Use of Hand Massage with Presence to Increase Relaxation in Korean -American Elderly (재미 한국이민노인에게 시행한 손마사지가 이완에 미치는 영향)

  • 간호사가 사용하는 중재로서 관심을 표현하는 방법이며 의사소통의 통로인 접촉의 한 형태로서의 마사지가 이완과 안녕감을 증진시킬 수 있는지를 알아보기 위해 간편하면서도 장소에 거의 구애를 받지 않는 손마사지를 미국 미네소타주에 살고 있는 22명의 한국 이민노인에게 시행하였다. 자료수집 기간은 1992년 12월부터 1993년 6월까지 약 6개월이 소요되었다. 편의표출방법으로 대상자를 선정한 원시실험연구설계로, 독립 변수로 손마사지를 사용하였고 이완행동, 수축기혈압, 이완기 혈압, 맥박수로 측정한 이완의 정도와 안녕감을 종속변수로 사용하였다. 손마사지는 Snyder등이 개발한 방법으로 한 손에 2분 30초씩 5분이 소요되는데 이틀에 한번씩 6회 실시하였다. 안녕 감을 측정하는 Cantril ladder는 10개의 사다리로 된 도구로 첫번 마사지전, 세번째 마사지후, 그리고 여섯번째 마사지후에 대상자로 하여금 직접 사다리 점수를 매기도록 하였다. Luiselli등이 개발한 이완행동 측정도구로 각 마사지 전후에 대상자의 이완정도를 관찰하였고, 마사지 전후에 혈압과 맥박수를 측정하였다. 자료는 paired t-test와 ANOVA, repeated measures ANOVA로 분석하였다. 연구의 결과는 다음과 같다. 1. 1-6회 동안의 마사지전의 이완점수의 평균값과 1-6회 동안의 마사지후의 이완점수의 평균값을 비교한 결과 마사지후에 이완의 정도가 유의하게 증가하였고 (t=10.70, p=.000), repeated measures ANOVA로 검증한 시간에 따른 효과도 유의한 차이를 보였다. 2. 1-6회 동안의 마사지전의 수축기 혈압의 평균값과 1-6회 동안의 마사지후의 수축기 혈압의 평균값을 비교한 결과 마사지후에 수축기 혈압이 유의하게 저하되었 다(t=6.09, p=.000). 3. 1-6회 동안의 마사지전의 이완기 혈압의 평균값과 1-6회 동안의 마사지후의 이완기 혈압의 평균값을 비교한 결과 유의한 차이를 보이지 않았다(t=.27, p=.793). 4. 1-6회 동안의 마사지전의 맥박수의 평균값과 1-6회 동안의 마사지후의 맥박수의 평균값을 비교한 결과 마사지후의 맥박수가 유의하게 감소하였다. (t=4.45, p=.001). 5. Cantril 안녕감의 점수는 유의한 차이를 보이지 않았다(F=2.42, p=1.01)

Effect of Health Behavior and Obesity Indices on Blood Pressure in 20s Man (20대 성인 남성에서 건강행태와 비만지표가 혈압에 미치는 영향)

  • This study was conducted to explore the effect of health behavior and obesity indices on blood pressure(BP) in 20s men. This was a retrospective exploratory study and the subjects were 214 men who participated in screening test for clinical trial at a tertiary hospital between October 2006 and April 2011. The proportion of normal SBP and DBP was 28.5% and 50.0%, respectively. BP was significantly higher in individuals who were hypertriglyceridemia and hypercholesterolemia than BP in subjects of opposing categories. DBP was significantly higher in individuals who were 25~29 years and smokers than DBP in subjects of opposing categories. The significant factors of increasing SBP were BMI and TG, and those for DBP were TC, TG, age, and smoking amount. In conclusion, these results demonstrate hypertention prevention program to include decreasing intake of TC and TG, maintaining normal BMI and quitting smoking needs to the young men in 20s.

A Study on Estimation of Systolic Blood Pressure using PTT (PTT를 이용한 수축기 혈압의 측정에 관한 연구)

  • Blood pressure (BP) is one of the important physiological parameters for diagnosing cardiovascula diseases by means of noninvasive method. Existing noninvasive methods for measuring arterial BP have to use cuff and difficult in measuring arterial BP continuously. Systolic blood pressure (SBP) and pulse transit time (PTT) have a kind of inverse relationship. We acquired PTT data when subjects were in relaxation and also after exercise. We performed the linear regression analysis for making the regression equations for each subject and the regression equation for all subjects. We compared the estimated SBP with the measured SBP to check the accuracy of our regression equations. From the result, the regression equations for each subject was appropriate according to the American National Standards Institute of the Association of the Advancement of Medical Instrument (ANSI/AAMI) which says that BP devices should have ±5mmHg mean of error and 8mmHg standard deviation of error. However, the regression equation for all subjects was not proper to ANSI/AAMI recommendation. The result means that, without cuff, we can continuously estimate each subject's SBP through PTT and indivisual calibration.

Relationship between Blood Pressure and Impairment of Cognitive Function In Some Rural Residents Aged 60-64 (일부 60-64세 농촌 거주자에서 혈압과 인지기능 장애와의 관계)

  • Objectives : Face-to-face interviews were conducted to investigate the relationship between blood pressure and the impairment of cognitive function in rural elderly (N=932) aged 60-64 in Dalsung County, April to September in 1996 Methods : Impairment of cognitive function was defined as a score of less than 23 by the Korean version of the Mini-Mental State Examination (MMSEK). Blood pressure was measured once in each subject using a portable automatic sphygmomanometer. Results : By univariate logistic regression on males, no category of systolic blood pressure bore statistical significance. Groups with diastolic blood pressures of, less than 80 mmHg, 90-94 mmHg, and more than 95mmHg had odds ratios of more than one compared with the reference group (80-89 mmHg). This was most significant in the group with blood pressures lower than 80 mmHg, which had a statistically significant odds ratio of 1.68 (95% confidence interval CI; 1.02-2.75). No category of blood pressure was statistically significant in females. Multivariate logistic regression for males, with adjustment for age, educational attainment, smoking, alcoholic drinking, body mass index, atherosclerotic disease, and antihypertensive medication use, did not alter the odds ratios significantly in terms of systolic blood pressure. However, the group with diastolic blood pressure of less than 80 mmHg had an increased odds ratio of 2.01 (95% CI; 1.15-3.52) compared with the reference group. In females, systolic blood pressure did not alter the odds ratio, but the group with a diastolic blood pressure of less than 80 mmHg had a statistically significant odds ratio of 0.57 (95% CI; 0.37-0.89). Conclusions : These results suggest that the relationship between blood pressure and cognitive function status is stronger diastolic than systolic blood pressure and that there is a complex relationship between blood pressure and cognitive function by sex.

The Effects of Red Ginseng on Blood Pressure and the Quality of Life in Essential Hypertensives (본태성 고혈압 환자의 혈압과 생활상에 미치는 홍삼의 영향)

  • We studied the effects of red ginseng on blood pressure (BP) and the quality of life (QOL) in 19 hypertensive patients treated with antihypertensive agents. Red ginseng was administered at a dosage of three grams a day for three months. Systolic blood pressure was significantly lowered during the administration period of red ginseng. while diastolic blood pressure and hear1 rate remained unchanged. QOL was significantly improved in $89\%$ of the patients. Among the QOL variables. sleep disorder. thirst. fatigue. mood. sexual life and general well-being were improved. Furthermore. an improved coefficient of variation of R-R intervals $(CV_{R-R})$ was observed during the period. A month after the cessation of red ginseng systolic blood pressure returned to the level before the administration and QOL variables detariorated to previous pre-treatment states. These results suggest that red ginsing may lower systolic blood pressure and may improve QOL in patients treated with antihypertensive agents.

Nutritional and Health Status of the Elderly Living in Songnam - I. Anthropometric Measurements and Biochemical Nutritional Status - (성남지역 노인의 영양 및 건강상태 조사 I. 신체 계측 및 생화학적 영양상태)

  • To study the nutritional and health status of the elderly in Songnam, the anthropometic measurments, fat composition of body(FAT), blood pressure(BP) and blood test for analyzing biochemical indies of the elderly in seongnam were carried out in 338 free-living elderly (161 men, 177 women) aged from 58 to 92. Anthropometric measurements and blood pressure were assessed, and body fat was analyzed by BI(Bioelectrical Impedence) method. Biochemical measurements including blood glucose(BG), GPT, serum total cholesterol(TC) and albumin(AL) were analyzed. Average heights of the subjects were lower than the standards established in the Korean Recommended Dietary Allowances, and average weights were similar to the standards. FAT were significantly higher in women(36.4%) than men(27.7%), and both groups showed higher values than the reference. Mean systolic BP levels of men and women were 153.3mmHg and 153.7mmHg, and diastolic BP levels were 87.5mmHg and 86.0mmHg, respectively, and there were no significant differences between the sex. Mean BG levels of men and women were 115.9mg/㎗ and 123.7mg/㎗, respectively, and there were no significant differences between the sex and among the age groups. Mean GPT levels of men and women were 19.4unit and 18.1unit, respectively, and women older than 75 years showed significanly low levels of GPT compared to other women's age groups. Mean TC levels of men and women were 186.0mg/㎗ and 206.6mg/㎗, respectively, and significantly higher TC levels were shown in women than men. Mean AL levels of men and women were 4.8g/㎗ and 4.8g/㎗, respectively, and men aged 50 ∼ 64 years showed significant high AL levels compared to other age groups of men. It was suggested that nutrition education program was a good way to improve health status of the elderly.

The Effect of Dietary Hizikia Fusiforme on the Reduction of Blood Pressure and Total Cholesterol in Jeju Women Divers (제주 해녀의 식이성 톳 섭취를 통한 혈압 및 총콜레스테롤 감소 효과)

  • The purpose of this study was to investigate the changes of blood pressure and total cholesterol after eating Hizikia fusiforme for 8 weeks with 100g of rice per day. As a result, the systolic blood pressure of the group ingesting Hizikia fusiforme significantly decreased. Second, diastolic blood pressure decreased significantly in both groups, but there was no difference between groups. Third, the systolic - diastolic blood pressure difference showed a significant decrease in the group of ingestion of Hizikia fusiforme, while that of the group without intake of Hizikia fusiforme increased. Fourth, total cholesterol decreased significantly in the group consuming Hizikia fusiforme but increased in the group not consuming Hizikia fusiforme. The above results indicate that total intake of 100 g of dried Hizikia fusiforme per day was effective in decreasing total cholesterol and reducing systolic blood pressure.

Risk Factors in Relation to Blood Pressure in Schoolchildren (국민학교 학동의 혈압에 관련된 요인분석)

  • This study was conducted to identify the factors related to the blood pressure in school children. The study subjects were 144 boys and 140 girls aged 10 years old, 4th grade student of one elementary school in Taegu City. Blood pressure and 18 variables for 284 school children were measured in May 1992. A weight-for-height index, R$\ddot{o}$hrer index(weight/$height^3$) was calculated for each individual as a measure of obesity. Body fat percent, fat weight, and lean body mass were measured by impedance fat meter(Model SIF-891). Serum total cholesterol, high-density lipoprotein, triglyceride, uric acid, total protein, and electrolyte were measured by automated clinical chemistry analyzer(Hitachi 4020). Low-density lipoprotein and atherogenic index were calculated by the equation. The variables assessed were sex, systolic blood pressure, diastolic blood pressure. R$\ddot{o}$hrer index, body fat percent, body fat weight, lean body mass, total cholesterol, high-density lipoprotein, low-density lipoprotein, uric acid, total protein, serum electrolyte and blood glucose. The mean systolic and diastolic blood pressures for boys were 104.1mmHg and 66.6mmHg and those for girls were 102.9mmHg and 67.5mmHg. Systolic blood pressure of boys was positively correlated with height, weight, R$\ddot{o}$hrer index, fat weight, lean body mass, and triglyceride. Systolic blood pressure of girls was positively correlated with height, weight, lean body mass, and uric acid. Diastolic blood pressure of boys was positively correlated with weight. R$\ddot{o}$hrer index, and lean body mass. Diastolic blood pressure of girls was positively correlated with height, weight, fat weight, and lean body mass and negatively with total serum protein. In multiple regression analysis for the systolic blood pressure, the significant independent variables were R$\ddot{o}$hrer index and serum sodium in boys, and serum uric acid, fat percent, and R$\ddot{o}$hrer index in girls. In multiple regression analysis of the diastolic blood pressure, the significant independent variables were R$\ddot{o}$hrer index in boys and total serum protein in girls. This study indicated that important factors influencing blood pressure in school children were R$\ddot{o}$hrer index, fat percent, serum sodium, uric acid, and total protein. R$\ddot{o}$hrer index was most important among these. It is recommended to enforce the nutritional education for diet control of obesity to prevent hypertention in children and to adopt healthy life-styles that promote good health and prevent development of cardiovascular risk factors.

  • We report the case of a 7-year-old boy who showed treatment-nonresponsive hypotension (59/29 mmHg) and decreased left ventricular systolic function (fractional shortening 22%) in the acute stage of Kawasaki disease (KD). The present case serves to highlight that methylprednisolone pulse therapy should be considered in patients with intravenous immunoglobulin nonresponsive symptomatic myocarditis during the acute stage of KD.

Effect of Supplementation of Fermented Milk Containing Active Peptides(IPP, VPP) in Accordance with Medical Nutrition Therapy in Pre- and Hypertension Subjects (고혈압 전단계 및 고혈압 환자에서 의학영양치료와 병행한 생리활성 펩티드 함유 유산균 발효유 섭취가 혈압에 미치는 영향)

  • It is well known non-drug therapy for hypertension patients can reduce blood pressure. These types of therapy include maintaining ones ideal body weight, quitting smoking, consuming large amounts of fruits and vegetables consuming low levels of saturated fat and salt and regular exercise. Fermented milk protein derived biologically active peptides such as isoleucine-proline-proline_(IPP) or valine-proline-proline_(VPP) have been shown to lower blood pressure in hypertensive subjects. This study was conducted to investigate the antihypertensive effects of medical nutritional therapy _(MNT) in accordance with the consumption of fermented milk enriched with IPP and VPP. To accomplish this, we conducted a randomized case-controlled study of 43 subjects who had blood pressure levels greater than 120/80 mmHg. The subjects in the study group were randomly allocated into two groups, an MNT + fermented milk (100mL/day) group (n=21) and an MNT+L. helveticus fermented milk with tripeptides (IPP=2.2mg, VPP=2.6mg/100mL) group (n=22). The MNT included weight management, reduction of sodium, total fat and saturated fat intake, increased intake of fruits and vegetables, and increased intake of low fat dairy products. The treatments were administered for 12 weeks during which time no drug interventions were conducted. The daily intakes of total calories, fats, cholesterol and Na decreased significantly after 12 weeks of MNT in the control and the experimental groups. In addition, the systolic blood pressure de creased significantly in the control and experimental groups; however, the diastolic blood pressure only decreased significantly in the experimental group. Overall, the results of this study indicate that the intake of fermented milk containing IPP and VPP in conjunction with MNT exerted positive effects on the blood pressure of pre- and hypertensive subjects.
