  • 구강은 신체의 관문과도 같은 기관으로 구강 안은 신체 다른 부위의 상태를 알려줄 수 있는 좋은 거울과도 같다. 에이즈 환자도 마찬가지이다. 에이즈는 면역체계의 기능을 약화시키는 질환으로 에이즈 환자에게서는 건강한 사람에게선 보기 힘든 질병들이 나타나기도 하고 일반인에게 나타나는 질병은 더 심하게 증상이 나타나기도 한다. 구강에서 나타나는 이러한 징후들은 HIV에서 AIDS로 진행되는 증후군으로 진단에 활용되기도 한다.

Immunoregulatory Mechanism of Polysaccharides from Natural Plant Resources (신기술 신소재/해외기술 - 천연소재 유래 다당체의 면역조절 기작)

  • 분자단계에서 천연소재 유래 다당체는 면역 세포 표면(예, 대식세포)에 있는 한가지 또는 여러가지 수용체(${\beta}$-glucan 수용체, mannose 수용체, complement 수용체 3, Toll-like 수용체, scavenger 수용체)와 면역 반응을 시작하기 위하여 결합할 수 있고, 그 후에 신호전달을 위하여 cytokine과 second messenger($Ca^{2+}$, cAMP, cGMP, NO 등등)들을 생성한다. 그 결과로 선천면역과 적응면역을 유도하여 다양한 질환에 영향을 미친다. 추가적으로 다당체들의 면역조절은 신경내분비 체계의 조절과도 연관되어 있다. 최근 수많은 천연소재유래 다당체의 생리활성에 관한 연구가 진행되고 있다. 다양한 연구를 통해 정확한 약효, 낮은 독성과 부작용을 가진 일단의 다당체가 발견되어 현재 임상에서 암 치료에도 사용되고 있다. 이러한 사실들은 면역증진 다당체들이 항 종양치료를 위한 새로운 치료제로서 큰 시장을 개척할 수 있다는 가능성을 보여주고 있다. 이러한 사실에도 불구하고 현재 다당체에 대한 연구는 다음과 같은 취약점을 내포하고 있다. 첫째 다당체의 생리활성에 대한 정해진 비교기준이 없다. 특별히 임상 환경에서는 이러한 연구들이 중국, 일본, 한국과 일부 다른 동양국가들에 국한되어 있어 유럽이나 미국에서의 연구 자료가 결여되어있다. 둘째 현재 연구에 사용되고 있는 대다수의 천연소재 유래 다당체는 순수한 다당체가 아닌 천연 그대로의 것을 사용하고 있어 소재로부터 유래하는 다른 활성성분에 의해 쉽게 영향을 받을 수 있어 순수 다당체가 면역체계에 어떠한 역할을 수행하는지 진단하는데 어려움이 있다. 셋째 면역조절 경로에서 다당체의 정확한 신호전달 경로가 아직 정확하게 밝혀지지 않았고, 또한 동물과 임상실험에서 다당체가 작용하는 수용체 및 신호전달 경로에 대한 상반되는 결과가 발표되고 있기도 하다. 따라서 다당체의 면역조절활성에 대한 연구에 있어 다음과 같은 점들에 주목해야 할 필요가 있다. 첫째 다당체의 구조와 활성간의 상관관계를 밝혀야 할 것이다. 특히 다당체의 세부 구조 및 활성 부위를 밝혀낸다면 분자수준의 면역조절 기작을 명확히 하는데 도움을 줄 수 있을 것이다. 이러한 정보를 확보할 수 있다면 다당체의 구조를 변형시켜 활성을 증진시킬 수 있을 것이며, 또한 활성이 증가된 다당체를 합성하는 일도 가능해 질 것으로 예상된다. 마지막으로 ${\beta}$-glucan과 lentinan과 같은 정제된 다당체의 연구를 지속적으로 수행하여 효과적인 동물실험 및 임상실험 protocol을 확보하고 연구결과들에 대한 database를 구축해야 할 필요가 있다. 이러한 노력들이 성공적으로 수행된다면 천연소재유래 다당체들이 가까운 장래에 암 치료를 포함한 다양한 질병에 적용 가능한 새로운 면역조절제로 사용될 수 있을 것이다.

The Evaluation of Recornbinant Circumsporozoite Protein in Malaria Diagnosis (말라리아 진단시 재조합 Circumsporozoite 단백질의 유용성 평가)

  • It had been evaluated the recombinant Circumsporozoite(CS) protein of Plasmodium viva in serologic diagnosis of vivax malaria. Western blot was done to analyse the sera of malaria patients according to the days after onset. The sera which have the terms within 15 days were shown 43.8%(14/32) of positive rates and the sera over the 16 days were shown 94.4%(17/18) of positive rates. So the total positive rate was 62%(31/50). It was 22.6%(7/31) which was shown negative response in Western blot, even though they were shown positive response in Immuuofluorescent antibody test(1FAT) using whole blood stage antigens. The positive rate of non-epidemic area(Yechon-gun, Kyongsangbuk-do) was 10.7%(3/28), and epidemic area(Kangwha-gun, Inchon-shi) was 27.6%(13/47) in Western blot analysis using recombinant CS protein. In order to applicate the recombinant CS protein in seroepidemiological survey, blood samples of 422 inhabitants were collected who lived in malaria epidemic areas, Chosm-ri, Majeong-ri, Hyangyang-ri and Noejo-n in Paju-shi, Kyonggi-do. All of them were negative in microscopic examination and two(0.5%) of them were positive in Polymerase Chain Reaction. 42(10.0%) of them were seropositive in FAT using whole blood antigens and 71(16.8%) of them were seropositive in Enzyme-linked immunosorbent assay using recombinant CS protein. It was figured out the positive rates were much higher according to the distances of villages which were closed to the demilitalized zone(DMZ) in all kind of diagnostic methods, respectively.

  • Enzymes are being used in various fields due to their unique property of substrate specificity. Enzyme-linked immunosorbent assay(ELISA) has enabled the detection of various antigens by reporting the binding event of antigen and antibody via enzyme-catalyzed reaction. However, the sensitivity improvement of conventional ELISA has been limited because only one enzyme molecule is conjugated to one molecule of antibody. To overcome this limitation and further improve the sensitivity of ELISA, there have been efforts to increase the number ratio of enzymes to antibody. Recently, the nanobiocatalytic approaches, with their successful enzyme stabilization, improved the performance stability as well as sensitivity in a modified protocol of ELISA. The present paper introduces the basic principle of ELISA, and the recent efforts to improve sensitivity and performance stability of ELISA by using the nanobiocatalytic approaches.

  • A hepatitis B Surface Antigen(HBsAg)-screening kit using immunochromatographic assay(ICA) method was developed by e employing two kinds of antibodies. One is mouse monoclonal anti-HBs for tracer antibody and the other is goat p이yclonal a anti-HBs for capture antibody. This capture antibody was immobilized on the surface of nitroceliulose(NC) membrane and the t tracer antibody was conjugated with g미d particles. When serum sample was added to the sample well, the $\infty$njugates d deposited in a dry state on the surface of glass fiber filter were reconstituted and then combined with HBsAg in serum. In 5 5 min after adding, the assay result was visible through the window, that is, the complexes composed of HBsAg and the c conjugates appeared as maroon line on the lower part of the NC membrane. The detection limit of the ICA kit was 2 ng/ml w when being tested with the reference HBsAg.

  • Sjogren's syndrome is a chronic autoimmune disorder of the exocrine glands with associated lymphocytic infiltrates of the affected glands. Primary SS presents alone as xerostomia, keratoconjunctivitis sicca and patotid gland enlargement, secondary SS occurs in connection with other autoimmune disorder such as rheumatitoid arthritis, systemic lupus erythematosus, or progressive systemic sclerosis. Among many other systemic complication, lymphoma is considered seriously. Patient with SS had a 44 times higher relative risk of lymphoma, and clinically identifiable lymphoma occurs in approximately 5% of patients with SS. So, patients with SS should be closely monitored. In dental office, diagnosis of SS is important in view of high risk of lymphoma. When a dentist diagnose and manage dry mouth, he or she should consider possibility of SS all the times and have knowledge of diagnostic criteria of SS.

  • Kawasaki disease (KD) is an acute, systemic inflammatory disorder that often targets coronary arteries. Being the common cause of acquired heart disease in children, timely diagnosis and intravenous immunoglobulin treatment are crucial. However, it is challenging for physicians to diagnose KD if it presents with atypical manifestations. We report the case of a 5-year-old boy who initially presented with appendicitis; after an appendectomy, he had a prolonged fever. He was finally diagnosed with atypical KD and successfully recovered after intravenous immunoglobulin treatment. Through a literature review, we found 21 cases of appendicitis associated with KD. In most cases, the patients were male with a mean age of 5.3 years. Most had higher proportions of incomplete KD and coronary artery complications than expected for typical KD. In conclusion, appendicitis could be a rare complication of KD; therefore, multidisciplinary cooperation and early recognition of atypical KD are essential for timely diagnosis.

  • Ten pigs infected with Aujeszky's disease virus (ADY) or hog cholera virus(HCV) were tested for the detection of virus antigens in frozens or paraffin-embedded sections by avidin-biotin-peroxidase complex(ABC) method. Tonsils, spleens, cerebra and buffy coats were examined for the immunohistochemical test. Where ADV antigen was detected by ABC, a dark brown deposit occurred in both the nucleus and the cytoplasm of lymphocytes and macrophages, however, HCV antigen was demonstrated in the cytoplasm of the infected cells. ADV-positive cells were most frequently detected in tonsils and cerebra, whereas, HCV -positive cells were frequently observed in spleens. And buffy coat were also good for both virus detection. The results suggested that ABC method is considered as an excellent and reliable tool for confirmative diagnosis of these viral diseases.

Clinical and Immunohistochemical Characteristics of Malignant Aortic Body Tumor in a Pekingese Dog (페키니즈에서 발생한 악성 대동맥체 종양의 임상양상과 면역조직화학적 특성)

  • A 12-year-old castrated male Pekingese dog was presented with weakness, exercise intolerance, and an episode of coughing started 2 weeks earlier upon presentation. Physical examination revealed a grade II/VI left systolic heart murmur. Echocardiographic examination revealed a pericardial effusion and a mass at the heart base located beside the left atrium. The mass beside the left atrium was also visible on the thoracic radiograph after drainage of the pericardial effusion. An aortic body tumor was definitely diagnosed based on postmortem histopathological examination and immunohistochemical staining for specific markers. The dog lived 234 days after diagnosis with only medical management and without recurrence of the pericardial effusion.
