Head Posture-Induced Phoria Deviation (머리 위치에 따른 사위도의 변화)

  • Purpose: The purpose of this study was to evaluate how phoria variations would vary with head positions, as well as to review the need to differentiate prism placement over either eye in a relation to the imbalance of the extraocular muscle. Methods: This study was carried out with 44 males (88 eyes) and 16 females (32 eyes) aged between 20 and 30. For all subjects, testings were performed using cover test, subjective refraction, phoria and vergence test. To analyze phoria variations comparatively for the head tilt test and the face turn test, phoria deviation were measured using the von Graefe. Results: For the phoria variation related with head tilt and face turn, a significant difference was observed in the group judged to be abnormal on the horizontal phoria of Morgan' norm (p<0.05); especially in patients who needed near prescription of Sheard's criterion, a higher variation was observed (p<0.05). The abnormal group was composed of patients who needed distance prescription of Sheard's criterion and ones who did not need the prescription, all of whom showed phoria variations, but the mean value was less than the near. Conclusions: With regard to the head tilt and face turn related phoria level, the patients who needed near horizontal prism prescription of Sheard's criterion showed a remarkably higher variation. Altogether, it is deemed to check whether extraocular muscles are ill-balanced in both eyes previously to determine the prism power on phoria level to the end that each of prism powers may be accurately determined.

The Study on The Property of the Phoria and Asthenopia Inducing Factor in Adults in Jeon-Buk Area (전북지역 성인의 사위특성과 안정피로 유발인자에 관한 연구)

  • We examined refractive error, the corrected visual acuity, phoria and fusional reserve of 114 healthy myopes and hyperopia who had no strabismus, no ocular and physical diseases. We evaluated the occurrence of asthenopia according to phoria and fusional reserve. 41 out of 114 subjects which exophoria was 69% and esophoria was 5.5%, orthophoria was 26% made a complaint against asthenopia. The subjects who had exophoria of $0-6{\Delta}$ in the range of normal state was 61.4%, while the subjects who had exophoria of $7{\Delta}$ or more in the range of abnormal state was 38.6%. Reducing fusional reserve was associated with increasing phoria. The fusional reserve twice or more than phoria were 27.2%, and twice or under were 72.8%. Futhermore, AC/A ratio for heterophoria was found from 1.0 to 12.6 and its relationship to asthenopia of phoria could not be determined.

Effect of Distance Phoria on Stereoacuity (원거리 사위도가 입체시에 미치는 영향)

  • Purpose: In this study we measured the mean value of the distance stereoacutiy for adult subjects and evaluated the variation of the distance stereoacuity according to correction of phoria using the Pola Test E-II of Carl Zeiss. Methods: We investigated the distance stereoacuity for 50 adults (male: 29, female: 21) ranging in age from 19 to 46 years using D6 Circle chart and D8 Line chart in the Pola Test E-II. Results: The mean values of the distance stereoacuity measured on the D6 Circle test and D8 Line test were 159.6 and 149.4 seconds of arc respectively. The mean variations of the distance stereoacuity according to correction of phoria in D6 Circle test were increased 43.2 seconds of arc in exophoria and 51 seconds of arc in esophoria, and those in D8 Line test were increased 42 seconds of arc in exophoria and 70.2 seconds of arc in esophoria. Conclusions: We knew from these results that the distance stereoacuity after prescription of phoria improved more esophoria than exophoria.

The Effects of Visual Training on Binocular Anomalies (양안시 기능 이상의 시훈련(Visual Training)의 효과)

  • In this study, we measured Near Point of Convergence(N.P.C) tests, Phoria tests using Von Grafe method and relative convergence tests on 138 men and 162 women, so a total of 300 subjects aged between 8~45 to examine the improvement of the fusion vergence through visual training and obtained as follows. 1. According to the results, the near point of convergence of 57 (19%) subjects were shorter than 7cm, and 243 (81%) were 7cm or longer, having a problem in convergence. After visual training, the number of subjects have the value shorter than 7 cm increased from 57 to 111 (37%), and the number of those have the value 7cm or longer decreased significantly form 243 to 189 (63%). 2. The results of the measure of lateral Phoria at far distance by Von Grafe method showed orthophoria 18 (6%), exophoria 198 (66%), esophoria 84 (28%). After phoria test, we examined the N.R.C and P.R.C test. The results showed that the hope finger was improved after V.T using B.l, B.O card. 3. The results of the measure of lateral Phoria at near distance by Von Grafe method showed orthophoria 6 (2%), exophoria 222 (74%), esophoria 72 (24%). After phoria test, we examine the N.R.C and P.R.C test. The results showed that the hope finger was improved after V.T using B.I, B.O card.

The Study on the Property of The Heterophoria and Fusional Reserve in Adults in Jeon-buk Area (전북지역 성인의 사위와 융합여력 특성에 관한 연구)

  • The aim of this study was to evaluate the property of heterophoria and fusional reserve in Adults in Jeon-buk Area. We examined the corrected visual acuity, corrected refractive error, heterophoria and fusional reserve of 116 healthy myopes aged from 20 to 44 old who had no strabismus no ocular and phyisical diseases. Using Von Graefe test of horizontal heterophoria Measurement, we measured orthophoria(26.7%), exophoria(52.5%) and esophoria(20.7%) for at near distance. The subjects who had exophoria of 0-6${\Delta}$ in the range of normal state was 38.8%, while the subjects who had exophoria in the range of abnormal state was 61.2%. Reducing fusional reserve was associated with increasing phoria. We found a relationship between asthenopia and fusional reserve of heterophoria and considered that fusional reserve must be examined when we preserve for a patient with heterophoria. Furthermore, Gradient method AC/A ratio was found 4.03 and its relationship to refractive error could not be determined.

Study of Heterophoria Using Torrington Method in Third-year Student High School (토링톤법을 이용한 고 3 수험생들의 사위에 대한 고찰)

  • This research was investigated to find out the examinees' refractive problems within heterophoria using Torrington method. The number of subjects ware approximately 327 and subjects were sourced from the 12th grade student at high school in Kwang-Ju city. Result obtained shows 21.4% of emmetropia and 78.6% of ametropia. Most of refractive errors were myopia and due to it's condition, frequency of ametropia and emmetropia at the similar rate. However, one that consist of refractive problems, it's myopia contains 74.9% of heterophoria. Due to emmetropia, horizontal heterophoria have 45.7% of exophoria and 4.3% of esophoria. Myopia shows 11.4% of esophoria and 63.5% of exophoria. Hozizonda heterophoria shows 5.7% of emmetropia and myopia of 22.4%, than average of esophoria shows $4.3P{\Delta}$ and case of myopia, it was $3.4P{\Delta}$. Case of Exphoria, emmetropia have $8.8P{\Delta}$ and within myopia $8.0P{\Delta}$. Vertical heterophoria owns $2.2P{\Delta}$ within emmetropia and case of myopia $4.3P{\Delta}$ was shown. Which shows myopia result value higher than emmetropia.

Research on Visual Function Before and After Prescription of Vertical Prism (수직사위 교정 전·후 시기능 변화에 대한 연구)

  • Purpose: This study was undertaken to study on visual function before and after prescription of vertical prism. Methods: 80 subjects (10-30ages) who had been precribed vertical prism and 1 year after they were represcribed vertical prism. Results: Phoria (p=0.000), supra vergence (p=0.000), NPC (p=0.003), NPA (right eye) (p=0.005), NPA(left eye) (p=0.000) showed a significant change. Conclusion: Vertical prism prescription was reduced during revisit. Supra vergence, NPC, NPA(right eye), NPA(left eye) showed a significant change. Binocular Accommodative lag was not associated with vertical prism prescription and binocular vision findings.

An Evaluation of Habitual Head Posture in Phoria (사위에서 습관적인 머리위치의 평가)

  • Purpose: The purpose of this study was to evaluate whether a relationship exists between head posture and phoria, and usefulness in examining habitual head posture. Methods: Twenty two subjects (20 males and 2 females, mean age $23.6{\pm}2.7$ years) with abnormal phoria from participants randomized were enrolled in this study. For all subjects, testing included case history, cover test, refraction, phoria and vergence test. Habitual head posture (head posture, head tilt and face turn) was measured by Impression IST, and observed by examiners. Results: The abnormal head posture was revealed in some subjects with abnormal phoria. Spearman' correlation ($\rho$=0.524, p=0.045) showed significant correction between face turn and phoria at distance in 15 subjects with prism prescription. No significant relationship between head posture and phoria was found in subjects with abnormal phoria. The objective measurement and subjective observation of head posture showed insignificant correction but there was a distinct difference. The former was detail and the latter was discriminate. Conclusions: Presence of abnormal head posture was found in phoric subjects. The results indicate the need to observe habitual head posture at all major positions of gaze in phoria.

The Study on Relation between Asthenopia of Lateral Phoria and Fusional Reserve (수평사위의 안정피로와 융합여력과의 관계)

  • The aim of this study was to evaluate the relation between Asthenopia of near lateral phoria and fusional reserve and also to provide fundamental clinical data. A total of 97 subjects, aged between 17 and 35 years old, who had no strabismus, an eye trouble or whole body disease, were examined nacked visual acuity, corrected visual acuity, corrected diopter, phoria, fusional reserve tests from October of 2005 to July of 2006. We excluded 8 subjects for the following reasons: if they had an amblyopia affecting binocular vision or inaccurate data. After these exclusions, 87 subjects remained. The results were as follow. According to interview results was that in near works, exophoria and esophoria with asthenopia was 59.6%, 64.7%, and 52.6% respectively. The subjects who have exophoria of $0-6{\Delta}$ in the range of normal state was 19.1%. The subjects who have exophoria of $7{\Delta}$ over in the range of abnormal state was 80.9%. The fusional reserve was in inverse proportion to phoria. The fusional reserve was twice over of phoria were 30.3%, and twice under were 69.7%. The asthenopia complain persons were 33.9% with the twice over fusional reserve of phoria. The asthenopia no complain persons were 66.1% with the twice under fusional reserve of phoria. In conclusion, our research has shown conclusively that there is a link between asthenopia of lateral phoria and fusional reserve and we also find that fusional reserve must be examined when we prescribe for a patient who has phoria.

A Study and Analysis of Accommodative Convergence/Accommodation Ratio by Measuring Methods (측정방법에 따른 AC/A비의 조사 분석)

  • Purpose: The aim of this study was to propose effective data for the clinical examinations of binocular vision by comparative analysis of measuring the near horizontal phoria according to accommodative stimulations. Methods: It carried out near horizontal phorias, targeting 104 college students (50 males and 54 females) aged between 19 and 24 ($20.27{\pm}1.31$). It made a comparative analysis of changes in near horizontal phoria according to accommodative stimulations by using the von Graefe, Maddox rod and the Howell phoria methods respectively. We have also investigated the AC/A ratio in all phoria groups. Results: As the additional lenses were changed to the negative (-) diopter, the near horizontal phoria changed to the esodeviation. At this time, the rate of change in the section signifies the AC/A ratio, and the values were not consistent based on the evaluating methods or on each section. The AC/A ratio of the esophoria group appeared the largest value in all groups. As a result of analyzing AC/A ratio at the exophoria group using the von Graefe method, the AC/A ratio was $1.568{\pm}1.937$ ${\Delta}/D$ on the additional lenses +2.00 D and $2.527{\pm}2.253$ ${\Delta}/D$ on the additional lenses -1.00 D and at the esophoria group using the Howell phoria method, the AC/A ratio was $5.521{\pm}1.337$ ${\Delta}/D$, $5.593{\pm}1.623$ ${\Delta}/D$ on the additional lenses +2.00 D, +1.00 D and $4.687{\pm}1.643$ ${\Delta}/D$ on the additional lenses -2.00 D. These were significant differences statistically. Conclusions: In the exophoria group, when the (-) lenses were added, the averages of the AC/A ratio were shown to be high but in the esophoria group, when the (+) lenses were added, the of AC/A ratio was high.
