신규 간호사가 인식하는 좋은 간호사 이미지: Q-방법론 접근 (A Positive Image of Nurses as Perceived by New Nurses: Applying Q-Methodology)

  • Purpose: This study verified the characteristics and types of perceptions and attitudes toward a positive image of nurses, as perceived by new nurses, by applying Q-methodology. Methods: Participants were 37 new nurses serving at tertiary hospitals accommodating over 300 beds. Data were collected from April 1~30, 2021. Results: A positive image of nurses was categorized into four types: professional leadership, skilled empathy, experienced responsiveness, and cooperative sincerity. It is believed that this study will help establish a professional view of nurses by verifying the positive image of new nurses, who are likely to lead the future. Conclusion: Furthermore, by reducing the turnover of new nurses and increasing their job satisfaction, it would be possible to secure professional nursing personnel and the public's perception of good nurses, thereby contributing to the enhanced social status of nurses.

졸업직후 신규간호사의 감성지능, 비판적 사고능력 및 임상수행능력 간의 관계 (The Relationship between Emotional Intelligence, Critical Thinking Disposition and Clinical Competence in New Graduate Nurses immediately after Graduation)

  • 본 연구는 졸업직후 신규간호사의 비판적 사고능력과 임상수행능력과의 관계에서 감성지능의 매개효과를 파악하고자 시도되었다. C시와 K시 소재 2개의 종합병원의 졸업직후 신규간호사 181명을 대상으로 설문지를 이용하여 2017년 2월 27일부터 3월 30일까지 자료를 수집하였다. 수집된 자료는 SPSS/WIN 23.0을 이용하여 기술통계, Independent t-test, ANOVA, 피어슨 상관분석과 다중회귀분석을 실시하였고, 매개효과의 유의성 검증은 sobel test로 실시하였다. 연구결과 감성지능은 비판적 사고능력(r=.62, p<.001), 임상수행능력(r=.60, p<.001)과 유의한 정의 상관관계가 있는 것으로 나타났으며, 비판적 사고능력과 임상수행능력과의 관계에서 부분매개를 하는 것으로 나타났다(Z=3.88, p<.001). 그러므로 졸업직후 신규간호사의 임상수행능력을 향상시키기 위해서는 비판적 사고능력 뿐만 아니라 감성지능을 향상 시킬 수 있는 프로그램의 개발 및 적용이 필요하다.

신규 간호사의 임상 경험 (The Clinical Experiences of Newly-qualified nurses)

  • Purpose: This phenomenological study is aimed at illuminating the lived experiences of newly-qualified nurses who working in general hospitals in C province. Method: The data was collected through unstructured, in-depth interviews with 10 new nurses from April to June 2001, and analysed by Colazzi' methodology. Results: Thematic content analysis identified 6 central themes in this experience-difficulties m making interpersonal relationship, negative feelings, fatigue and unskilled work performance, self-reflection, a sense of accomplishment, identity formation as a nurse. Also, this themes were constructed into 6 constructs explaining new nurses' experience. The results may contribute to a better understanding of what new nurses experienced and how their negative conflicts could be managed. Conclusion: New nurses need to get more education and supportive empowerment regarding diverse conflicts during training period. Educational programs like a preceptorship or individual counselling should be developed and adopted to all clinical settings.

2011~2015년 신규간호사와 경력간호사의 이동 경향과 지역 간 이동의 영향 요인 (Patterns and Influential Factors of Inter-Regional Migration of New and Experienced Nurses in 2011~2015)

  • Purpose: The purpose of this study was to analyze the migration patterns of new nurses and experienced nurses and to identify the factors influencing inter-regional migration for solving regional imbalances of clinical nurses in South Korea. Methods: This study involved a secondary analysis of data from the Health Insurance Review and Assessment Service (HIRA). Data were analyzed using descriptive statistics and multiple logistic regression analysis. Results: New nurses tended to migrate from Kyunggi to Seoul. However, experienced nurses tended to migrate from Seoul and Chungchung to Kyunggi. Significant predictors of inter-regional migration among new nurses were location and nurse staffing grade of hospitals. Significant predictors of inter-regional migration among experienced nurses were location, hospital type, nurse staffing grade, ownership of hospitals and age of nurses. Conclusion: Inter-regional migration occupied a small portion of total hospital movement among clinical nurses. The regional imbalances of nurses were not caused by the migration from non-metropolitan areas to Seoul. Nurse shortage problems in the small and medium hospitals of the non-metropolitan area can be solved only through improvement of work environment.

프리셉터의 신규간호사 교육경험 (Nurses' Lived Experience of Preceptorship for Newly Graduated Nurses in Hospital)

  • Purpose: This phenomenological study is aimed to describe nurses' lived experience of preceptoring in hospitals. Methods: Ten nurse preceptors who participated in this qualitative study shared their experience of the preceptor's role in hospitals. Data analysis was done using the Colaizzi' phenomenological method. In-depth personal interviews were conducted from September 1 to November 30, 2013. Results: Based on this study, seven consistent categories and twenty-two theme clusters emerged from collected data. The seven themes were: 'not knowing what to do', 'not understood', 'burnt out due to pressure from work', 'having difficulty in building relationships at work', 'becoming similar to their preceptor, personal growth through preceptorship', and 'feeling rewarded at the preceptee's improvement'. Conclusion: The result of this study suggests that further research is needed to improve preceptors' adaptations to the workplace and to reduce their stress.

지식경영 시스템을 활용한 신규간호사 e-Learning 프로그램 개발 및 평가 (The Development and Evaluation of an e-Learning Program for Novice Nurses before Beginning to Work in a Hospital Using KMS(Knowledge Management System))

  • Purpose: The purpose of this study was to develop and evaluate an e-Learning Program designed to enhance novice nurses's performance ability Method: The e-Learning program was designed for novice nurses before beginning to work in a hospital using a KMS(Knowledge Management System). This study was carried out from February 2006 to April 2007. The program had progressive steps including design, pilot test & modification, contents development, program application and evaluation. After the program design and pilot testing, the learner's satisfaction was analyzed. Result: The program site is http://kms.emc.ac.kr titled, 'Cyber Education Center'. The sixteen part course has been developed as follows: blood sugar test, skin test, insulin test 1, insulin test 2, blood culture, blood sampling, heat therapy, cold therapy, glycerin enema, preparation of operation, oxygen therapy, suction technique, transfusion, foley catheterization and aseptic technique. The overall learner's satisfaction score was 4.08 out of 5 points. Conclusion: E-Learning could be useful for education and nursing work. This method may be effective for clinical nurses.

암병동 신규간호사를 위한 행복 셀프코칭 프로그램 개발 및 효과 (Development and Evaluation of the Effect of a Happiness Self-Coaching Program for New Graduate Nurses Working in Cancer Care Unit)

  • Purpose: This study was conducted to examine the effect of a Happiness Self-Coaching program for new graduate nurses working in cancer care units. Methods: A quasi-experimental pretest-posttest, nonequivalent control-group design was applied to conduct the study. Study participants were new graduate nurses employed within 12 months and working as shift employees at G university hospital, located in J city. A total of 21 new graduate nurses participated in the study, 10 in the experimental group, and 11 in the control group. The happiness self-coaching program was conducted weekly for 70 minutes from February 1 to June 6, 2016 for 6 weeks. The experimental group received the Happiness Self-Coaching program through lectures, presentation, group activities, and strength card play. Data were analyzed by using repeated measure ANOVA and paired t-test. Results: The experimental group demonstrated significantly higher happiness (F=6.82, p=.003) and self-efficacy (F=3.38 p=.045) levels compared to the control group. Conclusion: The Happiness Self-Coaching program was effective in enhancing new graduate nurses' happiness and self efficacy. It is recommended that nursing organizations to apply the Happiness Self -Coaching program to facilitate adaption of new graduate nurses in cancer care units.

토픽모델링을 활용한 신규간호사 관련 국내 연구동향 분석 (Research trend analysis of Korean new graduate nurses using topic modeling)

  • Purpose: The aim of this study is to analyze the research trends of articles on just graduated Korean nurses during the past 10 years for exploring strategies for clinical adaptation. Methods: The topics of new graduate nurses were extracted from 110 articles that have been published in Korean journals between January 2010 and July 2020. Abstracts were retrieved from 4 databases (DBpia, RISS, KISS and Google scholar). Keywords were extracted from the abstracts and cleaned using semantic morphemes. Network analysis and topic modeling were performed using the NetMiner program. Results: The core keywords included 'education', 'training', 'program', 'skill', 'care', 'performance', and 'satisfaction'. In recent articles on new graduate nurses, three major topics were extracted by Latent Dirichlet Allocation (LDA) techniques: 'turnover', 'adaptation', 'education'. Conclusion: Previous articles focused on exploring the factors related to the adaptation and turnover intentions of new graduate nurses. It is necessary to conduct further research focused on various interventions at the individual, task, and organizational levels to improve the retention of new graduate nurses.

간호대학생의 프리셉터십 융복합 실습 경험 (A Convergence Work Experience of Nursing Students under Preceptorship)

  • 연구의 목적은 3년+1년 학제 재학 중인 간호대학생의 프리셉터십 실습 경험을 이해하고 이를 토대로 간호직의 중도 포기를 방지하기 위한 효율적인 실습방안을 제시하기 위함이다. 연구방법은 23명의 참여자를 네 집단으로 나누어 초점 그룹 면담으로 진행하였다. 자료 분석은 Corbin and Strauss 근거이론으로 분석하였다. 본 연구결과는 비교분석을 통하여 이 연구의 중심현상은 '등대 찾아가기'였다. 원인적 조건은 간호업무를 배우는 기회였다. 맥락적 조건은 진짜 신규 간호사가 된 기분, 임상현장의 생생한 정보획득, 학생들의 프리셉터십 실습 교육에 대한 인식 부재로 나타났다. 중재적 조건은 근무환경, 주도적 학습 및 애착심이 도출되었다. 작용/상호작용 전략으로 다양한 환자 및 보호자 맞추어 가기, 남의 일 같지 않은 일 및 아는 것만큼 보임으로 도출되었다. 작용/상호작용의 결과로 뿌듯함 및 신규간호사 역할 배움에 대한 여정이 도출되었다. 본 연구결과는 '등대 찾아가기' 과정에 있는 간호 학생들에게 유용한 임상 실습 향상을 위한 기초를 제공하는데 의미가 있다.

비계획적 발관 예방을 위한 교육프로그램 적용효과 (Study on an Unplanned Extubation Prevention Program (UEPP) for Intubated Patients in Intensive Care Unit)

  • 본 연구는 내과계 중환자실에 입원한 기관삽관 환자의 비계획적 발관 예방프로그램을 개발하여 비계획적 발관율을 감소시키고 간호사들이 실무에서 비계획적 발관에 적극적으로 대처할 수 있도록 도움을 주기 위해 수행되었다. 조사기간은 사전기간이 2008년 5월부터 2009년 4월이었고, 프로그램 적용기간은 2009년 5월부터 2010년 4월로 각각 1년씩이었다. 기간 내 조사 대상 총 삽관환자는 사전시기와 중재 시기에 각각 330명, 332명이었으며, 이들 중 비계획적 발관을 경험한 사전 29사례와 적용기간 23사례에 대해 발관경험 환자와 담당간호사의 발관 관련 제 특성을 추가적으로 살펴보았다. 통계분석은 SPSS 18.0을 이용하였다. 비계획적 발관을 경험한 환자와 간호사의 제 특성은 유의한 차이를 보이는 것은 없었으나 두 시기에 신규 간호사의 진입비율에 유의한 차이가 있었다(p=.039). 기관내관 삽입 1000일당 비계획적 발관율은 신규 간호사 진입이 많았음에도 적용 기간의 평균이 8.8을 보여 사전 관찰시기의 평균 발관율 12.3과 비교하여 유의하게 감소(t=-2.333, p=.040)하였으므로 본 연구의 결과는 중환자실 삽관환자의 비계획적 발관예방 활동을 통해 환자 안전에 기여할 수 있는 프로그램이라 사료된다.
