Evaluation of Applicability of Customized Bolus According to 3D Printer Material Characteristics (3D 프린터 소재 특성에 따른 맞춤형 볼루스의 적용성 평가)

  • Bolus is used in radiation therapy to prescribe an even dose to the tumor when the skin surface is inclined or has irregularities. At this time, the dose to the skin surface increases. Due to the patient's unique body structure and irregular skin, voids may occur between the bolus and the skin, which may reduce the accuracy of treatment. Therefore, in this study, the existing bolus and the self-produced bolus through 3D printing were applied to the nasal area, and the difference between the surface dose after treatment plan and the dose directly measured with an Optically Stimulated luminescence(OSL) dosimeter was compared to the existing bolus. The bolus rate was 97%, PLA 100.33%, ePETELA 75A 100.53%, and ePETELA 85A 100.36%. It was confirmed that there was little error in the measurement values and treatment plan values for each material. In addition, compared to when applying a conventional bolus, a difference of -3% to +0.5% for a 3D printed bolus can be confirmed, so a customized bolus produced through 3D printing can complement the shortcomings of the existing bolus.   • The picosecond time resolved fluorescence spectra of Hematoporphyrin Derivative (HPD) in both solutions and cancer cell are measured by a time correlated single photon counting system with a synchronously mode locked dye laser. Two exoponential decay components in the fluorescence spectra were observed. The slow decay(6.3 ㎱)and the fast one(350 ㎰)are attributed to be originated from monomers and dimers, respectively. The absorption and fluorescence measurements in steady state also showed the presence of a monomeric and dimeric forms of HPD molecules. The monomer lifetime in the cancer cell was measured to be longer than that in solution, which was expected from the blue shift and narrowing of the absorption spectra for HPD-treated in vitro. The relative amplitude of the fast component was found to be enhanced in cancer cell, strongly indicating the higher affinity of the dimer for the cancer cell.

The Association Between Neurodegenerative Diseases and Development of Type 2 Diabetes (신경퇴행성 질환과 제2형 당뇨병 발생의 연관성)

  • Objectives : A growing body of evidence links type 2 diabetes (T2D) with a neurodegenerative disease (ND) such as Alzheimer's disease and Parkinson's disease. The purpose of this study is to investigate the relationship between NDs and the development of T2D by comparing the incidence of T2D in a group of various NDs (ND group) and control group. Methods : A population-based 10-year follow-up study was conducted using the Korean National Health Information Database for 2002-2015. We used a retrospective cohort study design to investigate the association of ND with T2D occurrence. The study population included ND (n=8,814) and control (n=37,970) groups, all aged 60 years or over. The Kaplan-Meier method was used to estimate the risk of developing T2D as a function of time. Cox proportional hazards regression models were used to evaluate the relationship between ND and T2D. Results : T2D was developed in a significantly higher percentage of patients in the ND group (53.6%) than in the control group (44.7%). The ND group increased the risk of T2D (HR, 1.43; 95% CI, 1.38-1.47). About one-third of patients in both groups were additionally diagnosed with another ND before the occurrence of T2D during a 10-year follow-up period. When compared to those who did not have another ND during the follow-up period, the incidence of T2D in those who were additionally diagnosed with another ND was higher in both the ND and control groups. Conclusions : The ND group had about 1.4 times higher risk of developing T2D than the control group. Our results showed a positive association between ND and T2D.

  • The source position and source dwelling time in a given source arrangement in the applicators is very high effect to determine the expose time which in general is derived from the brachytherapy planning system. In high dose rate (HDR) intracavitary radiation therapy (ICRT), the treatment is often performed in based out-patient during the whole fractionation irradiations. However, the patient should be waited on coutch for ICR treatment in first start fraction as unconvinent and immobilized state until perform the dose plannings. In our experiments, the HDR source contributed dose for$55.89{\pm}4.20%$ for straight tandem source, $38.14{\pm}4.46%$ for the right ovoid soucre on the fornix and$5.97{\pm}0.50%$ for left ovoid source. It also showed the $60.33{\pm}6.53%$ for the tandem, $33.10{\pm}6.74%$ for right ovoid and $6.58{\pm}0.30%$ for the left ovoid source in 10 degrees of applicator. The authors designed the source template dose planning software for ICRT of uterine cervix results average $-0.55{\pm}2.15%$ discrepancy of the full charged brachytherapy dose planning. Developed Source temperate ICRT plaanning software guide a minimized the complains and operating times within a ${\pm}3%$ of dose discrepancies.

Perspective of a New Precision Medicine and Health Care Research (새로운 맞춤형 정밀의학과 보건의료 연구에 대한 조망)

  • The concept of precision medicine-prevention and treatment strategies that take individual variability into account-is hot issue of US in the year 2015. Precision medicine is a new concept that approach patients individually by there characteristics, such as genome, life style, environmental exposure, etc. For developing the precision medicine, National Institute of Health of US has been prepared the Precision Medicine Initiative Cohort Program, at least 1 million people cohort. The US President Obama announced the Precision Medicine Initiative on 30th January 2015. He announced that he will pioneer a new model of patient-powered research that promises to accelerate biomedical discoveries and provide clinicians with new tools, knowledge, and therapies to select which treatments will work best for which patients. Most medical treatments have been designed for the 'average patient.' As a result of this 'one-size-fits-all-approach,' treatments can be very successful for some patients but not for others. This is changing with the emergence of precision medicine, an innovative approach to disease prevention and treatment that takes into account individual differences in people's genes, environments, and lifestyles. Precision medicine gives clinicians tools to better understand the complex mechanisms underlying a patient's health, disease, or condition, and to better predict which treatments will be most effective. The healthcare researcher should prepare the new medicine era such as bio-information technology convergence, big data study.

The Effects of Individualized Cognitive Program on LOTCA-G and ADL in Elderly with Dementia and Mild Cognitive Impairment (맞춤형 인지프로그램이 치매와 경도인지손상노인의 LOTCA-G 및 일상생활동작에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate the influence of LOTCA-G and ADL by individualized cognitive program in elderly with vascular dementia, alzheimer dementia and mild cognitive impairment. Method : The subjects of this study, old man and woman diagnosed with vascular dementia, alzheimer dementia and mild cognitive impairment, 24 patients were picked up, who were agreed with this research and were having hospital care for 3weeks at nursing care centers. Individualized cognitive program was applied to 8 patients of vascular dementia, 8 patients of alzheimer dementia and 8 patients of mild cognitive impairment. Cognitive function measured by LOTCA-G and performance measured by FIM. The SPSS Ver. 22.0 statistical program was used for data processing. The significance level for statistical inspection was set as 0.05. Result : In comparison of LOTCA-G was significant increased after intervention and among three groups were significant difference. But FIM was no significant difference after intervention and among three groups were no significant difference. Conclusion : Therefore, the individualized cognitive program is useful to improve the cognitive function in elderly with dementia and mild cognitive impairment.

Effects of Korean Medical Treatments Combined with Foot Orthotics on Patients' Center of Gravity with Flat Foot : a retrospective study (후향적 연구를 통한 맞춤형 족부보조기와 한방치료를 병행한 편평족 환자의 인체중심선 변화에 대한 연구)

  • Objectives : The purpose of this study is to find out the effect of Korean Medical treatments combined with Orthotics on patients's center of gravity with flat foot. Methods : We reviewed the medical records of 24 patients diagnosed with flat foot. They had received Korean medical treatments combined with Foot Orthotics. To estimate the efficacy in rebalancing of body, we analyzed the changes in center of gravity. Results : Among the 24 patients, 13 were male and 11 were female. There was no statistically significant change in frontal angle at the time of treatment. But side angle was changed statistically significant(P=0.009). There was no difference according to sex of male and female. Conclusion : In this study, We found that Korean Medical treatments with Foot Orthotics was effective for correction of patients's center of gravity with flat foot.

A Study on the Field Application and Prospect of Artificial Intelligence and Bio-Sensing Technology in Physical Therapy: Focusing on Customized Rehabilitation Treatment (물리치료 분야에서 인공지능 및 바이오센싱 기술의 현장적용 및 전망에 관한 연구: 맞춤형 재활치료를 중심으로)

  • PURPOSE: This study analyzed the impact of AI and biosensors on physical therapy, identifying the stage of customized technology development and future prospects. AI and biosensors improve the efficiency, establish customized treatment plans, and expand patient treatment opportunities. The study employed a literature review by searching databases and collecting research. METHODS: This study searched various databases related to the topic, collected existing research, papers, and reports, evaluated the literature, and summarize the results. RESULTS: Exercise therapy utilizing artificial intelligence can provide personalized and optimal exercise plans while monitoring rehabilitation progress. In addition, biosensors such as EMG sensors and accelerometers can monitor the individual progress in physical therapy, particularly in stroke patients, which can help improve physical therapy strategy and promote patient recovery. CONCLUSION: This study suggested that artificial intelligence can be applied in many areas of physical therapy, such as exercise therapy, customized treatment plans, rehabilitation and management, pain management, neuro rehabilitation, and auxiliary devices. Using AI technology, it is possible to analyze and improve exercise and posture, retrain the central nervous system, establish customized treatment plans for individual patients, predict and compare patient progress before and after treatment, and provide customized pain analysis and treatment methods. In addition, AI can provide neuro rehabilitation programs and customized auxiliary devices.

The Effect of Cane Height on Walking and Balance for Stroke Patients (뇌졸중 환자의 지팡이 높이에 따른 보행과 균형에 미치는 영향)

  • Objecctives: This study investigates the effects of using customized walking aids individualized for stroke patients by measuring the effects of different cane lengths to determine the ideal length of walking aids for stroke patients. Methods: Cane lengths were determined from the greater trochanter with walking aids measured 5cm below, at the greater trochanter and 5cm above. All patients walked for ten meters with each cane length to measure speed. Then, we measured the opto gait, timed up go test, and electromyography three times each. Statistical analysis was performed using a linear mixed model, and in the case of significance, the p-value was corrected using the Bonferroni method. Results: There was a statistically significant differences in time up and go test(TUGT), 10m walking, stride and speed between the groups. Conclusions: Long cane length increases body symmetry, stride, increasing muscle activity, and short cane length increases balance and walking.

Career Guidance Program Design for the Graduate Medical School Students (의학전문대학원생을 위한 진로지도 프로그램 개발)

  • The purpose of this study was to design career guidance program based on the research of career demand in order to help students choose and prepare careers suitable to students' interests and aptitude. The study surveyed 211 students in the $3^{rd}$ and $4^{th}$ years of C medical school. The survey was about start time of career guidance by medical schools, information collecting method for major selection, conformity degree among interest-aptitude-major after graduation, demanded information for career guidance, considerations in selecting majors, the reasons for not being able to select majors, etc. The result showed that students thought that start time of career guidance should be good for juniors, seniors, or all grades in order. The students got the information about career selection mostly from the direct contact with seniors. Students who had already decided the career, showed the high conformity degree among interest-aptitude-major after graduation Students who had already decided the majors, highly considered the aptitude and interest, while students who had not decided the majors, did not know their aptitude or did not have enough information about majors. This study gave the basic direction for the career guidance and provided necessary information for designing customized career guidance programs by reconstructing contents of the programs according to school conditions.
