Anti-Stress Effect of Punica granatum L. Extract against Sleep Deprivation-Induced Impairment (석류 열수 추출물의 수면박탈을 유도한 Rat 모델에서의 항스트레스 효과)

  • The anti-stress effects of Punica granatum L. (family Lythraceae, PG) on $H_2O_2$/corticosterone (CORT)-induced stress in cells and sleep-deprived rats were investigated. The PG extract showed neuroprotective effects in SH-SY5Y cells against $H_2O_2$/CORT-induced stress. Sleep deprivation led to behavioral, hormonal, and biochemical alterations in the animal model. The effects of P. granatum on physiological, behavioral, and biochemical parameters aggravated by sleep deprivation were investigated. Sleep deprivation impaired physiological (survival, body weight, and drowsiness scores) and behavioral (rotarod, passive avoidance, hot hyperalgesia, and Y maze) parameters as well as biochemical factors (cortisol, serotonin, dopamine, testosterone, and growth factor I contents in serum). These parameters were significantly recovered by PG extract in a concentration-dependent manner. The PG extract also enhanced catalase, superoxide dismutase, and non-enzymatic antioxidative activities such as glutathione compared to sleep-deprived rats. On the basis of these results, our findings suggest that Punica granatum prevents impairment of body functions induced by sleep deprivation and related oxidative damage.

Health Risk Behavior and Oral Symptoms in Adolescents in Multicultural Families (다문화 가정 청소년의 건강 위험 행동과 구강 증상에 관한 연구)

  • This study used the raw data of the 15th (2019) Korean Youth Health Risk Behavior Survey to investigate the risk factors of the health-risk behaviors of Korean multicultural adolescents on the subjective oral symptoms. The scope of the multicultural family was limited to children of immigrant women's families married to Korean men and immigrant male families married to Korean women, and 572 were selected as the final study subjects. The sociodemographic variables were surveyed as gender, grade, economic level, and school performance. The health risk behaviors included drinking, smoking, medication, stress, sleep satisfaction, suicidal thoughts, depression, and brushing before bedtime. The oral symptoms examined were toothache, gum pain, and halitosis. As a result, toothache was related to drinking, toothbrushing before sleep, stress, sleep satisfaction, suicidal thoughts, and depression. Gum pain was associated with drug use, sleep satisfaction, suicidal thoughts, and depression. Halitosis was associated with medication, brushing before sleep, stress, suicidal thoughts, and depression. In terms of health risk factors, drinking, stress, and sleep satisfaction were risk factors for toothache. Smoking, sleep satisfaction, and suicidal thoughts were risk factors for gum pain. The presence of toothbrushing and stress were risk factors for halitosis. In conclusion, a policy system is needed to support oral health education programs nationally at times, such as after-school activities or club activities, to promote oral health for teenagers.

Classification of Behavioral Patterns Associated with Sleeping in Residential Space (주거공간에서 수면 전후의 행동유형 분류)

  • In this paper, we try to classify behavior patterns of a person around a bed based on a wireless sensor network system. We define five behavioral patterns and three states of a person around a bed which is described by a state machine. We collected data sensed by motion detection and vibration sensors installed around a bed from which a feature vector was extracted. Based on feature vector corresponding to behavioral patterns and the state machine, we established a model for behavioral patterns. To validate the model, experiments on subjects were performed and the model was fixed. These experimental results revealed that behavior patterns of a person around a bed can be classified well.

Effects of Food Behavior on Social Competence and Hyperactivity of Preschoolers (취학 전 아동의 식생활 행동이 사회적 능력 및 과잉 행동에 미치는 영향)

  • The purpose of this study was to examine the relationship between food behavior and social competence and hyperactivity among 330 children aged 5-6 years. Social competence and hyperactivity of the children were measured simultaneously by both children´s mothers and their teachers using the same checklists. A significant difference was noted between the teachers´ and the mothers´ assessment upon the association of food behavior with both social competence and hyperactivity (p<0.05). Health status was closely related to only the teacher´s food behavior assessment(p<0.05). Children´s social competence was significantly related with the degree of children´s activity and the degree of hyperactivity was negatively related with their sleep status(p<0.05). The higher the teachers´ assessments on the children´s food behavior were, the higher the children´s social competences were(p<0.05). However, this positive relationship between food behavior and social competence were not observed by the mothers´ assessment. These results indicate that food behavior assessed by the teachers, not by mothers, plays a role in children´s social competence and hyperactivity.

Clinical Characteristic and Respiratory Disturbance Index as Correlates of Sleep Architecture in Obstructive Sleep Apnea Syndromes Diagnosed with Polysomnography (수면다원기록법으로 확진된 폐쇄성 수면무호흡증 환자의 임상특성, 그리고 호흡장애지수와 수면 구조간의 상관관계)

  • Objectives: Obstructive sleep apnea syndrome is common and may produce various symptoms and serious complications. A substantial number of research articles on obstructive sleep apnea syndrome have been published in Korea. However, we found such limitations as lack of sufficient sample size and lack of polysomnography-proven cases. Therefore, we aimed at studying clinical features and sleep structure in a sufficient number of Korean patients with obstructive sleep apnea syndrome diagnostically confirmed with polysomnography. Methods: We studied 801 subjects referred to the Division of Sleep Studies, Seoul National University Hospital, who were diagnosed as having obstructive sleep apnea syndrome with polysomnography. Subjects were excluded if they had central sleep apnea syndrome, periodic limb movement disorder, narcolepsy or REM sleep behavior disorder. Foreign patients were also excluded. First of all, we studied the clinical features of the subjects. Secondly, we compared sleep-related parameters of the study subjects with those of age/sex-matched normal values. Thirdly, correlations of respiratory disturbance index (RDI) with each of the sleep-related parameters were calculated. Results: Among the 801 subjects, 668 were male subjects (83.4%) and 133 female subjects (16.4%). Their mean age was 46.6 years (${\pm}13.5$). The mean body mass index (BMI) was 25.8 (${\pm}3.8$) and subjects with BMI was over 28.0 accounted for 22.8% of the total. Fifty subjects (6.2%) were found to take benzodiazepines. Mean RDI and mean nocturnal oxygen saturation of all subjects was 31.2 (${\pm}24.4$) and 94.5% (${\pm}3.6$), respectively. In comparison with normal values, the subjects showed longer sleep latency, lower sleep efficiency, decreased total slow wave sleep % (TSWS %), and decreased total REM sleep % (TREM %)(p<0.01 in all). RDI had a negative correlation with each TSWS % and TREM % (p<0.01, p<0.01). However, RDI did not have significant correlation with either sleep latency or sleep efficiency. Conclusion: In this study, 6.2% of patients diagnosed as having obstructive sleep apnea syndrome were found to take benzodiazepines, although they are generally considered to be of litte benefit or even dangerous because of the respiratory suppressing effect. The proportion of obese subjects was only 22.8% and Korean patients with obstructive sleep apnea syndrome seem to be less obese than those described in foreign journals. This study also suggests that the severity of obstructive sleep apnea syndrome may have a more significant effect on sleep architecture defined as TSWS % and TREM % than on sleep efficiency.

Analysis of Sleep Questionnaires of Patients who Performed Overnight Polysomnography at the University Hospital (한 대학병원에서 철야 수면다원검사를 시행한 환자들의 수면설문조사 결과 분석)

  • Background : The objective of this study was to understand sleep-related problems, and to determine whether the sleep questionnaires is a clinically useful method in patients who need polysomnography. Methods : Subjects were patients who performed polysomnography and who asked to answer a sleep questionnaires at the Sleep Disorders Clinic of St. Paul's Hospital, Catholic University of Korea. Baseline characteristics, past medical illness, behaviors during sleep-wake cycle, snoring, sleep-disordered breathing and symptoms of daytime sleepiness were analyzed to compare with data of polysomnography. Results : The study population included 1081 patients(849 men, 232 female), and their mean age was $44.2{\pm}12.8years$. Among these patients, 38.9% had an apnea-hypopnea index(AHI)<5, 27.9% had $5{\leq}AHI<20$, 13.2% had $20{\leq}AHI<40$, and 20.0% had $40{\leq}AHI$. The main problems for visiting our clinic were snoring(91.7%), sleep apnea(74.5%), excessive daytime sleepiness(8.0%), insomnia(4.3%), bruxism(1.1%) and attention deficit(0.5%). The mean value of frequency of interruptions of sleep was 1.6 and the most common reason was urination(46.3%). Epworth Sleepiness Scale(ESS) had a weak correlation with AHI(r=0.209, p<0.01). When we performed analysis of sleep questionnaires, there were significant differences in the mean values of AHI according to the severity of symptoms including snoring, daytime sleepiness, taking a nap and arousal state after wake(p<0.05). Conclusion : On the basis of statistical analysis of sleep questionnaires, the severity of subjective symptoms such as ESS, snoring, daytime sleepiness and arousal state after wake correlated with the AHI significantly. Therefore the sleep questionnaires can be useful instruments for prediction of the severity of sleep disorder, especially sleep-disordered breathing.

Prevalence and Associated Factors of Snoring in School-Aged Children (학동기 아동에서 코골이의 유병률과 관련인자)

  • Purpose : We studied the prevalence of snoring and its association with diseases, obesity and environmental factors, as well as sleep disturbance arising from snoring, in school-aged children. Methods : The survey was performed by a special questionnaire on 1,707 children at elementary schools from 1 to 30 July 2001 in Gwangju City. The prevalence of snoring, associated factors, and sleep disturbance were evaluated. Results : 266(16.5%) of the children snored at least once a week. 73(4.3%) of the children snored almost every day. Regarding the associated factors of snoring : There was a statistically significant difference between snoring and nonsnoring groups due to gender, obesity, sinusitis, tonsillar hypertrophy, bronchial asthma and allergic rhinitis. Of them, male gender, tonsillar hypertrophy, and obesity were significant risk factors for development of snoring. Especially, the odds ratio increased to 1.048(CI, 1.004-1.659), 1.748(CI, 1.175-2.599), and 2.266(CI, 1.300-3.950) in children with an obesity index of 20-29%, 30-49%, ${\geq}50%$, respectively. There was a statistically significant association of snoring with decreased sleep duration, sleep talking, and drowsiness during the day as a result of sleep disturbance. Conclusion : 16.5% of school-aged children snored at least once a week. The association of snoring with tonsillar hypertrophy, obesity and gender as risk factors was demonstrated in this study.

Relationship among Cognition, Sleep Patterns, Salivary Melatonin Level and Sleep Disorder Inventory of Older Adults in Nursing Homes (요양시설 입소 노인에서 인지, 수면양상, 타액 멜라토닌 농도 및 수면장애행동의 관련성)

  • Purpose: The purpose of this study was to investigate sleep quality in older adults in nursing home with objective data collection. Methods: Participants included 74 older adults in nursing homes in Korea aged 65 years or above. Data were collected using a wearable device (Fitbit), salivary melatonin level and Sleep Disorder Inventory (SDI). The Pearson correlation coefficient was calculated to examine whether there was any correlation between sleep-related variables such as Total Sleep Time (TST), Rapid Eye Movement (REM) sleep, shallow sleep, deep sleep, salivary melatonin level and SDI. Results: There were distortion of sleep structure, as TST comprised short REM sleep (15.93±5.47%), long shallow sleep (74.18±8.08%) and short deep sleep (9.89±5.03%). Also, salivary melatonin levels were low (15.06±7.77 pg/mL). Moreover, we found than melatonin was significantly associated with TST (r = .251, p= .044), REM sleep (r= .294, p= .020) and deep sleep (r= .391, p= .002). But there was no correlation between SDI and other sleeprelated variables. Conclusion: These findings highlight that insufficient sleep structure is associated with the salivary melatonin level among older adults in nursing home. We suggest developing programs to promote sleep quality of older adults in nursing homes.

Sleep Disordered Breathing in Children (어린이의 수면호흡장애)

  • Sleep disordered breathing (SDB) is a disease characterized by repeated hypopnea and apnea during sleep due to complete or partial obstruction of upper airway. The prevalence of pediatric SDB is approximately 12 - 15%, and the most common age group is preschool children aged 3 - 5 years. Children show more varied presentations, from snoring and frequent arousals to enuresis and hyperactivity. The main cause of pediatric SDB is obstruction of the upper airway related to enlarged tonsils and adenoids. If SDB is left untreated, it can cause complications such as learning difficulties, cognitive impairment, behavioral problems, cardiovascular disease, metabolic syndrome, and poor growth. Pediatric dentists are in a special position to identify children at risk for SDB. Pediatric dentists recognize clinical features related to SDB, and they should screen for SDB by using the pediatric sleep questionnaire (PSQ), lateral cephalometry radiograph, and portable sleep monitoring test and refer to sleep specialists. As a therapeutic approach, maxillary arch expansion treatment, mandible advancement device, and lingual frenectomy can be performed. Pediatric dentists should recognize that prolonged mouth breathing, lower tongue posture, and ankyloglossia can cause abnormal facial skeletal growth patterns and sleep problems. Pediatric dentists should be able to prevent these problems through early intervention.

Relationship Between the Psychosocial Factors and Job Stress Among Clerical Public Officers (일부 공무원들의 사회심리적 요인과 직무스트레스와의 관련성)

  • This study was conducted to determine the levels of job stress, as well as its association with psychosocial factors (type A behavior pattern, locus of control, and self-esteem). Self-administered questionnaires were given to 634 clerical public officers in the C Provincial Office between February 18 and Mar. 10, 2013. As a result, the job stress levels of the subjects was significantly different according to age, sex, educational level, marital status, job position, job career, satisfaction in job life, smoking, alcohol drinking, sleeping time, type A behavior pattern, locus of control, and self-esteem. Regarding the correlation of job stress and psychosocial factors, the level of job demand was negatively correlated with the internal locus of control. The decision latitude was negatively correlated with the type A behavior pattern, internal locus of control and self-esteem. Supervisor support was negatively correlated with the external locus of control. In hierarchial multiple regression analysis, the affecting factors to the job stress selected variables, such as age, subjective health status, job position, overtime work, experience of sick absence, satisfaction in work, sleeping time, leisure time, visiting out-patient department, type A behavior pattern, locus of control, and self-esteem, and the explanatory powers of these factors was 32.1%. In particular, the factors related to the psychosocial factors (type A behavior pattern, locus of control, self-esteem) were strongly related to the job stress, increasing the explanation of factors up to 15.0%.
