Purpose: The purpose of this study was to investigate the extent of empowerment, task performance and job satisfaction perceived by hospital nurses and the relationship among those variables. Methods: The sample consisted of 261 nurses from a university affiliated hospital. Data were collected with self-administrated questionnaires including empowerment, performance and job satisfaction and analyzed using mean, standard deviation, ANOVA and $Scheff{\acute{e}}$ test. Results: The mean scores of variables were as follows. Empowerment was 4.6472, task performance was 3.7814, and job satisfaction was 3.1240. The older, more educated and more experienced nurses showed more empowered and higher job performed than the younger, less educated and less experienced nurses. Empowerment had a positive correlation with task performance and job satisfaction. Conclusions: The results imply that hospital should develop the empowerment program for nurses to be more job performed and satisfied.

Purpose: This study was to compare degree of stress, stress coping, adaptation and job satisfaction between Type A and Type B of new graduate nurses'personality. Methods: Seventynine new graduate nurses employed 2003 in an university hospital located in Gyeonggi-do were participated. Data for self-reported questionnaire were collected base on 5 point of time, 1st week and 3rd, 6th, 9th, 12th Month after their employment. Results: New Graduate Nurses' Stress were persistently increased. The statistically significance of positive correlations between stress and coping showed, but their correlations were getting reduced during 1 year. Type B was higher than Type A in stress and adaptation for 12 months. But, Type B of coping was higher up to 9th month, yet abruptly Type A was higher at 12th months. In Job Satisfaction, Type A was higher at 3rd and 6th months, but it was reversed from 9th months. Conclusion: Socialization education for new graduate nurses should be done in organization since their stress were continually increased and their job satisfaction was decreased after 9 months after employed.

Purpose: This study was done to examine the effects of mood state, resourcefulness, and health perception on health promoting behavior of hospital nurses. Methods: Convenience sampling was conducted for nurses working at three University hospital and 336 nurses were selected. Data were analyzed using SPSS 18.0. Results: The results of the analysis showed that the score for nurses' mood state was $41.76{\pm}18.90$; for resourcefulness, $11.30{\pm}20.63$; for health perception, $3.32{\pm}.77$, and for health promoting behavior, $111.55{\pm}17.76$. Mood state, resourcefulness, and health perception were significantly correlated with health promoting behavior. The overall explanatory power of the effects of nurses' mood state, resourcefulness, and health perception on health promoting behavior was 27.8%. Conclusion: Study results indicate that better mood state, higher resourcefulness, and higher health perception result in more health promoting behavior.

Purpose: The purpose of this study was to examine nurses' cognition of the diagnosis related group (DRG) in long-term care hospitals and changes in nursing care after application of the DRG system. Method: This study was a descriptive survey involving 161 nurses working in 12 long-term care hospitals located in Gwangju city and Chonnam area. Data were analyzed using the SPSS 18.0 version program. Data analyses utilized descriptive test, chi-square test, Fisher's exact probability test, t-test, and one-way ANOVA. Result: There was no change in cognition of DRG in 55.3% of the nurses, and 26.1% takes to 'change positively'. More than half of the respondents (57.8%) agreed to the application of DRG. After application of DRG, the nurses responded 'there were changes in nursing care' in 23 of the 25 care items. Two items had an increase in nursing care. Conclusions: No distinct changes in nursing care were evident after DRG application. Therefore, there is need to provide education programs related to DRG for nurse in long-term care hospitals.

Purpose: This study was done to examine the relationships among practice environment, nursing professionalism, career commitment, and turnover intention in nurses working in small-medium sized hospitals. Method: This study utilized a descriptive correlational design. Data were collected using structured questionnaires from 362 nurses in 7 small-medium sized hospitals and analyzed with SPSS 18.0. Result: The mean scores for practice environment, nursing professionalism, career commitment, and turnover intention were 2.29 on a 4-point scale and 3.13, 2.78, 3.51 on a 5-point scale, respectively. The practice environment showed significantly positive correlations with nursing professionalism and career commitment. Nursing professionalism showed a significantly positive correlation with career commitment. The highest significant negative correlation was between practice environment and turnover intention. Nursing professionalism and career commitment had negative correlations with turnover intention. Factors having significant influence on turnover intention included age, assigned ward, average wage, practice environment, and career commitment. These factors explained 43.0% of variance in turnover intention. Practice environment was identified as the most important variable in explaining turnover intention. Conclusions: The results of this study demonstrate the necessity of improving the practice environment and increasing nurses' career commitment if turnover intention in small-medium sized hospitals is to be lowered.

Purpose: This study was done to examine the level of use of essential oriental nursing terms and related characteristics among nurses in oriental medicine (OM) hospitals. Method: A cross-sectional survey design with convenience sampling was employed. Questionnaires were used to collect data from 315 nurses in seven university-affiliated OM hospitals. The questionnaire included 174 oriental nursing terms about actions, tests, drugs, and theories. Multiple regression analysis was performed to determine factors associated with the level of use. Results: The response rate was 98.1% (n=309). Overall mean score for use of essential oriental nursing terms was 2.7(${\pm}\;0.7$) on a five-point Likert scale. Significant factors associated with the level of use were workplace and knowledge of oriental nursing theory terms. Nurses who worked in nursing care wards and those who had a higher level of knowledge of oriental nursing theories had a higher level of use. Conclusions: These findings indicate that level of use of essential oriental nursing terms were below average. Educational programs to enhance knowledge of oriental nursing theories need to be implemented for nurses in OM hospitals, considering their workplace, in order to facilitate nursing data sharing and communication among healthcare providers.

Purpose: The purpose of this study was to explore nurses' experience of evidence-based nursing practice in general hospitals. Methods: Data were collected from 13 nurses through in-depth interviews about their experiences with evidence-based practice. The research questions were "What kind of evidence are you using in your practice?" and "What are the barriers to and enablers of evidence-based practice that you have experienced?" Qualitative data from field and transcribed notes were analyzed using qualitative content analysis methodology. Results: Major themes of using evidence were identified as 'research as primary valid evidence', 'information from local context and internet as realistic evidence', and 'clinical experience as pragmatic evidence'. Patient experience was not used as evidence in solving nursing problems. Barriers to and enablers of evidence-based practice were linked. They included both external, organizational factors and individual factors. Main issues were 'lack of evidence and poor work environment', and major facilitating factors were 'improving knowledge and skills related to evidence-based practice' and 'communicating and sharing evidence'. Conclusion: The study findings provide useful information for understanding nurses' experience of using external and internal evidence along with their meaning. A multidimensional approach is needed to overcome barriers to and implement evidence-based practice.

Purpose: The purpose of this study was to identify the relationship between patient characteristics and patient satisfaction. Methods: A cross-sectional questionnaire survey was conducted in an acute care hospital. The subjects were 317 patients discharged from general medical and surgical nursing care units during September, 2005. Patient satisfaction was measured using the short-form satisfaction scale of Hwang and Park(2001). Additional information about patient characteristics, including general demographics and health care utilization variables, was collected from the hospital information systems. Multiple regression analysis was performed to determine patient characteristics influencing patient satisfaction. Results: Patients were satisfied with hospital care with an average of 4.10 on a five-point Likert scale. Patient characteristics explained 13.5% of the variance of patient satisfaction. The significant factors influencing patient satisfaction were patients' age and perceived health status. There was no significant relationship between structural variables and patient satisfaction. Conclusion: This study showed that patients' characteristics were significant factors explaining patient satisfaction. Therefore, these characteristics should be adjusted in reporting patient satisfaction as an indicator for hospital-level or department-level rating.
This study analyzed the effect of foreign currency exchange rate on the increasing rate of medical care cost by items of fee schedule of Korean Medical Insurance. This study uses the data of cost analysis including cost of imported goods and the data of for a university hospital National Federation's Medical Insurance for a trend of claim. The method of cost analysis is as same as that used in the study of the development of Korean RBRVS(Resource Based Relative Valus Scale). The main findings of this study are as follows; 1. The proportion of imported goods in cost related to Medical Insurance fee schedule is 7.93%, and in case of substitution of available domestic goods 6.96%. 2. If foreign currency exchange rate changes from 800wen per $1 to 1,300won, the affecting rate of Medical Insurance fee schedules is 5.00%. If the imported goods will be substituted with available domestic goods, the rate 4.35%. Our results can be used a data for updating Medical Insurance fee schedule. But this result is limited to be generalized, because this study used the cost analysis for a university hospital.
This study was performed to investigate the practical oriental medical expenses by the use of internal data of an oriental hospital due to the bias of medical insurance program data. The purpose of this study was to describe prevalent diseases of clinical department in the studied hospital, to analyze medical expenses structure and to verify the each cost share ration of expenses on insurer to insuree. Under this purpose, we analyzed actual medical expenses data of 1,611 hospitalized patients of the oriental medical university hospital with 150 beds that can be approached to internal data from Jan. 1, 1994 to Dec. 31, 1994. The major findings are as follows : 1. Upper five of most frequent diseases of admitted patients were Joul-Jung-Pung(55.5%), Yoo-Kak-Tong(7.3%), Yoo-/Tong(7.1%), Gu-An-Wa-Sa(2.7%) and sequale of Joul- Jung-Pung(2.4%) 2. In medical expenses structure, hospital ward fee was 47.1%, medication fee 41.3%, fee for procedure(acupuncture, moxibustion, negative therapy, physical therapy, etc) 11.1% and consultation fee 0.5%. In addition to the cost share ration of insuree & that of insurer was 75:25 respectly.
