Sociomedical Study on the Person Recieved Permanent Sterilization Method in Busan Area (부산 일부 지역의 영구불임 피술자들에 대한 사회의학적 조사)

  • This study was carried out for the evaluation on the sociomedical characteristics of 1,580 cases who had recieved vasectomy and laparoscopic sterilization at the Busan Family Planning Clinic from January 1975 to December 1973. The results obtained were as follows: 1. In age distribution, the most predominant age group consited of 30.34 years as 44.7%. In regional distribution, the most predominant region was Youngdo Ku in the vasectomy group and Dong Ku in the laparoscopic sterilization group. 2. The educational level showed that the high school graduates(49.2%) in the vasectomy group and the primary school graduates(47.0%) in the laparoscopic sterilization group were the highest each other. 3. The most predominant experienced contraceptive method before accepted permanant sterilization operation was oral pills and non-experienced contraceptive method group was 54.1% of the total. 4. By the span of marital life before accepted permanant sterilization operation, the 5-9 years group was the highest. 5. The average number of living children per family was 2.54 in the vasectomy group and 3.0 in the laparoscopic sterilization group. 6. The average frequency of pregnancy per case was 2 in the vasectomy group and 3 in the laparoscopic sterilization group and the most predominant frequency of induces abortion was 1 per case. 7. The most predominant motive of accepting the sterilization operation were family planning education at the reserve forces training in the vasectomy group and at the mother's club in the laparoscopic sterilization group. 8. By the residing status of the cases, rented room was the highest as 69.4%.

  • 슬관절 전치환술은 관절염이나 사고로 인해 일상적인 활동의 제약을 받는 환자의 슬관절을 인공 관절로 대체함으로써 본래의 기능을 복원하고자 하는 수술이다. 이 수술은 인공 관절의 위치 및 정렬에 매우 민감하게 영향을 받기 때문에 수술이 잘못되는 경우 정렬 이상으로 인한 해리, 삽입물의 파손, 인공 슬관절 주위 골절, 슬개골 탈구, 굴곡 각도의 제한 등의 증상이 발생할 수 있다. 현재의 인공 관절은 임상에 적용되는 다양한 인공 관절 중에서 적당한 형상의 관절을 선택하여 시술되고 있지만 환자의 골 형상에 정확히 일치하는 인공 관절 선택의 어려움 때문에 종종 시술 후 부작용이 발생한다든지 심지어는 재수술을 해야 될 경우도 발생하게 된다. 본 논문은 Mechanical CAD 소프트웨어인 CATIA에서 제공하는 절단, Assembly, Analysis, Kinematic Simulation 기능 등을 이용하여 가상 수술을 수행하는 과정을 보여준다. 슬관절 전치환술 과정을 그대로 재현하여 절단량과 절단각을 결정하고 환자의 골격 형상에 적합한 최적의 인공 관절을 실제 수술 전에 미리 선정할 수 있다. CAD 시스템을 이용함으로써 외과의들이 실제 수술 시에 시행착오법을 통해 인공 관절을 선택하는 과정을 줄이고 수술의 정확도를 높일 수 있다. 향후 ADAMS나 ANSYS와 연계하여 수술 후 동작이나 하중을 분석할 수 있으며, 수술 과정에 대한 교육용으로 활용될 수 있다.

Factors Influencing on Medical Service Satisfaction of Patients Utilizing a Day-Case Surgery Center in Korea (국내 당일수술센터 이용환자의 의료서비스 만족도에 영향을 미치는 요인)

  • This study was attempted to identify the factors relating to the medical service satisfaction on the patients utilizing a day-case surgery center in Korea. A questionnaire survey was conducted to patients were utilizing a day-surgery center, independently operated by one general hospital from January 13 to February 13, 2009. The results of this study can be summarized as follows. First, in the characteristic related with health and medical situations, day-surgery patients had little experience with surgery or hospitalizing and much interest in their health. Secondly, there were significant differences in regard to the quality of medical service for surgery waiting time and surgery time(p<0.05). Thirdly, multiple regression analysis revealed that age and level of education affected patients perception of day surgery(p<0.05). In conclusion, in order to increase the rate of day-surgery, it is necessary to give information about the advantages of day-surgery center and preoperative education.

Effects of a Web-based Learning Contents in Operating Room Nursing for Nursing Students (간호학생을 위한 웹기반 수술간호 학습콘텐츠의 효과)

  • This study was conducted to develop a web-based learning contents that assists nursing students' clinical practice on operating room nursing and to examine its learning effects. Based on contents and need analysis, 6 learning modules were developed regarding nursing care in surgical operation and operating equipments. In order to verify the effects of the program, this study employed a quasi-experimental pretest-posttest control group design. Knowledge was significantly higher in experimental group that was enhanced by the web-based learning contents than that of the control group (p<.001). However, there was no significant difference between the two groups in self-directed learning. Experimental group had significantly higher motivation toward learning, which was examined posttest only, compared to control group (p<.001). These results support that the implementation of an web-based learning contents need to be continued as an effective educational material but we need more research to establish how this can be effectively implemented in a students' practicum.

The Effect of Preoperative Teaching on the Emotional Status of Patients Prior to Surgery (수술전 교육프로그램이 환자의 정서상태에 미치는 영향)

  • The purpose of this study was to examine the effect of preoperative teaching as a nursing intervention on the emotional status of the patients prior to surgery. The method of this study was Nonequivalent Control Group Non-Synchronize Design. The data were selected from H universuty hospital in Seoul, and they consisted of an experimental group-30patients, and a control group-30 patients. The questionnaire consisted of 28 Preoperative Stress Inventory(Kang, 1998), and 16 State- Trait Anxiety Inventory(Spielberger, 1972). The collected data were analized by means of frequency, percentage, $X^2$ test, t-test and paired t-test with SPSS WIN. The results of this study were as follows : 1. The level of stress prior to surgery was significantly lower for the preoperative teaching group than for the control group(t=2.61, p< .01). 2. The level of anxiety prior to surgery was not significantly lower for the preoperative teaching group than for the control group. 3. The difference level of the anxiety level the prior to surgery was significantly higher for the preoperative teaching group than for the control group(t=2.82, p< .01). In conclusion, preoperative teaching is a beneficial nursing intervention that has a positive effect on the stress and anxiety of patients prior to surgery. Therefore it is recommended to use the preoperative teaching program as a nursing intervention for patients prior to surgery.

The Effect of Education on Preoperative PCA upon Postoperative Pain (수술 전 통증자가조절기에 관한 교육이 수술 후 통증에 미치는 효과)

  • Purpose: The purpose of this study was to compare the effectiveness of a structured preoperative PCA education to that of the usual informal teaching provided by hospital staff in alleviating postoperative pain through more effective use of PCA therapy. Method: As an advanced research, the knowledge about the use of PCA therapy and the attitude about the use of the pain medicine were estimated, and then as a later research, the knowledge about the use of PCA therapy, the attitude about the use of the pain medicine, and the score of the pain were estimated. Result: The experimental group those who received structured preoperative PCA education had statistically significant higher knowledge regarding the use of PCA therapy(p=0.026) and more positive attitudes toward using pain medicine(p=0.004). The experimental group those who received structured preoperative PCA education reported better pain control 4(p=0.002), 8(p=0.014) and 24 hours(p=0.018) after the operation. Conclusion: The structured preoperative PCA education increases the knowledge for the use of patient's PCA therapy and changes their attitude toward the use of the pain medicine positively and consequently enable the use of PCA therapy to be more effective and then finally reduced the patient's pain after the operation.

The Effects of Nursing Education using CD ROM on the Anxiety and Knowledge of Patients having Minor Surgery (CD ROM을 이용한 교육프로그램이 단기입원수술 환자의 수술 전 불안과 수술 후 자가간호 수행을 위한 지식에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effect of nursing education using a CD ROM on the anxiety and knowledge of the patients having minor surgery. Method: Forty patients hospitalized in K hospital in Seoul from April to August 2002 participated in this study. In the experimental group, twenty patients received nursing education on the operational procedures and post-operational care. The control group, received conventional nursing care only. Anxiety experienced by patients was measured by Spielberger's State-Trait Anxiety Inventory, and knowledge was measured by an instrument developed by Rahe et al. The data were analyzed by SPSS statistical program. Result: There were no significant differences in anxiety level between two groups. However, subjects in the experimental group were found to have significantly higher postoperative knowledge levels than those in the control group, and were very satisfied with the CD ROM program. Conclusion: The nursing education program using CD ROM before minor surgery proved to be an effective nursing intervention to increase knowledge of patients and contribute to their self care after discharge. To decrease anxiety of the surgery, the nursing education program should be combined with supprortive emotional nursing intervention, such as touch, and massage.

The Effect of Pre-Operative Patient Controlled Analgesia Education on Elderly Patients with Total Knee Arthroplasty (슬관절 전치술 노인 환자의 수술 전 자가통증조절기 사용 교육이 수술 후 환자에게 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of preoperative patient controlled analgesia (PCA) education on total knee replacement arthroplasty (TKRA) patients' PCA usage, level of pain, the frequency of pro re nata (prn) administrations, number of ambulations/day and continuous passive motion (CPM) angle after TKRA. Methods: This research used the non-equivalent control group pre-test and post-test design. Forty-five TKRA patients at a hospital in Seoul, Korea were included for in the study. For the experimental group, a 20-minute education session was provided a day before surgery. Data were analyzed using Windows SPSS Statistics 21.0 program. Results: Knowledge was higher in the experimental group than in the control group (p<.001). Incidence of nausea was lower in the experimental group (p=.01). No significant differences were found in post-operative pain scores, the frequency of prn analgesics administrations, dizziness, number of ambulations/day and CPM angle. Conclusion: This study showed that pre-operative PCA education could be an effective nursing intervention for increasing patient knowledge on PCA and nausea reduction after TKRA.

The Effect of a Preparation Method using Hospital Play on Anxiety and Role Behaviors of Children Undergoing Tonsillectomy (병원놀이 교육이 편도선 적출술 아동의 수술 전 불안과 수술 후 환아 역할행위에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of a preparation method using hospital play on anxiety and role behaviors of children undergoing tonsillectomy. Method: Twenty-eight children aged 5 to 7 years who were admitted for tonsillectomy were assigned to either an experimental group or a control group purposefully. The preparation method included provision of information using hospital play. The data were analyzed using Mann-Whitney test and Chi-square test. Result: There was no Significant difference in anxiety before surgery between the experimental and control groups. Also, there was no Significant difference in role behaviors on the 1st and the 2nd day of surgery. Conclusion: Even though there was no statistical significance, the result showed that the preparation method had some positive effect on children and parents. It is needed to develop systematic preparation methods for children undergoing surgery to enhance their coping.

Effects of Preoperative Video Education on Delirium and Anxiety in Elderly Patients with General Anesthesia (수술 전 동영상 교육이 전신마취수술 노인환자의 섬망과 불안에 미치는 효과)

  • Purpose: This study aimed to verify the effects of preoperative video education on delirium and anxiety in elderly patients with general anesthesia. Methods: This study used quasi-experimental research with a non-equivalent control group non-synchronized design. Participants were 70 elderly patients who had undergone of surgery under general anesthesia (35 in the experimental group and 35 in the control group). Preoperative video education was given to the experimental group. Subjective anxiety, blood pressure, pulse and delirium were measured. Data were analyzed using descriptive statistics, test of homogeneity in pretest, independent t-tests, and two-way repeated measures ANOVA with the SPSS/Win 22.0 program. Results: Delirium of the experimental group was significantly lower than that of the control group. Subjective anxiety measured using visual analogue scale was not significantly different between the two groups. However, there was a significant difference in the interaction between the group and time. There was no significant difference in physiological anxiety measured using systolic blood pressure, diastolic blood pressure and pulse between the two groups. Conclusion: The findings suggest that preoperative video education may contribute to reducing delirium and subjective anxiety for elderly patients after surgery.
