Medical Convergence Analysis of Complaint about Medical Service in an Affiliated Hospital (일 상급종합병원 의료서비스 불만족 내용의 의료 융합적 분석)

  • Implementation of patient feedback is considered as a critical part of effective and efficient management. This study is a retrospective survey after to ensure the medical convergence analysis of contents on customer dissatisfaction using medical services. The data were collected from dissatisfaction 319 case at a affiliated hospital. The result indicate that a repeat visit than the initial visit, outpatients were more dissatisfied than the inpatients. The type of dissatisfaction were more dissatisfied of the in communication and explanation of medical service, nursing service and procedure of administrative services. In the classification of disease the percentages of dissatisfaction in the digestive system and neoplasm were high in both groups(p<0.05). Based on the study to improve the quality of medical service. Prevent the recurrence of dissatisfaction and to establish a customer-oriented business strategy. Characterized by dissatisfaction factors and contents of each patient and should be structured to specific disease types of services through continuous medical convergence research.

Gateway design and performance evaluation based on health information standards in multi-platform environment (멀티플랫폼 환경에서 의료정보표준 기반 게이트웨이 설계 및 성능 평가)

  • Recently, to enhance the quality and efficiency of health care services, smart health care services utilizing a smart phone and tablet pc are mainly being introduced into the large hospital and the interest for personal health record service which manages personal medical information utilizing PC and smart phone and technology development is actively underway. In order to build a next generation hospital information systems, the interoperability of data between the different hospital information systems and UX consistency between the various user devices must be assured. Therefore, in this paper, medical information standard and the study for improving the performance is conducted to design the gateway system which converting and integrating the medical information under multi-platform environment. In this paper, CCR standard is adapted to the mapping method between the data which is the key feature of the gateway system and design the system utilizing multi-table-based mapping method. To evaluate the performance of the system, experiment was done and the result has been improved approximately 20 percent for response rate.

Performance of Comprehensive Nursing Care Service in an Acute Care Hospital: Focusing on Accidental Falls and Pressure Injuries (급성기병원에서의 간호 · 간병통합서비스 운영 전후 낙상 및 욕창 발생 변화)

  • Purpose: This study aimed to investigate whether the comprehensive nursing care service positively affected accidental falls and pressure injuries. Methods: This study was a retrospective study that analyzed the accidental falls and pressure injuries cases in an acute care hospital located in Seoul and compared the rates of accidental falls and pressure injuries before and after the comprehensive nursing care service was operated. Results: Comparing the accidental fall incidence rates per 100 person-months between a comprehensive nursing care ward and a general ward, it showed fewer accidental falls by 0.44 in comprehensive nursing care wards, but the result was not statistically significant. In the case of pressure ulcers, the incident rate per 100 person-month was 6.17 in general wards and 4.77 in comprehensive nursing care wards, which showed that the number of pressure ulcer patients was lower in comprehensive nursing care wards, however it was also not statistically significant. Conclusion: It is not confirmed that the operation of the comprehensive nursing care service contributes to the reduction of accidental hospital falls or pressure injuries. Follow-up studies are recommended to determine the effectiveness of comprehensive nursing services in quality indicators.

A Development of Home Nursing Service System based Service Oriented Architecture (SOA) (SOA 기반의 가정간호서비스 시스템 개발)

  • To improve the quality of healthcare and build up the health delivery system, electronic health record system is important for healthcare institutions providing health services. However, the systems currently operated in domestic healthcare institutions use independent software interface with distributed environment for data search and process. Therefore, it raises an additional expense to buy or develop each interface module when there is a link to new system and the problem of complexity. To solve these problems, this study implemented the service oriented architecture basis for home nursing service and carried out an assessment. The study defined the process modeling and business requirements based on the scenario of service and drew 17 suitable services based on five verification items for service design. To draw the final service, seven services were selected by service litmus test (SLT). The study found that, by the improvement of business process for healthcare service support department, SOA based information system has an effect of time reduction of patient processing time. In conclusion, to enable the hospital information system to cope with various demands of healthcare consumers, it is necessary to consider the application of SOA technology which has excellent interoperability, reusability and maintenance.

Comparative Study on the Expertise, Convenience, and Interaction of Medical Treatment Systems Applied by Geriatric Hospital (요양병원 진료시스템에 따른 진료전문성, 편의성, 상호작용성의 역할 비교연구)

  • Medical treatment for the aged has become an increasingly important concern as the aged population continues to grow. However, little research has been done on the issue. This study examines the roles of the expertise, convenience, and interaction of medical treatment systems applied by geriatric hospital. Medical treatment systems include oriental medicine, western medicine and integrated(oriental and western) medicine practices. The empirical results of the study are as follows. First, the relative impacts of the expertise, convenience and interaction on customer satisfaction are different depending upon the medical treatment systems. Second, convenience and interaction are shown to be more important factors in western medicine, while interaction is the case in oriental medicine. As for the integrated medicine practices, expertise is considered more important. Thus, it is necessary for the western medicine to make more efforts on interaction and convenience, while it is critical to enhance medical personnels' service quality in the oriental medicine. In the integrated medical treatment, actual and effective cooperation should be achieved to secure expertise. Third, the data demonstrate that customer satisfaction has a positive influence on relationship quality which, in turn, impacts on repurchase intention. Finally, relationship quality has a negative influence on intention to switch in the western medicine, whereas the impact of relationship quality is not significant in the oriental and integrated medicine practices.

Comparison of Customer Satisfaction Before and After One-stop Service (당일 검사에 대한 원스톱 서비스 전과 후의 고객만족도 비교)

  • Purpose:Hospitals provide top medical service using exceptional manpower, medical technology, and state-of-the-art equipment, thus raising the standard of customer satisfaction. In addition, their medical service is becoming higher than before. One-stop service is a good way to improve the quality of customer-centered service as a qualitative marketing strategy. This study thus aims to facilitate subsequent research and compare customer satisfaction before and after one-stop service. Methods: The study included 72 patients who received the reserved examination and one-stop service for 20 days from April 23 to May 12, 2014. The surveyed questionnaire data were analyzed using SPSS 18.00. Results: The comparison results of customer satisfaction showed that the satisfaction score was generally high in the areas of kindness of examination staff, the speedy/accuracy of work processing of examination staff, and the kindness of reservation staff. The group before one-stop service showed their dissatisfaction with repeated visits and difficulty of booking a desired day. The group after one-stop service showed dissatisfaction with the long waiting time for examination or same-day treatment. Conclusion: The one-stop service showed good results, but new uncomfortable issues for the customer were revealed as well, which may result in more work of employees. Considering the characteristics of various clinical departments, the author hopes to find an efficient operation plan through the development and improvement of an appropriate one-stop service method.

Effects of National Evaluation of Long-Term Care Hospitals on Hospital Workers' Work Environment, Job Satisfaction, and Quality of Services (요양병원평가가 요양병원 종사자의 근무환경, 직무만족과 서비스 질에 미치는 영향)

  • Purpose: The purpose of this study is to evaluate the work environment and job satisfaction of hospital workers and to assess their effects on quality of services under the national long-term care hospital evaluation (hereafter, national evaluation), which has implemented since 2008. Methods: A self-administered survey was conducted on 178 hospital workers' at 18 hospitals in Chungcheong province, Korea. Survey questionnaires include questions about respondent' awareness of the national evaluation and any change in work environment and job satisfaction under the national evaluation. We used a path analysis to assess the effects of work environment and job satisfaction on quality of services. Results: Results showed that the effects of the national evaluation on work environment and job satisfaction were positive, which, in turn, leads to better quality of services. Improvements in the work environment under the national evaluation have resulted in increased job satisfaction. High scores for job satisfaction showed a significant association with the quality of services provided in long-term care hospitals. In addition, the national evaluation itself had a positive effect on improving quality of services. Conclusions: In order to facilitate quality improvement activities under the national evaluation, it is suggested that workers be provided with education and training. Continuous efforts to improve work environment and to enhance job satisfaction would lead to provision of better quality of services in long-term care hospitals.

  • Communication errors have severe consequences clinical quality and disputes in medical organizations. Thus, clinical communication has become a major practice recently, since clinical and managerial effectiveness can also be enhanced by improving the quality of the physician, medical staff, and laboratories interactions. Both medium and large-sized hospitals are increasingly adopting the philosophy to shorten clinical service time while enhancing higher medical service quality and lower healthcare service costs. From this motivation, this paper studies on the clinical collaborative communication concept and investigates approaches to the philosophy for implementation. The system has been theorized to improve communication and enhancing medical outcome qualities. In addition, to validate the system, the author analyzes the efficiency and effectiveness in S hospital, using statistical survey works.

  • Recently hospitals have divided into many divisions, specialized the medical service, and shown organic cooperation, all to provide patients with various and high quality medical service. They have also showed improvement in information protection by introducing an information protection system to regulate the access to patients' medical and personal information. The purpose of this paper is to present a case study to implement of a limited hospital medical information system that can regulate the access to medical information. For this, a router-based virtual network applying an ACL(: Access Control List) to regulate access to information was made using a packet tracer.

  • 오늘날 우리 사회의 생활과 문화의 질이 향상됨에 따라 공공 및 개인의 안전과 건강에 대한 관심이 증대되고 있다. 특히, 재난과 비상사태는 생명에 대한 희생뿐만 아니라 훨씬 더 심각한 경제적인 결과를 초래한다. 이러한 응급사태에 개비하여 적절히 준비되지 않았거나 재난의 예방 및 완화시킬 방법을 가지고 있지 못한다면 도시의 발전적인 프로젝트는 예고 없이 도시를 강타하는 재난 및 비상사태에 의해 무산되고 만다. 이러한 여건 하에 재난과 응급사고를 효율적으로 대처하는 응급처치와 서비스에 대한 급격한 관심과 함께 일정한 결정적 대응시간 내에 각종사고와 재해로부터 방어할 수 있는 공공서비스에 대한 필요성이 크게 증가되었다. 본 연구에서는 신고, 응급구조, 환자이송 등 도시의 병원 전 단계 의료환경에 대해서 조사하였다. 본 연구의 목적은 반응시간과 출동거리를 분석함으로써, 응급사고에 즉시 응급구조서비스를 제공하기 위해 안전센터(119센터)의 최적 입지를 확인하는 것이다. 이를 위해서 사고발생, 반응시간, 출동거리 등을 응급의료 활동과 관련된 연구논문, 대상지 전역의 소방기록물과 출동일지 등의 통계자료를 활용하여 신속한 응급의료 서비스를 위한 패턴을 분석 하였다. 또한 응급구조대의 조직과 활동의 분석을 통해 응급의료시설의 최적 입지설정을 위한 제도적 개선대안을 마련해 보았다. 대상지 연구를 위해, 구조활동 및 소방서비스의 최근 통계자료의 조사를 통해 진주시의 모든 119센터의 입지와 응급의료서비스 및 사고발생 현황을 나타내었다. 자료 분석을 위해서 SPSS14.0을 사용하였으며, 지역의 특성 분포, 안전센터별 대응시간, 이송시간 등을 소방 GPS시스템과 입지할당모형 중 Center Point Model(CPM)을 적용하였다. 결과적으로 본 연구를 통해 대상지역의 119안전센터의 서비스 질이 나쁜 지역을 지리학적인 관점에서 최적 입지/배분 패턴의 설정을 통해 확인할 수 있었으며, 이를 통해 미래 응급의료서비스의 효율적인 입지/배분을 통해 공공 및 개인의 안전과 건강에 기여할 수 있을 것으로 기대된다.
