Influential Factors for Professionalism of Community Health Practitioners(CHPs) (보건진료전담공무원의 전문직업성에 미치는 영향요인)

  • This study was conducted to determine factors affecting professionalism of community health practitioners (CHPs). The participants in this study were 153 community health practitioners working in G province with structured self-report questionnaires from 20 August to 25 September 2017. Data were analyzed by descriptive statistics, t-test, ANONA, Scheffe's test, Pearson's correlation coefficients and stepwise multiple regression using the SPSS 20.0 program. There were statistically significant differences according to income (F=6.951, p<.001), work experience (F=5.245, p=0.002) and motivation for choosing a community health practitioner (F=3.676, p=0.004). The highest related factors were job satisfaction (${\beta}=0.320$, p<0.001), individual disposition (${\beta}=0.291$, p<0.001), income (${\beta}=0.283$, p<0.001) and job continuance (${\beta}=0.176$, p=0.009). These variables explained 49.6% of the total variance in professionalism. These findings suggest that it is necessary to develop a convergence program and policy support at the individual and organizational level to strengthen the professionalism of community health practitioners.

Hypertension Management Status in Vulnerable Rural Areas and the Role of Community Health Practitioners (농어촌 의료취약지역에서의 고혈압관리 실태와 보건진료전담공무원의 역할)

  • Purpose: This study was a descriptive study to investigate correlations between self-care behaviors and quality of hypertension management by hypertensive patients being cared by community health practitioners. Method: Participants were 583 people who were prescribed antihypertensive medications at a health clinic located in South Chungcheong Province. The survey was done from June 1 to July 30, 2018. A self-report questionnaire was administered, and data analysis was performed using descriptive statistics and Pearson correlation coefficients with the SPSS 24.0 program. Results: The rate of awareness, treatment, and control of hypertension were 97.9%, 99.1% and 92.8%, respectively. The number of hypertensive self-care behaviors was 1.82 (${\pm}0.36$) out of 3 points. The quality of hypertension management was 3.22 (${\pm}0.46$) out of 4 points. There was a moderate correlation between hypertensive self-care behaviors and quality of hypertension management (r=.340, p<.001). Conclusions: Results of this study confirm that the quality of hypertension management by community health practitioners is related to self-care behaviors of hypertensive patients. Therefore, it is necessary to improve the quality of hypertension management by health care specialists for self-management of hypertension patients. In addition, a systematic program to improve the quality of hypertension management by community health practitioner is needed.

Job Competencies and Educational Needs Perceived by New Community Health Practitioners (신규 보건진료전담공무원이 인식한 직무역량과 교육요구)

  • Purpose: This study was performed to identify job competencies and needs for job education perceived by new community health practitioners. Methods: This study used a qualitative research design. Eight new community health practitioners participated in this study. Data were collected through in-depth interviews and analyzed using content analysis. Results: Five job competencies were identified in this study, including primary care, public health management, interpersonal relationship, teaching and counseling, and leadership. The contents of job education that they needed were management of major symptoms and chronic diseases, understanding and prescription of medications, emergency responses and care, management of endemic diseases, planning and management of public health programs, writing official documentsand computer works, and leadership training. The learning methods they preferred were connecting theory and practice, situation- or case-based learning, skill- or practice-based learning, and increased opportunities of clinical practice. Conclusion: The findings of this study provided the direction of job education for new community health practitioners. Job education for new community health practitioners needs to consider the job competencies and educational needs identified in this study.

Case Report on a Community Health Practitioner's Health Survey (보건진료소의 관할 지역 건강조사 사례)

  • Purpose: This case study was done to describe the health survey conducted by a community health practitioner. Methods: The community health survey was carried out from April 16 to May 31, 2018 with face to face interviews done by 48 trained senior nursing students. Results: Compared with other regions, rates for chronic disease prevalence, chewing discomfort, falls, and depression were higher than those of the relevant district/the relevant city, and the whole country. It is encouraging that the treatment rate for hypertension, diabetes, and hyperlipidemia, and walking practice rate were higher than those other regions. Those who participated in village events had low stress, and those who participated in health promotion programs had a higher quality of life. Conclusion: The community health practitioner in the public health center post needs to operate health promotion programs continuously. Programs including chronic disease management, fall prevention, depression control, and oral health management should be emphasized, and ways to increase social participation, including participation in village events should be developed.

The Effect of Health Promotion Program on the Frailty of Rural Elderly Women Implemented at Primary Health Care Posts (일부 보건진료소에서 실시한 건강증진프로그램이 농촌여성노인의 노쇠에 미치는 효과)

  • Objective: This study was conducted to examine the effects of a health promotion program on the health condition of rural elderly women implemented at primary health care posts using Korean Frailty Index. Methods: The research was designed a nonequivalent control group pretest-posttest setting. The participants in this study were 50 residents (intervention group: 25, control group: 25) selected from 4 villages 2 primary health care posts in J city Gyeongsangnam-do. The health promotion program was conducted at the primary health care posts twice a week for 12 weeks. This program consisted of basic exercises(Gukseondo + Theraband muscle strength training) and additional activities(including modified Theraband activity, rubber ball exercise, ball massage, nutrition class, singing class). Collected data were analyzed by descriptive statistics, paired t-test, and repeated measures ANOVA with SPSS 21.0. Results: Results of the health promotion program showed that the health conditions(measured by perceived health status, frailty score, upper/lower flexibility, maximum grip strength, dynamic balance test Timed Up and Go) of the experimental group(25) all statistical significantly improved. Conclusion: Study findings indicate that the health promotion program implemented at primary health care posts on rural elderly women is effective and can contribute to a developed health promotion program for local residents in the future.

Education Need of the Visit ing Health Service Workers in Gwangju and Jeollanam-do Public Health Facilities (일부 공공보건기관 방문보건요원의 교육요구도 조사)

  • This study was aimed to provide the basic data for the development of effective educational program by reflecting the opinions of the visiting health service workers. The subjects were 144 visiting health service workers in Gwangju and Jeollanam-do area who responded the mail questionnaire. The data were collected from June to July, 2001 using questionnaire composed of the education need, knowledge by subjective appraisal, and experience of education. The major findings of this study were as follows: 1. The number of respondents who received at least one education within recent three years were 43(29.9%) at the central level, 57(39.6%) at the provincial level and 53(36.8%) at the district level. The satisfaction with education was higher at the central level than at the provincial and district level. 2. Knowledges by subjective appraisal on the 'chronic degenerative diseases management' and 'health promotion' was relatively high. while that of 'rehabilitation' was low. 3. The knowledge by subjective appraisal of visiting health service was related with experience of education and license status. The knowledge was higher in registered nurses than in nurse aids. The curricula related to increased level of knowledge of visting health service workers were 'elderly health care', 'rehabilitation' and 'psychiatric-mental health nursing' educations at the central level 'continuing education for the community health practioners' and 'psychiatric-mental health education' at the provincial level and 'elderly health care', 'rehabilitation' 'psychiatric-mental health' and 'acute diseases control' educations at the district level. 4. The respondents preferred elderly health management as the contents of education, officer group education as the method of education, province(30.4%) as the main body of education, exercise and practice as the form of education, 2-3 times per year as the frequency of education, and 3-5days as the period of education. The findings of this study could suggest that future education program should be planned to increae the knowledge level of visiting health service workers by reflecting their educational need.
