The Differential Diagnosis between Allergic and Nonallergic Rhinitis and Management : Focusing on Current update of Medical History and Physical Examination (알레르기 비염과 비알레르기 비염의 감별 진단 및 관리 : 병력 청취 및 진찰 소견에 대한 최신지견 중심으로)

  • Objective : The purpose of this study is to summarize the differential diagnosis between allergic and nonallergic rhinitis and suggest management. Methods : We reviewed the current update of medical history and physical examination for allergic and nonallergic rhinitis. Then we analyzed clinical characteristics according to onset age, gender incidence and state of nasal symptoms, etc. Results : 1. Patients with nonallergic rhinitis tend to develop symptoms at a later age(>35 years of age), and there is a female-to-male incidence ratio for nonallergic rhinitis of 2:1 to 3:1. 2. Patients with nonallergic rhinitis report nasal congestion, nasal rhinorrhea and more often report postnasal drip rather than sneezing and itching, which are predominant symptoms of allergic rhinitis. And the nasal mucosa in nonallergic rhinitis usually looks normal. 3. Patients with nonallergic rhinitis have few complaints of concomitant symptoms of allergic symptoms and the absence of other atopic diseases in the patient or in the family supports the diagnosis of nonallergic rhinitis. 4. Common triggers of nonallergic rhinitis are nonspecific irritant exposures and many patients with nonallergic rhinitis find that antihistamines have no benefit. Conclusions : The differential diagnosis between allergic and nonallergic rhinitis is clinical and relies on a detailed medical history and physical examination.

Clincal Effect of Ketotifen in Allergic Rhinitis (알레르기성 비염에 대한 Ketotifen($Zaditen^{(R)}$)의 임상적 효과)

  • Ketotifen, a benzocycloheptathiophene, has an orally effective antiallergic as well as antihistaminic properties. In pervious studies, Ketotifen has shown encouraging results in patient with allergic rhinitis, either perennial or seasonal. 39 patients with allergic rhinitis had been treated with Ketotifen 1 mg twice daily for 8 weeks. And we obtained following results. 1) The efficacy rate in sneezing attack was 73.5%, in nasal discharge 71%, in nasal obstruction 58%. 2) Some improvements in at least one of three major symptoms were noted within 1 week in 30.7%, within 2 weeks in 55.8%, within 3 weeks in 66.7%, within 8 weeks in 87.2%. 3) Physical findings such as colour, swelling of turbinate, character of rhinorrhea were not improved significantly. 4) Side effect was observed only in one patient with abdominal pain and diarrhea, which was subsided after interruption of administration. These results suggested that Ketotifen was effective in treatment of allergic rhinitis.

The Effects of Gudambopae-Tang on Changes of Cytokines in Allergic Rhinitis Model (거담보폐탕(去痰補肺湯)이 알레르기성 비염 모델 생쥐의 cytokine변화에 미치는 영향)

  • In order to investigate the effects of Gudambopae-Tang on allergic rhinitis, laboratory study about cytokines and OVA-specific IgE was perfomed in the present study. The results are summarized as follow; 1. IL4, IL-5 was increased in medicating group than control group. 2. $IFN-{\gamma}$ was increased in medicating group than control group. 3. total IgE and OVA-specific IgE was significant decreased in medicating group than control group, statistically significant. This study shows experimentally that Gudambopae-tang could reduce allergic reaction in allergic rhinitis. Advanced studies are required to investigate the mechanisms of herbal medicine in allergic rhinitis model.

  • Objective : This study is to investigate the effect of 8 Constitution Acupuncture on allergic rhinitis patients Methods : We treated 3 allergic rhinitis patients diagnosed as Colonotonia with 8 Constitution Acupuncture by dry needle. The degree of improvement was evaluated by Visual Analogue Scale(VAS) Results & Conclusions : After taking 8 Constitution Acupuncture sessions, the symptoms of allergic rhinitis was improved. After 8 Constitution Diet, the effect of therapy was lingered. This study shows that the allergic rhinitis can possibly be treated not only with herb medicine, but also with acupuncture and diet.

  • Objective : Allergic rhinitis is an allergic reaction characterized by sneezing, coughing, itchy nose, mouth and throat, congestion and/or nasal discharge. We aim to observe effect of Sinichengpae-um on protective effects of nasal mucosal tissue in th allergic rhinitis. Method : For this purpose, we oberserved number of leukocyte and erythrocyte in blood, ratio of neutrophil and lymphocyte in leukocyte, activity of GOT and GPT, and histopathologic change of nasal mucose. Result and Conclusion : Sinicheogpae-um showed effects on immune reaction with no harms liver. And in histopathologic change of nasal mucosal tissue, Sinichengpae-um showed significant protective effet against allergic rhinitis.

  • Objective : We investigated the effects of BOM Gunbisoyeom-tang(Jianbixiaoyan-tang) Gamibang having prescribed clinically to patients with perennial allergic rhinitis. Methods : We prescribed BOM Gunbisoyeom-tang(Jianbixiaoyan-tang) Gamibang for 1 month to 77 patients visiting to KimKiJoon Orentalclinic BOM from January 1 to December 31, 2008 with perennial allergic rhinitis and analysised the questionnaire(symptom severity assessment) before and after treatment. Results : BOM Gunbisoyeom-tang(Jianbixiaoyan-tang) Gamibang improved significantly all nosal symptoms(sneezing, runny nose, congestion, itchy nose and postnasal drip), some non-nasal symptoms(eye and throat symptoms, headache and mental function) except chromic cough and ear symptoms and quality of life. Conclusion : BOM Gunbisoyeom-tang(Jianbixiaoyan-tang) Gamibang cured effectively patients with perennial allergic rhinitis by 1 month treatment but not perfectly. So further studies are needed for treatment duration and changes of pathological index before and after treatment.

  • Allergic Rhinitis is specified by three symptoms such as sternutation, rhinorrhea and rhinanchon. Besides these three major symptoms, it is characterized by sternutation, frontal headache, photophobia and epiphora. The allergic rhinitis occurs when inhaled allergen is in contact with nasal mucosa, which causes immune response. And Tonguetangmi is one of the most frequently used medical treatment for the allergic rhinitis. A study on the 20 patients with allergic rhinitis who took Tonguetangmi a year ago shows that the oral medication for 10 days of Tonguetangmi for an adult who has allergic rhinitis less than one year results in the best efficiency of the remedy.

A Clincal Study on the Effect of Intranasal Acupuncture Treatment on Pediatric Allergic Rhinitis (소아 알레르기 비염에 대한 비내침 치료의 임상적 연구)

  • Objectives: The purpose of this study is to investigate the effect of Intranasal acupuncture treatment in allergic rhinitis patients. Methods: This study was carried with the 42 pediatric allergic rhinitis who had been treated in our hospital. Inner young-hyang was used for the intranasal acupuncture points. Their nasal cavity was observed via anterior rhinoscopy. We induced patient's inferior turbiante bleeding by acupuncture. Patients nasal symtoms and QOL(quality of life) were assessed by the Total Nasal Symptom Score(TNSS) and Quality of Life Questionnaire for Korean Rhinitisian(QLQKR). Results: Total Nasal Symptom Score(TNSS) and Quality of Life Questionnaire for Korean Rhinitisian(QLQKR) were significantly improved. Conclusions: Intranasal acupuncture treatment is relatively effective in treating pediatric allergic rhinitis.

  • Objective : The purpose of this study is to analyze the trends of allergic rhinitis related articles that have been published in the Journal of Korean Medicine Ophthalmology, Otolaryngology & Dermatology(JKOOD). Method : We studied 32 articles of allergic rhinitis published in JKOOD from 1988 to 2018. 24 articles are classified into two categories; treatment-related article and diagnosis-related article which include pattern identification. And we reviewed the other 8 articles briefly in discussion. Results : 16 treatment articles are classified into two categories; 13 case reports and 3 retrospective observation studies. In case reports, the studies are classified into three categories; 3 acupuncture group, 3 herbal medicine group or 7 herbal medicine with other parallel treatments group. Conclusions : Further studies are needed on allergic rhinitis including treatment, alternative medicine and methods of evaluation in order to develop a level of evidence.

The Influence of Atopic Findings on Severity of Pneumonia in Children with 2009 Pandemic Influenza A (H1N1) Infection (2009 신종 인플루엔자 A (H1N1) 폐렴 환아에서 아토피 소견이 폐렴의 중증도에 미치는 영향)

  • Purpose : Atopic findings may be associated with severity of pneumonia in 2009 pandemic influenza A (H1N1) infection, which could suggest a possible association between atopic findings and the severity of viral infections. Thus, we studied association between atopic findings and severity of disease in children with H1N1 influenza infection. Methods : A retrospective study was performed in 74 children admitted in a single tertiary institute and confirmed as H1N1 patients by reverse transcriptase (RT) - polymerase chain reaction (PCR). They were divided into 2 groups according to the severity of pneumonia. We evaluated whether the atopic finding is risk factor between the two groups. Results : Children with severe pneumonia had higher percentages of serum eosinophilia (88% vs 40%, P <0.001), asthma (65% vs 35%, P =0.011), allergic rhinitis (71% vs 40%, P =0.009), and IgE level (P =0.007). We found positive correlations between aeroallergen sensitizations and severity of pneumonia (82% vs 53%, P =0.007). Conclusion : Among patients with H1N1 pneumonia, asthma and atopic findings are risk factors for severity of pneumonia.
