The Effect of Level of Dietary Protein on Kidney Development and Function in Growing Rats (식이내 단백질 수준이 성장기 흰 쥐의 신장 기능에 미치는 영향)

  • This study was performed to investigate the effect of protein intake on kidney development and function in growing rats. Fourty-two male Spraque-Dawley rats of weighing $97.5\pm1.9g$ were divided into 3 groups and given 5%, 15% or 50% casein diets for 6 weeks. Body weight gain was higher in the 50% group. The kidney weight was selectively affected more by the level of dietary protein compared to the other organs. DNA and RNA content were significantly higher in the 15% and 50% groups than in the 5% group but the differences disappeared when DNA and RNA were expressed per g of kidney weight. Protein and protein/g kidney content were increased with increasing level of protein in diet. GFR/animal and GFR/100gB. W. were significantly higher in the 50% group compared to the 5% and 15% groups. There was no differences in PAH clearence and RBF. Osmolality was not affected by dietary protein level. BUN and urinary nitrogen excretion were increased with the increasing dietary protein level. Although urinary Ca excretion was not significantly difference among 3 groups, the rats in the 5% group showed 30% less Ca excretion compared to the other groups. Above results suggest that dietary protein level has a great effect on the kidney weight and GFR in growing rats. Especially the hyperfiltration inhanced by high protein diet may accelerate the kidney senescence.

Importance of Target Blood Pressure Management in Diabetic Kidney Disease (당뇨병성 신장질환 환자에서 적정 혈압 관리의 중요성)

  • In diabetes mellitus, renal disease is a common complication, characterized by increased urinary albumin excretion and reduced eGFR. According to KDIGO CKD stage classification, Korean characteristics were analyzed according to urinary albumin and eGFR using the National Health and Nutrition Examination Survey VI raw data. According to KDIGO classification, diabetic patients were classified as Low risk 72.0%, Moderate risk 19.3%, High risk 5.6% and Very high risk 3.0%. Low risk decreased from 74.7% to 52.2%, and moderate to very high risk increased from 25.4% to 47.8% as the duration of diabetes mellitus was prolonged. The risk factors were CKD stage 1 (HR 2.064) to stage 4 (HR 11.049), the highest risk of hypertension. The incidence of renal disease was elevated according to duration of hypertension and HR 0.42 of kidney disease was decreased in the group maintaining proper blood pressure. In the hypertensive patients, the group administered with target blood pressure had a reduction of the kidney disease by 42% than the group with the hypertension. Therefore, controlling and managing hypertension to target blood pressure is important for the prevention of kidney disease.

Differences in Urine Cadmium Associations with Renal Damage Markers According to the Adjustment with Specific Gravity or Urinary Creatinine (요비중 또는 크레아티닌 보정에 따른 요중 카드뮴과 신장손상지표와의 관련성 비교)

  • In general, specific gravity (SG) and urinary creatinine (CR) have been used to adjust urinary cadmium (Cd) concentrations. However, the validity of correction methods has been controversial. We compared the two adjustments to evaluate associations between urinary Cd and various renal damage markers and to evaluate the relationship between urinary Cd concentration and renal disease markers, such as estimated glomerular filtration rate (eGFR), in a relatively large general population sample. Among the 1,086 volunteers who were enrolled in this study, 862 healthy volunteers who did not have kidney disease were included in the final analysis. Urinary Cd, malondialdehyde (MDA), and N-acetyl-${\beta}$-D-glucosaminidase (NAG) concentrations were measured, the creatinine-based eGFR was calculated, and the relationships between these markers were subsequently analyzed. This study showed the use of urinary Cd concentration adjusted with SG rather than with urinary creatinine may be appropriate in studies evaluating renal function based on Cd exposure. Urinary Cd concentration adjusted with SG had a positive correlation with urinary MDA levels and a negative correlation with eGFR. This relationship was relatively stronger in women than in men. This study showed that urinary Cd level was associated with decreased eGFR in the general population, and oxidative stress was likely to act as an intermediator in this process. These results suggest that eGFR can be a very good indicator of kidney damage caused by Cd exposure in the general population.

Relationship of Glomerular Basement Membrane Alterations to Epithelial Cell Structure and Clinical Parameters in Alport Syndrome (Alport 증후군에서 사구체 기저막의 형태학적 변화와 사구체 상피세포의 구조 및 임상지표와의 관계)

  • Purpose : This study was performed to evaluate the relationship between glomerular basement membrane (GBM) alterations to epithelial cell (EpC) structure and renal function in Alport Syndrome (AS) patients. Methods : Fifteen patients diagnosed with AS (4-26yrs) were examined. The GBM in AS was categorized as : C1) normal, C2) minor alterations (widening of lamina rara interna or externa without lamina densa change), C3) nonspecific splitting of lamina densa, C4) basket-weaving pattern of lamina densa splitting. The length of each GBM portion along the epithelial side was measured on the systematically obtained electron microscopic photographs. Furthermore to obtain an objective assessment of the degree of glomerular EpC foot process change, the number of slit pores along $10\;{\mu}m$ of peripheral GBM in each category was obtained. Results : The percentage of normal GBM portion (C1) correlated inversely with daily protein excretion (g/day/$m^2$, P<0.05) and sum of the percentage of abnormal GBM portion (C2+C3+C4) had direct correlation with daily protein excretion (g/day/$m^2$, P<0.05). There were no significant relationships between the percentages of other categories of GBM alterations and creatinine clearance or protein excretion. There were no significant relationships between of creatinine clearance in relation to normal GBM(C1) portion as well as that in relation to sum of the percentage of abnormal GBM portion (C2+C3+C4). GBM abnormality did not correlate with age at biopsy. Conclusion : The extent of GBM structural abnormality is related to proteinuria in AS but the epithelial response is uniform even though the GBM ultrastructural lesions are not.

The Factors Affecting Accurate Quantitaion of Proteinuria Using Sp ot Urine Protein/Creatinine Ratio in Children (소아 단백뇨 검사에 있어서 단회뇨 단백 크레아티닌 비에 영향을 미치는 요인)

  • Purpose : Many results have reported a correlation between the spot urine protein/creatinine ratio(P/C ratio) and 24-hour urinary protein(24UP) amount. This study was designed to evaluated correlation between 24UP amounts and P/C ratio in children and to find the factors that affect this correlation. Methods : 210 patients who visited the Department of Pediatrics in Busan Paik Hospital from september 2003 to december 2007 were included in this study. All the patients were divided into I, II, III/A, B, C group[I:24UP(mg/$m^2$/day)]<100, II: 100$\leq$24UP<1,000, III: 24UP$\geq$1,000, A: Cr excretion(mg/kg)<15, B: 15$\leq$Cr excretion<25, C: Cr excretion$\geq$25)]. Pearson correlation analysis was performed between 24UP and P/C ratio to evaluate the relationship. We defined fractional difference between 24UP and P/C ratio, and then performed multiple regression analysis. Results : There was a strong positive linear correlation between 24UP and P/C ratio in all patients, and the correlation was also good in each group. The factors affecting accurate quantitation of proteinuria using spot urine P/C ratio was creatinine excretion. Conclusion : Spot urine P/C ratio is a useful test to predict proteinuria roughly. Therefore, we expect that urine P/C ratio can be used as parameter instead of 24UP, if we set cutoff value of P/C ratio considered to creatinine excretion according to age and sex in large pediatric population.

A Study of Bone Uptake According to Renal Function in the Whole Body Bone Scan (전신 뼈 검사에서 신장 기능에 따른 뼈 섭취율에 대한 고찰)

  • Whole body bone scan has been used to confirm bone metastasis and follow-up study with radio isotope. However, if the factors related to $^{99m}Tc$ uptake and waiting time for study are inappropriate, it would be image of low quality. The purpose of present study was to investigate correlation between the evaluation index of renal function and uptake of radiopharmaceuticals. The population for this retrospective study consisted of 387 patients who underwent whole body bone scan between June 2012 and December 2012. As a result of quantitative and qualitative analysis, we were able to confirm that GFR of less than normal range and creatinine levels in blood of more than average are more likely to be under the mean uptake rate. As a result of analysis on the indicator affecting soft-tissue and bone uptake, the correlation of all elements was somewhat low. Also there are no statistically significances due to the other parameters we did not deal with. Therefore, further research on additional factors is needed for exact study and improvement of the image quality.

Development of a Small Animal Positron Emission Tomography Using Dual-layer Phoswich Detector and Position Sensitive Photomultiplier Tube: Preliminary Results (두층 섬광결정과 위치민감형광전자증배관을 이용한 소동물 양전자방출단층촬영기 개발: 기초실험 결과)

  • Purpose: The purpose of this study was to develop a small animal PET using dual layer phoswich detector to minimize parallax error that degrades spatial resolution at the outer part of field-of-view (FOV). Materials and Methods: A simulation tool GATE (Geant4 Application for Tomographic Emission) was used to derive optimal parameters of small PET, and PET was developed employing the parameters. Lutetium Oxyorthosilicate (LSO) and Lutetium-Yttrium Aluminate-Perovskite(LuYAP) was used to construct dual layer phoswitch crystal. $8{\times}8$ arrays of LSO and LuYAP pixels, $2mm{\times}2mm{\times}8mm$ in size, were coupled to a 64-channel position sensitive photomultiplier tube. The system consisted of 16 detector modules arranged to one ring configuration (ring inner diameter 10 cm, FOV of 8 cm). The data from phoswich detector modules were fed into an ADC board in the data acquisition and preprocessing PC via sockets, decoder block, FPGA board, and bus board. These were linked to the master PC that stored the events data on hard disk. Results: In a preliminary test of the system, reconstructed images were obtained by using a pair of detectors and sensitivity and spatial resolution were measured. Spatial resolution was 2.3 mm FWHM and sensitivity was 10.9 $cps/{\mu}Ci$ at the center of FOV. Conclusion: The radioactivity distribution patterns were accurately represented in sinograms and images obtained by PET with a pair of detectors. These preliminary results indicate that it is promising to develop a high performance small animal PET.

Therapeutic Effect of Plasmapheresis in Relapsed Focal Segmental Glomerulosclerosis after Steroid Pulse Therapy (스테로이드 충격요법 후 재발된 국소성 분절성 사구체 경화증에서 혈장반출법의 치료 효과)

  • Purpose : Focal segmental glomerulosclerosis(FSGS) is known to have a poor prognosis and a poor response to treatment. We performed a combination therapy of plasmapheresis, steroid pulse and immunoglobulin in 4 patients with relapsed steroid dependent(SD) or steroid resistant(SR) FSGS after steroid pulse therapy. Materials and Methods : 8 cycles of plasmapheresis were performed in 4 patients with biopsy proven FSGS who had relapsed after steroid pulse therapy from March 1988 to July 2002. Clinical findings and treatment courses were reviewed retrospectively. Results : Among the 4 patients, there were 3 males and 1 female. After 8 cycles of plasmapheresis, clinical remissions were obtained. Two of the four patients had two relapses and received 2 more cycles of plasmapheresis which resulted in remissions. One of these patients had two further relapses and was treated with oral steroid resulting in clinical remission. Three patients have maintained normal serum creatinine level and glomerular filtration rates during the follow-up period of 10 years, and the other 1 patient for 5 months. Conclusion : A combination therapy of plasmapheresis, steroid pulse and immunoglobulin led to a complete remission in patients with FSGS who were SD or SR and was effective in maintaining normal renal function.

Analysis of the Correlation Between Kidney Function Indicators and Kidney Size According Age Groups in Ultrasonography (신장 초음파 검사에서 연령대에 따른 신장 기능 지표와 신장 크기 간의 상관관계 분석)

  • The purpose of this study was to analysis the correlation between kidneys function indicators and these size in ultrasonography. A total of 170 (male:86, female:84) patients of sex and age groups were examined by abdominal ultrasonography. The patients classified as those in their 20's, 30's, 40's, and over 50's. We measured the length, width, and cross-sectional height of the kidneys twice. At this time, the length of these were measured from the maximum upper to the maximum lower pole and the widest width in the same ultrasonography was measured to obtain the cross-sectional area. Other relevant indicators included body surface area, serum creatinine, glomerular filtration rate (GFR), MDRD (Modification of diet in renal disease) and C-G (Cockcroft-Gault). Significant comparisons of differences between relevant factors by age groups and sex were conducted with a one-way distribution analysis. Correlation analysis was also performed between relevant factors by using Pearson and Spearman correlation coefficient. It was defined as meaningful when the p-value was less than 0.05. As a result, the length, the width, and the cross-sectional area of kidneys were correlated with GFR, C-G, MDRD. Therefore, it is expected that the accuracy of diagnosis of kidneys disease will be increased if the relevant indicators are evaluated together rather than measuring only length of these in ultrasonography.

A Case Report of Improvement of Glomerular Filtration Rate and Anemia in Chronic Renal Failure through Herbal Medication (만성신부전 환자에게 한약투여 후 사구체여과율 및 빈혈 호전 1례)

  • Renal failure means irreversible damage and depression of renal function, represented by glomerular filtration rate(GFR). A renal failure patient with uremia and anemia is presented. Anemia, in this patient, appeared before uremia did. A 55 year-old male patient was treated for renal failure with Weilingtang. Improvement was observed in, not only GFR, blood urea nitrogen(BUN), and creatinine, but also in such symptoms as difficulty in urine excretion, dyspepsia and abdominal swelling. But anemia appeared during treatment, so a mixture of Dangguibuxuetang(當歸補血湯) and Xiongguitang(芎歸湯) was prescribed. After this treatment, improvement in hemoglobin, and hematocrit was observed.
