Usefulness of DFOV Changes in Pediatric PET/CT Image Reconstruction (PET/CT에서 소아환자 영상 재구성 시DFOV 변화의 유용성)

  • Purpose: There have been something difficulties in locating focuses and quantitative analysis in case of pediatric patients because of the relatively small body compared to adults. This author of this study, therefore, evaluated the usefulness of DFOV (Display Field Of View) according to its changes in PET/CT image reconstruction by means of the phantom experiment and pediatric patients examination. Materials & Methods: 0.023 MBq/cc of $^{18}F$-FDG was put into the uniform NU2-94 phantom, and then emission scan was acquired for 10 minutes. For reconstruction, DFOV values were changed to 50, 45, 40, 35, 30, and 25 cm respectively. As for patient images, 20 patients who were diagnosed as the one or suspicion of the children tumor are targeted from Oct 2007 to Jan 2008. For image reconstruction, 50 cm was the basis of DFOV, and the value was adjusted to DFOV 45 cm to 25 cm respectively. In the phantom and the reconstruction image of pediatric patients, the changes in pixel size and $SUV_{max}$ according to DFOV changes were analyzed. Results: As DFOV decreased to 50, 45, 40, 35, 30, and 25 cm by means of the phantom, the pixel size was changed to 3.906, 3.515, 3.125, 2.734, 2.343, and 1.953 mm respectively. Besides, as a result of reconstruction DFOV in images of pediatric patients to 50, to 25 cm, the different values of $SUV_{max}$ are shown as 3.3, 7.3, 12, 14, 18% and 2.6, 4.3, 5.0, 7.0, 10.0% on respectively when 50 cm was the standard. Conclusion: In $SUV_{max}$ using the phantom, as DFOV decreased every 5 cm, the mean value gradually increased. With 50 cm as the standard, the increase rates were 3.7, 6.5, 11.2, 19.5, and 32.1% respectively. As for pediatric patients image too, as DFOV decreased, the rates increased as in the phantom experiment. In image reconstruction, since DFOV decrease regardless of matrix size change reduced the pixel size, the image quality can be improved. This would be more useful than reconstruction and enlarge images of pediatric patients in the same way of examining adults. However, when the value of 35 cm DFOV was applied, this may result in truncated artifact, and thus the application should be properly controlled. Change of DFOV may produce better image for pediatric patients, but changes of SUV values according to DFOV change should be considered in reading.

  • The purpose of this study was to understand the change of demands in young patients and to suggest a directional shift for the pediatric dental practitioners who are facing these changes. This study collected data on new patients who had visited Chonnam National University Dental Hospital, Department of Pediatric Dentistry from 2003 to 2010 and the researchers were able to find out the following information. Total number of dental visits has been increasing since 2009 and 2010, which includes an increase of male patients in the 13~15 years age group showed significant increase and that in the 3~4 years age group showed a significant decrease. December showed the largest number of total dental visits, of which 69.6% of newly visit patients were living in Gwang-ju. Patients who had systemic diseases were 10.7% of total newly visit patients and 19.1% of patients received sedative treatment. Considering the chief complaints of new patients, 36% of patients visited because of dental caries. Regarding the dental treatment of patients, 38.4% had restoration treatment in 2003 and 30.4% had restoration treatment in 2010.

Impact of Influenza Infection on Febrile Seizures: Clinical Implications (인플루엔자 감염과 연관된 열성경련의 임상적 특징)

  • Purpose: Febrile seizures (FSs) are the most common type of seizure in the first 5 years of life and are frequently associated with viral infections. Influenza infection is associated with a variety of neurological conditions, including FSs. The purpose of this study was to evaluate the clinical implications of influenza infection in FSs. Methods: In total, 388 children with FS were divided into two groups: FS with influenza infection (n=75) and FSs without influenza infection (n=313). Their medical records, including seizure type, frequency, duration, and familial history of FSs or epilepsy, were retrospectively reviewed and the clinical characteristics of the two groups were compared. Results: In total, 75 of the 388 children (19.3%) had FSs associated with influenza infection; such children were significantly older than those with FSs without influenza infection ($34.9{\pm}22.3$ months vs. $24.4{\pm}14.2$ months; P<0.001). The children who had more than two febrile seizures episodes were more prevalent in children with FS with influenza infection [40/75 (53.3%) vs. 92/313 (29.4%); P<0.01]. Children older than 60 months were more likely to have influenza infection compared to those aged less than 60 months [11/22 (50%) vs. 64/366 (17.5%); P=0.001]. Conclusion: Influenza infection may be associated with FSs in older children, and with recurrence of FSs. Its role in the development of afebrile seizures or subsequent epilepsy requires further investigation with long-term follow-up.

AN EPIDEMIOLOGIC STUDY ON THE PEDIATRIC PATIENTS IN DEPARTMENT-OF PEDIATRIC DENTISTRY, CHOSUN UNIVERSITY DENTAL HOSPITAL FOR LAST 10 YEARS(1990-1999) (최근 10년간 조선대학교 치과병원 소아치과에 내원한 신환에 관한 역학적 연구(1990-1999))

  • The purpose of this study was to make public oral health plan more effective and the improvement of the hospital management system for better clinical dental practice. Distribution and trends were examined in all patients who had been examined and diagnosed at Department of Pedodontics, Dental Hospital, Chosun University over 10 year-period from 1990 to 1999 Results were as follows, 1. The number of patients per year was increasing trend after 1996 and higher visiting rate in male(55.9%) than in female(44.1%). 2. Age distribution had shown $3\sim4$ year-old cup being the largest (23.4%) and each percentage of $5\sim6,\;7\sim8,\;0\sim2,\;9\sim10,\;11\sim12,\;13\sim14,\;above\sim15$ year-old group was 19.9%, 17.7%, 14.6%, 12.6%, 8.0%, 3.3%. 0.5%. 3. Geographic distribution showed a majority of patients in Kwang-Ju (83.0%). Group in the suburbs of Kwang-Ju(Jang-Sung group) was 5.4%. 4. Dental caries showed the highest percentage(40.5%) in chief complaints and percentage of oral exam, orthodontic problem, oral pain, trauma, supernumerary teeth, swelling was 13.9% 12.6%, 8.8%, 7.4%, 5.5%, 4.9%. 5. In all patients with traumatic problem, crown fracture showed the highest percentage(41.4%) and percentage of subluxation was 19.4%. 6. Majority(78.7%) of patients were not refered, and percentage of patients refered from local clinic was 20.5%. 7. Patients who had periodic check-up comprised 19.6%, and percentage of after 3, 6, 9, 12, 18, 24month was 36.7%, 22.2%, 13.5%, 11.3%, 5.4%, 11.0%.

Childhood Tuberculosis Contact Investigation and Treatment of Latent Tuberculosis Infection: a Single Center Study, 2014-2017 (소아청소년 결핵 접촉자 검진 및 잠복결핵감염의 치료 현황: 2014-2017 단일 기관 연구)

  • Purpose: In order to prevent tuberculosis transmission early, it is important to diagnose and treat tuberculosis infection by investigating people who have contact with patients with active tuberculosis. Methods: From July 2014 to June 2017, the intrafamilial childhood contacts of the patients who were diagnosed with active tuberculosis at Chungnam National University Hospital were investigated for the presence of tuberculosis infection. We also retrospectively analyzed the treatment status of children treated with latent tuberculosis infection (LTBI) during the same period. Results: Among the 269 children who had intrafamilial contact with active tuberculosis patient, 20 (7.4%) did not receive any screening. At the first screening, one (0.4%) was diagnosed with pulmonary tuberculosis, seven (2.8%) had a previous history of tuberculosis infection, and 42 patients (16.9%) were diagnosed with LTBI. At the second screening, 29 patients (11.6%) were diagnosed with LTBI, and 61 patients did not finish the investigation. Only 188 (69.9%) out of 269 patients completed the investigation. Ninety patients received treatment for LTBI and 83 patients (92.2%) completed the treatment, of which 18 patients had side effects such as rash, fatigue, and gastrointestinal symptoms. However, there were no serious side effects requiring treatment discontinuation. Conclusions: The completion rate of childhood tuberculosis contact investigation was low, but the completion rate of LTBI treatment was high in children without serious side effects. In order to prevent and manage the spread of tuberculosis, active private-public partnership efforts and education of the patient and guardian are needed.

Investigation of Stress-Inducing Factors and Occupational Stress Levels in General and Pediatric Dentists (일반 진료 치과의사와 소아 진료 치과의사의 스트레스 유발 요인과 직무 스트레스 정도에 대한 연구)

  • The purpose of this study was to compare factors causing stress in pediatric and general dentists when treating pediatric patients and to evaluate their overall occupational stress level. A total of 191 dentists participated in the online survey, consisting of 66 pediatric dentists and 125 general dentists. The questionnaire was conducted using Google Form. Both groups were stressed due to poor cooperation of patient and caregiver, uncertain prognosis of treatment, and low cost of pediatric dental treatment. The pediatric dentists felt relatively high stress due to poor cooperation from caregivers and an uncertain prognosis of treatment (p < 0.05). Overall occupational stress was high in both groups in the order of patient, time, job environment, and income-related stress, and the degrees of pediatric dentists were lower than general dentists. Among the 3 sub-factors of occupational burnout, more than 98% of both groups showed burnout in "depersonalization", and more than 69% of both groups showed burnout in "emotional exhaustion". Both groups showed a low burnout rate in "reduced sense of accomplishment" and pediatric dentists showed a higher sense of accomplishment than general dentists (p < 0.05). This study showed that both groups were under high occupational stress due to various factors, and efforts were required to relieve stress.

Dental Treatments and Oral Health Status of Adolescent Girl Inmates in a Juvenile Detention Facility for the Last 5 Years (최근 5년간 소년원 여학생의 치과치료 및 구강건강실태)

  • This research investigates into overall oral health status of adolescent girl inmates in a juvenile detention facility and ordinary school students in the same age range of 11 to 18 years old and compares the results focusing on the difference between the two groups. The last 5 years dental records for girl inmates in Juvenile detention facility and for students who have visited the Department of Pediatric Dentistry at Ajou University Hospital was colleted and analyzed. The restorative treatments took up the greatest part in their history of treatments. While the percentage of extractions of permanent teeth due to dental caries had been gradually increased, in Juvenile detention facility students, throughout the transition period from middle school to high school, in dental hospital patients, the phenomenon of the same kind has hardly been detected. After that record analysis, oral examination for Juvenile detention facility students and dental hospital patients was conducted. DMFT index for dental hospital patients are 4.15, whereas, the value for Juvenile detention facility students are as twice as high to be 7.53.

  • Air abrasion technology can prepare enamel and dentin for bonding, similar to etching by acidic gels and solutions. Longer treatment can excavate pit and fissures, preparing the tooth for immediate placement of bonded resin materials. Although not appropriate for every clinical situation, the air abrasive technology minimizes heat, vibration and bone-conducted noise associated with conventional means of caries removal since the cutting is accomplished by air pressure. Also, patients treated with the air-abrasion technology rarely request anesthesia. Air abrasion technology was more effective in treating early carious lesions and stains compared to lesions where caries had already progressed to produce soft dentin and the strong air stream and noise caused by the evacuation system was a major discomfort to pediatric patients, and the experience and skillfulness of clinician should be required for accurate and proper tooth preparation.

Changes in Exposure Dose and Image Quality due to Radiation Shielding in Pediatric Patients (소아 환자에서 방사선 차폐체로 인한 피폭선량과 화질의 변화)

  • This study was conducted to observe the changes in radiation exposure dose and image quality of pediatric patients according to the presence and size of the gonadal shield when using the AEC system. X-ray equipment was used to measure the radiation exposure dose in the abdominal and gonads of the pediatric phantom when no shielding body was used and when three different sizes of shielding body were used, and SNR and CNR were measured through the obtained images. As a result of the study, the radiation exposure dose to the gonads decreased in proportion to the size of the radiation shield, but the radiation exposure dose to the abdomen was rather increased, and the image quality did not change. It is recommended to use a shield with a size optimized for the age, weight, and body size of the pediatric patient so as not to be overexposed by the increased radiation due to the radiation shield due to the use of the AEC System. For this purpose, information about the pediatric patient with the nurse It is believed that exchange is necessary.
