceptions were assessed by the self perceptions profile for children. Mother's beliefs questionnaire, developed by Okagaki and Sternberg, was used to obtain the data on mother's beliefs of parenting. The methods of analyses included basic descriptive categorical analysis as well as t-test, one way ANOVA-test, and multiple regression analyses. The major findings from the analyses are as follows: First, a significant difference was found in the degree of mother's beliefs about child rearing by mother's religion(p<.01), mother's education(p<.05), income(p<.001), level of life (p<.0l), mother's job, mother and fathers' agreement of child rearing(p<.05), relationship between mother and child(p<.05) in independence but a significant difference was not found in the degree of mother's beliefs about child rearing in accommodation. Second, the levels of children's scholastic performance were related to higher levels of mother's education(p<.001), mother's income(p<.001), mother and fathers' agreement of child rearing(p<.01). The levels of children's social competence was related to higher, child's gender (p<.01); girls were higher than boys. The levels of children's athletic abilities were not significant. The levels of children's physical appearance was related to higher levels of mother's education(p<.01), mother's income(p<.01), level of life(p<.05), mother's marriage(not divorce)(p<.01). The levels of children's behavioral conduct were related to higher levels of child's gender (p<.05), mother's education(p<.01), mother's income (p<.01), relationship between father and child(p<.0l). The levels of children's global self worth were related to higher levels of mother's age(p<.05), mother's education(p<.001), level of life(p<.05). Third according to the multiple regression analyses for the relationships between mother's beliefs about child rearing and children's self perceptions, mother's beliefs on the creativity were related to higher levels of children's scholastic performance, social competence, and mother's beliefs on the problem solving skills to higher levels of children's behavioral conduct and mother's beliefs on the accommodation to lower levels of children's scholastic performance, social conduct.

감각통합치료가 뇌성마비 아동의 감각.운동발달 및 적응행동에 미치는 영향 (Effects of Sensory Integration Therapy on Sensory. Motor Development and Adaptive Behavior of Cerebral Palsy Children)

  • The purpose of this study was to examine the effects of sensory integration therapy (SIT) on sensory' motor development and adaptive behavior of cerebral palsy children. The design of this study was quasi experiments with a non-equivalent pre- and post-test control design. Subjects of the study were arbitrarily chosen based on predetermined selection criteria among the cerebral palsy children who were treated as out-patients at two rehabilitation hospitals one in Seoul, and the other in Kyunggi-do. The study was conducted between early April and late July in 2000. Fifteen children were in the experimental group and eleven in the control group. The allocation was done based on ease of experimental treatment. A five-step SIT program was devised from a combination of SIT programs suggested by Ayres(1985) and Finks(1989), and an author-designed SIT program for cerebral palsy children. The experimental group was subjected to 20 to 30 minutes of SIT per session. two sessions a week for ten -week period. The effects of SIT were measured with respect to 9 sub-areas that can be administered to cerebral palsy children out of a total of 17 sub-areas in the Southern California Sensory Integration Test (SCSIT) developed by Ayres (1980). In addition. the scale developed by Russell (1993) for Gross Motor Function Measure (GMFM). and Perception Motor Development Test developed by 中司利一 et al.(1987) were also applied. Adaptive behavior was analyzed using guidelines in two unpublished documents - School-Age Checklist for Occupational Therapy by the Wakefield Occupational Therapy Associates, and the OTA-Watertown Clinical Assessment by the Watertown Occupational Therapy Associates-, and an author-developed Adaptive Behavior Checklist. Collected data were statistically analyzed by SPSS PC for chi square test, Mann-Whitney test, Wilcoxon signed rank test, and paired t-test. The results were as follows: 1. In sensory development, the experimental group exhibited a score increase compared to the control group, but the difference was not statistically significant, Although the experimental group showed improvements in all. 9 sub-areas compared to the control group, only right-left discrimination exhibited statistically significant change. 2. In gross motor development, the experimental group showed improvements in score compared to the control group, but it was not statistically significant. In fine motor development, the experimental group exhibited statistically significant improvements compared to the control group. In sub-area analysis, figure synthesis showed positive change. 3. In adaptive behavior development, post-experimental adaptive behavior scores were higher compared to pre-experimental scores with statistical significance. Furthermore, sub-areas emotional behavior, perception behavior, gross-fine motor function, oral-respiration function, motor behavior, motor planning, and adaptive response exhibited higher scores after SIT. In conclusion SIT was found to be partially effective in sensory and fine motor development, effective in all adaptive behavior areas, and not effective in gross motor development. Thus, this study has shown that SIT is an effective intervention for sensory development, fine motor development, and adaptive behavior for cerebral palsy children. But, for the effectiveness of SIT on gross motor development, further studies employing longer-time experiments are recommended.

언어네트워크분석을 통한 유아놀이 관련 연구동향 탐색 (Research Trends of Young Children's Play Using the Semantic Network Analysis)

  • 누리과정 개정으로 인해 유아놀이에 대한 관심이 높아지면서 관련 연구의 양적 증대와 더불어 다각적인 연구가 진행되고 있다. 이에 본 연구에서는 최근 10년간 교육학 및 유아교육학을 포함한 사회과학, 생활과학 등 다양한 연구분야에서 발행된 등재후보지 이상의 학술지를 대상으로 언어네트워크분석을 적용하여 시기별 유아놀이 관련 연구의 핵심단어를 도출하고 핵심단어의 중심성 분석을 하고자 하였다. 분석결과로 나타난 주요특성은 다음과 같다. 첫째, 유아놀이관련 연구는 1기(2009~2013년)에 비해 최근 5년 간 큰 폭의 키워드 증가를 나타내었고 주제 분야별 학술지 게재현황을 살펴본 결과 교육학, 유아교육, 생활과학, 아동보육, 사회과학 분야의 순으로 나타났다. 둘째, 키워드 네트워크를 분석한 결과 1기(2009~2013년)에는 놀이성, 역할놀이, 유아, 창의성, 놀이, 또래놀이상호작용 등이 중점 키워드로 나타났으며 2기(2014~2018년)에는 놀이성, 유아, 놀이, 또래놀이행동, 또래놀이상호작용 순으로 높은 빈도를 나타냈다. 또한 1기와는 다르게 2기에는 교사·유아 상호작용, 언어능력, 행복감, 인지능력, 유아교육 등의 키워드가 새로이 등장하였다. 이러한 결과를 바탕으로 향후 관련 연구의 발전 방향을 모색하였다.

초등학교 아동의 성역할 정체감 및 가정환경변인에 따른 의복행동연구 (A Study on the Clothing Behavior of Elementary School Students in Connection with Sex-Role Identity and Home Enviornments)

  • Clothing is considered as a means of self-projection in the childhood and children show clear preference on their own favorite clothing on which their latent identity is reflected. Changing sex-role identity, in particular, in the modern complex society specially affects the clothing behavior of children. The purpose of this study is to investigate the clothing behavior of children in conjunction with sen-role identity and home environment variables. This study will provide fundamental information on the clothing behavior in the childhood and be of help in selecting appropriate clothing in accordance with the variation of sex-role identity. Necessary information for this study was obtained through a questionaire. 'Bem sex-role identity inventory' was used to investigate the effect of sex-role identity and the clothing behavior was studied in conjunction with the factors of conformity, dependence, aesthetics and interest. The subjects of investigation are composed of three hundred male and female elementary school students in the 5th and 6th grades. For the statistical analysis of the collected data, percentage(%), mean value(M), standard deviation(SD) and Pearson's correlation coefficient were calculated and t-test, $x^2$-test, and Duncan's multiple range test were performed. The results form this study are as follows : 1. Sex-role identity shows a little difference according to gender, order of a birth and standard of living. This small difference is shown to be statistically insignificant. 2. In clothing behavior in connection with the sex-role identity, statistically significant trend was not found in typical sex-role identity group. However, clothing behavior in modernistic sex-role identity group exhibits statistically significant results in the factors of dependence, aesthetics and interest at the level of P<.001. 3. As concerns the clothing behavior in connection with gender, boy students show statistically more significant tendency in aesthetics and interest than girls do. However, boy students show statistically more significant tendency in aesthetics than girls do in connection with the standard of living. Although the clothing behavior associated with the level of parents education, mother's age and existence of mother's job shows a little difference in mean values, statistically significant trend was not detected. 4. Clothing behavior of the children shows a meaningful correlation between the factors. 5. Through 3-dimensional variables analysis, it is found that there is no mutual interaction between gender, sex-role identity and standard of living on the clothing behavior. The results from MCA supplementary analysis reveal that there is difference according to the gender, sex-role identity and standard of living.

감각통합 중재 효과에 대한 체계적 고찰 (The effectiveness of Sensory Integration : Systematic Review)

  • 본 연구의 목적은 감각통합 중재 효과에 관해 체계적 고찰을 통해 최근 연구 동향을 살펴보고, 감각통합 중재가 어떤 영역에서 효과가 나타나는지 객관적인 근거를 찾고자 함이다. 연구 분석을 위해 데이터베이스 Medline과 EMBASE에 "Occupational therapy", "Sensory integration therapy", "Sensory processing", "Weighted vest", "Wilbarger protocol"를 검색하였다. 2010년부터 2015년까지의 연구 중 감각통합 중재 효과를 보고한 14개의 연구를 분석하여 PICO원리에 따라 정리하였다. 연구결과 근거수준I과 근거수준V의 연구가 각각 4개(28.6%)로 가장 많았고, 중재 효과를 측정하기 위해 사용된 평가도구를 빈도 분석한 결과 GAS와 VABS-II가 4개(11.8%)의 연구에서 사용되었다. 분석 연구들에서 자폐스펙트럼 장애 아동들이 71.4%로 주요 대상군이었으며, 감각통합 중재가 운동수행, 감각처리, 행동, 학습과 관련한 교육, 작업수행영역에서 효과가 있었다. 본 연구결과를 통해 작업치료 임상에서 감각통합을 중재 계획으로 수립하는데 유용한 자료로 활용할 수 있을 것이다. 향후 연구에서는 폭넓은 재활 분야에서 사용되고 있는 감각통합 중재 효과에 대한 연구를 포함하여 감각통합 중재의 효과성 검증에 대한 연구가 이루어져야 할 것으로 사료된다.

12주 OROS-Methylphenidate 약물 치료 후 ADHD 아동의 삶의 질 변화 (Change of Quality of Life in Children with ADHD after 12 Weeks OROS-Methylphenidate Treatment)

  • Objectives : The aim of this study is to investigate the effectiveness of treatment with osmotic-release oral system methylphenidate (OROS-MPH) on quality of life (QOL) in children with attention-deficit hyperactivity disorder (ADHD). Another aim is to assess the relationship between change in QOL and other factors including children's symptoms and academic performance or parents' depression and parenting stress. Methods : A total of 111 medication-naive children with ADHD in a multicenter, open-label, 12-week trial of OROS-MPH completed an evaluation using diverse rating scales at two time points; at baseline and after 12 weeks of treatment. Scales for investigation of children included the Parent Report Form-Children's Health and Illness Profile-Children's Edition (PRF-CHIP-CE) on QOL, the ADHD Rating Scale-IV on symptoms, and the Academic Performance Rating Scale (APRS). The Beck Depression Inventory and Parenting Stress Index were used for assessment of their parents. Results : Total scores for mean PRF-CHIP-CE increased from $207.9{\pm}26.7$ at baseline to $226.3{\pm}25.9$ after 12 weeks of treatment (p<.001). The change of APRS showed the strongest correlation with the increment of PRF-CHIP-CE scores (Pearson coefficient=0.561, p<.001), even after controlling for other factors (partial correlation coefficient=0.420, p<.001). Conclusion : Treatment with MPH-OROS results in improvement of QOL in children with ADHD in Korea. The advance in academic performance plays a key role in this change of QOL.
