Full mouth rehabilitation with reorientation of occlusal plane using facial scan: a case report (교모 환자에서 안면 스캔을 활용하여 교합 평면을 재설정한 전악 보철 수복 증례)

  • The most critical aspect of full-arch prosthodontic treatment is evaluating whether the patient's vertical occlusal dimension is appropriate, and if necessary, restoring it through increasing vertical dimension. If the vertical occlusal dimension is too low, it can lead to reduced chewing efficiency, as well as not only aesthetic concerns but also potential issues like hyperactivity of muscles and posterior displacement of the mandible. This report is about the patient dissatisfied with pronunciation and aesthetics due to an inappropriate vertical occlusal dimension resulting from prior prosthetic interventions, underwent full-arch prosthodontic restoration treatment. Through the utilization of digital diagnostic apparatus, a comprehensive evaluation was undertaken for patient's vertical occlusal dimension, occlusal plane orientation, and the condition of prosthetic restorations. Through 3D facial scanning, the facial landmarks were discerned, and subsequently, the new occlusal plane was established. This provided the foundation for a digitally guided diagnostic wax-up. An elevation of 5 mm from the incisor was determined. Comprehensive dental rehabilitation was then executed for all remaining teeth, excluding the maxillary four incisors. The treatment protocol followed a systematic approach by initially creating implant-supported restorations on both sides of the dental arch to establish a stable occlusal contact. Subsequently, prosthetic restorations for the natural dentition were generated. Diagnostic and treatment planning were established through the utilization of facial scanning. This subsequently led to a reduction in treatment complexity and an expedited treatment timeline.

  • Physical factors and intra- and extra-oral factors of a patient have to be considered in order to decide a treatment plan for the skeletal class III malocclusion patient. Most of cases, the pre-prosthodontic treatment requires the orthodontic approaches and maxillofacial surgery. However, in some cases, patients' economic or medical condition makes impracticable situation for the orthodontic or surgical intervention. For those cases, the compromised prosthetic treatment which provides more stable and persistent occlusal stabilization is recommended. In this case report, a woman patient has a skeletal class III maxillomandibular relationship and misses multiple teeth. The prosthetic treatment without orthodontic and surgical intervention is performed due to her physical problem. The functional and esthetic results are achieved by the fixed prosthesis.

  • In edentulous patients, the advantages of prosthodontic therapy using oral implants include both stability and comfort. Advantages suggested for this approach include the prevention of mandibular bone resorption and increased masticatory function. Implant hybrid prostheses place 4 to 6 implants between the mental foramens, and cantilevers are used to replace the posterior teeth at both ends. The 85 - year - old female patient visited our clinic with complete edentulism in the maxilla and mandible. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using removable complete denture in the maxilla and implant hybrid prosthesis in the mandible.

  • 상악골이 부분적으로 결손된 환자의 증가와, 수술 후 수명의 연장으로 좀 더 확실한 보철물이 요구되고 있다. 본 증례에서는 종양으로 인해 상악골의 Leoisection을 시행한 2명의 환자에 대하여 obturator를 제작, 창착하여 발표, 심미, 기능 및 심리적인 개선이 이루어졌다. 그러나 악골 자체의 부분의 소실로 외치의 지지 및 유지등에 한계성이 있으므로, 장래의 Hemimaxillectomy가 시행된 악궁에 대한 의치의 설계에 관한 더 많은 연구가 진행되어야 하겠다.

Modified resin-bonded fixed partial dentures utilizing tooth undercuts: a clinical case study (서로 다른 삽입로를 이용한 최소 삭제 접착성 고정성 보철물의 임상 연구)

  • The object of this clinical study was to evaluate the short-term outcome of modified resin-bonded fixed partial dentures which utilizes the original tooth undercuts and different path of insertion of components. Materials and methods: 71 units of modified RBFPDs that were used in 21 patients at the Department of Prosthodontics, College of Dentistry, Yonsei University were evaluated. The follow-up period was up to 25 months, the mean being 7 months. Survival rate, mobility, percussion, probing depth, bleeding on probing, plaque index was recorded and radiographs were taken to monitor alveolar bone loss. Results & Conclusion: Within the limits of this short term retrospective study, it was concluded that: 1. No mechanical failure such as debonding or fracture of the framework was found during the follow-up period. 2. The periodontal apparatus was stable and no clinical change was observed after prosthetic treatment. 3. No significant marginal bone loss was found in the radiographic evaluation.

  • Oral lichen planus (OLP) is a common mucocutaneous disease, which presents as bilateral or multiple lesions. The several factors are implicated in etiology of OLP such as dental restorations, systemic disease, drugs and stress. Especially the influence of dental restorations and plaque control for OLP has been the interest in dentistry. This case is about OLP patient having poor contoured and ill-fitting metal ceramic restorations. The patient in this study has suffered from OLP for a long time, particularly after restoration of metal ceramic restorations on both posterior teeth of maxilla and mandible. This study reported that OLP lesion recovered effectively by improving the contour and fitness of restorations with plaque control.

  • When restoring edentulous patients with lower complete denture, the smaller supportive and retentive area of mandible can lead to poor support and stability, denture dislodgement and pain resulting discomfort. In this situation, implant prosthesis can improve esthetics, stability and occlusal force. Whereas, patients with a upper complete denture can adjust more easier because of palate. Therefore, it is suggested to rehabilitate fully edentulous patients with lower implant-supported, upper complete denture as one of the treatment options. So, we are going to report the case and literature review about how the lower implant prosthesis opposing to upper complete denture affects the bone resorption of maxillary residual ridge.
