Impact of Influenza Infection on Febrile Seizures: Clinical Implications (인플루엔자 감염과 연관된 열성경련의 임상적 특징)

  • Purpose: Febrile seizures (FSs) are the most common type of seizure in the first 5 years of life and are frequently associated with viral infections. Influenza infection is associated with a variety of neurological conditions, including FSs. The purpose of this study was to evaluate the clinical implications of influenza infection in FSs. Methods: In total, 388 children with FS were divided into two groups: FS with influenza infection (n=75) and FSs without influenza infection (n=313). Their medical records, including seizure type, frequency, duration, and familial history of FSs or epilepsy, were retrospectively reviewed and the clinical characteristics of the two groups were compared. Results: In total, 75 of the 388 children (19.3%) had FSs associated with influenza infection; such children were significantly older than those with FSs without influenza infection ($34.9{\pm}22.3$ months vs. $24.4{\pm}14.2$ months; P<0.001). The children who had more than two febrile seizures episodes were more prevalent in children with FS with influenza infection [40/75 (53.3%) vs. 92/313 (29.4%); P<0.01]. Children older than 60 months were more likely to have influenza infection compared to those aged less than 60 months [11/22 (50%) vs. 64/366 (17.5%); P=0.001]. Conclusion: Influenza infection may be associated with FSs in older children, and with recurrence of FSs. Its role in the development of afebrile seizures or subsequent epilepsy requires further investigation with long-term follow-up.

  • Various treatments can be attempted in patients with intractable epilepsy, in whom the symptoms of seizures are not controlled by various drugs. On the other hand, in patients requiring a surgical method, a preoperative examination is needed to determine the portion of seizure site to be resected. Electrodes are inserted into the cerebral cortex for accurate lesion measurements and safe operation. The electrodes inserted in the cortex not only record the electroencephalography (EEG), but also allow various tests to confirm the function of the part. One of these methods is the evoked potential test. From January 2015 to December 2018, the trends of measured waveforms in were analyzed 70 patients. The somatosensory evoked potential (SSEP) recorded on the electrode inserted in the cerebral cortex can be searched for the pathway of the central sulcus to avoid the primary motor area and primary sensory area. In addition, using the middle latency auditory evoked potentials (MLAEP) and flash visual evoked potentials (FVEP), the functional cortex in the auditory cortex and the visual cortex were compared with the seizure focus point on the EEG to help determine the location of the ablation and minimize functional impairment after surgery.

  • 진단기술과 약물치료기법 및 신경학적 과정에 대한 이해가 발전됨으로 인하여 치과의사는 간질환자를 더 잘 이해하고 치료할 수 있게 되었다. 간질환자는 몇 가지 주의사항만 지킨다면 일반치과에서도 쉽게 치료할 수 있다. 즉, 의학적 병력과 함께 매 번 방문 시 마다 치과치료 전에 현재상태에 대한 평가가 필요하다. 기왕력 중 발작은 치료계획수립시 반드시 고려되어야 한다. 치과의사는 간질이나 경련성질환에 대하여 포괄적이며 통합적 이해를 통하여 환자의 구강건강뿐 아니라 나아가 전신건강을 증진하는 데 기여할 수 있다.

Detection and Diagnosis of Neonatal Seizures (신생아 발작의 발견 및 진단)

  • Seizures are the most common clinical manifestation of a neurologic insult during the neonatal period. Neonatal seizures continue to present a diagnostic and therapeutic challenge to pediatricians because the recognition and classification of neonatal seizures remains problematic, particularly when clinicians rely only on clinical criteria. Neonatal seizures can permanently disrupt neuronal development, induce synaptic reorganization, alter plasticity, and "prime" the brain to increased damage from seizures later in life. Since neonatal seizures, particularly status epilepticus, predict an increased risk for later epilepsy and other neurologic sequelae, accurate diagnoses are needed for aggressive antiepileptic drug use. The present review summarizes the pathophysiology, etiology, and diagnosis of neonatal seizures.

Hypoglycemic Convulsive Seizure Due to Late Dumping Syndrome (후기 덤핑증후군으로 유발된 저혈당성 경련발작)

  • Dumping syndrome is a common complication of esophageal or gastric surgery. Patients with late dumping syndrome usually suffer from hypoglycemic symptoms such as palpitation, tremor, and general weakness. Hypoglycemia induced convulsive seizure due to late dumping syndrome is rarely reported. We report a 46-year-old man with postprandial hypoglycemic convulsive seizure as the first symptom of late dumping syndrome.

Meningoencephalitis in Dachshund Dog with Canine Distemper Virus Infection: 1.5 T and 7.0 T MRI Findings (닥스훈트견의 바이러스성 뇌수막염에서 1.5T와 7T 자기공명영상을 이용한 진단 증례)

  • A 6-year-old, intact female Dachshund was presented with generalized seizure for 2 days. Based on the neurologic and physical examinations, intracranial diseases were suspected. 1.5T and 7.0T magnetic resonance imaging (MRI) of the brain were taken. The MRI results revealed diffuse hyperintense lesions in the area of the diencephalon to the medulla oblongata in the T2-weighted images. Canine distemper virus-induced meningoencephalitis was confirmed by the result of RT-PCR of the cerebrospinal fluid (CSF). The dog was euthanized 7 days after diagnosis due to poor prognosis and clinical deterioration. Postmortem histopathologic examination was consistent with the MRI findings. This is the first case report using 1.5T and 7.0T MRI to compare the virus-induced intracranial lesions in meningoencephalitis.

Hepatocellular Carcinoma in a Cocker Spaniel Dog with Hypoglycemia (저혈당증을 동반한 코커스파니엘견에서의 간선암종 증례)

  • A 5-year-old male Cocker spaniel was presented with an abrupt seizure episode. The dog was moderately dehydrated, hypersalivation and loss of consciousness at the presentation. Tentative diagnosis was made through history taking, clinical signs, physical examination, neurologic examination, basic laboratory examinations, radiological examination and cytologic examination. In addition, serum alpha-fetoprotein (AFP) value was measured to diagnose in this case. The serum AFP concentration was marked elevated (1513.5 ng/ml). The patient was suspected as HCC and medical therapy was initiated to control hypoglycemic-associated seizure. However, the dog was euthanized because of the owner's request. It was diagnosed as hepatocellular carcinoma (HCC) based on the histopathologic examination. This case indicates that AFP measurement might be valuable to diagnose HCC in dogs.

The Effect of Seizure-Related Variables on Interictal Psychiatric Symptoms in the Intractable Epileptic Patients (난치성 간질환자에서 간질관련요인이 발작간 정신증상에 미치는 영향)

  • The aim of this study is to find the relationship between interictal psychiatric symptoms and seizure-related variables such as structural changes, regional interictal perfusion changes, the number of interictal epileptic discharges and the presence of accompanying generalized tonic clonic seizure(GTC). The subjects were 75 patients (47 males : mean age $28.3{\pm}7.7$) with intractable epilepsy, Interictal psychiatric symptoms were rated by Symptom Checklist-90-R(SCL-90-R). Each of 4 seizure variables was measured by MRI, $^{99m}TC-HMPAO$ SPECT and the prolonged EEG monitoring. The mean SCL-90-R subscale T-scores and the ratio of the patients with definite psychiatric symptoms(T-score > 65 at any one subscale of SCL-90-R) were compared among the groups with different seizure variables. Demographic variables and clinical variables were not statistically different among the groups with different seizure variables. The patients with right mesial temporal sclerosis had higher mean SCL-90-R scores for obsessive compulsive symptom, interpersonal sensitivity, depression, hostility and psychotic symptoms than the patients with left mesial temporal sclerosis, and they also showed higher ratio of definite psychiatric symptoms. The presence of interictal epileptic discharges was related with higher T-scores of paranoid subscale. The areas of hypoperfusion, the presence of the GTC, and other clinical seizure variables showed no significant influences on the mean SCL-90-R subscale T-scores. from these results, the authors speculated that interictal subictal epileptic dischrages may be related with psychiatric symptoms of the intractable epileptic patients, especially with right mesial temporal sclerosis.

One Case of Insulinoma (인슐린 분비성 췌장세포암 1례)

  • Insulinoma is a rare tumor, occurring more often in women and in the older age range. Eighty percent of patients have a single benign tumor, usually 2cm in diameter, located with about equal frequency in body, head or tail of pancreas and amenable to surgical cure. About 10% have multiple tumors. The remaining 10% of patients have metastatic malignant insulinoma. The symptom of insulinoma is characterized by the periodic attack of hypoglycemia of blood sugar level below 50mg%, by fasting or exertion, and rapid relief of symptom by oral or intravenous administration of glucose. Symptom often lead to misdiagnosis as a neurologic or psychiatric disorder. A case described by authors was 44-year old female with the chief complaints of the loss of consciousness, epileptic seizure although she has been treated by anticonvulsants. Serum blood sugar and insulin level during fasting sugested insulinoma but abdominal computed tomography shows no definitive mass in pancreas. Celiac angiography revealed insulinoma. She transfered to the defartment of General Surgery and was performed enucleation. Microscopic findings shows the islet cell tumor of pancreas. A brief review of the literature was made.
