  • 응급의학은 여러 가지 질환이나 외상, 사고 등의 원인으로 생명이 위독한 상태에 있는 긴급한 사람을 신속히 진단하고 치료하는것이 주 역할이나, 때로는 사람이 사망하였을경우 이를 다시 살리는 이른바 심폐소생술을 시행함으로써 죽은 사람을 살리는 일도 취급한다. 이러한 행위를 심폐소생술이라 일컫는 이유는, 사람이 죽으면 심장이 멎고 폐를 통한 호흡이 중단되므로 호흡과 심장박동을 인위적으로 다시 소생시켜 주는 시술이기 때문이다. 심폐소생술에는 기본심폐소생술과 전문심폐소생술로 구분한다.

The Effect of a Mechanical Chest Compressions for Out-of-hospital Advanced Cardiac Life Support (병원 전 전문심장소생술을 위한 기계적 가슴압박기의 효과)

  • The purpose of this study is to evaluate the quality of chest compression by conducting comparison research between mechanical chest compressor(LUCAS) and manuale cardiopulmonary resuscitation(CPR) in a out-of-hospital environment and suggest effective advanced cardiac life support using mechanical chest compressors. For this, a out-of-hospital cardiac arrest was simulated with a team of 3 ambulance workers, and manuale CPR and CPR using LUCAS were performed on site and during transport in an ambulance. The research results are as follows: the comparison of manuale CPR between on site and in an ambulance revealed that on-site manuale CPR showed significant differences in the average compression depth, compression rate, and relaxation rate. Second, the comparison between manuale CPR and LUCAS in an ambulance showed significant differences in the average compression depth, compression rate, the number of compression per minute.

  • 기본인명구조술은 의식이 없는 환자를 발견한 후 구조를 요청하고 기도유지, 구조호흡, 흉부압박을 시행하는 심폐 소생술의 초기단계이다. 이번 호에는 심정지가 의심되는 환자를 발견하였을 때의 행동요령과 심폐소생술에 대하여 알아보고자 한다. 심정지의 발생은 예측이 불가능하며 심정지가 발생한 후 4~6분이 경과하면 치명적인 뇌손상이 발생하므로 환자가 발생한 현장에서부터 목격자에 의한 심폐소생술을 시행함으로써 폐와 심장의 기능을 유지하고, 뇌의 생리학적 사망을 연장시켜 심정지 환자의 생명을 살릴 수 있다. 그러므로 환자발견 현장에서 빠른 시간 내에 심폐소생술을 시행하는 것이 중요하다. 응급의료 선진국에서는 현장에서의 빠른 응급처치로 인해 20%의 생명을 살릴 수 있는 반면, 우리나라는 4%에 불과하다.

  • 연구의 목적은 간호대학생의 영아심폐소생술에 대한 지식, 태도, 수행능력의 정도를 확인하고 그 관계를 분석하기 위함이다. 이 연구에 참여하기를 동의한 간호대학생 151명을 대상으로 자가보고식 설문지를 이용하여 자료를 수집하였다. 자료는 SPSS 21.0 Win을 이용하여 t-test, ANOVA, peason's correlation coefficients을 이용하여 분석 하였다. 영아심폐소생술의 대한 지식과 수행능력, 태도와 수행능력은 긍정적 상관관계가 있었다. 본 연구결과를 토대로 간호대학생들의 영아 심폐소생술 수행능력을 증진시키기 위해 지식과 태도를 강화할 수 있는 반복적 영아 심폐소생술 교육프로그램을 개발할 필요가 있다.

Effects of Basic Life Support Education Program in Middle School Students' Basic Cardiac Life Support Knowledge, Attitude and Performance (기본심폐소생술 교육프로그램이 중학생의 기본심폐소생술 관련 지식, 태도 및 수행능력에 미치는 효과)

  • The purpose of this study was to evaluate the effects of BLS education program in middle school students and their retention period. The participants were 70 middle school students in D와 Y middle school in D city. Data were collected from 16th July to 26th November in 2012. Students were tested for their knowledge, attitude before, immediately after, 8 weeks after, 16 weeks after BLS eduction. Data were analyzed frequency, percentage, $x^2$-test, t-test and repeated measured ANOVA using SPSS/PC 21.0 statistical program. Knowledge, attitude, performance ability were significantly increased experimental group compared to control group. Knowledge and attitude 8 weeks after, and performance ability were significantly decreased compared to 8 weeks after. And Knowledge, attitude, and performance ability were significant decreased among groups. Therefore, to maintain the knowledge attitude and skills of BLS, appropriate renewal time are necessary in regular school health curriculum.

Nurses' Emotional Responses and Ethical Attitudes towards Elderly Patients' DNR Decision (노인환자 심폐소생술금지 결정에 대한 간호사의 윤리적 태도와 정서상태)

  • Purpose: The purpose of this study was to examine nurses' emotional responses and ethical attitudes towards elderly patients' Do-Not-Resuscitate (DNR) decision. Methods: Data were collected using a questionnaire which was filled out by 153 nurses who worked in nursing homes and general hospitals. Data were analyzed using real numbers, percentages, means, standard deviations and Pearson's correlation coefficients with SPSS 19.0 program. Results: The average score for ethical attitudes towards the DNR decision was 2.68 out of 4. Under the ethical attitudes category, the highest score was found with a statement that said 'Although they will not perform cardiopulmonary resuscitate (CPR), it is right to do their best with other treatments for DNR Patients'. Items regarding emotional responses to the DNR decision, the average score was 2.36 out of 4. Among them, the highest score was achieved on 'I understand and sympathize'. No significant correlation was found between ethical attitudes and emotional responses in relation to patients' DNR decision (r=-0.12, P=0.13). Conclusion: Regarding elderly patients' DNR decision, nurses showed somewhat highly ethical attitudes and slightly positive emotional response. A follow-up study is needed to investigate variables that affect our results.

Changes in Life-sustaining Treatment in Terminally Ill Cancer Patients after Signing a Do-Not-Resuscitate Order (심폐소생술금지 동의 전·후 말기암환자의 연명치료 변화)

  • Purpose: This study investigated changes in life-sustaining treatments in terminally ill cancer patients after consenting to a do-not-resuscitate (DNR) order. Methods: Electronic medical records were reviewed to select terminally ill cancer patients who were treated at the oncology unit of the Asan Medical Center, a tertiary hospital in South Korea and died between January 1, 2013 and December 31, 2013. Results: The median (range) age of the 200 patients was 59 (22~89) years, and 62% (124 persons) were male. Among all patients, 83.5% were aware of their medical condition, and 47.0% of the patients had their DNR order signed by their spouses. The median of the patients' hospital stay was 15 days, and time from admission to DNR decision was 10 days. After signing a DNR order, 35.7~100% of the life-sustaining treatments that had been provided at the time of the DNR decision making were administered. The most commonly discontinued interventions were transfusion (13.5%), blood test (11.5%) and parenteral nutrition (8.5%). Conclusion: It is necessary to define the scope of life-sustaining treatments for DNR patients. Treatment guidelines should be established as well to secure terminal patients' death with dignity after their consent to a DNR order, thereby avoiding meaningless life-sustaining treatments and allowing administration of active terminal care interventions.

Outcomes of Out-of-hospital Cardiopulmonary Resuscitation and ROSC by Fire EMTs (119 구급대원에 의해 시행된 심폐소생술 및 자발순환회복 결과)

  • 이 연구는 하트세이버를 배지를 수여 받은 구급대원과 심정지 환자 중 심폐소생술로 자발순환이 회복된 후 환자를 대상으로 심정지 생존의 요인을 파악하고 분석한 결과 심정지에서 소생한 환자의 75%가 가정에서 발생되었으며, 출동에서 현장까지 도착 소요시간이 6.1분, 현장심폐소생술 소요시간 6.2분, 출동에서 병원 응급실 도착까지 소요시간 23.0분이 소요되었다. 또한 목격자에 의해 시행된 심폐소생술은 50.0%, 발견 당시 심장리듬은 심실세동이 87.5%, 무수축 12.5%였고, 심폐소생술과 동시에 자동제세동기를 사용한 환자는 87.5%, 심폐소생술만 시행한 환자 12.5%, 기관내삽관 시행이 12.5%였다. 자발순환이 회복된 환자에게 시행된 심폐소생술 소요시간은 10.9분 이었으며, 책임구급대원은 1급응급구조사가 75.0%, 출동 구급인원은 3명 출동이 87.5%였다.

Willingness to Perform Cardiopulmonary Resuscitation (CPR) in Emergency Situation: Convergence factor in Undergraduate Nursing Students (간호대학생의 심폐소생술 수행의도에 영향을 미치는 융합적 요인)

  • The purpose of this study was to investigate the correlation between basic self - confidence and CPR - related factors and to investigate factors affecting CPR intention in nursing college students. Data were collected between January to March, 2018. Using SPSS(ver, 24.0), independent t-test, Pearson's correlation and logistic regression were used for the statistical analysis. Among the variables related to cardiopulmonary resuscitation, there was a high correlation (r = .575~.738). In the case of AED use, there was a low correlation with self-confidence of core nursing skills, CPR confidence, proficiency, and self-efficacy (r = .163~.301). A moderate correlation (r = .415~453) was found between the variables related to core nursing skills and CPR related variabls. Logistic regression analysis showed that CPR Self-efficacy (OR=1.22, 95% CI: 1.07~1.38), and AED application Competency (OR=1.72, 95% CI: 1.09~2.75) are effect factors. Since there is a correlation with AED, which is the main tool in CPR, it is necessary to include not only the self - efficacy but also the use of AED which is closely related to CPR in the actual situation as an essential element of education.

Impact of Biomedical Ethics Awareness and Ethical Values in Nursing Student on Their Attitudes towards DNR (간호대학생의 생명의료윤리의식과 윤리적 가치관이 심폐소생술금지 태도에 미치는 영향)

  • Purpose: The purpose of this study was to identify how nursing students' awareness of biomedical ethics and ethical values affect their attitudes towards a do-not-resuscitate (DNR) order. Methods: This cross-sectional correlation study was conducted with 275 nursing students enrolled at two universities in North Chungcheong Province and North Gyeongsang Province of South Korea. Data were collected in April 2017 using a self-reported questionnaire. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression with the SPSS/WIN 23.0 program. Results: The students' attitudes towards DNR were positively correlated with attitudes that seek ethical values but negatively correlated with their biomedical ethics awareness. The explained variance for attitudes towards DNR was 20%, which was significant (F=13.01, P<0.001). Conclusion: These findings suggest that nursing students' biomedical ethics awareness and ethical values were associated with their attitudes towards DNR. Curriculum organization and various educational programs should be developed and applied to help nursing students develop ethical values and awareness of biomedical ethics.
