Two cases of varicella zoster meningitis in immunocompetent children (면역 기능이 정상인 소아에서의 대상포진 수막염 2례)

  • Varicella-zoster virus is a human herpesvirus that causes chickenpox (varicella), becomes latent in cranial nerve and dorsal root ganglia, and frequently reactivates to produce shingles (zoster) and postherpetic neuralgia. Varicella zoster meningitis is a rare complication after varicella zoster infection. It usually affects a patient of immunocompromised or impaired cellular immunity, is rare in a immunocompetent child. We report two cases of aseptic meningitis in association with varicella zoster, not having any complication in the immunocompetent children.

  • Herpes zoster is caused by the varicella-zoster virus (VZV), which affects nerve ganglions. VZV infection may be associated with neurologic complications, which are usually observed after vesicular exanthem. Acute aseptic meningitis is a rare complication of VZV reactivation. We report the case of a previously healthy 14-year-old boy who suffered from aseptic meningitis that was attributed to reactivated VZV infection with exanthem; the patient had undergone vaccination against varicella. This condition can be confirmed by the detection of VZV DNA in the cerebrospinal fluid. The patient was treated with acyclovir and recovered fully.

  • The electrochemical approach to the characteristics of black chrome solar selective coatings has been reviewed. Based on the formula of the ChromOnyx bath solution manufactured by Harshaw Company, the chromic acid-propionic acid bath solutions have been prepared by KIER. Comprehensive review of the black chrome electrodeposition mechanism also has been performed on the role played by constituents in the bath solution during electrodeposition. The optical properties such as absorption and emittance of electrodeposited black chrome coatings obtained from the new formula and their surface analysis will be reported in detail.

  • A 13-year-old, neutered male, chihuahua was referred to the Veterinary Medical Teaching Hospital of Chungnam National University with a history of a bilateral mandibular mass, intermittent vomiting, cough, and left sided staggering starting ten days prior. Clinical examination along with radiography, neurologic examination and magnetic resonance imaging (MRI) demonstrated a well-circumscribed and solitary mass in the brain stem. The dura mater was thickened with marked linear enhancement after contrast administration. Based on diagnostic image analysis and histopathologic examination, this case was diagnosed as intracranial meningioma. The patient's symptoms were controlled for 18 months by a combination therapy of prednisolone and lomustine (CCNU), and then he died of at home.

  • 암반 내에 공동을 굴착하여 LPG 혹은 원유를 저장하는 경우 공동에서의 지하수 유입량은 공동상부의 수압과 공동내의 가스압과의 관계를 파악할 수 있는 정량적인 지표가 된다. 공동내의 유입량은 되도록 일정하게 유지되는 것이 굴착등의 시공단계와 공동 운영 및 유지관리면에서 유리하며, 유입량의 급증 혹은 급감이 일어나는 경우는 그 원인을 조기에 규명하여야 한다. 이를 위해서는 지하수위, 가스저장압, 수막공 주입압 등에 따른 공동주변의 유동장 해석, 공동내로의 지하수 유입량 해석을 실시해야 한다. 지하저장공동의 유입량 해석에 있어서는 공동의 정확한 형상을 반영하기 위해서 유한요소법이 보편적으로 사유되어 왔으나 한번 설정한 유한요소망으로부터 공동의 설계요소를 변경하는 작업은 수원하지 않아 설계전단계에서 공동 및 수막 시설의 다양한 배치에 따른 모의를 수행하는데는 다소 무리가 있다. 이러한 불편함은 경계부의 형상과 조건만으로 내부점에서의 미지변수 계산을 효과적으로 수행할 수 있는 경계요소법을 도입함으로써 극복할 수 있다. 따라서 본 연구에서는 지하공동으로 배수되는 유입량 산정을 위해 경계요소법을 근간으로 한 2차원 지하수 흐름모형을 구성하였고, 이를 지하저장공동이 위치한 A기지에 적용하여 상부경계조건인 지하수위의 변화, 수막공 주입압 등에 따른 공동내의 유입량과 공동저장압과의 관계를 정량적으로 분석하였다. 분석 결과를 지하저장공동의 운영 및 유지관리에 활용될 수 있도록 수식화하여 제시하였다.

  • Bacillus cereus causes serious central nervous system infections, especially in immunocompromised patients. Successful treatment requires adequate antimicrobial concentrations in the cerebrospinal fluid; however, in some cases, achieving this with systemic treatment alone is difficult. We treated intractable B. cereus ventriculitis with intraventricular vancomycin, with no major adverse events.

  • Meningococcal infections can be associated with abnormalities of the complement system, which contains 5 terminal complement proteins. Furthermore, deficiencies in 1 of these 5, complement component 7 (C7), leads to the loss of complement lytic function, and affected patients show increased susceptibility to recurrent meningococcal meningitis and systemic Neisseria gonorrhoeae infection. In September 2003, an 11-year-old female patient presented at our outpatient department with high fever, lower leg pain, headache, and petechiaes. She rapidly progressed to coma but later achieved full recovery due to prompt treatment. Her final diagnosis was meningococcal sepsis and arthritis. Her elder brother also had a similar bacterial meningoencephalitis history, which encouraged us to perform analyses for complement component and gene mutations. Resultantly, both the brother and sister were found to have the same mutation in the C7 gene. Subsequently, vaccinations of the meningococcal vaccine meningococcal vaccine ($Menomune^{(R)}$) were administered. However, in September 2006, the brother expired due to acute micrococcus meningoencephalitis. At present, the 16-year-old female patient is healthy. Here, we report a Korean family with a hereditary C7 deficiency with susceptibility to meningococcal infections due to C7 gene mutation.

Fire Experiment for a Water Curtain Nozzle in a Double Glazed Curtain Wall System (유리커튼월 수막시스템 분무헤드의 화재 실험 연구)

  • Recently, a curtain wall system was applied to skyscrapers to prevent fire spreading along the vertical direction due to stack effect and outside air flow when a fire breaks out in skyscraper. Conventional sprinklers cannot prevent the spread of fire to the upstairs areas, which increases the loss of life and property. In this study, a water curtain nozzle that creates wider water coverage on the surface of glass and can prevent the spread of fire to the upstairs areas was developed. The spray pattern of the water curtain nozzle was compared with that of a sprinkler to determine performance of the water curtain nozzle. A fire experiment was constructed on the second floor to verify performance of the water curtain nozzle in preventing the spread of fire to the upstairs areas. The fire and watering experiment proved that the water curtain nozzle prevented more effectively the spread of fire than the conventional sprinkler. The position of first crack and critical temperature of fracture were also analyzed using numerical analysis.

Recurrence and Extraneural Metastasis in 31 Meningeal Hemangiopericytomas (31예 수막 혈관외피세포종에 있어서의 재발 및 신경계외 전이)

  • Purpose : Meningeal hemangiopericytoma(M-HPC), characterized by a high local recurrency and metastatic potential, is a rare neoplasm arising from perivascular pericytes. A retrospective study was performed to identify the recurrence and extraneural metastasis in M-HPC. Materials and Methods : We reviewed the records of 31 M-HPC patients treated from 1982 through 1999 at our institution. The time to recurrence and the various parameters affecting recurrence were determined. Extreneural metastasis was also analyzed. Results : The rate of local recurrency was 38.7%(12/31). The overall average recurrence-free period(RFP) before the first recurrence was 104 months, with overall recurrence-free rates(RFRs) at 5 and 10 years after first surgery of 59.2% and 33.6%, respectively. Of the 12 patients who experienced local recurrence, 4 had recurrences 5 years later after the first surgery. Complete excision at the first operation significantly extended the average time before first recurrence from 43 to 111 months. The 5-year RFRs for the groups of complete excision and incomplete excision were 72.7% and 20.8%, respectively(p=0.0060). Although there was no statistical significance, complete excision followed by adjuvant radiotherapy of more than 50Gy extended the RFP. The 5-year RFRs for the groups of complete excision and complete excision with adjuvant radiotherapy were 70.3% and 100%, respectively(p=0.3359). Four patients(12.9%) presented one or more extraneural metastases that were developed at an average of 107 months after the first operation with the 5- and 10-year metastasis rates of 4.4% and 24.9%, respectively. Conclusions : M-HPC has a propensity to recur either locally or at distant sites after surgical resection. Complete excision is the most important factor to reduce recurrence. However, even with complete excision, adjuvant radiotherapy of more than 50Gy significantly reduces the risk of recurrence. Local and distant recurrences may occur after a prolonged disease-free interval, emphasizing the need for long-term follow-up.

Bacterial Meningitis in Children in One Tertiary Hospital (소아의 세균성 수막염)

  • Purpose : We performed a retrospective study on bacterial meningitis in children, pertaining to the causative organisms, bacterial resistance to antibiotics and the effect of recent introduction of Haemophilus influenzae type b(Hib) vaccine. Methods : We analysed the forty-three cases of bacterial meningitis which had been treated at the Inha University Hospital from June 1996 to June 2003. Results : Nineteen cases(44.2%) of them were infants younger than 2 months of age, and 29 cases(67.5%) younger than 1 year of age. The common causative organisms under 2 months of age were group B streptococcus(GBS)(47.4%), E. coli(21.1%), and Klebsiella pneumoniae(21.1%). In the age group beyond 2 months of age, S. pneumoniae were seen in 50 %, H. influenzae in 16.7% and N. meningitidis in 16.7%. All of the five cases of Hib meningitis had not been vaccinated for Hib. There has been no Hib meningitis cases since 2001. Overall fatality rate was 4.5%, and complication occurred in 39%. Complications was significantly less frequent in patients resumed to be treated within 48 hours after onset compared to after 48 hours after onset. Penicillin resistance of S. pneumoniae and GBS isolated among bacterial meningitis cases was high. Conclusion : Timing of treatment after the onset of the disease appeared the most important factor for prognosis of bacterial meningitis. The cases of H. influenzae meningitis have decreased probably due to Hib vaccination.
