SUPPLEMENTAL PERMANENT MAXILLARY LATERAL INCISORS(DOUBLE LATERAL INCISORS) : CASE REPORT (상악 측절치에 발생한 보충치형의 과잉치 : 증례보고)

  • Supernumerary teeth are in excess of the normal number of teeth in either the primary or permanent dentitions. They are classified into supplemental teeth resembling those of the normal series and rudimentary teeth with abnormal shapes, according to their form. Most of the supernumerary teeth are rudimentary form, and supplemental teeth are much less common. Sulppemental teeth are most common in the permanent maxillary lateral incisor area and clinicians should be careful with differential diagnosis from normal teeth. Unerupted supernumerary teeth may produce several complications such as delayed eruption, displacement of permanent teeth, diastema, root resorption and cyst formation. Early detection and proper treatment plan according to the tooth alignment and root formation stage are important. Here we report 3 cases of unilateral or bilateral normal incisor shaped supernumerary lateral incisors treated by eruption observation, surgical extraction and orthodontic treatment with resin build-up.

  • Supernumerary teeth are teeth which result from the continued budding of the enamel organ of the preceding tooth or from excessive proliferation of cells. They are most often found in the maxillary anterior region. And they can be responsible for a variey of irregularities in the primary and transitional dentition. There are two morphological types of supernumerary teeth, supplemental and rudimentary. Supplemental teeth have normal shape and size. In contrast, rudimental teeth have abnormal shape and smaller size. Supplemental supernumerary teeth are most common in permanent lateral incisor area. Its extraction must be decided more carefully with differential diagnosis between normal teeth, because it has normal shape and size. We reports 3 cases of the normal incisor shaped teeth in the maxillary anterior region. In all cases, we used the 3D Dental-CT as well as the conventional plain film such as periapical, occlusal, and panoramic radiograph. Consequently, 3B Dental-CT was valualble to figure out the exact position and morphology of supernumerary teeth, to do more conservative surgery and to reduce surgery stress and time.

  • Supernumerary teeth are dental abnormalies in the permanent dentition and the primary dentition. The etiology is unclear, but it may occur due to dichotomy of the tooth bud or hyperactivity of dental lamina. They occur more in the permanent dentition than in the primary dentition, with the most common site being the premaxillary area. Supernumerary teeth can be classified by morphology and position. Supplemental tooth refers to normal shape tooth. The treatment of supernumerary teeth depends on its shape, position, effect on dentition, and child's physiological condition. In this case, supernumerary primary tooth in the maxillary molar area was revealed by radiographical and clinical examination, but it was difficult to determine which is the supernumerary tooth. The tooth on the mesial side was extracted to induce the formation of adequate space and to prevent excessive space loss, and the result was favorable.

  • A child diagnosed with congenital hypothyroidism after newborn screening and follow up thyroid function test at 1 month of life in another general hospital demonstrated euthyroid state with thyroxine( $T_4$) supplementation until the age of 22 months of life, when he was transferred to our hospital, where he was diagnosed as thyroxine binding globulin(TBG) deficiency with low $T_4$ and TBG. Withdrawal of $T_4$ at age of 26 months was associated with hyperthyrotropinemic hypothyroidism. This patient is a case of TBG deficiency associated with hypothyroidism, and in rare instances, TBG deficiency may lead to hypothyroidism requiring hormone supplementation.

  • 암석의 인장강도 실험법의 하나로써 새롭게 개발된 Hoop Test에 대한 이론적 뒷받침을 하고자 유한요소법을 사용하여 응력분포를 해석하였다. 간단하며 사용하기 편리한 실험장치이지만 시료가공시 생길 수 있는 내부공의 지름에 대한 오차, 재부공벽면과 하중장치외벽과의 마찰 등이 있어 이들로부터 발생할 오차를 평가해 보았다. 또한 시료의 안지름과 바깥지름의 비율이 응력의 크기에 끼치는 영향을 조사하였다. 응력해석결과, 일반적인 시료준바에 필요한 주의만 기울이면 실험치에 별다른 영향을 주지않는 것으로 제안되었고 이는 그동안의 실험결과의 신뢰성을 이론적으로 보충해 주었다.

Portland-Blended Cement with Reduced CO2 using Trass Pozzolan (화산회 가루를 사용함에 의한 CO2-저방출 포트랜드-혼합 시멘트 제조)

  • This paper reports the use of supplementary cementing materials (SCMs) derived from local resources, for the partial replacement of Portland cement to reduce $CO_2$ emission during cement production. Replacement of Portland clinkers up to 20 wt.% with SCMs in normal cements reduced $CO_2$ emission by 0.18 kg $CO_2$/kg. The compressive strength exceeded the standard specification for Portland cement ASTM C-150. Blended cement samples containing 20% Portland clinker replacement had compressive strengths of 37 MPa after 28 days of curing time. The microstructure evolution of blended cement at a composition of 80:20 was similar to that of the 100% Portland cement, where the structure between days 28 and 56 reached a steady state. Blended cements with compositions of 70:30 and 60:40 still showed progress of CSH plate formation and the lack of massive structure development. It is shown that the use of supplementary cementing materials could be as one of alternative ways to reduce $CO_2$ emissions during cement production.

Effect of Enzyme Replacement Therapy on Growth in Korean Patients with Mucopolysaccharidosis Type I (I형 뮤코다당증 환자들에서 효소 보충 요법이 성장에 미치는 영향)

  • Purpose: Mucopolysaccharidosis type I (MPS I) is a lysosomal storage disorder caused by deficiency of the enzyme ${\alpha}$-L-iduronidase, which leads to a broad spectrum of multisystemic manifestations. Short stature and decreased growth velocity are prominent features of MPS I. The aim of the present study was to evaluate the effect of enzyme replacement therapy (ERT) on growth of Korean MPS I patients from a single center. Methods: Height data were obtained by retrospective chart review of 10 Korean patients with MPS I who had received ERT for a minimum of 3 years. Height was expressed as standard deviation scores (SDS) based on normative data. Annual growth rates were calculated before and during ERT. A piecewise regression model was used to analyze height z-scores before and after treatment. Individual analysis was performed for impact of phenotype [(severe (Hurler) versus attenuated (Hurler-Scheie, Scheie)] on growth. Results: Annual growth was 3.3 cm (z-score= -0.21) in the year before ERT and 6.2 cm (z-score= 0.17), 5.8 cm (z-score= 0.07), and 3.8 cm (z-score= -0.4) in the first, second, and third years of ERT, respectively. Regression analysis showed improvement in the slope after ERT (difference= 0.04; P=0.022). Estimated slope differences between severe and attenuated phenotypes were statistically significant before (P=0.001) and after treatment (P<0.0001), although no significant difference was noted when stratified by phenotype. Conclusion: ERT with aldurazyme appears to have a positive impact on linear growth in patients with MPS I.
