The Effect of Urinary Elimination Facilitation Program for Stroke Patients During Rehabilitation: Residual Urine, the Number of Urinary Catheterization, Urinary Discomfort, and Urinary Tract Infections (배뇨중재 프로그램이 뇌졸중 재활대상자의 잔뇨량, 도뇨횟수, 배뇨불편감, 요로감염에 미치는 효과)

  • Purpose: The aims of this study were to develop and apply the urinary elimination facilitation program for stroke patients with urinary disorders and evaluate the effectiveness of the program. Methods: This study was conducted using the non-equivalent control group pre- posttest design. The participants included 23 patients in the control group and 22 in the experimental group. Data were collected from Nov. 25, 2013 to Nov. 25, 2014 in acute rehabilitation inpatient wards. For the final analysis, 35 patients' data were utilized 17 in the control group and 18 in the experimental group. The developed urinary elimination facilitation program was continued for 7 days and the program consisted of logging urination, monitoring timely voiding, assisting urinary elimination, and stimulating urination using sound. The data were analyized using the SPSS-win (version 21). Results: The number of urinary catheterization and the urinary discomfort were significantly improved in the experimental group compared to the control group. However residual urine and the occurrence of urinary tract infections were not significantly different between the experimental and control groups. Conclusion: The results of this study indicated that the urinary elimination intervention program for stroke patients with urination disorder could be useful in decreasing the number of urinary catheterization and the urinary discomfort.

Factors Influencing Physical Function and Treatment of Colorectal Cancer Patients (대장암환자 치료와 신체기능과의 관련 요인)

  • 본 연구는 대장암환자 치료와 신체기능과의 관련요인을 규명하고자 시도되었다. 2009년 12월 15일부터 2010년 1월 30일까지 한 종합병원 외과에 내원한 대장암환자 110명을 대상으로 설문조사를 하였다. 자료분석은 치료시점과 신체기능과의 관련요인은 ANOVA를 이용하여 분석하였으며 방사선치료 시기 및 장루유무 등과 신체기능과의 요인은 t-test로 하였다. 연구결과 첫째, 치료시점은 성기능에서 유의한 차이를 보였다(F=3.482, p<0.05), 둘째, 오심구토, 식욕저하, 치료부작용은 복원수술 전, 성기능은 복원수술 후에 유의하게 높게 나타났다(p<0.05). 셋째, 장루유무의 경우 성적만족은 장루가 없는 경우가 63.40점으로 장루가 있는 경우의 45.69점 보다 높은 반면 배뇨문제에서는 장루가 있는 경우가 28.17점으로 장루가 없는 경우의 14.69점보다 유의하게 높은 불편감을 가진 것으로 나타났다.(t=1.548, p<0.05). 결론적으로 대장암 환자 치료의 신체기능을 향상하기 위해서는 기능상태와 증상불편감을 개선시킬 수 있는 중재프로그램의 개발과 적용에 관한 연구가 필요하다.

Relationship of Urinary Symptom, Urinary Discomfort and Quality of Life in Bladder Cancer and Benign Prostatic Hypertrophy of Male Patients (남성 방광암환자와 전립성비대증 환자에서 배뇨증상, 배뇨 불편감 및 삶의 만족도와의 관계에 관한 연구)

  • Purpose: The purpose of this study was to identify relationship of urinary symptom, urinary discomfort and quality of life among the bladder cancer patients and benign prostate hypertrophy patients, and to contribute health promotion of such patients and nursing intervention development based on this results. Method: Study sample recruited bladder cancer patents(n=49) and benign prostate hypertrophy patients who admitted Seoul National University Hospital from June, 2002 to June, 2003. Both group patients were operated, and prostate hypertrophy patients group (mean 67.8 years old) were older than bladder cancer patients group(60.82 years old). Instruments was composed of general characteristics, urinary symptom scale(19 items), urinary discomfort scale(19 items) and quality of life scale(21 items). Data was analysed SPSS PC + 10. using mean, standard deviation, pearson correlation coefficient. Result as follows: 1. There was a statistically significant difference in occupation between two groups (p=.027). Hypertrophy patients group's age was more older than bladder cancer patients group. 2. The prostate hypertrophy patients group had the significantly higher score in urinary symptom (p=000) and nighttime urination frequency. However, there was no significant difference in incontinence symptoms and the symptoms associated bladder cancer between two groups. 3. The prostate hypertrophy patients group had significantly higher score in urinary discomfort (p=000) than the bladder cancer patients group. However, there was no significant difference incontinence discomfort and the discomfort associated bladder cancer between two groups. 4. The prostate hypertrophy patients group suffered more urinary discomfort than the bladder cancer patients group did. The quality of life the prostate hypertrophy patients group was lower than the quality of life the bladder cancer patients group. Quality of life was no statistically significant difference between two groups (p=000). 5. There was a positive correlation between urinary symptoms and urinary discomfort. However, there was a negative correlation between the quality of life and urination symptoms and discomfort. Conclusions: The prostate hypertrophy patients group had significantly higher score in urinary symptom and urinary discomfort (p=000) than the bladder cancer patients group. The quality of life the prostate hypertrophy patients group was lower than the quality of life the bladder cancer patients group. This means that urinary symptom and urinary discomfort in prostate hypertrophy patient group is more important problem. So, prostate hypertrophy patient group need to control the symptom. Therefore, nurses will be provide the intervention program to improve the bladder function after prostate hypertrophy surgery.

Effects of Foot Bath Therapy on Menopausal Symptoms and Sleep in Women in Their 50s (족욕요법이 50대 여성의 갱년기 증상 및 수면에 미치는 영향)

  • Purpose: Purpose: This study was to study the effect of foot bathing on menopausal symptoms and sleep disorders in female subjects in their 50s with menopausal symptoms. Methods: After 12 weeks of foot bath therapy three times a week, pre- and post-menopausal signs were measured. Results: Subjects with hot flashes, heart discomfort, sleep problems, depression, irritability, anxiety, physical and mental fatigue, sexual problems, urination problems, vaginal dryness, and joint and muscle discomfort significantly improved after than before(p < .001). Subjective sleep quality, sleep incubation period, sleep duration, sleep disturbance score, sleep drug use, and sleep dysfunction significantly decreased after foot bath than before(p < .001). Habitual sleep efficiency increased considerably. Conclusions: The subjects showed overall improvement in menopausal symptoms and sleeping quality after a foot-bath. Therefore, foot bath therapy is evaluated as a natural healing therapy suitable for improving menopausal symptoms and sleep.

The Effect of Physical Function on Operative Treatment of Colorectal Cancer Patients (대장암환자 수술치료가 신체기능에 영향을 미치는 요인)

  • The purpose of this study was attempted to identify the effect of physical function on operative treatment of colorectal cancer patients. Colorectal cancer patients(110 persons) who visited a general hospital in Seoul were examined with questionnaires and interview from December 15, 2009 to January 30, 2010. The results of this study are as follows ; First, operative treatment side-effects were significantly higher in patients before ileostomy repair than in patients after ileostomy repair(t=5.284, p<0.001). Secondly, sexual satisfaction were higher in pre-operative radiotherapy patients than in post-operative radiotherapy patients(t=1.946, p<0.05). Thirdly, micturition problems were significantly higher in discomfort symptom of stoma patients than in nonstoma patients(t=1.537, p<0.05). In conclusion, in order to improve physical function on operative treatment of colorectal cancer patients, it is necessary to develop intervention program about the improvement of function status and discomfort symptoms.

Frequency, Intensity and Daily Life Distress of Urinary Dysfunction in Women with Cervical Cancer after Radical Hysterectomy (근치적자궁절제술을 받은 자궁경부암 여성의 배뇨장애 빈도, 강도 및 일상생활 불편감)

  • Purpose: This study was done to identify frequency, intensity of urinary dysfunction and daily life distress in women after a radical hysterectomy for cervical cancer. Methods: One hundred and fifty seven women who had undergone a radical hysterectomy and one hundred and sixty five women as healthy controls completed questionnaires on intensity of urinary dysfunction and daily life distress caused by urinary dysfunction. Results: Women with cervical cancer showed higher frequency of urinary dysfunction than healthy controls. Major urinary dysfunction for women with cervical cancer in order of frequency were night-time incontinence (odds ratio=10.39, p<.001), difficulty in starting urination, weak urine stream and sense of incomplete emptying of bladder. The highest score on intensity was difficulty in starting urination, followed by urgency, weak urine stream, daytime frequency and sense of incomplete emptying. Night-time incontinence was the urinary symptom causing the most daily life distress for cervical cancer women followed by difficulty in starting urination, urgency, sense of incomplete emptying, and night-time frequency. Conclusion: Results suggest that nurses should address the potential postoperative urinary complications and develop long term interventions to decrease urinary dysfunction and daily life distress for women who have had a radical hysterectomy for cervical cancer.

Prevalence of Lower Urinary Tract Symptoms and Association of Hypertension with I-PSS (국제 전립선 증상 점수(I-PSS)를 이용한 하부요로증상의 유병률 및 고혈압과의 관계)

  • Benign prostatic hyperplasia (BPH) is a highly prevalent, age-related disorder in men which place a considerable burden on health care resources worldwide. While BPH and hypertension are apparently diverse disease processes, they have some features in common(e.g. underlying etiology of the sympathetic nervous system). The purpose of this study is to estimate the prevalence of lower urinary tract symptoms using I-PSS(international prostate symptom score) and to investigate the association of hypertension with I-PSS. This study was carried out to 390 men(40~86 year old) at 3 Myun in Chungju City from July to August, 1997. Subjects answered on questionnaire for I-PSS and were checked age, education, marital status, annual income, blood pressure. The mean age of subjects was 59.8 year old. The mean of I-PSS were increased by age decades(40~49, 50~59, 60~69, 70~86) and it's scores were 4.4, 7.0, 8.2, 12.3 respectively. Strong correlations were observed between the I-PSS and the QUL(quality of life)(P=0.0001). I-PSS(mean-value) were 7.85 in hypertensive group and 8.39 in normotensive group but there was no statistically significance between the two groups(P>0.05). The proportion of mean scoring greater than 8 was 42.5%. There have been reports of association between lower urinary tract symptoms and hypertension, but there was no consistent suggestion that such an association could be casual. The need for high-quality epidemiological information and consequent increased prospects for prevention is obvious.

Study of Post Procedural Complications Associated with Voiding Cystourethrography (소아에서 시행한 방광요도 조영술 이후 발생한 합병증에 대한 고찰)

  • Purpose : Voiding cystourethrography(VCUG) is a commonly performed diagnostic procedure in children with urinary tract infections. Recently, with the widespread use of prenatal ultrasonography, VCUG is performed as part of the postnatal radiological evaluation of asymptomatic infants with prenatally detected hydronephrosis. The procedure is relatively simple but it involves discomfort and some complications. We studied post procedural symptoms and complications in children who underwent VCUG. Methods : This study reviewed 259 patients who underwent VCUG in our hospital between October 2005 and September 2006. We did a chart review and a telephone interview with the patients' parents about symptoms and complications associated with VCUG. Results : Among 269 children, 217 patients(80.7%) were under 2 years of age and 5 patients (1.9%) were over 8 years of age. Their mean age was $13.1{\pm}22.9$ months. After VCUG, dysuria was found in 49 patients presented with dysuria, and irritability in 36 patients with irritability. Other complications were hematuria, fever, frequency, bladder rupture and urinary tract infection. Mean symptoms duration was $1.4{\pm}0.7$ days. There was no significant relationship between prophylactic antibiotics use and complication rate associated with VCUG. Conclusion : Our study demonstrated that 32.7% of patients showed complications including bladder rupture and urinary tract infection after VCUG. We also found that prophylactic antibiotics use did not prevent urinary tract infection nor decrease the rate of complications associated with VCUG. Therefore, we suggest that the procedure must be done carefully and aseptically, and we should closely observe the children who undergo VCUG for development of possible complications.
