Medical Application of the Nondestructive Ultrasonic Tests: Diagnosis of Micro Bone Fractures using Ultrasonic C Scan Images (비파괴 초음파 검사법의 의학적 활용: 초음파 C 스캔 영상을 이용한 미세 골절의 진단)

  • Ultrasonic tests employing non-ionizing radiation are preferred in nondestructive examinations since they are safe and simple in use. The same principles of the techniques have been taken as valuable tools in medical area for the diagnoses of diseases, in other words, defects of the human body. The paper overviews the principles of the medical diagnosis based on nondestructive ultrasonic tests, and then evaluates experimentally the clinical potential of C scan images not popular in medicine, for detecting the micro fractures of the cortical bone. In the experiment the micro bone fractures were created on the femurs of porks by loading three point bending forces (2-4kN) with the speed of 1 mm/min. As the extent of the fracture was altered, not only X ray images but also ultrasonic C scan images using a focused ultrasonic probe resonated at 25 MHz were obtained. The results showed that ultrasonic C scan images were capable of detecting the micro bone fractures which were not possible to identify by conventional X ray images.

Effects of Non-Invasive Constant Microcurrent Stimulation on Expression of BMP-4 After Tibia Fracture in Rabbits (비침습식 미세전류자극이 토끼 경골의 골절 후 BMP-4 발현에 미치는 영향)

  • This study aims to examine the effects of non-invasive constant microcurrent stimulation on expression of Bone Morphogenetic Protein(BMP) 4 after tibia fracture in rabbits. Twenty four rabbits with tibia fracture were randomly divided into control and experimental groups. Each group was divided into four subgroups, based on the duration of the experiment (3, 7, 14, 28 days). The experimental groups received a constant microcurrent stimulation of $20{\sim}25{\mu}A$ intensity with surface Ag-AgCl electrode (diameter 1cm, Biopac, U.S.A.) for 24 hours a day. Cathode of the microcurrent stimulator located on the tibia directly, anode of it did on the gastrocnemius muscle. Rabbits were sacrificed on each of the postoperative days 3, 7, 14, 28. To investigate how non- invasive constant microcurrent stimulation affects bone healing, immunohistochemical analysis of BMP-4 was performed at each point. After evaluation, the test results are as follows: Comparisons of immunohistochemical observation of BMP-4 in 7 days after tibial fracture show that there was shown to be a moderate positive reaction (++) on concentric circles of Harversian system andt he interstitial lamella in the control group, while there was a very strong positive reaction (++++) on concentric circles of Harversian system and interstitial lamellain the experimental group. These results suggest that applying non-invasive constant microcurrent stimulation on fractured bone is helpful to bone healing.

Effects of Microcurrent and High Voltage Pulsed Galvanic Current Stimulation on Fibular Fracture Healing of the Rabbits (미세전류자극과 고전압 맥동직류 통전이 토끼의 비골 골절치유에 미치는 효과)

  • This study investigated the degree of fracture healing using cathode stimulation of microcurrent, cathode and anode stimulation of High Voltage Pulsed Galvanic Current (HVPGC). Measures were performed by X-ray test and Hematoxylin-Eosin stain and Masson's trichrome stain and osteocalcin-positive immunoreactivity. In the measure of X-ray, microcurrent stimulation group revealed more rapid recovery than the groups of HVPGC's cathode and anode stimulation in bone union degrees. Microcurrent group showed significant difference statistically (p<0.05). However, the groups of HVPGC's cathode and anode stimulation didn't show significant difference statistically(p>0.05). In the histologic examination with Hematoxylin-Eosin and Masson's trichrome, microcurrent stimulation group was observed more proliferation of irregular woven bones than the groups of HVPGC's cathode and anode stimulation. Osteocalcin-positive immunoreactivity was observed more osteoblast, osteocyte, osteoclast, bone matrix than the groups of HVPGC's cathode and anode stimulation. Microcurrent stimulation can be considered an effective way during healing of fresh fracture and it can show more effective method than HVPGC's cathode and anode stimulation in the fracture healing.

Arthroscopic Treatment of Symptomatic Shoulders with Minimally Displaced Greater Thberosity Fracture (상완골 대결절의 미세전위골절의 관절경적치료)

  • Twenty-three patients with chronic shoulder pain beyond 6 months after the fracture of the greater tuberosity underwent arthroscopic treatment and were retrospectively assessed after an average of 29 months(range, 22 to 40 months). There were 18 men and 5 women with the average age of 39 years(range, 24 to 61 years). Fourteen were isolated fractures and nine were related to acute anterior instability episode. The average displacement of the fracture was 2.3mm(range, 0 to 4mm) on the anteroposterior view of the plane radiographs. At the time of arthroscopy, all patients had partial thickness rotator cuff tears in the articular surface. The cuff tears were located on the tuberosity fracture area and were an Ellman's grade I to n in depth. With the arthroscopic debridement or repair of the tear depending on the condition of the tear itself, as well as the subacromial decompression, the UCLA score revealed good to excellent results in 20 and fair in 3 patients. Nineteen of the patients had returned to the previous level of activities. The patient with a higher activity demand revealed a lower level of activity return(p=0.034). The partial thickness rotator cuff tear should be considered in patients with chronic shoulder pain after the minimally displaced fracture of the greater tuberosity, and arthroscopic debridement or repair is an appropriate procedure.
