To reevaluate the clinical usefulness of erection penography for diffential diagnosis of impotence, we reviewed retrospectively the data of penography in 56 patients who were diagnosed as impotence by various diagnostic workup. Twelve normal males were studied as control group. Papaverine HCl 30mg was injected into the corpus cavernosum and simultaneously $^{99m}Tc$-HSA 20mCi was injected via an antecubital vein. After injection radioactivities in the penile area were detected for 30 minutes by gamma camera and time activity curve was displayed. We defined that transit time(TT) is the time to reach peak activity and erection persistent time(EPT) is the duration of time to maintain peak activity and venous index(VI) is the ratio of radioactivity($R_{30}/R_{max}$). The results were as follows. The TT of arteriogenic group($10.7{\pm}2.8min$) was significantly increased compared with those of control and venogenic groups(P<0.05, respectively). The EPT of venogenic group ($6.2{\pm}6.8min$) is significantly decreased compared with those of control and arteriogenic groups(P<0.05, respectively). The TT of psychogenic($15.2{\pm}5.5min$) is significantly increased compared with those of control and arteriogenic groups(P<0.05, respectively). In conclusion erection penography was very useful for the diagnosis of vascular and psychogenic impotence and for differentiation between arteriogenic and venogenic but it could not differentiate mixed type or neurogenic from vascular or psychogenic.

발기부전이란 과거에는 임포텐스라고 불리어 왔던 질환으로 임포텐스 보다는 발기부전이 정확한 명칭이다. 발기부전의 정의는 만족할 만한 성생활을 영위할 수 없을 정도로 발기가 안 되거나, 발기가 되도 끝까지 유지 못하는 상태가 지속되는 것이다.

5년 전부터 경구혈당강하제를 먹고 있는 56세 남자로 발기부전을 주소로 비뇨기과 외래를 방문했다. 약 3$\sim$4년 전부터 성기의 강직도가 좀 떨어졌지만, 부부관계를 끝까지 할 수는 있었는데, 최근 1년 전부터는 관계 도중에 사정을 하지도 않았는데 발기가 소실되어 버리는 경우가 생겨 당황하게 되기도 하였다. 급기야는 발기가 제대로 되지 않아 부부관계를 전혀할 수가 없어서 몇 번 시도 하다가 실패한 후로는 시도조차하지 않고 지내고 있다고 한다. 정력에 좋다는 건강보조식품을 먹어도 별로 도움이 되지 않는 것 같아서, 친구에게서 얻은 비아그라를 한알 먹어 보았는데, 효과를 보지는 못했다고 한다. 간혹 새벽발기도 되는 때가 있고, 이전 같지는 않지만 마음이 동할때도 있지만, 몇 번의 좌절을 겪은 후라 선뜻 시도하기가 두렵다고 하였다. 성기능을 평가하는 설문인 국제발기능지수로 보았을 때 11점으로 중등도의 발기부전이었고, 기본적인 혈액검사와 남성호르몬 수치는 정상이었다. 위와 같은 경우가 당뇨병에 의한 발기부전의 가장 흔한 예 중 하나인데, 대개 발기력의 약화가 서서히 진행되고, 성관계의 실패로 몇 번 당황한 후로는 의기소침 해져서 잠자리를 멀리하게 된다. 남성호르몬 수치가 정상이고 새벽발기도 간혹 되므로 경구용 발기부전 치료제가 우선적으로 선택될 수 있다. 이전에 친구에게서 얻어 먹은 비아그라는 약이 진짜인지 가짜인지가 불분명 하고, 무엇보다 복용법을 정확히 지키지 않아서 효과가 없을 수 있다.

당뇨병에 성기능장애가 많다는 것은 옛날부터 알려져 있지만 실제로는 그릇된 지식과 선입관에 의해 과장되게 잘못알고 있는 경우가 많다. 당뇨병이라고 모두 발기부전 환자는 아닌 것이며 정상 건강인보다 조금 높은 빈도를 보이는 것뿐이다. 식사요법과 운동요법으로 혈당조절을 잘하고 합병증을 예방하면 정상인과 다름없는 성생활을 할 수 있으며 인슐린 치료를 계속하면서 주의 깊게 혈당을 조절하고 합병증을 예방하면 60세 이후까지도 정상적인 성생활이 가능하다. 정상인도 노쇠하면 자연현상으로 언젠가는 모두 발기부전이 되므로 당뇨병환자만이 발기부전이 오는 것은 아니므로 미리 겁내고 불안해할 필요는 없는 것이다. 당뇨병환자중에는 선입관에 의해 당뇨라는 진단을 받고 정신적 충격과 그릇된 지식으로 심인성 발기부전에 빠지는 사람이 상당히 많다. 그러므로 당뇨병환자는 정상인보다 더 많은 노력을 하며 합병증의 병발을 미연에 방지하는 것이 가장 중요하다.

Erectile dysfunction (ED), also known as impotence, is the inability to attain and sustain an erection firm enough to have sexual intercourse. Frequent ED may be a symptom of health problems including heart disease, obesity, alcoholism, stress, smoking, and depression, that need treatment. This study aimed to effect of complex extract (CPL) including Cornus officinalis on sexual function factor in the erectile dysfunction rat model. The erectile dysfuction rat model was induced by cimetidine (500 mg/kg in 5% ethanol, oral injection 2 weeks). Rats were oral administered with different concentration of CPL in rat erectile dysfunction model. As a results, sexual function factors (NO, cGMP) significantly improved in CPL treated groups (CPL-300, 600, 900 mg/kg) compared to CON group. Serum testosterone was increased in a dose-dependent manner after CPL treatment. Furthermore, administrations of CPL restored lumen areas of the prostate in the erectile dysfunction rat model. These results indicated that CPL alleviated erectile dysfunction by increasing sexual function factor and testosterone in rat model. CPL could be used to natural treatement for erectile dysfunction. However, further study is required to identify active ingredient and its mechanism of erectile dysfunction.

The safety of fake anti-impotence drugs (fake Viagra : 26 samples, fake Cialis : 25 samples) distributed in Gyeonggi province was studied by monitoring the concentrations of anti-impotence pharmaceutical ingredient and their analogues. The concentrations of anti-impotence pharmaceutical ingredient 4 specis and their analogues 17 specis were estimated using by HPLC/PDA, LC-MS/MS. The range of concentration of sildenafil in fake viagra was 40~199 mg/tablet, among them the portion of the concentrations of sildenafil over 150 mg/tablet exceeded 65%. 3 cases in tested samples contained sildenafil and tadalafil. The range of concentration of sildenafil in fake cialis was 102~249 mg/tablet, among them the portion of the concentrations of sildenafil over 150 mg/tablet exceeded 88%. One case in tested samples contained demethylhongdenafil (90 mg/tablet). These results indicate that there were many fake anti-impotence drug contained high level of anti-impotence pharmaceutical ingredients, a sustainable monitoring and the blocked distribution of fake anti-impotence drugs recommended.
