Effects on Changes in Femoral Vein Blood Flow Velocity with the Use of Lower Extremity Compression for Critical Patients with Brain injury (하지압박요법이 중증 뇌손상 환자의 대퇴 정맥 혈류 속도변화에 미치는 영향)

  • Purpose: This study was done to evaluate the mean venous velocity (MVV) response with knee and thigh length compression stockings (CS) versus intermittent pneumatic compression (IPC) devices in immobile patients with brain injuries. Methods: We carried out a randomized controlled study. We analyzed both legs of a randomly chosen sample of 43 patients assigned to one of 4 groups (86 legs). The patients were sequentially hospitalized in the intensive care unit (ICU) in "S hospital" from November 2005 to December 2006. The base line and augmented venous velocity was measured at the level of the common femoral vein. We applied leg compression 42 times over 7 days (for 2 hours at a time at 2 hour intervals). Results: There was a statistical difference among the 4 groups. The difference for the "IPC" group was more significant than the "CS" group. Conclusion: These results indicate that the application of IPC can be considered as an effective method to prevent deep vein thrombosis for immobile patients with brain injury.

A Case of Phlegmasia Cerulea Dolens as a Complication Caused by Placement of Inferior Vena Cava Filter (하대정맥 필터 삽입 후 합병증으로 발생한 Phlegmasia Cerulea Dolens 1예)

  • Inferior vena cava filters are increasingly being used as an alternative to anticoagulation therapy for the prevention of pulmonary embolism. However, using an Inferior vena cava filter may result in clinically significant complications. Phlegmasia cerulea dolens is a rare disease that presents with acute complete venous occlusion due to extensive thrombosis in the lower extremity. It is characterized by intense pain, edema, decreased pulses and a cyanotic extremity. We report here on a case of phlegmasia cerulea dolens that was accompanied with disseminated intravascular coagulation (DIC) as a complication of the placement of an inferior vena cava filter in a patient who had been previously diagnosed with pulmonary embolism, and the patient had recently developed a cerebral hemorrhage due to a traffic accident.

  • Incidences of pulmonary thromboembolism markedly increase with age. Risk factors of pulmonary thromboembolism are surgery, trauma, acute medical illness, immobilization, pregnancy, usage of hormone, and advanced age. In the cases of thrombomembolism occurred in young age, the possibility of thrombophilc state is needed to be investigated. Among many diseases or state associated thrombophilic state, homocyteinemia should be considered a cause of thromboembolism before fifth decade. Homocyteinemia is caused by deficiency of N-5-methyltetrahydrofolate, cystathionie ${\beta}$-synthase and vitamin B12. The presence of the mutation of 5,10-methyleneterahydrofolate lead to homocyteinemia by deficiency of N-5-methyltetrahydrofolate. Homocysteine is acknowledged the risk factor of cardiovascular event, and storke. Homocysteinemia can be the cause of thromboemboism via damaging endotheial cell. We present two cases of pulmonary thromboembolism in young age which seem to be associated with homocysteinemia precipitated by mutation of 5,10-methyleneterahydrofolate.

Deep Vein Thrombosis after Total Knee Replacement in a Patient undergoing Korean Medical Rehabilitation (슬관절 전치환술 후 한방재활치료 중 발생한 심부정맥 혈전증 보고)

  • Objectives The objective of this study is to report the deep vein thrombosis (DVT) after total knee replacement (TKR) in a patient undergoing Korean medical rehabilitation. Methods A female patient who had total knee replacement (TKR) was treated by Korean medical rehabilitation with protocol. After fifteen days treatment, deep vein thrombosis (DVT) was suspected. Therefore, CT venography was performed. Results After Korean medical rehabilitation, pain was decreased. But also occurred deep vein thrombosis (DVT) as a complication. Conclusions We present a case of deep vein thrombosis (DVT) after total knee replacement (TKR) in a patient undergoing Korean medical rehabilitation. There is a limit that this study hasn't sufficient number of cases. Further studies will be needed.

Treatment of Deep Thrombosis (심부정맥혈전증의 치료)

  • During the past 4 years, 18 cases of deep vein thrombosis were treated. Diagnosis was based on clinical symptoms, phlebography and vascular Doppler examination. Etiologic factors were mainly trauma, operation and immobilization of the lower extremities. Seven patients were treated surgically and eleven patients medically. The indications for surgical thrombectomy included phlegmasia cerulea dolens [N=6], and phlemasia cerulea dolens with focal venous gangrene[N=1]. The comparative analysis of each treatment methods was done after a mean observation period of 6 months. Good clincal results were observation in 4 patents in surgically treated [57.1%], and 4 in conservative management group[36.3%]. There were no mortality in the both surgical and medically treated group but one patient with phlegmasia cerulea dolens and focal venous gangrene was dischared in moribund state at the third postoperation day due to sepsis and multiorgan failure. We believe that aggressive early surgical thrombectomy should be stronly considered for patients of phlegmasia cerulea dolena.

  • 1. Objectives The purpose of this study is to evaluate the effects of Sasang Constitutional medical diagnosis and treatment on Soyangin Edema caused by Deep Vein Thrombosis. 2. Methods The Subject is a 75 years old woman who has a left lower limb edema caused by Deep Vein Thrombosis, we had diagnosed her as Soyangin and prescribed Sasang Constitutional Medicine. We evaluated with length of patient's calf, thigh, ankle circumference and pitting edema, skin flare after medication. 3. Results Lower limb edma of a Soyangin patient responded well to the treatment with Dojeokganggi-tang(導赤降氣湯). 4. Conclusions This case study shows an efficient result by using Dojeokganggi-tang in treatment of Soyangin edema patient.

Case Study of a Deep-vein Thrombosis Patient Treated with Combinatorial Hyulbuchuko-tang (심부정맥혈전증에 대한 혈부축어탕의 치험 1례)

  • Objective: This study reports on the clinical effects of combinatorial Hyulbuchuko-tang on a deep-vein thrombosis patient.Method: The deep-vein thrombosis patient was treated using combinatorial Hyulbuchuko-tang and pharmacopuncture (water-soluble carthami flos, WCF) three times a day for 33 days. The effects of deep-vein thrombosis were measured using a 0-10 numerical rating scale (NRS) and external appearance.Result: Improvements in the NRS from 7 to 2 were observed after the combinatorial Hyulbuchuko-tang treatment.Conclusions: From these results, combinatorial Hyulbuchuko-tang may be considered to show improvement for patients with deep-vein thrombosis.

  • Objectives : The purpose of this study is to examine the significance of Korean Medical literature on representative symptoms of postpartum deep venous thrombosis (DVT), such as lower extremity edema and pulmonary embolism. Methods : Pattern differentiation, cause, symptoms and treatment related to DVT and pulmonary embolism as written in Korean Medical texts were studied comparatively. Results : In Korean Medical understanding, DVT could be linked to lower extremity edema due to blood stagnation, while pulmonary embolism could be linked to coughing, asthma and nose bleed due to stagnated blood. Conclusions : To prevent relapse and manage its after-effects, Korean Medical treatment for DVT and pulmonary embolism is much in need.

  • May-Thurner syndrome (MTS) is a rare cause of deep vein thrombosis. Several cases of MTS because of various causes have been reported. However, MTS due to osteophytes in patients with underlying diffuse idiopathic skeletal hyperostosis (DISH) has not been reported in English literature, to the best of our knowledge. Here, we present the CT findings of MTS in patient with DISH.

Prevention Effects of Graduated Compression Stockings and Intermittent Pneumatic Compression on Deep Vein Thrombosis in SICU Patients: Pilot Study (항혈전스타킹과 간헐적공기압박기 적용이 중환자의 심부정맥혈전 발생 예방에 미치는 효과: 예비조사)

  • Purpose: The purpose of this pilot study was to investigate the effects of mechanical interventions for deep vein thrombosis (DVT) prophylaxis in surgical intensive care unit (SICU) patients. Methods: The participants were assigned to the intermittent pneumatic compression (IPC) and graduated compression stocking (GCS) intervention. Patients who met the criteria were selected for comparison from our previous study. Data for 140 patients were included in the final analysis. Results: The mean age was 57.5 (${\pm}15.7$) and 61.4 % were men. About forty-seven percent of the participants were 61 years or over. In the second duplex scan, 3, 2 and 1 critically ill patients developed deep vein thrombosis in the control, GCS, and IPC groups, respectively. Incidences of DVT were 6.0%, 5.0%, and 2.0% for the control, GCS, and IPC groups, respectively. This difference was not significant. Relative risks of no intervention were 3.0 and 1.2 compared with IPC and GCS application. There were no significantly different variables among the three groups before the intervention except for diagnosis on admission. Conclusion: Although it may difficult to conclude that mechanical prophylaxis effectively prevents DVT among SICU patients because there was no statistical significance in this study, but incidence rates among the three groups differed greatly. The findings reveal that further study should be conducted with larger samples and randomized controlled trial for SICU patients.
