Fibromyalgia from the Psychiatric Perspective (정신과적 관점에서의 섬유근통)

  • Fibromyalgia is a disorder characterized by the core symptom of chronic widespread pain, along with fatigue, sleep disturbances, mood changes, and cognitive difficulties. The etiology of fibromyalgia involves a combination of biological factors, such as genetic vulnerability, alterations in pain processing and stress response system ; psychological factors, such as anxiety, depression, anger, and perceived stress ; environmental factors, such as infections, febrile diseases, and trauma. Central sensitization, which is amplified in the process of sensory stimulation, has been emphasized as a key etiological factor, as supported by enhanced wind-up, delayed aftersensation, decreased nociceptive flexion reflex threshold and functional imaging studies. Several guidelines recommend that a multimodal approach be used to treat fibromyalgia, including both pharmacological and non-pharmacological treatments, tailored to each individual, and that clinicians should provide an intellectual framework through sufficient education and emphasis on the importance of self-management. The prevalence of mood disorders, anxiety disorders, and other psychiatric problems is 7-9 times higher in patients with fibromyalgia than in the general population ; moreover, the association between fibromyalgia and certain psychopathologies or sleep problems has also been suggested. Since psychiatric problems, with shared vulnerabilities and risk factors, interact with fibromyalgia bidirectionally and also affect the disease course, an integrated management approach is needed to determine the risk of comorbidities.

Impact of Aged-related Experience on Knowledge and Attitude toward Aged - Dental Hygiene Students - (일부 치위생과 학생들의 노인관련 경험이 노인에 대한 지식 및 태도에 미치는 영향에 관한 연구)

  • To investigate effect of aged-related experience on the knowledge and attitude of department of dental hygiene students for the old people. The the survey, which is done by 292 students in D health college, is enforced from August to October 2009. Following result is the data. Analyzed by using SPSS 14.0 version. 1. For the knowledge toward aged, in case of educated has the high level of knowledge (p < 0.05) in Physical-physiological, Social-familial field. But there's no some difference in Psychological field. 2. Whether doing education or not, there's no some difference toward attitude to aged. 3. Whether living together or not, there's no difference about knowledge or attitude. But people who has experience as volunteer shows positive attitude to aged.(p < 0.05) 4. The factors which affects to knowledge about aged are education and attitude for aged. 5. The factors which affects to attitude to aged are volunteering experience and knowledge for aged.

The Effects of Adlerian Community Feeling Program on the School Life Satisfaction and Depression among Elementary School Students (아들러 심리학에 기초한 공동체 의식 프로그램이 초등학생의 학교생활 만족도와 우울에 미치는 효과)

  • The purpose of this study was to examine the effects of Adlerian Community Feeling Program on the school life satisfaction and depression among elementary school students. Subjects were 38 students who were the fifth grader students and all participants were asked to complete the school life satisfaction scale and depression scale. The pretest and posttest were implemented in order to verify the effectiveness of the program. The program consisted of a total 9 sessions, focusing on building the relationship, self-acceptance, respect for others, cooperation and contribution. The employed statistical analysis was ANCOVA. The results of the study are as follows: First, the Community Feeling Program had an influence on significant improvement in school life satisfaction and its sub-factor, general classes, teacher-student relationships and the physical environment of a school. Second, the Community Feeling Program had an influence on significant effect in depression and its sub-factor, negative mood/physical symptom. Thus, the Community Feeling Program based on Adlerian Psychology has been found to be effective in improving overall school life satisfaction and reducing depression.

The Impact of Cognitive Load Factors and Arousal Levels of Galvanic Skin Response on Task Performance in Computer Based Learning (컴퓨터 기반 학습에서 인지부하 요인과 GSR의 각성수준이 과제수행에 미치는 영향)

  • The purpose of this study was to verify the impact of cognitive factors and GSR on the task performance. For this study 64 students participated. Multiple regression and repeated measures were applied to analyze the data. The result for the survey indicated that previous knowledge, physical efforts, and task difficulty had significant impacts on task performance. Particularly, task difficulty has a negative impact. This can be interpreted as someone who has high prior knowledge inputs higher physical efforts with low task difficulty perception will show high performance. On the other hand, the low arousal level of GSR in the evaluation stage is a prediction variable of task performance. This result shows that high prior knowledge and low arousal level of GSR produces high performance. However, the analysis of difference in GSR between learning and evaluation stages does not show significant difference. It suggests that physiological measure such as GSR is reliable index of cognitive load; however, it partially represents cognitive load. Other crucial factors should be added for comprehensive measures.

  • 노인에게서 두드러지게 나타나고 있는 저근육형 비만은 근육감소를 동반한 체지방의 증가로 신체상의 뚜렷한 체성분의 변화를 야기 시킨다. 이때 골감소증을 동반하여 신체기능의 감소 및 골절장애 그리고 대사성 관련 질환의 위험도가 올라가는 것으로 보고되고 있다. 노화로 인한 체성분의 변화는 단순한 저근육형일 경우와 비만일 때 보다 급격히 증가된 복부내장 지방조직에서 분비되는 염증성 사이토카인, C-반응성 단백질(CRP), 인터루킨(IL)-6, IL-8 및 종양 괴사 인자(TNF-${\alpha}$)들이 단백질 대사를 저해하여 근육량의 감소를 더욱 촉진시키며, 염증관련 대사질환의 유병률에 중요한 요인이다. 본 연구에서는 DNA 메틸화가 당뇨병, 심혈관질환, 암과 같은 만성염증성 질환에 관계하고 있다는 최근 연구 결과를 기초로 하여 항염증 영양소와 생리활성을 갖는 식품인자들의 충분한 섭취가 염증조절에 중요하게 기여할 것으로 생각되며, 또한 염증성 질환의 주요 표식자인 DNA 메틸화와 히스톤 변형을 유발하는 효소의 활성 또는 비 암호화된 RNA의 발현을 조절함으로써 근육량 증가와 체지방 감소에 중요한 역할을 하는 것을 살펴보았다. 따라서 최근 새롭게 인식되는 후생유전학적 연구의 중심에 있는 항염증 영양소의 효과와 체성분 변화와의 긍정적 관계를 중심으로 저근육형 비만의 예방 및 인구고령화에 건강한 노화를 위한 효과적인 방법을 제시하였다.

Physical, Physiological, and Psychological Factors Determing Sleep Quality in Patients on Maintenance Hemodialysis (혈액투석 환자의 수면의 질에 영향을 주는 신체적, 심리적, 생리적 요인)

  • Purpose: The study aimed to examine quality of sleep in hemodialysis patients and explore physical, physiological, and psychological factors determining sleep quality. Methods: The survey was conducted among 165 patients on maintenance hemodialysis at a university hospital. Data were collected from July to October in 2015. The sleep quality and its related factors were assessed by standardized questionnaires, anthropometric measures, and lab tests after obtaining a written consent from the participant. Results: The mean age of the participants was 62.58 years old, and 67.9% of the participants experienced sleep disorders such as 'not falling asleep within 30 minutes' or 'frequently waking up at night'. Pain (t=-3.29, p=.007) and depression (t=-6.028, p<.001) were significantly different between the group with sleep disorder and the other without. The demographic factors (age, shared room, sleep during the day), physical factors (pain and symptom), and psychological factors (depression and trait and status anxiety) together explained 20.0% of variance in sleep quality. Conclusion: Sleep disorder was frequently experienced among the patients on maintenance hemodialysis. The effective nursing strategies should focus on sleep environment, pain and depression management. Further study is warranted to explore the effect of physiological factors related to the disease on sleep disorder in this population.

  • The rotator cuff is situated in a potential tight subacromial space and undergoes senescent structural changes commonly observed in other joints of the body. When the cuff fails, spontaneous healing of the torn tendon is not expected to occur, and multiple factors may be responsible. Its fibers are under tension and typically retract on tearing. The subacromial bursal inflammation and alterations in normal glenohumeral kinematics have been considered in the development of symptoms. Controversy continues to exist concerning the pathogenesis of rotator cuff disease. The heterogeneity of the disorder, as well as the notion that rotator cuff disease may not actually represent a continuum of the same process, but rather, is a compilation of independent disorders, may partly explain the differing viewpoints on its origin. Two contrasting pathogenetic mechanisms have been extensively described and include vascular, or intrinsic, causes and impingement, or extrinsic, factors. Other etiologies have also been reported that include trauma, congenital or developmental factors, and instability. For successful treatment of the rotator cuff diseases, it is essential to understand the structure and function of rotator cuff and to clarify the pathogenesis and natural history of its disorder.

Relationship among Anger Expression Mode, Depression, and Blood Pressure in Korean Male Military Draftees Subjected to the Examining Procedure (징병신체검사 대상 남성의 분노표현 양식, 우울, 그리고 혈압간의 관계)

  • Objectives : The purpose of this study was to investigate the relationship among anger expression mode, depression, and blood pressure. Method : Eight hundred sixty-eight male military draftees were asked during the examination procedure to answer Spielberger's Anger Expression Inventory, MMPI Repression Scale, and Beck Depression Inventory(BDI). Blood pressures, weight, and height were also measured. Results : 1) No significant difference was found in the anger-in and anger-out subscales of Spielberger Anger Expression Inventory, MMPI Repression Scale, and BDI between the hypertensives and the normotensives. 2) No significant difference of blood pressure was found between the groups determined by upper and lower 25% of each of the above scales and inventory. 3) The interaction effect of anger-in and anger-out on depression was found to be significant. Conclusion : These findings suggest that there is no relationship among anger-in, anger-out, depression, and blood pressure in young male military draftees. Interestingly, ambivalence of anger expression, i.e. the interaction of anger-in and anger-out, was found to be an important factor related to depression.

Socio-economic status is associated with the risk of inadequate energy intake among Korean elderly (노인의 에너지 섭취 부족과 관련된 사회환경요인 분석)

  • Purpose: This study was conducted to evaluate the factors influencing inadequate energy intake among Korean elderly. Methods: Our study included 1,869 elderly people (over 60 years old) who completed a dietary survey from the fifth Korean NHANES (2010). Factors that could affect the nutritional status of the elderly included age, family status, socio-economic factors (education, family income, livelihood security, employment), and health related factors (having chronic disease, functional status, diet therapy, depression, and suicidal thoughts). Energy and protein intake were assessed using the dietary intake data from 24hr recall method. Results: The percentage of people who consumed energy less than 75% of EEA for Koreans was 23.7% in men, 31.1% in women. The carbohydrate contribution to the total energy intake in the inadequate energy intake group was significantly higher than that in the adequate intake group (p < 0.05). Factors significantly related to inadequate energy intake after adjusting for age, family status, education, family income, employment, functional status, and suicidal thoughts were education (OR: 1.480 in men, 1.614 in women) and employment (OR: 1.751 in men, 1.464 in women), age 70 years or older in men (OR: 1.475), and living with family but without spouse in women (OR: 1.496). Conclusion: In summary, the results imply that energy intake of elderly would be affected by the status of social environment with aging and nutrition-related policy for Korean elderly should be based on the social status as well as health related conditions.

The comparative study of activity ability of daily livings, body pain and emotional states between elders living alone and those living with others (독거노인의 생활활동능력과, 신체통증과 정서상태에 관한 비교연구)

  • In this research, one's ability to perform activities of daily living, body pain, and emotional states of elders who live alone and elders who live with others were compared to investigate which factors affect senior's health state. The data was collected from 88 seniors and analyzed for the comparison. Compared to seniors who live with others, elders who live alone had lower activity of daily livings, stronger body pain, and instability in emotional states. These 3 aspects showed a statistical significance of correlation. As a result of further analysis, not only residential types(living alone or living with others), but also life pattern(indoor or outdoor) is affected on the results. It is expected that the results of this research could be used as basic data for physiological index to evaluate senior's health state and care service for seniors who live alone.
