A Study on the Performance Evaluation of Effectiveness and Satisfaction of Veteran Medical Service Delivery System : Focused on the Perspective of Provider and Beneficiary (보훈의료서비스 전달체계의 효과성과 만족도에 관한 성과평가 연구 : 공급자 측면과 수요자 측면을 중심으로)

  • This study examines determining factors of effectiveness and satisfaction of Veteran Medical Service Delivery System. Especially, the association between the relevant variables of the effectiveness of the Veteran Medical Service Delivery System and the variables of the satisfaction from the perspective of beneficiaries was studied. Multi-level analysis was utilized to separate results of the evaluation of effectiveness in organizational-level and the evaluation of satisfaction in individual-level. This study tests key posited hypothesis by using survey data collected from 5 medical center of country(Seoul, Busan, Daejeon, Daegu, Gwangju). In terms of the result of the hypothesis testing on the effectiveness variable, integrity(${\beta}=.156$), accountability(${\beta}=.376$, financial sufficiency(${\beta}=.109$), and adequacy (${\beta}=.367$) are the determinants among various factors in evaluating veteran medical service delivery system, statistically reflecting the perception of directors of the veteran medical service delivery facilities on effectiveness. In other word, professionalism variable(${\beta}=0.99$) and effectiveness variable(${\beta}=-1.09$) are statistically reflecting the perception of directors of the beneficiaries satisfaction with employee. The findings suggests that the theoretical and practical implications will improve Effectiveness and Satisfaction of Veteran Medical Service Delivery System.

The Effect of Dentist's Attitudes on Patient Satisfaction (치과의사의 태도가 환자 만족에 미치는 영향)

  • This study amis to investigate the effects of dentist's attitudes on patient satisfaction. To achieve research purpose, we used 2020 Medical Service Experience Survey (MSES) carried out by Korea Institute for Health and Social Affairs(KIHASA). The data of 560 patients was analyzed using SPSS Ver. 26.0. The main results are as follows. According to the regression analysis, 'politeness', 'empathize with anxiety', 'enough conversation', and 'easy-to-understand explanation' are influential factors to patient satisfaction. This study is meaningful in that it provided basic data for improving the satisfaction of patients who visited the dentist and the service quality of dental medical institutions.

A Study on Drug awareness information, Usage and Misusage on Elderly Inpatients of Veterans Hospital (보훈병원입원노인환자의 약물정보인식, 약물사용 및 오용실태에 관한 연구)

  • This study is descriptive research, attempts to determine the status of elderly inpatients of veterans hospital' of drug awareness information, drug use status and drug Misusage. The subjects were 220 patients of aged 65 and over only five metropolitan areas nationwide in drug use in the Veterans Hospital, national merit, the study is from May to June. SPSS Win 18.0 program was used as the data analysis and a statistical statement by mean, standard deviation, t-test and ANOVA, and multiple regression analysis. As a results, there was showed a statistically significant level 'age', 'type of marriage', 'education', 'triage', 'health status' as the general characteristics of the subjects. It showed the 'age' of the variables that influence recognition in the area of drug awareness information, and drug Misusage and drug use status factors seemed to affect the 'health status'. Therefore, It need for a comparative study of elderly inpatients and inpatients of veterans hospital, the research should be qualitative research of veterans medical services to the on reasonable support measures in the Veterans Affairs health care system and national merit.

The Effect of integrated health and welfare services on the control of medical use and medical expenses in Korean Veteran's Hospitals (보훈병원 통합의료복지서비스가 의료이용과 의료비 절감에 미치는 효과 분석)

  • This study aims to examine the effect of integrated health and welfare(IHW) services on medical use and medical expenses in Korean Veteran Hospitals. Data on the volume of medical use and medical expenses were collected from two patient groups of 5 Veterans' Hospitals: the beneficiary group who has received IHW services and the non-beneficiary group who has not. Each group was composed of 265 patients respectively. The results of the study revealed that the volume of home nursing services, home rehabilitation services, ambulatory services have increased significantly, whereas the average length of stay, number of using emergency services, and average medical expenses have decreased after receiving integrated health services in the beneficiary group. In contrast, the non-beneficiary group showed much more increase both in the volume of inpatient and outpatient services, as well as the average medical expenses, compared to those of the beneficiary group. This results imply that the provision of IHW services have positive impacts on the enhancing the effectiveness of the medical resources utilization for the veteran patients.

Characteristics and Death Risk Factors of Patients in Long-Term Care Hospital Connected to Special Hospital (일개 특수 요양병원 환자들의 특성 및 사망 위험요인)

  • The objective of this study is to provide high-quality medical service, by understanding the characteristics and death risk factors of patients at V long-term care hospital, that has established and operated the cooperative treatment system in connection with a public institution, the S veterans hospital. A follow-up survey targeting a total of 850 discharged patients was conducted for the year 2017. Average age of patients was 79.2 years, with 86.4% belonging to the 70 years or over group. The men of national merit were 75.4%, which was more than three times higher than general patients. Hospitalization for 180 days or more was 46.4%, which was higher than general long-term care hospitals (37%). The major death risk factors were age and the number of hospitalized days. Our data indicate that transfer to S veterans hospital results in providing quicker treatment and higher effectiveness of the numerous recovery therapies. We therefore propose that the cooperative treatment system should be expansively operated to increase the quality of happy life, by alleviating the health of patients with chronic diseases in a long-term care hospital.

No-Show Related Factors for Outpatients at a Hospital (병원 외래환자의 예약부도 요인)

  • In order to maximize the efficiency and the quality of care in a hospital, hospitals in general operate by appointments. Patient no-shows or missed appointments waste the time of staffs who prepare for appointments, deprive patients of needed care, and increase burden on clinic capacity. The purpose of this study is to examine the importance and significance of factors that affect patient no-shows. The actual data of 50,000 outpatient appointments is compiled and analyzed from a VAMC in the United States with aims to identify the factors that contribute to outpatient no-shows and to draw meaningful implications. Random forest along with logistic regression analysis identify the factors affecting no-shows; appointment leadtime, show-up rate, travel distance, no-show rate in previous period, patient age, severity, complexity of disease, chronic pain, depression, drug dependence. The appointment leadtime, show-up rate, travel distance and previous no-show rate can be improved by enhancing SMS pre-notification and by improving afterwards telephone counseling. For other factors, it needs to study on the service differentiation with the characteristics of each patient group.
