수부 봉합 수술 환자, 뇌졸중과 중풍으로 인한 수부 마비 환자, 깁스 후 수부 근육 수축 환자 등 수부 재활 치료가 필요한 환자의 수는 계속해서 증가하고 있지만 그에 비해 재활 치료사의 수는 현저히 부족하다. 또한 수부 재활에 대한 예산 부족으로 환자들의 경제적 부담이 증가하고 있다. 이를 해결하고자 본 논문에서는 모션인식 수부 재활 콘텐츠를 리듬 게임의 형태로 구현하였다. 추후 이 콘텐츠를 통해 수부 재활 치료가 필요한 사람들이 비교적 저렴한 가격으로 지속적인 자가 재활 치료를 진행할 수 있을 것으로 기대된다.

The patients who have illness of hand and upper limb should be received rehabilitation treatment to recover such illness. The rehabilitation treatments is a treatments designed to facilitate the process of recovery from injury, illness, or disease to as normal a condition as possible. This should be done continuously and repeatedly. In this paper, we describe hand-rehabilitation system which provides a treatment method improving and recovering the function of injured hands. Expecially, this system is using a leap motion device which can easily and properly identify and trace a hand motion and provide six treatment patterns for hand rehabilitation. By using this system, the patients can do rehabilitation treatment easily and continuously in their daily life and in result, the achievement of treatment will be improved.

This study is a study to recognize hand movements using a neural network for wrist rehabilitation. The rehabilitation of the hand aims to restore the function of the injured hand to the maximum and enable daily life, occupation, and hobby. It is common for a physical therapist, an occupational therapist, and a security tool maker to form a team and approach a doctor for a hand rehabilitation. However, it is very inefficient economically and temporally to find a place for treatment. In order to solve this problem, in this study, patients directly use smart devices to perform rehabilitation treatment. Using this will be very helpful in terms of cost and time. In this study, a wrist rehabilitation dataset was created by collecting data on 4 types of rehabilitation exercises from 10 persons. Hand gesture recognition was constructed using a neural network. As a result, the accuracy of 93% was obtained, and the usefulness of this system was verified.
Objective: The purpose of this study is to ob serve effects of CPM(Continuous Passive Motion) on hand functional and upper-extremity muscular strength rehabilitation for stroke patients. Method: Objects of this study, three patients have the symptoms of hemiplegia due to stroke. These are acute patients, within a 18 months after treatment and correspond in Brunnstrom stage 4~6. This study used single subject (A-B) design for three patients with a stroke and the effect of CPM was measured using Jebsen-Taylor hand function test, Purdue Pegboard test, hand muscular strength test. They received CPM for 3 weeks, 2 per day, 30 minutes for each, total 30 times. Results: Two patients' results of fingertip grip test in hand strength measurements did not change. Results of Jebsen-Taylor hand function test, Purdue Pegboard test and other hand muscular strength test were improved. To validate statistical results nonparametric statistical method, Wilcoxon signed ranks test was performed. P-Values are greater than 0.05 so difference between be fore and after treatment is not statistically significant result. Conclusion: Despite of limitation of short program period and fewer participants, CPM which has been conducted for stroke patients showed the effect on improvement of hand function and muscle strength. This study shows that CPM which is mainly used to treate lower-extremity rehabilitation can be use to improve performance of hand function and strength for patients with stroke.
Hand rehabilitation training tools are used in hospitals and at home for patients and users who require recovery of disabled hands and improvement in overall hand function. However, existing training tools are not organized into a progressive system, and they lead to repeatability operations over a period of time. As a result, patients feel free and cannot be motivated by rehabilitation, and continuous rehabilitation training is difficult. Based on this argument, the study combines one of the elements of the game called the "Cat's cradle" to enable the user to feel achievement through play and to achieve natural rehabilitation through unconsciousness. After examining the characteristics of the tool, the user's environment, the relevance of the Cat's cradle game to the training tool and to the patient's continued rehabilitation was established. And design elements were derived through professional interviews. Later, design guidelines and prototypes have been created to complement the problems associated with guidelines and prototypes by conducting usability testing and design element assessment.

수부는 일상생활동작(ADL, activities of daily living)을 위한 중요 부위로 손상이나 장애가 있는 경우 재활치료를 통해 과제수행능력을 향상시킨다. 그러나 치료 과정은 주관적으로 진행되어 정확한 치료 성과를 위한 객관적 지표가 필요하다. 본 논문에서는 실제 수부 움직임을 추적할 수 있는 동작인식 장비인 립모션 컨트롤러와 Unity3D로 가상공간 상에서 구현된 Box&Block Test를 이용하여 수부 움직임에 대한 객관적인 데이터를 얻었다. 기존 Box&Block Test와 제안한 모션인식과 추적에 의한 수지손상 환자용 진단 및 치료시스템의 성능을 T검정 수행한 결과 통계적으로 유의미한 차이가 없음을 보였으며 수부 움직임의 정량적인 데이터를 통해 정확한 치료성과 판단의 가능성을 확인하였다.
Purpose: The purposes of this study were to explore changes in pain, mood and the level of posttrauma risk for 2 weeks after acute hand microsurgery and determine predictors of pain 2-week after microsurgery. Method: Using a sample of 84 patients with hand microsurgery, pain, mood, and posttrauma risk were measured by the Brief Pain Inventory-Korea, the modified Profile of mood states, and the Posttrauma risk checklist at 1-day, 1-week and 2-week post-microsurgery. Repeated measures ANOVA and Multiple regression analysis were conducted to evaluate changes in pain, mood, and posttrauma risk over time after the surgery and determine predictors of pain 2-week after the surgery. Results: Pain significantly decreased (F=63.22, p<.001), mood significantly improved (F=41.04, p<.001) 2 weeks after microsurgery and interestingly, posttrauma risk significantly decreased from baseline to 1-week microsurgery but increased at 2-week after microsurgery (F=24.66, p<.001). Approximately 57% of the variance of pain 2-week post-microsurgery was explained by pain at 1-week post-microsurgery, mood and the numbers of injured fingers. Conclusion: The findings suggest the pain control for 1week after acute surgery being critical and posttrauma rehabilitation of injured finger being necessary. Developing nursing interventions is urgently needed to help individuals with impending hand microsurgery.

Unexplained pain and weakness, i.e., without obvious predisposing factors, are often encountered by physiatrists and efforts should be made to determine the cause. A 63-year-old male presented with radiating pain in his right arm and mild weakness of the right hand. An electrodiagnostic examination revealed distal symmetric sensory polyneuropathy in the upper and lower extremities, and denervation potentials in the forearm muscles, which were inconsistent with the cervical spine MRI images and symptoms. A predisposing undiscovered disease was revealed, i.e., squamous cell carcinoma in the lung; brain metastasis affecting the left primary motor cortex was also detected. Therefore, we concluded that the pain and weakness were related to paraneoplastic syndrome and brain metastases of the hand knob. The observed denervation potentials were characterized as trans-synaptic changes in the brain metastasis. This case highlights the importance of unexplainable focal pain and weakness in the increasing prevalence of cancer.

Recurrent motor branch entrapment syndrome is a compressive mononeuropathy of recurrent motor branch of median nerve. It is a rare condition as a cause of thenar muscle wasting and may have different pathogenesis. If such an anatomical variation is the cause, there is a possibility that thenar muscle atrophy remains if only the transcarpal ligament release is performed. We report a 25-year-old male patient with carpal tunnel syndrome with thenar muscle wasting 1 month ago.
There are many wrist and hand diseases in which ultrasound can help in the diagnosis and treatment. Because many small structures are located, identifying anatomical locations and pathways is especially important. In De Quervain's syndrome, it is necessary to find tendon lesions located in the first compartment of the wrist. If injection therapy is required, administer the regimen accurately within the tendon sheath through ultrasound. In carpal tunnel syndrome, there are several methods to diagnose a disease by measuring cross-sectional area of the median nerve. Ultrasound has the advantage of administering injection therapy without damaging the nerve. Intersection syndrome can be diagnosed by observing swelling and hypoechoic appearances at the point where the tendons of the first compartment cross over the second compartment of the wrist. Ultrasound-guided injection is also safe and efficient. If there is a trigger finger lesion, the most representative findings is to observe a nodular hypoechoic thickening of the involved A1 pulley. When injection therapy is performed, it is effective to administer medication between pulley and flexor tendons as much as possible to reduce pressure on the attached structures.
