The use of surgical guide stent for implant placement (임플란트 식립을 위한 수술 가이드의 사용)

  • Surgical guide not only provide diagnosis and treatment plan, but even location and direction of implantation. Surgical guide could be divided into non-limited design, partially limited design, and completely limited design. Partially limited design is easily manufactured and inexpensive but less accuracy, compared to completely limited design. From this approach, partially limited design may be particularly effective in patients who present with a single missing tooth or partially edentulous teeth. Completely limited design is anatomically accuracy, esthetical and functional, optimized treatment for prosthetic and biomechanical perspective, and also minimizes discomfort for post-treatment. The purpose of this study is to review previous studies of various surgical guides and applying in clinic.

Finite element analysis of cortical bone strain induced by self-drilling placement of orthodontic microimplant (Self-drilling 방식의 마이크로임플란트 식립에 의해 발생하는 피질골 스트레인의 유한요소해석)

  • Objective: The aim of this study was to evaluate the strain induced in the cortical bone surrounding an orthodontic microimplant during insertion in a self-drilling manner. Methods: A 3D finite element method was used to simulate the insertion of a microimplant (AbsoAnchor SH1312-7, Dentos Co., Daegu, Korea) into 1 mm thick cortical bone. The shape and dimension of thread groove in the center of the cortical bone produced by the cutting flute at the apical of the microimplant was obtained from animal test using rabbit tibias. A total of 3,600 analysis steps was used to calculate the 10 turns and 5 mm advancement of the microimplant. A series of remesh in the cortical bone was allowed to accommodate the change in the geometry accompanied by the implant insertion. Results: Bone strains of well higher than 4,000 microstrain, the reported upper limit for normal bone remodeling, were observed in the peri-implant bone along the whole length of the microimplant. Level of strains in the vicinity of either the screw tip or the valley part were similar. Conclusions: Bone strains from a microimplant insertion in a self-drilling manner might have a negative impact on the physiological remodeling of cortical bone.

Three-dimensional finite element analysis according to the insertion depth of an immediately loaded implant in the anterior maxilla (상악 전치부 즉시하중 임플란트의 식립 깊이에 따른 삼차원 유한요소 분석)

  • Purpose: The purpose of this study was to investigate the effects of the insertion depth of an immediately loaded implant on the stress distribution of the surrounding bone and the micromovement of the implant using the three-dimensional finite element analysis. Materials and methods: A total of five bone models were constructed such that the implant platform was positioned at the levels of 0.00 mm, 0.25 mm, 0.50 mm, 0.75 mm, and 1.00 mm depth from the crest of the cortical bone. A frictional coefficient of 0.3 and the insertion torque of 35 Ncm were simulated on the interface between the implant and surrounding bone. A static load of 178 N was applied to the provisional prosthesis with a vertical load in the axial direction and an oblique load at $30^{\circ}$ with respect to the central axis of the implant, then a finite element analysis was performed. Results: The implant insertion depth significantly affected the stress distribution on the surrounding bone. The largest micromovement value of the implant was $39.34{\mu}m$. The oblique load contributed significantly to the stress distribution and micromovement in comparison to the vertical load. Conclusion: Increasing the implant insertion depth was advantageous in dispersing the concentrated stress in the cortical bone and did not significantly affect the micromovement associated with early osseointegration failure.

A total of 1,800 analysis steps was used to simulate the 10 turns and 5 mm advancement of the microimplant. A series of remesh in the cortical bone was allowed to accommodate the change in the geometry accompanied by the implant insertion. Results: Bone strains of well higher than 4,000 microstrain, the reported upper limit for normal bone remodeling, was observed in the bone along the whole length of the microimplant. At the bone in the vicinity of the screw tip, strains of higher than 100% was recorded. The insertion torque was calculated at approximately 1.2 Ncm which was slightly lower than those measured from the animal experiment using rabbit tibias. Conclusions: The insertion process of a microimplant was successfully simulated using the 3D finite element method which showed that bone strains from a microimplant insertion might have a negative impact on physiological remodeling of bone.

  • Implant assisted removable partial denture (IARPD) has been practiced in various forms for a long time, and among them, implant surveyed crown RPD is gaining predictability as well as being considered as a treatment option for patients with anatomical and financial disadvantages. The position of implant could be divided as posterior placement or anterior placement according to the purpose of the treatment and should be planned in consider to the alveolar ridge of patient, anticipated prognosis of remaining teeth, and opposing dentition. This case report describes a treatment for mandibular Kennedy class I partial edentulous patient with two implant-supported surveyed crown and implant assisted removable partial denture. Given the difficulty of posterior placement in this patient and the prognosis of the residual teeth, the plan was to place two implants in close proximity to the residual teeth, which were placed in the planned position, angle, and depth using guided surgery. The process of fabricating the fixed prosthesis was carried out in parallel with the maxillary edentulous tooth arrangement process to increase predictability, and when fabricating the localized tooth, the implant was designed in a form that allows the patient to perform functional movements by preventing excessive loading as the last supporting tooth, and was fabricated through a secondary impression process. Each treatment procedure was proceeded as planned, with aesthetically and functionally satisfactory results for both patient and operator.

The effects of tissue punch diameter on healing around implants in flapless implant surgery (무피판 임플란트 수술에서 연조직 펀치의 크기가 임플란트 주위 조직의 치유에 미치는 영향)

  • Statement of problem: Flapless implant surgery using a soft tissue punch device requires a circumferential excision of the mucosa at the implant site. To date, Although there have been several reports on clinical outcomes of flapless implant surgeries, there are no published reports that address the appropriate size of the soft tissue punch for peri-implant tissue healing. Purpose: In an attempt to help produce guidelines for the use of soft tissue punches, this animal study was undertaken to examine the effect of soft tissue punch size on the healing of peri-implant tissue in a canine mandible model. Material and methods: Bilateral, edentulated, flat alveolar ridges were created in the mandibles of six mongrel dogs. After a three month healing period, three fixtures (diameter, 4.0 mm) were placed on each side of the mandible using 3 mm, 4 mm, or 5 mm soft tissue punches. During subsequent healing periods, the peri-implant mucosa was evaluated using clinical, radiological, and histometric parameters, which included Gingival Index, bleeding on probing, probing pocket depth, marginal bone loss, and vertical dimension measurements of the peri-implant tissues. Results: The results showed significant differences (P <0.05) between the 3 mm, 4 mm and 5 mm tissue punch groups for the length of the junctional epithelium, probing depth, and marginal bone loss during healing periods after implant placement. When the mucosa was punched with a 3 mm tissue punch, the length of the junctional epithelium was shorter, the probing depth was shallower, and less crestal bone loss occurred than when using a tissue punch with a diameter $\geq$ 4 mm. Conclusion: Within the limit of this study, the size of the soft tissue punch plays an important role in achieving optimal healing. Our findings support the use of tissue punch that 1 mm smaller than implant itself to obtain better peri-implant tissue healing around flapless implants.

  • Skeletal anchorage is recommended as an orthodontic treatment for an impacted immature permanent tooth. Among these methods, C-tube is relatively safe because it is fixed to the cortical bone of interdental and the lower part of the root with several short miniscrews, which causes less damage to the root in patients of early permanent dentition. As it can be easily bent, the traction direction can be adjusted to favorable bone density sites. However, patient cooperation is important and traction based on physiological force in order to gain root and dentoalveolar tissue development in immature permanent teeth is required. Periodic follow-ups should be mandatory.

Interrelationship between periodontal parameters for the evaluation of clinically stable dental implants (인공매식치의 평가를 위한 치주지수간의 상관관계)

  • 자연치의 경우에는 치주지수간의 상관관계가 정립되어 병적인 상태에 대한 진단과 그에 따른 치료방법을 결정하는데 많은 정보를 제공하나, 인공매식치에서는 아직 논란의 여지가 많다. 이에 임상적으로 안정화되어 기능하고 있는 인공매식치에서 치주지수간의 상관관계에 관하여 연구하고자 하였다. 총 43명의 환자(평균 44.7세)에 식립된 178개의 인공매식치를 연구에 이용하였다. 인공치아매식술을 시행한 후 1년에서 6년이 경과한 인공매식치에서 상부보철물을 제거한 후 치태지수, 치은열구출혈지수, 각화점막지수, 치주낭깊이, Periotest Values(PTVs) 등을 측정하여 각각의 분포상황과 상관관계를 분석하였다. 이 연구의 결과는 다음과 같다. 1. 치태지수의 평균값은 $0.83{\pm}0.82$이었다. 2. 치은열구출혈지수의 평균값은 $1.04{\pm}0.86$이었다. 3. 각화점막지수의 평균값은 $2.47{\pm}0.95$이었다. 4. 치주낭깊이의 평균값은 $3.12{\pm}1.14\;mm$이었다. 5. Periotest Values(PTVs)의 평균값은 $-0.66{\pm}4.28$이었다. 6. 치태지수, 치주낭깊이가 증가함에 따라 치은열구출혈지수는 유의성있게 증가하였다(P<0.01). 7. 각화점막지수가 증가함에 따라 치은열구출혈지수는 유의성있게 감소하였다(P<0.01). 8. 치은열구출혈지수와 매식치동요도 사이에서는 유의성있는 상관관계를 발견할 수 없었다(P>0.05). 9. 치주낭깊이와 매식치동요도 사이에서도 유의성있는 상관관계를 발견할 수 없었다(P>0.05). 10. 인공매식치의 직경과 길이가 증가함에 따라 매식치동요도는 유의성있게 감소하였다(P<0.01). 11. 하악에서의 매식치동요도가 상악의 경우와 비교하여 유의성있게 작았다(P<0.01). 결론적으로, 인공매식치에서도 자연치에서와 동일한 양상으로 치태가 많을수록, 그리고 치주낭 깊이가 깊을수록 염증의 심도와 관련이 깊은 것으로 생각된다. 특히, 인공매식치의 경우에는 같은 양의 치태가 존재시에 각화치은이 충분히 있는 쪽이 염증발생이 적은 것으로 나타났다. 치주조직에 염증이 존재하는 경우, 자연치아에서는 치아의 동요도가 증가하는 것으로 알려져있으나, 이 실험의 인공매식치에서는 유의성있는 상관관계를 발견할 수 없었다. 임상적으로 안정화된 인공매식치의 동요도는 염증정도에는 큰 영향을 받지 않고 인공매식치의 직경과 길이가 증가함에 따라 감소함을 보여주고 있다. 또한, 인공매식치의 동요도는 상하악골의 골질에 따라 차이가 있음을 명확히 보여주고 있다.

COMPARATIVE STUDY ON THE EFFICACY OF DIGITAL IMAGING FIBER-OPTIC TRANS-ILLUMINATION AND LASER FLUORESCENCE IN MONITORING THE REMINERALIZATION PROCESS OF INCIPIENT SMOOTH SURFACE ENAMEL LESIONS (Digital Imaging Fiber-Optic Trans-Illumination과 Laser Fluorescence를 이용한 평활면 초기우식증의 재광화에 관한 비교 연구)

  • Through out the world dental caries seems to be decreased as it is difficult to make an accurate diagnosis for dental caries. The traditional diagnostic method which is probing and x-ray taking has many limitations to diagnose the early caries, so there were recommendations for the needs of new equipments such as laser fluorescence(LF), digital imaging fiber-optic trans-illumination(DIFOTI), and quantitative light fluorescence (QLF) which were developed from various study results. Also confocal laser scanning microscopy(CLSM) and ultrasonics are used for research progression. This study is to evaluate whether it is possible to monitor accurately for remineralization amount of enamel surface early caries using DIFOTI or LF After inducing artificial caries to bovine teeth to 10 participants remineralization was enhanced by 0 ppm and 500 ppm fluoride mouth rinse solution for 3 weeks. Then they were cross sectioned and analyzed using gold standard of the lesion depth measured by CLSM. The following results were obtained: 1. The measured percentage of light intensity(luminosity ratio) by DIFOTI increased with remineralization period, and showed significant reverse correlation with lesion depth measured by CLSM (p<0.01). 2. The measurement of laser fluorescence increased with remineralization period, and showed significant correlation with lesion depth measured by CLSM (p<0.01). 3. To the result for CLSM, 500 ppm fluoride mouth rinse group showed rapid rate for decreased tendency of lesion depth than 0 ppm fluoride mouth rinse group. In conclusion DIFOTI system was used to measure accurately for the remineralization amount of early surface caries, it is a very useful equipment to detect precisely the changes for early enamel caries remineralization during treatments.

Evaluation of the accuracy of two different surgical guides in dental implantology: stereolithography fabricated vs. positioning device fabricated surgical guides (제작방법에 따른 임플란트 수술 가이드의 정확성비교: stereolithography와 positioning device로 제작한 수술 가이드)

  • Purpose: Recently implant surgical guides were used for accurate and atraumatic operation. In this study, the accuracy of two different types of surgical guides, positioning device fabricated and stereolithography fabricated surgical guides, were evaluated in four different types of tooth loss models. Materials and methods: Surgical guides were fabricated with stereolithography and positioning device respectively. Implants were placed on 40 models using the two different types of surgical guides. The fitness of the surgical guides was evaluated by measuring the gap between the surgical guide and the model. The accuracy of surgical guide was evaluated on a pre- and post-surgical CT image fusion. Results: The gap between the surgical guide and the model was $1.4{\pm}0.3mm$ and $0.4{\pm}0.3mm$ for the stereolithography and positioning device surgical guide, respectively. The stereolithography showed mesiodistal angular deviation of $3.9{\pm}1.6^{\circ}$, buccolingual angular deviation of $2.7{\pm}1.5^{\circ}$ and vertical deviation of $1.9{\pm}0.9mm$, whereas the positioning device showed mesiodistal angular deviation of $0.7{\pm}0.3^{\circ}$, buccolingual angular deviation of $0.3{\pm}0.2^{\circ}$ and vertical deviation of $0.4{\pm}0.2mm$. The differences were statistically significant between the two groups (P<.05). Conclusion: The laboratory fabricated surgical guides using a positioning device allow implant placement more accurately than the stereolithography surgical guides in dental clinic.
