Effects of MBO and Job Commitment on Nursing Performance in Operating Room Nurses (수술실 간호사의 목표관리특성과 직무몰입이 간호업무성과에 미치는 영향)

  • This study aimed to examine the effect of management by objectives(MBO) and job commitment on the nursing performance of operating room(OR) nurses. The study was conducted on 226 OR nurses from tertiary healthcare institutions in Seoul. Multiple regression analysis was performed to determine the effect of OR nurses on nursing performance. In results, the factors that affect the nursing performance of OR nurses included the concreteness of goals, job commitment, and measurability of objectives in that order, and these variables explained 25.9% of the nursing performance. Therefore, in order to improve the performance of the nursing work, it is necessary to make efforts to create a work environment where nursing can be carried out due to the nature of the nursing operation. In addition, it is essential to develop a strategy to further the influence of the concreteness and measurability of goals among other attributes, thereby enhancing the job performance of OR nurses.

  • This study is a qualitative study conducted to understand and explain the operating room practice experience of nursing college students. Participants in this study were 67 nursing college students in 3rd and 4th grade at a university in C-gun, Gyeongsangbuk-do who participated in the practice of operating rooms. The data collection period was from January 14, 2019 to January 13, 2020, and the data collection was carried out through an open self-report-style reflection log, and the collected data was analyzed using the traditional content analysis method of Krippendorff [21]. Analysis of the operating room practice experience of nursing college students resulted in 27 sub-themes, 12 themes and 5 categories. The five categories are "Being seized with complicated feelings," "Being faced with dissection body," "Learning the characteristics of the operating room, " "Being confronted with the limits of clinical practice" and "Self-reflection." The results of this study provided an understanding of the operating room practice experience of nursing students and are expected to be used as basic data to improve the quality of practice of nursing students.

The Effect of Nursing Work Environment on Job Involvement in Operating Room Nurses in small- and medium-sized hospitals: Mediating Effect of Psychological Empowerment (중소병원 수술실 간호사의 간호근무환경이 직무몰입에 미치는 영향: 심리적 임파워먼트의 매개효과)

  • The purpose of this study is to investigate the mediating effect of psychological empowerment (PE) in the relationship between nursing work environment (NWE) and job involvement (JI) of nurses in operating rooms of small- and medium- hospitals. Subjects were 150 nurses in operating rooms of 24 small- and medium- hospitals nationwide, and data were collected from July to September 2018 using a structured questionnaire. Data were analyzed by t-test, ANOVA, Pearson's correlation coefficients and multiple regression. As a result of the study, JI was positively correlated with NWE or PE, PE was positively correlated with NWE. PE was found to partially mediate the effect of NWE on JI. In order to help nurses in small- and medium-sized hospital operating rooms to positively recognize the NWE and immerse themselves in the job, it is necessary to develop convergence program to increase PE.

The Influences of the Awareness of Patient Safety Culture on Safety Care Activities among Operating Room Nurses (수술실 간호사의 환자안전문화에 대한 인식이 안전간호활동에 미치는 영향)

  • Purpose: This study was aimed to provide information on the awareness of patient safety culture and safety care activities among operating room (OR) nurses and to analyze the factors influencing the safety care activities. Methods: For this descriptive research, self-reported questionnaires were administered to 168 OR nurses who were working at the university-affiliated and general hospitals. The collected data were analyzed using descriptive statistics, t-test, ANOVA, $Scheff{\grave{e}}$ test, Pearson's correlation coefficient and Stepwise multiple regression with SPSS/WIN 17.0. Results: The mean score of the awareness on patient safety culture was 3.27 out of 5 points and that of safety care activity was 4.31 out of 5. The statistically significant difference was found between experience of safety education and the awareness on patient safety culture. Also, the scores of safety care activities were significantly different according to OR nurses' position, education levels, and experience of safety education. There was a positive correlation between the awareness of patient safety culture and safety care activity. Their explanatory power on safety care activity was 8.8%, which includes working environment in operating room 3.3% and nursing position 5.5%. Conclusion: Compared to the level of patient safety activities, the OR nurses' awareness on patient safety culture was low. Given the specific characteristics and conditions in each hospital, it needs to increase the OR nurses' awareness on patient safety culture and activities related to patient safety.

The Effects of Web-based Video Program Selection Attributes of Confidence in Nursing Performance-Mediating Effects of Learning Flow (웹 기반 동영상 프로그램 선택속성이 수술실 신규간호사의 간호 수행능력에 미치는 영향 -학습몰입의 매개효과 중심으로-)

  • This study aimed to examine the effect of selected factors on confidence in nursing performance among new operating room nurses, with a focus on the mediating effect of learning flow. Data was collected from July to August 2019 using structured questionnaires. The participants were 250 new operating room nurses from university hospitals located in Incheon and Gyeonggi. The collected data was analyzed using the IBM SPSS Statistics 25.0 and AMOS 24.0. Structural equation modeling was performed to examine the effect of the selected factors namely web-based video programs on confidence in nursing performance, and the mediating effect of learning flow. Results: The factors of 'hygiene & safety' and 'patient care' from the web-based video programs had a positive effect on the learning flow. The higher the learning flow of the nurses, the greater their confidence in nursing performance. Just the 'hygiene & safety' factor significantly increased nursing performance. Conclusion: The study results suggest that new nurses perceive that 'hygiene & safety' is an important factor in building their confidence in nursing performance, even without the learning flow. Therefore, it is necessary to develop web-based video programs based on nurses' needs and to emphasize the importance of postoperative care.

Development of Case-based Multimedia Learning Contents for Preventing Malpractice in Operating Room (수술실 간호오류 예방을 위한 사례중심 멀티미디어 학습콘텐츠 개발)

  • Purpose: This study was conducted to develop case-based self-learning multimedia contents for preventing malpractice that frequently occurred among nurses working in the operating room. Methods: Based on the learning needs of operating room nurses, real case reports, and literature reviews, the case-based multimedia learning contents were developed according to the instructional design procedure. The assessment of learning needs was performed by the combination of surveys using structured questionnaire and of interviews for 40 operating room nurses. Results: The learning contents included four learning modules with real malpractice cases from the areas of operating preparation, nursing skills during operation, environmental management of operating room, and patient safety and observation-related. The 80 minute long case-based multimedia learning contents were finally developed after content validity tests from clinical experts. Each module contained photos, sounds and flash animation with voice recording on the contents of nursing error cases and standardized protocols. Conclusion: The developed multimedia learning contents based on real error cases in this study can be utilized as an educational hands-on training materials for nurses to prevent malpractice in the operating room.

Effects of a Web-based Learning Contents in Operating Room Nursing for Nursing Students (간호학생을 위한 웹기반 수술간호 학습콘텐츠의 효과)

  • This study was conducted to develop a web-based learning contents that assists nursing students' clinical practice on operating room nursing and to examine its learning effects. Based on contents and need analysis, 6 learning modules were developed regarding nursing care in surgical operation and operating equipments. In order to verify the effects of the program, this study employed a quasi-experimental pretest-posttest control group design. Knowledge was significantly higher in experimental group that was enhanced by the web-based learning contents than that of the control group (p<.001). However, there was no significant difference between the two groups in self-directed learning. Experimental group had significantly higher motivation toward learning, which was examined posttest only, compared to control group (p<.001). These results support that the implementation of an web-based learning contents need to be continued as an effective educational material but we need more research to establish how this can be effectively implemented in a students' practicum.

A Survey of Nursing Practice Guidelines for Prevention of Pressure Injury in the Operating Room ([수술실] 욕창예방 간호 실무지침 현황 및 적용실태 조사)

  • This descriptive study examined the practical nursing guidelines for the prevention of pressure injuries (PI) in operating rooms and their application. Method: Four general hospitals and three specialized hospitals located in Seoul, Incheon, and Gyeonggido province were selected through an online randomization program and nurses at these hospitals who participated in operations and had at least six months work experience were surveyed. Data were collected from January to April 2018 through a survey distributed to 150 nurses, 129 of whom returned the survey (86% recovery rate). The results showed that 43 nurses provided PI prevention care for patients in a lateral position (33.3%), 37 for patients in a supine position (28.7%), and 36 for patients in a prone position (27.9%). Gel was most widely used as the supporting surface material (102 nurses; 79.1%), followed by sponge (62 nurses; 48.1%), and cotton (47 nurses; 36.4%). Skin was often inspected twice, before and after the use of a supporting surface (90nurses; 69.8%), but no designated tool was used to determine the risk of PI developing (76 nurses; 58.9%). Additionally, the patient's position during surgery (83 nurses; 64.3%) and the length of the operation (i.e., more than two hours, in this case) (49 nurses; 38.0%) were determining factors for whether to use a supporting surface. The operating room nurses used the gel, sponge, and cotton as the supporting surfaces for the prevention of PI and confirmed that the cutoff operation time for the use of a supporting surface was more than 2 hours.

  • 심한 화상환자를 치료하는 동안이나 피부이식을 하는 동안에 어떻게하면 환자를 따뜻하고 안전하게 보호할 수 있을까? 사용할 용액들, 홑이불, 수술대, 회복실 침대를 따뜻하게 준비해 놓고 에어컨을 끈다. 너풀거리는 꺼즈를 잘 다듬어 놓고 stockinette도 준비한다. 피부이식을 위하여 피부를 떼어낸 부위에도 화상부위와 마찬가지로 질산은 드레싱을 하며 피부에 식염수나 기름종류는 사용하지 말아야 한다.
