Purpose: The purpose of this study was to identify the effects of psychosocial intervention on depression, hope and quality of life of home-based cancer patients. Methods: The study design was a nonequivalent control group pretest-posttest design. Data were collected from September 21 to November 13, 2009. The subjects consisted of 81 cancer patients randomly selected who were registered at four public health center in Daegu, Korea. The 39 subjects in the experimental group received a psychosocial intervention and the 42 subjects in the control group received the usual nursing care. The weekly psychosocial Intervention protocol was comprised of health education, stress management, coping skill training and support (60 min) for eight weeks. Data were analyzed by using the SPSS/WIN 12.0 program. Results: Depression (F=23.303, p<.001) scores in the experimental group were significantly less than that of the control group. Further, hope (F=58.842, p<.001) and quality of life (F=31.515, p<.001) scores were significantly higher than those reported by the control group. Conclusion: The findings indicate that the psychosocial intervention was an effective intervention in decreasing depression and increasing hope and quality of life of home-based cancer patients.
Purpose: This study has aimed to examine the relationship among self-esteem, Activities of Daily Living (ADL) and depression reported by hospitalized elderly patients with chronic diseases. Methods: A descriptive correlation study by means of a self-report questionnaire or face-to-face interview was used to collect data from 119 elderly patients who were hospitalized in a General Hospital from January 5 to February 25, 2010. Their levels of depression was measured using Short-Form Geriatric Depression Scale, self esteem using Jeon's, and activities of daily living using K-ADL. Results: 80.7% of the subjects experienced depression. Depression correlates with self-esteem (r=-.67) and ADL (r=.45). The influencing factors on depression were self-esteem, ADL, subjective health status, and family support satisfaction ($R^2=.57$), while self esteem in itself explained 45% of variance in depression. Conclusion: These findings indicate the importance of early detection of depression, which starts from the admission of patients and the continuing evaluation/management in daily life after discharge to ensure their well-being and quality of life. The development of program empowering self esteem, ADL and subjective health status with adequate family support during hospitalization and in daily life is indispensible.
Purpose: The purpose of this study was to explore the life experiences of patients with a severe Chronic Obstructive Pulmonary Disease (COPD). Methods: The data were collected through in-depth interviews of six patients suffering from severe COPD. The interviewed data were audio-recorded and transcribed verbatim and checked for accuracy. The Giorgi method of phenomenology was used for analyzing data. Results: Eight themes forming the, units of meaning, were: Repeated and Unpredictable Suffering of Dyspnea, Confidence Loss/Exhaustion Life due to non-efficient breathing, Gradually Deprived Liberty, Absolute Being to Sustaining my life, Source of Burden but Significant Person I am in the Family, Endless Tug-of-War-Capability/Endeavor to Breath, Longing for my Life, and Dead-end of breathing. Conclusion: The study results provide an in-depth understanding of life experiences of patients suffering from severe COPD. The findings will be useful to nurses caring for this population.
Purpose: The purpose of this study was to investigate health literacy, diabetic knowledge, and diabetic self-care among foreign diabetic patients at a hospital in South Korea. Methods: Participants was 134 foreign patients diagnosed with diabetes and who understand English. Data were analyzed by SPSS version 20.0. Results: Health literacy significantly differed with HbA1c levels, whereas diabetic knowledge significantly differed with education levels, and time since diagnosed. Diabetic self-care significantly differed with time since the diagnosis. Health literacy significantly differed with diabetic knowledge and diabetic self-care. There was a correlation between health literacy and diabetic knowledge and between diabetic knowledge and diabetic self-care but not between health literacy and diabetic self-care. Conclusion: There are significant results on health literacy, diabetic knowledge and diabetic self-care. Subject with adequate health literacy had high scores on diabetic knowledge and self-care.
Purpose: The purpose of this study was to identify pain intensity according to activities over postoperative days and to identify factors of patient satisfaction with pain management after abdominal surgery. Methods: The subjects were 123 patients who were admitted at a general surgical ward and had abdominal surgery from Mar. 2005 to June 2005. The collected data were analyzed using the SPSS 12.0 program. Results: The average postoperative pain scores were 6.13 at POD 1, 5.46 at POD 2, and 4.22 at POD 3. In stepwise multiple regression analysis a total of 40% of variance in satisfaction with pain management was accounted for by pain intensity on POD 3 at rest (29.4%) and attitude toward using pain medicine (6.6%), and side effects of pain medicine (4.0%). Conclusion: According to the findings of this study, patients had inadequate pain management after abdominal surgery. Therefore, nurses need to provide educational programs focused on changing attitudes toward using pain medicine, to reduce side effects of pain medicine, and to develop nursing intervention for relieving pain.
Purpose: The purpose of this study was to propose a theoretical model of hope commonly held by the cancer patients just after surgery, under the assumptions that hope of those patients is not only realistic and disease oriented but in dialectical circulation. Method: A theoretical model was generated through 4 steps: exploring a hope structure by synthesizing the relevant hope structures expressed in Kim and Tae's studies, in-depth literature review, examining the meanings of the concepts consisted of the structure in use and their causal relations in logical adequacy, proposing a theoretical structure through synthesizing the causal relations, and diagramming the structure. Results: The proposed theoretical model involves concepts such as Cancer Related Uncertainty (CRU), Efforts to Find out the Possibility of Cure or Recovery (EFPCR), and Hopefulness or Hopelessness. The 'EFPCR' is stipulated as 'Behaviors Related to Looking for Evidences or Cues (BRLEC)' and 'Formation of Cognitive Schema (FCS)'. In the model, Hopefulness is directly influenced by 'CRU in low', which is affected by 'FCS in good' from the result of EFPCR started with 'CRU in increase' while 'CRU with increase' from the result from EFPCR has direct effect on Hopelessness. Conclusion: The theoretical model would be used to enhancing hope of the cancer patients in post-operation.
Purpose: The objective of this study was to examine patient safety culture (PSC) and patient safety initiatives (PSI) according to IT-based medication errors prevention system which is constructed in this study, and to identify the relationships among system construction, perception to the usage, PSC and PSI. Methods: The subjects were 180 nurses who work at 12 different hospitals with over 300 beds. The questionnaire included the characteristics of participants, a system construction status, the perception to the usage using electric pharmacopoeia (EP), a drug dose calculation system (DDCS), a patient safety reporting system (PSRS) and a bar-code system (BS). The data were collected from July 2011 to August 2011. Descriptive statistics, ANOVA, Pearson correlation and MANOVA were used for data analysis. Results: Systems were constructed in participating hospitals; For EP and PSRS, 83.9%, DDCS, 50%, and BS, 18.3%. The perceptions on the usage of the system were marked highest in BS as 4.54 followed by EP as 3.85. There were significant positive correlations between PSI and EP construction (r=.17, p=.028); PSRS (r=.17, p=.028) and DDCS (r=.23, p=.002). Conclusion: The developed system for improving the user experiences and reducing medication errors was found out well accepted. It is hoped that the system is helpful for PSC and PSI improvement in clinical settings.
Purpose: The purpose of the study was to validate fall risk assessment scales among hospitalized adult patients in South Korea using the electronic medical records by comparing sensitivity, specificity, positive predictive values, and negative predictive values of Morse Fall Scale (MFS), Bobath Memorial Hospital Fall Risk Assessment Scale (BMFRAS), and Johns Hopkins Hospital Fall Risk Assessment tool (JHFRAT). Methods: A total of 120 patients who experienced fall episodes during their hospitalization from June 2010 to December 2013 was categorized into the fall group. Another 120 patients, who didn't experience fall episodes with age, sex, clinical departments, and the type of wards matched with the fall group, were categorized to the comparison group. Data were analyzed for the comparisons of sensitivity, specificity, positive and negative predictive values, and the area under the curve of the three tools. Results: MFS at a cut-off score of 48 had .806 for ROC curves, 76.7% for sensitivity, 77.5% for specificity, 77.3% for positive predictive value, and 76.9% for negative predictive value, which were the highest values among the three fall assessment scales. Conclusion: The MFS with the highest score and the highest discrimination was evaluated to be suitable and reasonable for predicting falls of inpatients in med-surg units of university hospitals.
The purpose of this study was to identify the effect of structured information provided on knowledge and self-care behavior. The subjects of this study were both hospitalized patients and outpatients in K university hospital. The instrument use for this study were the knowledge assessment tool and self-care behavior assessment tool by Eom Soon-Ja(1998) and they were modified for liver cirrhosis patients. The data were analyzed by t-test, Chi-square test, Pearson correlation coefficients using SAS program. The results of this study were as follows. The experimental group which had received structured information provided showed greater increased knowledge of liver cirrhosis(P=.001). The experimental group which had received the structured information provided indicated increased self-care performance rate, especially after information about diet(P=.001), activity and bed rest(P=.001), drug therapy and visiting the hospital(P=.001), prevention of a complication and observation(P=.001). In conclusion, structured information provided showed increased in the degree of knowledge and self-care behavior, so information showing is an effective nursing intervention. It is much needed to employ information showing for chronic patients.
