Radiation dose reduction effectiveness of a male gonadal shield during 128-MDCT using Glass Detector (유리선량계를 이용한 128-MDCT 검사시 생식선 차폐 선량 감소 효과)

  • When abdomen and pelvic were scanned with 128 channel MDCT, the gonadal exposure dose was measured with and without gonadal shield and the obtained images were evaluated. As a result, during abdominal MDCT scan, the gonadal exposure dose was measured $16.5{\pm}0.5$ mGy when the gonad shield was not used, and it was $7.5{\pm}0.3$ mGy when the large gonad shield($650m^2$) was used, which showed the effect of reduction in the gonadal exposure dose by 54%. During pelvic MDCT scan, the gonadal exposure dose was $9.5{\pm}0.3$ mGy when the gonad shield was not used, and it was $2.8{\pm}0.2$ mGy when the large gonard shield($650m^2$) was used, which showed the effect of reduction in the gonadal exposure dose by 70%. The images were obtained when using the gonad shield and when not using it during MDCT scan, and as a result of analyzing them with 5-point Likert scale, in the abdominal image, it was 4.1 points irrespective of whether using the gonad shield or not. And also, in pelvic scan, it was 1.2 points when the gonad shield was used, and 4.1 points when it was not used. With the results above, it is considered that during the abdominal 128-MDCT scan, by using the gonad shield, the images should be obtained without being degraded and the exposure dose must be reduced.

  • Upper gastrointestinal series is an examination that uses X-rays. It is important to defend against exposure to radiation during upper gastrointestinal examination because the organs, such as thyroid gland, lens, breasts, and gonads, with relatively high biological sensitivity to radiation are distributed on the examination area. We have made a whole body phantom that can change the depth of organs. radiation dose of eye, thyroid gland, breast and gonads were measured by the same procedure as the actual upper gastrointestinal examination. When performed only fluoroscopy the mean dose reduction of lens, thyroid gland, breast and gonads was 62.2%. The mean dose reduction of lens, thyroid gland, breast and gonads was 59.0% when both fluoroscopy and spot shoot were performed. Therefore, when performed upper gastrointestinal examination it was confirmed that shielding of the lens, thyroid gland, breast and gonads was effective in decreasing the exposure dose. The manufactured human phantom can be used in measuring radiation dose for deep organ because it can adjust the height corresponding to the organs located in the human body.

Study on Gonad Dose and Utility according to Use of Filters During the Defecography (배변조영촬영에 있어 필터사용에 따른 생식선량 및 유용성에 관한 연구)

  • The study is to measure a variation of exposed dose on genital glands (ovary, testis) which are exposed to radiation during the defecography to diagnosis domain according to use of filters and to look into its utility. whose results are as follows: The measured values of dose were the left ovary 23.4mGy, the right ovary 7.5mGy, the testis 10.3mGy in case of not using filter at all, the left ovary 22.4mGy the right ovary 7.0mGy, the testis 9.5mGy in case of using an additional filter only, the left ovary 26.7mGy, the right ovary 8.4mGy, the testis 11.5mGy in case of using a defeco filter only and the left ovary 20.5mGy, the right ovary 6.2mGy, the testis 7.5mGy in case of using both an additional filter and a defeco filter, respectively. When comparing with the value in case of not using filter at all, the dose to the left ovary decreased by 10%, the dose to right ovary by 5% and the testis by 8% respectively in case of using an additional filter only. While the dose to the left ovary increased by 33%, the dose to right ovary by 9% and the testis by 12% respectively gonad a defeco filter only. And in case of using both an additional filter and a defeco filter, the dose to the left ovary decreased by 29%, the dose to right ovary by 13% and the testis by 28% respectively. In other words, the dose increased in case of using a defeco filter only while the dose decreased markedly on the rest conditions such as using an additional filter only, using a defeco filter only and using both an additional filter and a defeco filter.

Exposure Dose of Thyroid, Breast, and Sexual Gland using a Personal Dosimeter in Multiple CT Examinations (다중 CT 검사 시 개인선량계를 이용한 갑상선, 유방, 생식선의 피폭선량)

  • In this study, a dose assessment was conducted on the exposure dose of thyroid, breast and sexual gland using a personal dosimeter in multiple CT examinations currently being conducted in health examinations. The dose assessment was measured by attaching TLD and EPD to the locations of the thyroid, breast and sexual gland during CT examinations of Brain, Brain + C-S, Brain + Low lung, Brain + L-S among CT items. The generated dose of equipment, CTDIvol and DLP, was measured. The study found that effective doses were rated 41.7% higher for thyroid TLD in Brain + C-S CT examinations than for the general public, 156% higher for EPD, 10% for breast EPD in Brain + Low Lung CT examinations, 124.4% higher for reproductive TLD and 339.8% higher for Brain + L-S CT examinations. The CTDIvol and DLP analysis results showed that C-S CTDIvol values were higher than the diagnostic reference levels at 0.6%, Low Lung CTDIvol values at 5.7%, DLP values at 11.8% and L-S CTDIvol values at 1.2%. In order to reduce the exposure dose of patients, indiscriminate examination is reduced and dose limit setting is needed in health examination.

Changes in Exposure Dose and Image Quality due to Radiation Shielding in Pediatric Patients (소아 환자에서 방사선 차폐체로 인한 피폭선량과 화질의 변화)

  • This study was conducted to observe the changes in radiation exposure dose and image quality of pediatric patients according to the presence and size of the gonadal shield when using the AEC system. X-ray equipment was used to measure the radiation exposure dose in the abdominal and gonads of the pediatric phantom when no shielding body was used and when three different sizes of shielding body were used, and SNR and CNR were measured through the obtained images. As a result of the study, the radiation exposure dose to the gonads decreased in proportion to the size of the radiation shield, but the radiation exposure dose to the abdomen was rather increased, and the image quality did not change. It is recommended to use a shield with a size optimized for the age, weight, and body size of the pediatric patient so as not to be overexposed by the increased radiation due to the radiation shield due to the use of the AEC System. For this purpose, information about the pediatric patient with the nurse It is believed that exchange is necessary.

  • 하지전신계측검사(low extremity teleography)에서 자세 변화에 따른 중요장기의 방사선량을 측정하고 영상을 비교 분석하여 검사방법에 따른 유용성을 알아보고자 하였다. 대상은 하지전신계측검사를 시행한 성인남자 10명을 대상하였고 촬영조건은 관전압 73 kVp, 관전류량 32 mAs, SID 180 cm로 설정하였다. 방사선량 측정은 란도 팬텀을 이용하여 수정체, 갑상선, 생식선 부위에 유리선량계(ion chamber)를 부착한 후 전후방향자세와 후전방향자세를 각각 5번씩 시행하여 부위별로 방사선량을 측정한 후 Paired T-test로 비교 분석 하였다. 영상평가는 전후방향자세와 후전방향자세를 시행한 영상을 blind test를 실시하여 5점 척도로 평가하였다. 결과적으로 전후방향자세검사에 비해 후전방향자세검사가 수정체 약 6%, 갑상선 약 6%, 생식선에 미치는 방사선량을 약 27% 감소시킬 수 있으며 영상평가에서도 두 그룹 간에 큰 차이가 없어, 하지전신계측검사에서 전후방향자세검사보다 후전방향자세검사가 유용할 것으로 사료된다.

MOSFET Dosimetry for Evaluation of Gonad Shielding during Radiotherapy (방사선 치료시 생식선 차폐체 성능 평가를 위한 MOSFET 선량 측정)

  • In order to confirm feasibility of MOSFET modality in use of in.vivo dosimetry, evaluation of gonad shielding in order to minimize gonadal dose of patients undergoing radiotherapy by using MOSFET modality was performed. Gonadal dose of patients undergoing radiotherapy for rectal cancer in the department of radiation oncology of Seoul National University Hospital since 2009 was measured. 6 MV and 15 MV photon beams emitted from Varian 21EX LINAC were used for radiotherapy. In order to minimize exposed dose caused by the scattered ray not only from collimator of LINAC but also from treatment region inside radiation field, we used box.shaped lead shielding material. The shielding material was made of the lead block and consists of $7.5\; cm\;{\times}\;9.5\;cm\;{\times}5.5\;cm$ sized case and $9\;cm\;{\times}\;9.5\;cm\;{\times}\;1\;cm$ sized cover. Dosimetry for evaluation of gonad shielding was done with MOSFET modality. By protecting with gonad shielding material, average gonadal dose of patients was decreased by 23.07% compared with reference dose outside of the shielding material. Average delivered gonadal dose inside the shielding material was 0.01 Gy. By the result of MOSFET dosimetry, we verified that gonadal dose was decreased by using gonad shielding material. In compare with TLD dosimetry, we could measure the exposed dose easily and precisely with MOSFET modality.

  • Sitting position upper extremity X-ray examinations (SUEX) is the most widely used patient positioning method for upper extremity X-ray examinations. For this method, the radiation dose is considerable for relatively less interesting organs. We investigated whether patients need to wear the apron during the examination or not. We also studied the examination methods which can reduce the radiation dose. The results showed that radiation dose was reduced as the distance of source to patient becomes longer and the thickness of object grows higher.
