Mandibular implant-natural tooth retained overdenture using magnetic attachment with stress breaker (하악에서 완압형 자성 어태치먼트를 부착한 임플란트-자연치 피개 의치: 10년 증례보고)

  • Overdenture has the advantage of improving the stability and retention of the denture but the abutment may be easily affected by caries or periodontal disease and the thin denture can be easily broken. The magnetic attachment overdenture has a high vertical retention but a low horizontal retention, thus, exerting a less disruptive force to the abutment or implant and shows less abrasion or damage compared to other mechanical retainers. Denture fractures in overdenture is caused by the thin denture base as the attachment is inserted, but it may also be caused by the difference in detrusion between soft tissue and hard tissue, and between an implant and a natural tooth. To compensate this shortcoming, a magnetic attachment with a silicone ball inserted in the magnet was developed as we report a successful case using this specific type of magnetic attachment overdenture.

  • Compared with traditional full dentures, mandibular implant overdentures have the advantage in that good support and retention can be obtained even with two implants. When manufacturing a mandibular implant overdenture using two implants, it is important to place the implant in the correct position. The long-term prognosis of overdenture is good when two implants are placed in the direction vertical to the occlusal plane and parallel to each other at the canine position. However, it is difficult to place two implants in the correct position in edentulous patients, and if you use surgical guides in these cases, you can get help in placing the implants in a prosthetically advantageous position. This case, a 57-year-old male patient, came to our hospital with all upper and lower teeth removed due to periodontal disease. Therefore, the maxilla was restored with a traditional full denture and the mandible was restored with an implant overdenture using two implants, which resulted in satisfactory functional and esthetic results.

Treatment and posttreatment changes following intrusion of maxillary posterior teeth with miniscrew implants for open bite correction (Miniscrew implant를 이용한 상악 구치부 압하와 개방교합 치료의 효과 및 안정성)

  • Objective: This study investigated the skeletal and dentoalveolar changes after intrusion of maxillary posterior teeth using miniscrew implants during the treatment and posttreatment period. Method: The subjects consisted of 11 adults (1 male, 10 females) whose open bites were treated by posterior teeth intrusion with miniscrew. The lateral cephalometric x-rays of pre-treatment, post-treatment, and the retention period were evaluated. Results: The average intrusion of maxillary posterior teeth at the end of the treatment period was 2.22 mm (p < 0.001) and the average extrusion of maxillary posterior teeth at the end of the 17.4 month retention period was 0.23 mm (p = 0.359). The relapse rate was calculated at 10.36%. A decrease in overbite after the 17.4 month retention period was 0.99 mm (p < 0.05). The relapse rate was calculated at 18.10%. The skeletal changes and vertical height change of maxillary posterior teeth during the retention period was statistically insignificant, but there was a significant decrease in overbite (mean 0.99 mm, p < 0.05). The amount of intrusion and the overbite improvement from treatment significantly correlates to the degree of relapse. Conclusions: The results indicate that intrusion of the maxillary posterior teeth using miniscrews is an effective, non-surgical treatment modality which can reasonably be used to address adult open bite.

Changes of Occlusion following the Placement of Preformed Metal Crown to the Permanent First Molar in Children and Adolescents (소아청소년에서 제1대구치의 기성금속관 수복 후 교합 변화)

  • The purpose of this study is to evaluate changes of occlusion following the placement of preformed metal crown in the permanent first molar affected by severe caries or hypomineralization. The study was conducted on 24 patients who met the criteria among the patients with occlusion in the permanent first molar reaching the occlusal plane. Restorative treatment was performed on one first molar using preformed metal crown. Before treatment, immediately after treatment, after 4 weeks, and after 8 weeks, the T-scan III was used for measuring distribution of bite force at the maximum intercuspal position, and overbite was measured with digital vernier calipers and change of dental midline was surveyed. After 8 weeks, questionnaires survey was conducted. In most cases, immediately after treatment, the distribution of bite force between the restored side and non-restored side of arch was reversed compared to before treatment, and the overbite was decreased. However, after 4 weeks, bite force of the left and right side was balanced and the distribution of bite force on the restored and non-restored permanent first molars had similar values. Also, the changed overbite and midline were returned to pre-treatment status and there was no significant temporomandibular joint and masticatory discomfort after treatment. In this study, it was confirmed that spontaneous occlusal equilibrium was achieved one month after the placement of preformed metal crown.

Implant-retained overdenture with CM LOC® Pekkton® in maxillary edentulous patient (상악 완전 무치악 환자에서 CM LOC® Pekkton® attachment를 이용한 임플란트 유지 피개의치 수복 증례)

  • Treatment options for edentulous patients are complete denture and implant prosthesis. A two implant-retained overdenture can be considered the first treatment in the edentulous mandible, but there is no clear consensus of treatment for edentulous maxilla. Implant-retention/support overdenture shows better retention and stability than complete denture and is less expensive and more esthetic than implant-supported fixed prosthesis. CM $LOC^{(R)}$ $Pekkton^{(R)}$ attachment is a solitary type attachment and evaluated to have excellent abrasion resistance and retention with a female part made of poly-ether-ketone-ketone. Meanwhile, SR Ivocap system is injection molding method and discussed to show few changes in the vertical dimension of denture and have excellent fracture resistance. In this case, we restored maxillary arch with a four implant-retained overdenture using CM $LOC^{(R)}$ $Pekkton^{(R)}$ and SR Ivocap system, and mandibular arch with a removable partial denture. Through this procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

  • Extracting mandibular incisors for orthodontic treatment may adversely affect the occlusion. However, when properly used, extraction of mandibular inciors is a selection for the correction of the malocclusion. Generally, treatment for crowding needs to select between nonextraction and four premolar extraction. Approaches for crowded mandibular incisors include distal movement of posterior teeth, lateral movement of canines, labial movement of incisors, interproximal enamel reduction, removal of premolars, removal of one or two incisors, and various combinations of the above. Extraction of incisors is used in case of crowding, anterior tooth size discrepancy, absent of maxillary lateral incisors, and ectopic eruption. But severe overjet. overbite, and space are the contraindication of it. A patient had severe crowding on upper anterior teeth, impacted upper left lateral incisor, palatal ectopic eruption of upper right incisor and severe crowding on lower anterior teeth. Lower lateral incisors are extracted for space availability and facial esthetics. We report the case of orthodontic treatment of upper and lower anterior crowding through extraction of lateral incisor.

A Comparative Study on the Retention of Implant Overdenture According to the Shape and the Number of Magnetic Attachment (자성 어태치먼트의 형태와 수에 따른 하악 임플란트 피개의치의 유지력에 대한 비교 연구)

  • The aim of this study was to compare the retention and stability of implant overdenture according to the shape and the number of magnetic attachment. The experimental groups were designed for the number of implants(1, 2, 4) and shape of magnetic attachments(flat, cushion, dome type) resulting in 9 subgroups. 45 attachments were tested attached to $Br{\aa}nemark$ system implants which were planted on a mandibular model. Each attachment was composed of the magnet assembly embedded in a overdenture sample and the abutment keeper screwed into the implants. Dislodging tensile forces were applied to the overdenture samples using an Instron(cross-head speed 50.80mm/min) in 3 directions simulating function: vertical, oblique, and anterior-posterior. The loading was repeated 10 times in each direction for 45 samples. The values of maximum dislodging force of each subgroup were processed statistically using SPSS V. 12.0 at the 0.05 level of significance. The results of this study were as follows: 1. Flat type magnetic overdenture was the most retentive when subjected to vertically directed forces and dome type was the lest retentive when subjected to obliquely directed forces(p<0.05). 2. In case of planting one implant, flat type had a higher vertically retentive force than anterior-posteriorly retentive force. In case of planting two implants, flat type and dome type had a higher vertically retentive force and in case of planting four implants, flat type and cushion type had a higher vertically retentive force than anterior-posteriorly retentive force(p<0.05). 3. The incremental number of dental implant, without regards to the three types of magnetic attachment shapes, showed higher retention of overdenture(p<0.05). From the results, if a patient need much more retention of implant overdenture, flat type magnetic overdenture would be a good treatment. In case of the bruxism where excessive lateral forces are already present, dome type could be expected to produce better results. In case of planting one implant, flat type is more stable than the other shape of magnet and in case of two implant, flat type and dome type are more stable and in case of four implants, flat type and cushion type are more stable. Planting more than two implants and using flat type magnetic attachment would provide better retention and stability of implant overdenture

  • Overdenture using dental implants could improve the problems of conventional complete denture function which are pain during mastication, insufficient retention and stability. Locator attachment used widely for implant-retained overdenture has advantages that it needs the smallest vertical space and also its nylon male cap allows personalized retention for each case. However its retention force decreases rapidly with function rather than the bar and ball attachment. So, implant fixture should be positioned as parallel as possible.

The Prevalence of Malocclusion and Related Self-Esteem of Adolescents in Yangsan (양산시 청소년의 부정교합 유병률 및 관련 자존감 조사)

  • For the purpose of evaluating the prevalence of malocclusion and self-esteem in adolescents, 1,380 middle-school students in the Yangsan area were surveyed by oral examination and questionnaires and the obtained results were as follows: The distribution of 1st molar occlusion by Angle's classification was 69.0, 19.4, 10.6% for Class I, II and III respectively. In the horizontal relationship, the prevalence of normal overjet and crossbite was 86.9% and 5.6% respectively whereas larger and extremely larger overjet was found in 6.6% and 0.8%. In the vertical relationship of anterior teeth, normal, deep overbite and openbite was shown in 94.1%, 4.7%, and 1.2%. For the midline discrepancy, the distribution of groups with 0~1 mm, 2~4 mm and over 5 mm was 98.2%, 1.4%, and 0.4%. Crowding only in the maxilla was found in 9.6%, while that only in the mandible and in both arches was 14.1% and 24.1% respectively. Spacing only in the maxilla was seen in 3.0%, while that only in the mandible and in both arches was 2.4% and 1.7% respectively. Significant difference in self-esteem was revealed in female and malocclusion groups of crossbite and openbite(p < 0.05).

Validity of Arch Relationship Measurements in Digital Dental Models (디지털 치열 모형에서 악궁 관계 지표 측정의 타당성)

  • The aim of the present study is to evaluate the validity of orthodontic measurements including tooth width, Bolton ratio, overjet and overbite on the digital dental models. Dental models of the subjects aged 12 to 18 were obtained in 3 different forms, which were conventional stone model, digital model created with Freedom HD model scanner, and digital model produced with CS3600 intraoral scanner. After measurements were made on the models, reliability and reproducibility of the measurements were evaluated by using intraclass correlation coefficient, while validity was assessed with paired t-test. As a result, significant reliability and reproducibility were verified, with intraclass correlation coefficient exceeding 0.750 in all groups. Measurements of the model scanned group showed an adequate validity in overall and anterior Bolton ratio, overjet, and overbite. Intraoral scanned models showed an adequate validity in anterior Bolton ratio, and overjet. Measurement on intraoral scanned digital models can be considered as an alternative for young children who have difficulty in taking impression. Furthermore, careful considerations on measurement error should be made in clinical situations.
