A Survey on Recognition, Attitude and Demand toward Natural Family Planning Method in Female Adult (성인 여성의 자연가족계획방법 사용에 대한 인식, 태도 및 요구도 조사)

  • Purpose: This study was attempted to investigate the demand, the recognition, and the attitude of general people on Natural Family Planning Method(NFP), and to provide basic data for a development of a program to establish a healthy sexual life. Method: This is a descriptive correlative study, collecting data by questionnaires. The subjects were 130 people of three urban areas from March 1 to April 15, 2004. The measurement tool used was about demand, recognition, and attitude to the natural birth control method. The data analysis was done by number, percent and T-test, Pearson's correlation coefficient, using SPSS program. Results: The recognition, attitude, and demand to natural birth control method was 3.01, 2.84 and 2.73 respectively. The higher recognition of correlation for natural family planning, the higher attitude(r=.864, p=.000) and demand(r= .525, p=.000) of correlation for natural family planning. It was confirmed that the study subjects almost produced self esteem and confidence recognizing at the idea of life and self value, and positively thought of need, recognition, and attitude of natural family planning method which could keep their own health management. Conclusion: As the result above, we suggest that an educational program should be developed, diffused and supported, and culturally formed by nation and social group to insist the benefit of conjugal relations in natural family planning as well as to support the knowledge deficit.

A Study on Physiological Parameters, Health Perception, and Health Behavior of Adults in Kyungi Province Area (경기지역 일부성인의 생리적 지표와 건강지각, 건강행위에 관한 연구)

  • Purpose: The purpose was to investigate the physiological parameters and health perception of adults in Kyungi province area, and the correlation between these variables. The subjects were 95 adults who participated voluntarily in the health examination program. Methods: The instruments for this study were physiological parameters, which were composed of blood pressure, blood sugar, and body fat. The health perception scale developed by Ware (1976), and the health practice performance scale developed by Chang et al. (1999). Results: The average blood pressure was 137/85mmHg, the average blood sugar was 108.56mg/dl, and the average body fat arm sunt was 27.08% in males, and 30.07% in females. There were significant negative correlation between health perception and blood sugar (r=-2.36, p=.01), and body fat (r= -2.77, p=.001). There were significant positive correlations between blood pressure and health behavior (r=2.76, p=.001). However a significant correlation between health perception and health behavior was not found. Of the sociodemographic characteristics, sex (F=5.42, p=.01), and marital status (F=3.29, p=.04) were related to blood pressure. Also sex (F=4.17, p=.04) was related to body fat, and sex (t=4.04, p=.03) was related to health perception and disease status (t=4.33, p=.040). Thus having a relationship with health behavior. Conclusion: It is suggested that health perception provide important information about health status. However, this study has not shown a correlation between health perception and health behavior. Therefore further research is needed to find the other variables related to health behavior.

Structural Equation Modeling on Health-Related Quality of Life in Adults with Epilepsy (성인 뇌전증 환자의 건강관련 삶의 질 구조모형)

  • Purpose: This study aimed to identify variables influencing the health-related quality of life (HRQoL) of adults with epilepsy in order to establish a structural model and design an intervention strategy to improve patients' HRQoL. Methods: The selected subjects were 212 patients with epilepsy aged between 18 and 70 years who were currently receiving treatment from hospital, general hospital, and clinic. They were surveyed using a structured questionnaire. Results: The goodness of fit measures of the final hypothetical model were as follows: ${\chi}^2/df=2.51$, GFI=.91, AGFI=.90, CFI=.96, SRMR=.04, NFI=.93, and RMSEA=.08. The major variables influencing the HRQoL of adults with epilepsy were epilepsy self-efficacy, depression, social support, and side effects of anti-epileptic drugs (AEDs), which were significant in the mentioned order, whereas the duration of AEDs use and perceived stigma did not show any effects. Six variables accounted for 75.6% of HRQoL. Variables having a direct and total effect on the HRQoL of adults with epilepsy were the side effects of AEDs, social support, epilepsy self-efficacy, and depression, and those with an indirect effect were the side effects of AEDs and social support. Conclusion: It is necessary to accurately identify the side effects of AEDs in adults with epilepsy and accurately observe the physical changes caused by depression. In addition, it is imperative to establish an active and effective nursing intervention program to strengthen the self-efficacy of the patients and to improve their quality of life through social support provided by family members and medical professionals.

A Study on Occurrence of Backache Related to Types of Daily Activities in Adulthood (성인의 활동형태와 관련된 요통발생에 관한 조사연구)

  • For the study on occurrence of backache related to types of daily activities in adulthood Nine hundred twenty six adults living in Seoul were randomly selected as subjects from the eight kinds of occupation; housewives, teachers, nurses, officers, physicians, labourers, beauticians & barbers, and students. The purpose of this study was to identify various factors related to backache in daily activities and to apply these factors in clinical situations as well as in the community situations. This survey was conducted from September 21 through October 8, 1976. Under the ten hypothesis designed for this study, the. followings were obtained; 1. Significant difference on occurrence of backache was revealed according to sex difference at 0.5% level. (x$^2$= 27.85, df: 1, p<0.005) 2. Significant difference on occurrence of backache was revealed according to age groups at 2.5% level. (x$^2$= 11.13, df= 3, p<0.025) 3. Significant difference on occurrence of backache was revealed according to types of occupation at 2.5% level. (x$^2$= 18.04, df: 7, p<0.025) 4. Significant difference on occurrence of backache was revealed according to types of movement originated by hand during occupational activities at 1% level. (x$^2$= 14.23, df= 4, p<0.01) 5, Significant difference on occurrence of backache was revealed according to types f movement of legs during occupational activities at 0.5% level. x$^2$= 20.78, df: 3, p<0.005) . No significant difference on occurrence of backache was revealed according to length f daily working hours. . Significant difference on occurrence of backache was revealed according to sleeping posture at 1% level. x$^2$= 9.61, df: 2, p<0.01). Comparison of blood glucose concentrations from capillaries and veins in SMBG (자가혈당측정시 채혈방법에 따른 혈당치의 비교)

  • 당뇨병환자의 자가혈당조절에 있어서의 기준이 되는 혈당측정에 있어서 하나의 지침을 제공하기 위해 본 연구는 말초혈관과 상완정맥에서의 혈액을 채취하여 이들 검사물들의 혈당치를 직접적으로 비교했다. 30명의 건강한 한국인 성인 남자(연령 : 평균=30세)로부터, 공복시의 혈액을 세가지의 서로 다른 방법들(우선 손가락 끝에 말초혈관을 울혈이 되도록 한 뒤에, 이후 같은 편의 팔의 상완정맥에서, 마지막으로 다시 처음 검채를 시행했던 바로 그 말초의 천자부위에서 혈액을 짜내는 방법으로.)로 각각의 대상에서 검채하였다. 검사물들의 혈당치는 간편측정용 혈당측정기(Glucocard, KDK Corp., Japan. 1995)로 측정한 결과 상완정맥에서 채취한 혈액의 혈당치(107.58$\pm$21.03mg/dl)가 말초에서 채취한 혈액(울혈 후 : 100.21$\pm$24.03mg/dl, .짜낸 경우 : 97.84$\pm$22.45mg/dl)들 보다 높았다(T=2.86. p=.0081 : T=3.084. p=.0055). 그러나 이 결과는 기타의 연구들에서의 결과와는 일치하지 않았다. 이러한 불일치는 자가혈당측정기가 인슐린 의존형 당뇨환자는 물론이고 비의존성 당뇨환자들의 자가관리에 결정적인 지침이 된다는 점을 고려할 때, 좀 더 통제된 상황에서 더욱 많은 사람을 대상으로 한 임상시험을 통해 자가 혈당측정치에 있어서 기계요인에 대한분석이 이루워질 필요를 시사하고 있다. 또한 동일한 부위에서 서로 다른 방법들(혈관을 울혈을 시킨 천자하여 얻은 경우와 천자 후 그 부위를 짜서 검사물을 얻는 방법)에 의해서 얻어진 혈액의 헐당치 측정 결과 간에 차이가 없는 것으로 나타나서(t=0.72, p=.4791), 일반적으로 혈당치의 측정 시 검채한 양이 부족할 때, 새로이 천자하여 검사를 시행하는 것을 원칙으로 인식하고 있으나 본 연구결과를 볼 때, 이러한 채혈의 방식에 의한 차이는 환자의 안녕의 차원에서 고려하여 필요하다면 간과될 수도 있음을 의미하고 있다.

Predictive Factors of Aspects of the Transtheoretical Model on Smoking Cessation in a Rural Community (범이론 모형을 기초로 한 농촌지역 성인의 금연행위에 영향을 미치는 요인)

  • Purpose: This study was done to evaluate the predictive value of aspects of the Transtheoretical model (TTM) of behavior change as applied to smoking cessation in a rural population. Method: A convenience sample was recruited from a public health center in a community. A total of 484 participants were recruited, including 319 smokers, 116 ex-smokers and 49 non-smokers. A cross-sectional and descriptive design was used in this study. Data was analyzed using descriptive statistics, frequency statistics, ANOVA and Logistic regression. Result: The major findings were 1) The participants were assessed at baseline for their current Stage of Change resulting in a distribution with $42.1\%$ in Precontemplation, $24.1\%$ in Contemplation, $9.7\%$ in Preparation, $6.2\%$ in Active, and $17.9\%$ in the Maintenance stage. 2) There were statistically significant differences of processes of change, decisional balance and situational temptation across the stages of change. 3) The main factors that affect smoking cessation were age, number of years smoking, age when began smoking, self-liberation and negative/affective situations, which combined explained $33.2\%$ of the smoking cessation. Conclusion: TTM variables measured prior to a smoking cessation program added little predictive value for cessation outcome beyond that explained by demographic and smoking history variables.

Validity Test of Korean Pain Measurement Tool Using Normal Adult Individuals (정상성인에서의 한국어휘를 이용한 통증척도의 타당도 조사)

  • The main purpose of th study was to evaluate he validity of Korean Pain Measurement Tool composed of pain terms. The specific purposes of this study were 1. to examine whether pain intensities of pain terms are congruent with those classified in three previous studies. 2. to evaluate the relative intensity of each term by panel of judges. 3. to explore the difference of ranks of pain terms according to the sex, education, and ages. One hundred and sixty normal individuals were selected by 2$\times$2$\times$4 sampling design. Sex (male, female), education (high school, college), and age (20s, 30s, 40s, 50s) were matched. Each individual was asked to rate the ranks of 3~8 pain terms in each subclass. The data measured by ordinal scale were transformed to the interval scale to compare with the pain intensities gained from the previous study. The pain ranks different from previous results were finally rearranged or cancelled through the consultation of 4 panel of judges and sunmed up to 91 pain terms in the scale. As a result, the ranks of pain terms within each of eleven subclasses among the twenty subclasses completely were congruent with the Previous pain ranks, while the ranks of nine subclasses were different from the previous pain ranks. In addition, there was significant relation between sex and pain ranks in skin punctuate pressure pain and cavity pressure. (sp : $\chi$$^2$=5.18 ø=0.26; cp : $\chi$$^2$=5.83 ø=0.24) In conclusion, seven terms from subclasses of inflammatory repeated pain, traction pressure pain, fatigue-related pain, fear-related pain, dull pain, and pulsation. related pain were cancelled. The ranks of four terms in subclasses of incisive Pressure pain and constrictive pressure pain were tentatively rearranged. Ranks of two terms in the tract pain were left as shown in the third study. As a result, six terms must be studied repeatedly for obtaining exact scores from ratio scale.

Prediction of Optimal Gluteal Intramuscular Needle Length by Skinfold Thickness Measurements in Korean Adults (피부주름두께 측정을 통한 성인의 둔부 근육주사 바늘의 최적 길이 예측)

  • Purpose: This study was conducted to assess optimal needle length for gluteal intramuscular injections (IM) via simple skinfold thickness (SFT). Methods: For this study, 190 healthy adults were recruited and grouped into eight groups according to gender and body mass index (BMI) (kg/$m^2$). The Korean Society for the Study of Obesity criteria defines a BMI under 20 as underweight, 20.1-22.9 as normal, 23-24.9 as overweight and over 25 as obese. For each participant, the SFT of dorsoguteal (DG) and ventrogluteal (VG) sites were measured using a caliper. Subcutaneous tissue thickness was acquired through ultrasonic images. Results: For men in the overweight and obese groups at the DG site, for the obese group at the VG site, and for women in the normal weight, overweight and obese groups at both sites, the mean subcutaneous tissue thickness exceeded 1.84 cm, the minimal length for a 1 inch needle used for IM. At the DG site, optimal intramuscular needle length (OINL) was 1.4 times in women and 1.0 times in men compared to SFT. At the VG site, OINL was 1.3 times in women and 0.9 times in men compared to SFT. Conclusion: The results of this study suggest that SFT is a reliable index to determine optimal needle length with minimal effort prior to IM.

Comparison of Cardiovascular Health Status and Health Behaviors in Korean Women based on Household Income (소득수준에 따른 성인여성의 심혈관계 건강상태와 건강행위)

  • Purpose: In this study cardiovascular health status and health behavior of Korean women based on their household income were explored. Methods: For this cross-sectional study, 91 women residing in the community were recruited to complete survey questionnaires and biophysical tests including blood pressure (BP), body mass index (BMI), body fat rate, waist circumference (WC), and blood chemistry tests. Results: Compared to non-low income women (NLIW), low income women (LIW) were more likely to be older, less educated, and jobless, and further more LIW were postmenopause and reported having been diagnosed with hypertension or hypercholesterolemia. Significant differences were found in systolic BP, triglyceride level, BMI, body fat rate, and WC between the groups. Two fifths of the LIW had indications for metabolic syndrome. Their 10-yr risk estimate of myocardioal infarction or coronary death demonstrated a higher probability than that of NLIW. Although these significant differences were due to age gap between the groups, advanced age is known to be one of the key characteristics of LIW as well as a non-modifiable risk factor. Conclusion: Effective community programs for vulnerable women at risk of cardiovascular disease should be based on strategies targeting unhealthy behaviors and modifiable risk factors.

A Survey Study on Urinary incontinence of Adult Women (일부지역 여성의 스트레스성 요실금 경험에 대한 조사연구)

  • The purpose of this study were to identify the characteristics of urinary incontinence in adult women and to identify it's relating factors, daily living and sexual problem related urinary incontinence. Subjects were 60 community dwelling women in the age of 25-60 years old at 1 cities in Korea. The data were collected from January 1988 to May. 1998. Subject were interviewed with structured questionnaire. Questionaire was composed of item of general characteristics, obstetrical characteristics, measuring tool of severity of urinary incontinence, discomfort due to urinary incontinence. The data were analyzed with SAS program, descriptive statistics, T-test, ANOVA test. The results of study were as followings. 1) Most of incontinence women were mildly incontinent subjects(mean=7.40). 2) The common activity related to the urinary incontinence were coughing, sneezing, laughing aloudly exercising. 63.5% of women reported small volume accidents of only 1 to 2 drops. 3) The incidence of urinary incontinence was significantly higher in woman who had more pariety and older age of last delivery, menopause, itching sense of vagina. 4) 73.3% of the episodic urinary incontinence were not treated because the felt that urinary incontinence was not disease(70.4%) was shamful(18.1%), was incurable inspite of treatement attempt(4.5%). 5) Daily living problem related to urinary incontinence were as follows : Voiding before going out(66.6%), odor of urine(10.0), frequent underwear change.(88.3%). 6) Sexual intercourse problem related to urinary incontinence were as follows : Pain during sexual intercourse(30.0%), sexual life trouble,(8.3%), urine leakage.(11.7%) The results indicate that urinary incontinence is common in adult women. Health care provider should develop and provide adequate nursing intervention for prevention and early treatment of urinary incontinence.
