  • 마그네슘은 나트륨, 알루미늄과 함께 지구상에서 가장 풍부한 금속 중 하나로서 밀도가 약 $1.74g/cm^3$으로서 구조용 금속재료 중 가장 가볍고 우수한 비강도를 지니고 있으며, 우수한 열전도도, 전기전도도, 전자파 차폐능을 지닌다. 최근 마그네슘 및 그 합금은 항공기, 자동차, 전자제품, 기계류 및 생활용품 등에 쓰이고 있으며, 사용량 및 적용범위가 매년 급격히 증가되고 있는 추세이다. 그러나 마그네슘합금은 매우 낮은 표준 환원전위와 치밀하지 못한 표면 산화막으로 인하여 부식에 대한 저항성이 매우 취약하다는 한계를 가지고 있다. 따라서 마그네슘합금의 표면처리 가운데 부식에 대한 저항성을 보완할 수 있는 방법은 활발한 마그네슘합금의 응용에 필수적이다. 이러한 마그네슘합금의 내식성을 향상시키고자 전기화학적 플라즈마 전해 산화처리 (Plasma Electrolytic Oxidation)를 하게 되는데, 아노다이징, 화성피막처리 등 과 같은 기존의 산업적 표면처리 방안으로는 불가능한 수준의 표면경도를 확보할 수 있을 뿐만 아니라 두꺼운 산화피막 형성을 통해 이들 합금이 가진 기본적 취약점인 내식성 문제를 보완할 수 있는 장점이 있지만, 다공성 산화피막 형성만으로 기대할 수 있는 내식성 향상 효과가 매우 크지는 못하다. 따라서 다공성의 양극산화피막의 단점, 즉 다공성 물질로 부식성 물질의 이동을 허용할 수 있는 공간을 가지는 구조를 개선시킬 수 있는 추가적인 처리를 필요로 한다. 본 연구에서는 발수성 표면처리를 이용하여 다공성 구조물의 표면이 물에 대한 저항성을 가지도록 함으로써 초발수성 표면을 구현하고자 하였다. 이러한 방법은 기존의 후처리 방법인 봉공처리로는 얻을 수 없었던 다공성 구조물로의 부식성 물질의 침투를 억제할 수 있었으며, 상당한 수준의 내식성 향상 효과를 보여주었다. 또한 물에 대한 반발성은 표면에 물의 이동성을 높이는 효과를 보여주며 이로 인하여 자기세척 효과도 보여주었다.

  • Wegener's granulomatosis is characterized by necrotizing granulomatous vasculitis affecting upper and lower respiratory tracts and kidneys. Vascular lesions commonly involve capillaries and small vessels but, less commonly larger vessels. We report a 46-year-old male patient of Wegener's granulomatosis associated with paranasal sinusitis, pulmonary consolidations, glomerulonephritis, skin lesions with obstruction of both anterior tibial and peroneal arteries. Several necrotic lesions of the toes had progressed to gangrene and both transmetatarsal amputations were done. The patient continued to receive cyclophosphamide and prednisolone.

Clinical trial for efficacy and safety with acupuncture & suction in nasal cavity on snoring patients (코골이 대상자에게 비강사혈흡인요법의 안전성과 유효성을 평가하기 위한 다기관 임상시험)

  • Objectives : The study is to investigate clinical trial for efficacy and safety with acupuncture & suction in nasal cavity on snoring patients. Methods : 24 subjects were recruited from the patients received a hospital visit to the Semyung University Chungju oriental medical center and Jaechon oriental medical center from 1 October 2014 to 31 December 2014. According to the clinical protocol, we treated the patients with acupuncture and suction in the nasal cavity 8 times for 4 weeks. We compared their symptoms between before and after treatment. Results and Conclusions : 1. There was no significant decrease in the mechanical measurement of the reduction of snoring in the effectiveness of snoring patients, but there was a significant effect on the improvement of sleep quality and it was effective in improving the symptoms such as eye fatigue. 2. The safety of nasal treatment for snoring patients was normal in pre- and post-treatment Lab tests, and there was no adverse reaction before and after treatment. Acupuncture & suction in nasal cavity on snoring patients is efficient and safe.

Three Cases of Pityriasis Rosea in Child treated with Eunkyo-san (은교산으로 호전된 소아 장미색 비강진 3례)

  • Objectives : The purpose of this study is to report the effect and safety of Eunkyo-San on pityriasis rosea in children and to suggest antiviral effect of Eunkyo-san. Methods : We performed a retrospective chart review of three cases of pityriasis rosea in children younger than 10 years who treated with Eunkyo-san. Results : One patient recovered after taking Eunkyo-san 10 times for 10 days. The others recovered after taking Eunkyo-san 30 times for 22 days and hyperpigmentation was observed in one of them. Conclusions : It is expected that Eunkyo-san is thought to be effective and safe methods for the treatment of pityriasis rosea in children. Based on existing research, it is thought that Eunkyo-san has antiviral effect through the activation of immunological mechanism, and thus Eunkyo-san appears to have a therapeutic effect on pityriasis rosea.

  • Objective : The purpose of this study is to report a case that has an important meaning as a result of treating Hunter syndrome patient with oriental medicine for elevation of quality of life and continuous health care. Methods : The patient complained of abdominal dropsy, abdominal pain, constipation, frequent gases and the common cold. So we treated him with herbal medicine, acupuncture, infra red, laser, CEP nebulizer, aroma massage and cupping. Results : After treatment, the girth of abdomen didn't increse any more and abdominal pain, constipation, frequent gases and common cold were improved. Conclusion : This study shows that oriental medicine can elevate the Hunter syndrome patient's quality of life with continuous heath care and treatment for chief complaint. The further study is needed with more cases.

Detection of Human papillomavirus & Ebstein-Barr virus in Sinonasal Inverted papilloma (비강 및 부비동의 반전성 유두종에서 인형유두종 및 Ebstein-Barr 바이러스의 검출)

  • The etiology of inverted papilloma(IP) remains unknown, but several studies have reported that Human Papillomavirus(HPV) may play a role in the pathogensis of sinonasal inverted papilloma(IP). And recent reports demonstrate the possible etiologic role of Epstein-Barr virus (EBV) in sinonasal IP. The aim of this study is to detect HPV and EBV in sinonasal IP, to examine the relationship between HPV subtype and sinonasal IP, to investigate the relation between HPV and EBV. We reviewed 30 cases of sinonasal IP(simple IP 19 cases, IP with dysplasia 8 cases, IP with squamous cell carcinoma 3 cases). Paraffin embedded archival tissue was used in this study. Detection of HPV, EBV were examined by in situ hybridization(ISH) using HPV type 6/11, 16/18, 31/33/35 DNA probe and EBER probe. The HPV was detected in 6(20%) out of 30 cases. The HPV 6/11 was dectected in 4 out of 19 cases of simple IP, HPV 16/18 in 1, HPV 31/33/35 in 1 out of 8 cases of IP with dysplasia respectively. The EBV was not detected in 30 cases. HPV may play a role in the pathogensis of sinonasal inverted papilloma. But EBV is not a etiopathologic factor to be considered in the development of sinonasal IP.

Histopathological observations and virus detection by in situ hybridization in wild rats intranasally infected with Aujeszky's disease virus isolated in Korea (국내분리 오제스키병 바이러스로 비강접종한 야생집쥐(Rattus norvegicus)의 병리학적 소견 및 in situ hybridization에 의한 바이러스 동정)

  • The present study was carried out to investigate the pathogenicity and pathogenesis of wild rats(Rattus norvegicus), trapped in nature, intranasally infected with Aujeszky's disease virus(ADV/NYJ-1-87) by histopathology, immunohistochemistry and in situ hybridization(ISH). Fifteen rats inoculated intranasally were roughened haircoat, anorexia, listlessness, and depression second day after inoculation, and three rats died in 66-72 hours. Eight rats showed severe pruritus at the face that was accompanied by frequent face-washing movements of the forelegs, and then became violent and spasmodic for an hour or until they died. Four rats slowly recovered after showing mild clinical signs of the disease. Microscopic lesions in infected rats were characterized by meningitis, perivascular round cell infiltration, focal gliosis, and neuronal degeneration and necrosis. And intranuclear inclusion bodies were frequently detected in the cerebral cortex and medulla. Positive reaction to ADV by immunohistochemistry and ISH were detected in the following areas : trigermimal ganglion, brain, tonsil, nasal mucosa, spleen, lung and liver. The result has suggested that ADV intranasally infected in wild rats is followed by replication in epithelial calls of nasal mucosa and tonsil, then invade local lymph nodes by way of the lymphatics. It is also believed that the virus invades bipolar olfactory cells and trigerminal ganglion; and then spread into central nervous system.

Nasal adenocarcinoma in a dog (개의 비강샘암종 증례)

  • A 10-year old female Yorkshire terrier with nasal discharge and swelling was referred to the local animal hospital. Abnormal mass of right nasal cavity was detected in physical examination and radiography. According to the radiographs of the head, there was an evidence of bony destruction in right nose. Oronasal fistula was detected in right maxillary canine teeth. After surgical excision, the sample of nasal mass was refereed to Pathology Department of Veterinary Medicine in Jeju National University. Grossly, the enlarged mass was soft and 3 ${\times}$ 3 cm in size. Histopathologically, the neoplastic mass was composed of tubular to tubulopapillary structures which were lined by single to 6~7 layers of cuboidal to ciliated columnar cells. These neoplastic cells showed invasive tendency to adjacent normal parenchyma. They had uniform, round to oval nuclei, cytoplasm with small vacuoles and indistinct cellular margin. The number of mitotic figures was varied in different areas, ranged from 0 to 4 per high power field. Necrotic foci and infiltration of inflammatory cells including neutrophils, lymphocytes, and plasma cells also presented in the mass. Immunohistochemically, the neoplastic cells demonstrated strong positive reaction for cytokeratin (CK) 18 but were negative for CK 7 and 8. Based on the gross, histopathology and immunohistochemistry, this mass was diagnosed as nasal adenocarcinoma originated from respiratory epithelium.

Intranasal Phototherapy for Allergic Rhinitis : a systematic review (알레르기 비염의 비강 내 광 치료 : 체계적 문헌고찰)

  • Objectives : We investigated the effectiveness and safety of intranasal phototherapy for allergic rhinitis (AR). Methods : We searched 8 electronic databases (PubMed, Cochrane Library, CNKI, CiNii Articles, OASIS, NDSL, KISS, KMbase) to identify randomized controlled trials (RCTs) that reported the use of intranasal phototherapy for AR from their inception until May 30, 2020. Two investigators independently searched, collected, and screened the RCTs. We performed data extraction and evaluation for risk of bias using the Cochrane risk-of-bias tool. Results : This study included 12 RCTs; six studies compared intranasal and sham phototherapy, of which four studies reported a significant inter group difference and two studies reported a significant difference partially. No significant changes in symptoms were observed between the phototherapy and conventional therapy groups. The phototherapy and concurrent acupuncture treatment group showed a significantly higher effectiveness rate compared with the group that received only acupuncture. Both the phototherapy and laser acupuncture group showed significant improvement in the symptom severity scale scores. Six studies reported mild adverse effects, such as dryness and nasal pain in the intranasal phototherapy group; however, no severe adverse effects were reported. Conclusions : This study confirmed the safety and effectiveness of intranasal phototherapy for symptom relief and improved quality of life in patients with AR. However, further studies are needed on this topic in order to demonstrate it clearly.

A Case of Respiratory Difficulty Due to Congenital Tracheal Calcification and Nasal Pyriform Aperture Stenosis (선천성 기관 석회화와 조롱박 구멍 협착 (Nasal Pyriform Aperture Stenosis)에 의한 호흡 곤란증 1례)

  • Nasal obstruction is a cause of respiratory distress in newborns. The congenital nasal airway obstructive abnormalities are classified into three forms according to the location: posterior choanal atresia, nasal cavity stenosis and congenital nasal pyriform aperture stenosis(CNPAS). CNPAS is located at the anterior part of the nasal fossa. CT is the study of choice to make the diagnosis of CNPAS and rule out other causes of nasal obstruction. Though conservative management of CNPAS is recommended, in cases of severe CNPAS surgical treatment should be considered. Calcification of cartilage in the larynx, trachea and bronchi is extremely rare in children. Such calcifications are generally discovered in young children with congenital stridor. The clinical course is favorable. No case with CNPAS and tracheal calcification is reported in newborn. We report a one-day-old girl with CNPAS and tracheal calcification who presented with respiratory difficulty immediately after birth.
