  • The study was performed to identify the functional status change of beneficiaries of Long-Term Care Insurance and its related factors. We conducted the logistic regression with 17,652 beneficiaries during August and September in 2008. As a result, activities of daily living(ADL), behavioral changes, rehab, instrumental activities of daily living(IADL) and cognitive function, followed by nursing care area were improved in a greater degree. For the institutional service, level-1 beneficiaries was significantly improved in rehab area and level-2 beneficiaries was improved in ADL. For the home-visit care service of in-home services, level-1 beneficiaries was improved in ADL, level-2 beneficiaries was improved in ADL and rehab area, level-3 beneficiaries was improved in ADL, cognitive function and behavioral changes. For the day-and-night care service, level-1 beneficiaries was improved in ADL, IADL, behavioral changes and rehab area, level-2 beneficiaries was improved in behavioral changes, level-3 beneficiaries was improved in cognitive function and behavioral changes. For the short-stay service, level-3 beneficiaries was improved in behavioral changes. By the above results, there was a difference in a functional improvement by level and used services. Therefore, government need to provide the personalized service system based on the objective and comprehensive understanding for health and functional status of beneficiaries.

Out of the 360 questionnaires responded to, only 308 were used for data analysis because 52 were responded to incompletely or incorrectly. The survey questionaries were used to collect data. The questionary for physical function state were developed and standardized by kung-hee chung(1993), yang-lae Kim(2005). And mental function state were divided two parts; self-perceived scale and depress scale. The questionary for self-perceived scale were based on MMSE-K(Mini Mental State Examination-Korea). The questionary for Depression were based on Depression Scale developed by Beck(1978), adapted in Korean and standardization by Lee Young-ho & Song Jong-yong(1991), regulated by Jo Gyu-whang(2004). From the analysis of this study, the following conclusions were obtained: First, according to the elderly people's participation in leisure sport activities, it partially affected physical and mental function state. Second, according to the degree of elderly people's participation in leisure sport activities, it partially affected physical and mental function state.

  • The purpose of this research is to test the relationship between ACLT(Allen Cognitive Level Test) and FIM(Functional Independence Measure), MMSE-K(Mini-Mental State Examination), and MFT(Manual Function Test). 38 elderly persons with stroke were recruited. Pearson Correlation Coefficients is used for correlation test. There is statistically significant correlation between ACLT and FIM, MMSE-K, and MFT(p<.05). This results identified the relationship between cognitive function and activities of daily living, indicated the usability of ACLT as a cognition assessment tool and the need of research for it's application in stroke patients.

  • The purpose of this study was to investigate the relations between the cognitive functions, health conditions, activities of daily living, depression and sleep states among the elderly at care facilities. The subjects include 204 elderly people aged 65. The data collected were analyzed using descriptive statistics, the t-test, ANOVA, Pearson correlation, and hierarchical multiple regression. The findings show that the cognitive functions of the elderly at care facilities are related to their engagement in regular exercise, duration of residency, grade of care, lack of physical freedom, listening ability, state of teeth, urinary incontinence, activities of daily living, and state of sleep. Their cognitive functions had positive correlations with activities of daily living and negative correlations with state of sleep. Their cognitive impairment was significantly influenced by their engagement in regular exercise, duration of residency, grade of care, listening ability, ADL, IAD dependency, and sleep disorder. In short, the cognitive functions of the elderly at care facilities are highly related to their health conditions as perceived and felt by them. It is thus required to develop, apply, and consistently assess and manage cognitive rehabilitation training programs to provide interventions for the factors that influence their cognitive impairment.

  • The purpose of this research is to investigates 10-year trajectories of cognitive functions among older adults in their 70s to understand changes in cognitive functions as a continuum until very late life. This study also examines differences in trajectories of cognitive functions by gender and by changes in marital status, especially widowhood. Among participants of the Korean Longitudinal Study of Ageing(KLoSA), the sample of this study includes 800 older adults in their 70s during the first study wave (2006) and those who reported their cognitive functions for six consecutive study waves (2006, 2008, 2010, 2012, 2014, and 2016). The analyses were conducted in two steps. First, we conducted Latent Class Growth Analyses(LCGA) to investigated heterogeneous trajectories of cognitive functions in 10 years. Then, we performed multinomial logistic regression. Three heterogeneous trajectories of cognitive functions were identified. One group of 48.7% of older adults showed high cognitive function at baseline and maintained it over 10 years. Second group of 14.7% of older adults reported low cognitive function scores at baseline and showed continuous decline over time. Third group of 36.6% were showed mid-level cognitive functions and maintained their functions over time. We also found significant gender differences but not significant differences in marital status when we consider both in our model; however, the we found significant differences in changes in marital status when we did not consider gender in the model. The results suggest that the importance of considering dynamics of gender and changes in marital status to understand changes in cognitive functions in later life.

This was a descriptive study. Data were collected for two years with 973 older adults who enrolled in a health center at A city. Trained nurses visited subjects' home and collected data using Resident Assessment Instrument MDS-HC. The average age of the subjects was 76.8 (±7.6) and female was 74.8%. 20.7% of subjects reported fear of falling. In terms of the fear of falling, two groups were compared. There were significant differences in vision, urinary incontinence, pain that interrupts daily activity, unstable gait, comorbidities, frequency of depressive or anxious sign. The greater the age, IADL and ADL dependency, comorbidities, unstabled gait, the more fear of falling. In conclusion, this study reassured the importance of assessing the major factors related to fear of falling such as age, IADL and ADL, comorbities, gait among older adults. This study will contribute to establish strategies for preventing fall. Reducing the fear of falling through the comprehensive nursing intervention will ultimately improve the quality of life in the elderly.
