A Study on Sleep-Wake Assessment for Substantiation of Sleep Products (수면 제품 실증을 위한 수면-각성 평가에 대한 고찰)

  • In this paper, we reviewed domestic and foreign cases and evaluation methods for validation of sleep products for development of the domestic sleep industry. Foreign companies and organizations are trying to verify products relatively systematically for demonstration purposes, but they are using different methods depending on the institution, and standardized validation guidelines have not been established. In Korea, there has been little evaluation including objective verification for sleep products. Sleep-wake evaluation for validation of sleep products requires expert evaluation of the product and of the product effectiveness by users, and subjective and objective sleep-wake evaluations and circadian rhythm evaluation methods can be used. For more accurate verification, experimental designs such as randomization method, control product utilization method, and cross-experiment design can be used.

Sex differences of children's facial expression discrimination based on two-dimensional model of emotion (정서의 이차원모델에서 아동의 얼굴표정 변별에서 성 차이)

  • This study explores children's sex differences of emotion discrimination from facial expressions based on two dimensional model of emotion. The study group consisted of 92 children, of 40, 52, and 64 months of age, and the rate of male and female children was male children (50%) and female children (50%). Children of 92 were required to choose facial expressions related the twelve emotion terms. Facial expressions applied for experiment are used the photographs rated the degree of expression in each of the two dimensions (pleasure-displeasure dimension and arousal-sleep dimension) on a nine-point scale from 54 university students. The experimental findings appeared that the sex differences were distinctly the arousal-sleep dimension than the pleasure-displeasure dimension. In the arousal-sleep dimensionoussleepness, anger, comfort, and loneliness' emotions showed large sex differences over 1 value. Especially, while male children showed high arousal more than female children in the emotions like 'sleepiness, anger and loneliness', female children showed high arousal more than male children in 'comfort' emotion.

  • To control arousal level using physiological index, electrodermal activity signal was detected and separated into skin impedance level and response. Arousal state decision and control algorithm was studied to implement and evaluate real time portable arousal control system. The implemented system can detect and control arousal state from initial drowsy state. This system will be applicable to the evaluation of the effect of warning signal, driver's drowsy detecting system, and sleep study.

The Effects of Modafinil on Clinical Features and Sleep Structure of Narcolepsy Patients and Healthy Controls (모다피닐이 기면병 환자와 대조군의 임상 양상과 수면구조에 구조에 미치는 영향)

  • Introduction: Excessive daytime sleepiness and cataplexy are key features of narcolepsy. Modafinil is psychostimulant used in the treatment of narcolepsy. In this study, we evaluated effects of modafinil on nocturnal sleep structure and sleep latency in multiple sleep latency test and clinical features. Methods: Twelve narcoleptic patients (7 male, age: $22.9{\pm}2.6\;yrs$) were participated in the study. All of them had done nocturnal polysomnography (nPSG), multiple sleep latency test (MSLT), clinical symptoms scales and have repeated same procedure after taking 200 mg of modafinil. We have done linear mixed model analysis to describe effects of group, medication and nap time on these measures. Results: Modafinil did not affect clinical scales except PSQI which had been reduced after medication. In this study, Modafinil reduced total sleep time, sleep efficiency and increased wake after sleep onset and percent of arousal during sleep in nocturnal polysomnography and prolonged mean sleep latency in multiple sleep latency tests in both group. Discussion: Modafinil has stimulant effect of central nervous system but its effect on night sleep is less than other psychostimulants such as methylphenidate. We ascertained that modafinil affected total sleep time, sleep efficiency and percent of wake during sleep but did not effect on sleep structure. Modafinil was effective in the management of day time sleepiness. Modafinil can enhance alertness of control group without day time sleepiness.

  • 본 연구에서는 피부전기반사를 이용하여 각성도를 측정하기 위한 측정 시스템을 구성하고 이를 이용하여 피부 전기활동의 변화가 뇌파를 동시에 측정하여 각성수준의 변화에 따른 세 측정 요소간의 상관관계를 검토하고 측정에 따른 문제점들을 분석하여 각성도 측정 및 평가와 모니터 개발 을 위한 기초적인 연구를 하였다. 분석 결과 기존의 피부임피던스 만을 이용한 수면상태 판정은 불완전하며 피부임피던스 반사도 포함하면 더욱 정확한 판정을 할 수 있슴을 밝혔다.

Relatively Decreased Level of Subjective Daytime Sleepiness and Its Associated Factors in Patients with Primary Insomnia (일차성 불면증 환자에서의 주간 졸림증과 연관된 수면변인에 대한 연구)

  • Objectives: The purpose of this study was to assess the difference of subjective daytime sleepiness level between primary insomnia patients and healthy control subjects. We also investigated the relationship between subjective daytime sleepiness level and variables of nocturnal polysomnograghic sleep architecture of insomnia patients. Method: Total subjects were 87 patients with primary insomnia diagnosed with polysomnography and 88 normal controls. The daytime sleepiness level in each group was measured by Korean version of Epworth Sleepiness Scale (ESS). The correlations of ESS score and nocturnal polysomnographic variables were calculated in the patient group. Results: Patients with insomnia had the lower ESS scores than the control group. In patients group, the ESS score showed significant negative correlations with total sleep time, sleep efficiency%, and stage 2 sleep time%. The ESS score also showed significant positive correlations with number of awakenings, number of awakenings more than 2 minutes, and wake after sleep onset time. Conclusions: Insomnia patients showed lower level of subjective daytime sleepiness that may indicate their higher alertness comparing to control subjects. Daytime sleepiness of patients with insomnia was associated with polysomnographic variables including total sleep time, sleep efficiency%, stage 2 sleep time% and disrupted continuity of nocturnal sleep.

Association of Sleep Perception With Objective and Subjective Sleep Variables in Insomnia Patients (불면증 환자의 수면인식과 객관적, 주관적 수면변수의 연관성에 대한 연구)

  • Objectives: Our study aims to investigate the clinical and polysomnographic variables associated with subjective sleep perception. Methods: Among the patients who underwent nocturnal polysomnography (PSG) at the Center for Sleep and Chronobiology of Seoul National University Hospital from May 2018 to July 2019, 109 diagnosed with insomnia disorder based on DSM-5 were recruited for the study, and their medical records were retrospectively analyzed. Self-report questionnaires about clinical characteristics including Pittsburgh sleep quality index (PSQI), Beck depression inventory (BDI), and Epworth sleepiness scale (ESS) were completed. Subjective sleep quality was measured using variables of subjective total sleep time (subjective TST), subjective sleep onset latency (subjective SOL), subjective number of awakenings, morning feeling after awakening, and sleep discrepancy (subjective TST-objective TST) the morning after PSG. Pearson and Spearman correlation analyses were used to determine the factors associated with subjective sleep perception. Results: In patients with insomnia, subjective TST was negatively correlated with Wake After Sleep Onset (WASO) (p = 0.001) and N1 sleep (p = 0.039) parameters on polysomnography. Also, it was negatively correlated with PSQI (p < 0.001) and BDI (p = 0.014) scores. Sleep discrepancy was negatively correlated with PSQI score (p = 0.018). Morning feeling was negatively correlated with PSQI (p = 0.019) and BDI (p < 0.001) scores. Conclusion: Our results demonstrated that subjective sleep perception is associated with PSG variables (WASO and N1 sleep) and with PSQI and BDI scores. In clinical practice, it is helpful to assess and manage insomnia patients in consideration of objective sleep variables, subjective sleep quality, and depressed mood, which can influence subjective sleep perception.

  • 수면단계는 수면감성을 평가하는데 있어서 중요한 생리지표로서 사용되어왔다. 그러나 수면다원검사를 이용한 전통적 수면단계 분류방법은 뇌전도, 안전도, 심전도, 근전도 등을 종합적으로 측정하므로 수면단계를 비교적 정확히 분류할 수 있지만 피험자에게 심한 구속감을 주는 문제가 있다. 본 연구에서는, 각성상태에서 교감신경계가 지배적인 반면에 수면 중에는 부교감신경계가 더 활동적인 점에 착안하여 수면단계를 간단히 분류할 수 있는 방법을 찾고자 수면단계에 따른 심박동변이도(heart rate variability: HRV)를 분석하였다. 단일채널 심전도를 이용하여 수면단계별로 HRV 의 교감신경계/부교감신경계 활성도의 비율을 분석한 결과, W(wakefulness) 단계가 NREN(non REM) 2 단계, 3 단계, 4 단계에 비하여 높게 나타났으며, NREM 4 단계는 REM(rapid eye movement) 단계와 NREM 1단계에 비하여 낮게 나타났다. 또한 교감신경계/부교감신경계 활성도 비율의 수면단계에 따라 변화하는 양상은 W, REM, NREM 1, 2, 3, 4 단계의 순으로 단조 감소하였다.

The Effects of Circadian Rhythm in Subjective Alertness on the Occurrence of Traffic Accidents (주관적 각성도의 일주기(日週期) 리듬이 교통사고 발생에 미치는 영향)

  • In order to examine the effects of circadian rhythm in subjective alertness on the occurrence of traffic accidents, the authors investigated the occurrence rate of traffic accidents adjusted to traffic volume in Seoul and the relative rate of fatal accidents versus total traffic accidents in Korea at different times of day in 1991. We analyzed these data in relation with the circadian rhythm in subjective alertness. The results were as follows. Both the occurrence rate of traffic accidents adjusted to traffic volume and the relative rate of fatal accidents versus total traffic accidents were the highest at 3-4 a.m., known as the time period of the lowest subjective alertness. They were negatively correlated with subjective alertness (p<0.05, p<0.001). In conclusion, it is suggested that the circadian rhythm of subjective alertness should have a significant effect on the occurrence of traffic accidents despite many confounding factors such as driving environments.

Performance evaluation of sleep stage classifier for the sleep-inducing portable neurofeedback system (포터블 수면유도 뉴로피드백 시스템 구현을 위한 수면뇌파 상태 분류기 성능 평가)

  • Recently, many people have suffered from insomnia, labor loss, cognitive decline, and mental illness. The solution to this problem is almost entirely cognitive therapy or medication, but it is not recommended in the long term due to side effects and dependency problems. Therefore, in this paper, we propose a neuro feedback system based on portable EEG that helps induce sleeping. We design and evaluate the EEG classifier, which is the most important function to implement the system, and propose an optimized classifier modeling method for various factors that can affect performance. When using the proposed classifier, we could distinguish 97.9% of awakening and sleep phase in portable EEG.
