Fibrous Dysplasia Associated with Primary Hyperparathyroidism Absent of McCune-Albright Syndrome: Tc-99m MIBI and Tc-99m MDP Findings

  • 섬유성 골이형성증은 비정상적인 섬유성 골조직으로 대체되며 느리게 진행하는 골병변이다. 섬유성 골이형성증과 부갑상선기능항진증은 흔하게 관찰되는 질환이나 McCune-Albright 증후군 없이 일차성 부갑상선기능항진증에 동반된 섬유성 골 이형성증은 거의 보고된바가 없다. Tc-99m MDP 골스캔이 섬유성 골이형성증의 진단에 유용하다고 알려져 있으나, Tc-99m MIBI 영상은 아직 보고된 바가 없다. 저자들은 McCune-Albright 증후군이 없이 부갑상선기능항진증에 동반된 섬유성 골 이형성증의 Tc-99m MIBI 스캔과 Tc-99m MDP 골스캔의 영상을 비교하였다. Tc-99m MDP 골스캔상 병변 부위에서 증가된 섭취 소견을 보였으며, Tc-99m MIBI 조기영상에서도 동일한 위치에서 섭취증가가 관찰되었다. 2시간 지연 Tc-99m MIBI 영상에서는 Tc-99m MIBI의 섭취가 배출되는 소견을 보였다. 섬유성 골이형성증 병변에서 Tc-99m MIBI 스캔이 유용할 것이라고 생각하며, 섬유성 골이형성증 병변에서 Tc-99m MIBI의 섭취를 증가시키는 인자에 대한 더 많은 연구가 필요할 것으로 생각한다.

  • Selective Laser Melting is one of the representative 3D printing techniques for handling metal materials. The main factors influencing the characteristics of structures fabricated by the SLM method include the build-up angle of structures, laser power, laser scan speed, and scan spacing. In this study, the tensile properties of AlSi10Mg alloys were investigated by considering the build-up angle of tensile test specimens, laser scanning speed and scan spacing as variables. The yield stress, tensile strength, and elongation were considered as tensile properties. From the test results, it was confirmed that the yield stress values were lowered in the order of 0, 45, and 90 based on the manufacturing direction of the tensile specimen. The maximum yield stress value was obtained at 1870 mm / min based on the laser scan speed. The yield stress size decreased with decreasing scan speed. Based on the laser scan spacing, as the value increases, the yield stress increases, but the variation is smaller than the other test criteria. The tendency of the tensile strength and elongation variation depending on the test conditions was difficult to understand.

A Study on Dose Response of MAGAT (Methacrylic Acid, Gelatin Gel and THPC) Polymer Gel Dosimeter Using X-ray CT Scanner (X-ray CT Scanner를 이용한 MAGAT (Methacrylic Acid, Gelatin Gel and THPC) 중합체 겔 선량계의 선량 반응성 연구)

  • In this study, we evaluated the dose response of MAGAT (Methacrylic Acid Gelatin gel and THPC) normoxic polymer gel dosimeters based on the X-ray CT scanner. To perform this study, we determined the proper ratio of the gel composition and acquired X-ray scan parameters. MAGAT gel dosimeters were manufactured using MAA (MethacrylicAcid) and gelatin of various concentration, irradiated up to 20 Gy. We obtained the 20 CT images from the irradiated gel dosimeters by using on a Phillips Brilliance Big Bore CT scanner with the various scan parameters. This CT images were used to determine the $N_{CT}$-dose response, dose sensitivity and dose resolution As an amount of MAA and gelatin were increase, the slope and intercept were increase in each MAGAT gel dosimeter with various concentration of the $N_{CT}$-dose response curve. The dose sensitivity was $0.38{\pm}0.08$ to $0.859{\pm}0.1$ and increased were amount of the MAA was increased or the gelatin was decreased. However, the change of gelatin concentration was very small compare to MAA. The Dose resolution ($D_{\Delta}^{95%}$) varies considerably from 2.6 to 6 Gy, dependent on dose resolution and CT image noise. The slope and dose sensitivity was almost ident verywith the variation of the tube voltage, tube current and slice thickness in the dose response curve, but the noise (standard deviation of averamalg CT number) was decreased when the tube voltage, tube current and slice thickness are increase. The optimal MAGAT polymer gel dosimeter based on the CT were evaluated to determine the CT imaging scan parameters of the maximum tube voltage, tube current and slice thickness (commonly used in clinical) using the composition ratio of a 9% MAA, 8% gelatin and 83% water. This study could get proper composition ratio and scan parameter evaluating dose response of MAGAT normoxic polymer gel dosimeter using CT scanner.

Value of Bone Scan in Addition to F-18 FDG PET/CT and Characteristics of Discordant lesions between F-18 FDG PET/CT and Bone Scan in the Spinal Bony Metastasis (척추골전이에 있어 F-18 FDG PET/CT에 대한 골스캔의 추가적 역할 및 F-18 FDG PET/CT와 골스캔간에 불일치 병소에 대한 연구)

  • Purpose: Our purpose was to evaluate spinal bony metastasis which could be missed on an F-18 FDG PET/CT (FDG PET/CT) alone, and to characterize discordant metastatic lesions between FDG PET/CT and bone scan. Material and Methods: FDG PET/CT and bone scans of 43 patients with spinal bony metastasis were analyzed retrospectively. A McNemar test was performed comparing the FDG PET/CT alone to the FDG PET/CT plus bone scan in the spinal bony metastases. A one-way chi-square test was performed to characterize the metastases that were missed on the FDG PET/CT alone. To evaluate discordant lesions between FDG PET/CT and bone scan, we performed logistic regression analyses. The independent variables were sites (cervical, thoracic, and lumbar), size (large and small), and maximum SUVs, and the dependant variable was bone scan uptake (positive and negative MDP uptake). Results: A significant difference was found between the FDG PET/CT alone and the FDG PET/CT combined with the bone scan (p < 0.01). Using the FDG PET/CT only, diffuse osteoblastic metastasis was missed with a significantly higher frequency (p = 0.04). In the univariate analysis, cervical vertebra and small size were related to negative MDP uptake, and thoracic vertebra and large size were related to positive MDP uptake. However, in the multivariate analysis, only the large size was related to positive MDP uptake. Conclusion: A bone scan in addition to the FDG PET/CT increased the ability to evaluate spinal bony metastases, especially for diffuse osteoblastic metastasis. Large metastasis was related to positive bone scan uptake in spinal bony metastasis.

Impact of Contrast agent for Attenuation Correction Using CT Scan in PET/CT System (PET/CT 시스템에서 CT 영상을 이용한 감쇠 보정 시 조영제가 PET 영상에 미치는 영향)

  • Experiments and simulation were done to study the impact of contrast agent when CT scan was used to attenuation correction for PET images in PET/CT system. Whole body phantom was imaged with various concentration of iodine-based contrast agent using CT. Mathematical emission and transmission density map with liver were made to simulate for whole body FDG imaging. Various transmission density maps was generated with non-uniform enhancement of contrast agent, hypo-attenuating of contrast agent for tumor, different concentration of contrast agent, and so on. Attenuation correction was done with all transmission maps. In the experiments, we confirmed that attenuation coefficient was changed by concentration of contrast agent. From the simulation data, image quality of attenuation corrected images was affected by contrast agent and artifact was produced by contrast agent. These results indicated that the contrast agent should be used with a full understanding of its potential problem in PET/CT system.

Relationship between Early Postoperative Renal Scintigraphy and Long-term Transplant Survival (신 이식 직후 신장 스캔 소견과 이식신 장기 생존의 관계)

  • Purpose: We investigated the possibility of early postoperative Tc-99m DTPA scintigraphy in predicting long-term renal transplant survival. Materials and Methods: 64 living donor (LD) grafts were divided into two groups according to the graft function on early post-operative renal scintigraphy. Survival analysis was performed using Kaplan-Meier method and Cox proportional hazard model. Chi-square test was performed to evaluate the difference in the frequency of acute rejection. Results: Cumulative renal transplant survival was decreased in 11 patients with abnormal renal scintigraphy, but it was not statistically significant. Decreased graft function on early post-operative renal scintigraphy was not a predictor of long-term graft survival. The frequency of acute rejection was higher in abnormal scintigraphy group, and the difference was statistically significant. Conclusion: Decreased graft function on early post-operative renal scintigraphy has no direct effect on long-term renal transplant survival in LD transplantation. But it may have an indirect elect through increasing the frequency of acute rejection.

  • Skip lesion is not uncommon feature in osteosarcoma and considered to be importantly associated with poor prognosis factor, and thus, should be excised with the main mass. The accurate pre-operative evaluation of the intramedullary extent of osteosarcoma is essential, because it determines the level of bone resection. Among the reliable detection methods, bone scan has a drawback of high rate of false negative results and regional MRI has a difficulty to cover the whole involved lesions without clinical suspicion. The authors report a case of osteosarcoma of the distal femur with a proximal skip lesion that was not detected by either regional MR imaging or by bone scan, but which was visualized by FDG-PET/CT.

  • The roughness changes on a fracture surface were analyzed via a multi-stage compression test under high temperatures to assess how the cracks in a rock mass affect groundwater movement. The analyzed samples consist of coarse granitic rocks from approximately 40 and 270 m depth, and fine granitic rocks from 500 m depth. The compression test was conducted on $20{\times}40{\times}5mm$ samples using a loading system where the pressure increases in 10 MPa increments to 120 MPa. A high-resolution 3D confocal laser scanning microscope (CLSM) was used to observe the surface changes, including the roughness changes, at each pressure step. The roughness change was calculated based on the roughness factor. The experimental results indicate that the roughness of the fracture surface varies with rock type under the stepwise pressure conditions. These data provide a basis for predicting groundwater flow along rock fractures.

Factors associated with Renal Scarring in Children with a First Episode of Febrile Urinary Tract Infection (소아의 첫 발열성 요로감염에서 신 반흔에 영향을 미치는 인자)

  • Purpose : Development of renal scarring is associated with delayed diagnosis and treatment of urinary tract infection(UTI). This study was performed to clarify how soon treatment should be started to Inhibit renal scarring after onset of UTI and the factors associated with renal scarring in children with a first episode of febrile UTI. Methods : We retrospectively reviewed 163 patients with a first episode of febrile UTI under the age of 2 years from April 2000 to Ap,il 2004. All patients had a DMSA renal scan and voiding cystourethrogram done in the diagnostic period, 6 months after which a follow-up renal scan was done. After patients wet-e divided into 2 groups according to the duration of fever prior to start of treatment, the duration of fever after start of treatment, and total duration of fever, initial and follow-up DMSA scan findings were analyzed among the different groups. We compared the factors associated with renal scars between the groups with and without renal scars. Results : The initial DMSA renal scan identified abnormal finding in 23% of the patients who were treated $\leq$24 hr from the onset of disease and in 43% of those with fever more than 24 hr. Renal scars developed in 33% of patients who were treated $\leq$24 hr and 38% of those with fever >24 hr prior to treatment. Renal scars developed in 34% of patients with remission of fever $\leq$48 hr after treatment and ill 50% of those with fever >48 hr after treatment. The risk for renal scars was significantly higher in children who had total duration of feyer >72 hr(67%) than in those with shorter duration(19%). In children with renal scars, VUR was most highly associated with an increased risk of renal scar formation. Conclusion : Although children with a first episode of febrile UTI are treated within 24 hr after onset of the fever, renal damage cannot be prevented completely and it is mainly associated with VUR. Scan Time Analysis Using 4D Phase-Contrast MRI According to Scan Parameter: A Phantom Study (스캔 인자에 따른 4D 위상 대조 자기공명영상을 이용한 스캔 시간 분석: 팬텀 연구)

  • Purpose: The purpose of this study was to evaluate the effect according to the NEX, VENC, targeted cardiac phases on the velocity measurement of 4D phase-contrast MRI. Materials and Methods: The abdominal aortic phantom was made to experiment. The working fluid was mixed with water and glycerin to mimic the density and viscosity of human blood. The inlet velocity was Reynolds number 2000. The experimental conditions were NEX 1 and 4, VENC 102 cm/s and 200 cm/s, and 10 and 15 targeted cardiac phases, respectively. The average flow rate, average velocity, maximum velocity, and cross-section area were measured. Results: As a result of the case-by-case comparison, the error rate was less than 5%. There was no significant difference (p > 0.05). Conclusion: It is expected that this result will be useful for acquiring blood flow information in clinical practice.
