Early loading on a maxillary posterior single implant with deepened threads: a case report (깊은 나사선을 갖는 임플란트를 이용한 상악 구치부 조기 하중: 증례보고)

  • This case report shows an early loading at the maxillary posterior area with the wide diameter implant which has a deepened threads after removal of failed implant. Implant Stability Quotient (ISQ) value has represented favorable result for one year. This clinical report describes the potential of early loading on a maxillary posterior single implant with deepened threads.

  • Stable posterior occlusal support is crucial for adequate masticatory function and facial aesthetics. In elderly patients over the age of 65, masticatory ability has a significant impact on nutritional intake and overall health. This case report presents a prosthetic treatment of an elderly patient with edentulous maxilla and bilateral posterior edentulous mandible. The upper jaw was restored with a complete denture to establish an ideal occlusal plane, and the three-dimensional positions of the mandibular implants were determined accordingly. The implants were placed through computer-guided implant surgery and were immediately loaded with fixed provisional restorations. The implant-fixed zirconia final restorations were inserted in the lower posterior region, and the occlusal surface of posterior artificial teeth in the upper denture was substituted with cobalt-chrome alloy to resist occlusal wear. The patient's posterior occlusal support and masticatory function were promptly restored through this treatment process, and long-term stability against wear was also obtained.

  • The atrophy of edentulous ridge and pneumatization of the maxillary sinus often limit the volume of bone available for implant placement on maxillary posterior teeth. Most clinicians suffer difficulties from poor bone quality and quantity on maxillary posterior site. Thus, the success of maxillary posterior implant surgery depends on the increase of the available bone and obtaining a good initial stability of the implant after maxillary sinus reconstruction. The maxillary sinus augmentation methods include a crestal approach and a lateral approach. Less morbidity and complications after operation is major advantage to sinus augmentation using crestal approach than lateral approach. However, when the residual ridge height is ${\geq}6mm$, it is known that crestal approach is appropriate. Also delayed implantation after sinus augmentation is recommended in severely atrophic ridge. We present the three cases of implant placement simultaneously sinus augmentation using crestal approach in posterior maxilla site with ${\leq}3mm$ of residual alveolar bone.

Oral Hygiene Management and Factor Analysis of the Community People (일부지역 주민들의 구강환경관리 및 요인분석)

  • The subjects of this study were patients at scaling practice, the total number of the subjects was 249 of 138 men and 111 women. The results were analyzed using SPSS 12.0. 1. In oral health care, among tooth brushing methods, scrub method was found to be 65.9%(164 patients), and 83.9%(209 patients) did not use oral care aids. 2. Mean calculus index was $0.57{\pm}0.22$. 3. For calculus index per tooth brushing method, there was statistically significant difference in all of the upper jaw, the lower jaw, the anterior portion, and the posterior portion(p < 0.05). 4. For calculus index per frequency of tooth brushing, there was statistically significant difference in the upper jaw, the lower jaw, and the posterior portion(p < 0.05). 5. For calculus index per time of tooth brushing and per use of oral care aids, there was statistically significant difference in all of the upper jaw, the lower jaw, the anterior portion, and the posterior portion(p < 0.05). 6. For factors influencing calculus index, as a result of applying stepwise method based on 0.05 of significance level, age, use of oral care aids, sex, and time of tooth brushing were found to influence calculus index. Final regression model was calculus index = $0.362^*age+0.216^*$use of oral care aids - $0.161*sex-0.127^*$time of tooth brushing, and explanatory power of the model was 23.4%. $0.362^*age+0.216^*$use of oral care aids - $0.161^*sex-0.127^*$time of tooth brushing, and explanatory power of the model was 23.4%.

The evaluation of rotational movements of maxillary posterior teeth using three dimensional images in cases of extraction of maxillary first premolar (3차원 영상을 이용한 상악 소구치 발치공간 폐쇄 후 상악 구치부 회전양상의 평가)

  • Cephalometric superimposition is unable to evaluate tooth movement along the occlusal plane and the errors of photographing and superimposition of the occlusogram is also inevitable. The purpose of the present study was to evaluate the rotational movements of the maxillary posterior teeth following space closure after extraction of maxillary first premolars. using 3D dental images. In 19 adult females. analysis of rotational movements of the maxillary posterior teeth was performed between the initial and final 3D dental images superimposed on the maxillary hard palate. The results showed a diversity of rotational movements of posterior teeth. The causes of various rotational movements may be different rotation of molars in each case at the beginning of treatment. individual characters of the dental arch and tooth morphology. The results of the study indicate that accurate evaluation of rotational movements of posterior teeth following space closure after extraction of premolars using 3D dental images can be obtained through the subdivision of tooth movement during the initial aligning, space closure and finishing stages.
