Verification of Validity on the Manual Ability Classification System in Children With Spastic Cerebral Palsy (경직형 뇌성마비 아동의 손 기능 분류 체계의 타당도 검증)

  • The purpose of this study was to examine the validity of the Manual Ability Classification System (MACS) by analysing of relation between MACS and Jebsen-Talyor Hand Function Test. The concurrent validity was examined by calculation of correlation between MACS and Jabsen-Talyor Hand Function test and the discriminant validity was examined by measurement of hand function difference according to MACS level. For this, eighty-one children with spastic cerebral palsy were employed in this study. The children were evaluated by using the MACS and Jebsen-Taylor Hand Function Test for their hand function. There were a significant correlation between the MACS and Hand function (r = .870, p < .05). The good correlation between the MACS and subtest of Jabsen-Talyor Hand function (p < .05). The hand function according to the MACS level were different significantly (p < .05). The MACS is valid classification system for assessment of hand function of children with cerebral palsy. The MACS in practice will provide usefulness for assessment of hand function in children with spastic cerebral palsy.

  • 본 연구에서는 사지마비 장애인이 활용할 수 있는 동작을 이용하여 마우스, 문자 입력, 전동휠체어 제어 등에 활용될 수 있는 인터페이스 기술을 개발하였다. 사지마비 장애인이 활용 가능하면서도 착용이 쉽고 데이터 처리가 용이한 동작으로서 이물기 동작을 제안하였다. 좌측, 우측 및 양측 이물기 동작 및 이물기 시간의 조절을 통해 사지마비 장애인의 경우에도 다양한 명령어 형성이 가능하다. 이때, 이물기 동작의 인식은 관자놀이 부근에 위치한 관자근에서의 근전도 신호를 이용하였다. 본 연구에서는 데이터를 획득하여 전송하고 처리하기 위한 하드웨어와 소프트웨어를 함께, 전동휠체어 제어를 위한 응용 시스템도 개발하였다. 또한 개발된 시스템을 사지마비 장애인에게 적용하여 그 효용성을 평가하였다.

  • Bell's palsy is an acute-onset unilateral peripheral facial neuropathy. For patients with sequelae of facial paresis, the successful rehabilitation of fully edentulous arches is challenging. This case report described the treatment procedures and clinical considerations to fabricate complete dentures of a patient who showed unilateral displacement of mandible, unilateral chewing pattern and parafunctional jaw movement due to sequelae of Bell's palsy. Gothic arch tracing was used to record reproducible centric relation and lingualized occlusion was performed to provide freedom to move between centric relation and the patient's habitual functional area in fabricating satisfactory dentures in terms of function and esthetics.

  • In this paper, we analyze the interaction features using eye-tracker and the characteristics of attention concentration of children with intellectual disabilities, and present gamification elements that can be used when designing digital contents for them. For children with quadriplegic cerebral palsy, eye-trackers will increase the accessibility of digital contents. Foreground gamification can be used for selective attention concentration, background gamification and intrinsic motivation to sustain attention concentration, and foreground gamification and extrinsic motivation for mobility.

Clinical Aspects in Patients with Thyrotoxic Periodic Hypokalemic Paralysis (갑상선 중독성 주기성마비 환자의 임상적 고찰)

  • Background: Thyrotoxic periodic paralysis is an uncommon illness characterized by intermittent flaccid paralysis of skeletal muscle, usually accompanied by hypokalemia, in patient with hyperthyroidism. However, the pathophysiology of thyrotoxic periodic paralysis remains largely unexplained and controversial. This report describes the clinical and biochemical findings in 19 patients with thyrotoxic periodic paralysis who were examined at the Yeungnam University Medical Center(YUMC) during the past decade. Methods: The medical records of 997 YUMC patients, seen between 1986 and 1996, with diagnosis of hyperthyroidism were reviewed. Nineteen patients out of 997 hyperthyroidism patients were diagnosed, and examined by history, physical examination, serum electrolyte value, and thyroid function test during paralysis. On the basis of these results, comparisons were made on age, sex, precipitating factors, timing, affected limbs, prognosis, serum potassium and serum phosphate and thyroid hormone levels. Results: The prevalence of periodic paralysis in hyperthyroidism was 1.9 percent and the male to female prevalence ratio was 30:1 and in all patients, the development of perodic paralysis was correlated with hyperfunctional state of the thyroid gland. Eleven cases of periodic paralysis were associated with hypokalemia and their thyroid hormone levels were significantly more increased than those of the patients without hypokalemia. Interestingly, our study shows the recurrence of paralysis after treatment. Conclusion: Although the precise pathophysiology of the disease is as yet undefined and controversial, it occurs primarily in Asians with an overwhelming male preponderance and prevalence of 2 percent in hyperthyroidism. The interactive roles of thyroid hormone, Na-K pump, and genetically inherited defect in the cellular membrane potential of the skeletal muscle can be speculated. Further investigation will be needed to firmly establish the mechanism of thyrotoxic periodic paralysis.

A Systematic Review of Modified Constraint- Induced Movement Therapy in Children With Hemiplegic Cerebral Palsy (뇌성마비 유형 중 편마비 아동을 위한 수정된 강제유도 운동치료의 효과에 대한 체계적 고찰)

  • Objective : This research investigated the intervention effects, protocol of modified constraint-induced movement therapy in children with hemiplegic cerebral palsy. Methods : For the key words of a database search, "Cerebral Palsy", "Hemiplegia", "Constraint Induced Movement Therapy", "modified Constraint Induced Movement Therapy" were used. We examined papers published in journals from January 2001, when the modified Constraint Induced Movement Therapy was first suggested, to May 2011, using PubMed, Medline. Ovid. Results : A total of 10 papers were analyzed and results of modified Constraint Induced Movement Therapy were an effective therapeutic method to improve motor function, quality of movement and that they also increased the frequency of functional use of the affected hands of hemiplegic cerebral palsy. Conclusion : This paper conducted a systematic review of the research literature reporting on the effects of modified constraint-induced movement therapy in children with hemiplegic cerebral palsy. Analysis of the fewer number of papers, there was limitation that we consider studies at all levels of evidence. However the restraint methods should be decided according to the characteristics of the individually and apply a variety of therapeutic activities, there is positive support for the use of modified constraint-induced movement therapy to improve the recovery of the paretic upper extremity with cerebral palsy.

The Effect of Day-Hospital Rehabilitation Program on Gross Motor Function for Children with Cerebral Palsy (재활 낮병동 프로그램이 뇌성마비아동의 대동작기능에 미치는 효과)

  • The purpose of this study was to find the effect of Day Hospital Rehabilitation programs on gross motor function in children with cerebral palsy. The subjects were 57 children with spastic cerebral palsy with no previous botulinum toxin injection or operation history within 6 months. The Gross Motor Function Measure(GMFM) score and Gross Motor Function Classification System(GMFCS) were used to evaluate as functional change and functional level. The Programs for Day Hospital Rehabilitation period for cerebral palsy children was 8 weeks. The results of the study are as follows: 1, GMFM Score of pre- and post-programs for Day Hospital Rehabilitation showed the statistically significant difference(p <.001). 2, The age group in 1-2 was higher than age 5-6 group, 3-4 was higher than age 5-6 group by Post-hoc analysis. Lastly, there was significantly different of GMFM in GMFCS level(p<.05), especially mild(GMFCS level I) and moderate(GMFCS level II, III) of CP showed that gross motor function was significantly different than severe(GMFCS level IV, V). The programs for Day Hospital Rehabilitation was effective on gross motor function for children with cerebral palsy, and for early intervention needs to mild, moderate CP, and below age 4 group.

Effect of Vibration Stimulation Training on Upper Extremity Function in Children with Cerebral Palsy (뇌성마비 아동의 진동 자극 훈련이 상지기능에 미치는 영향)

  • Objective : The purpose of this study is was to find ouetermine whether training to applying vibration stimulation to the biceps brachii of children in the late stages of spasticity hemiplegic cerebral palsy can helps to improve the function of the upper extremity. Methods : This The study was conducted on with three children with cerebral palsy, all between the ages of 13 and 15 years. Among the experimental research methods of used with individual subjects, an AB research design using multiple basic baseline individual experimental studies was used, and vibration stimulation was provided to the paralyzed hand during the intervention period. The An evaluation was conducted before and after each session used to measure the function of the upper extremity using was conducted after each session of the Rapael Smart Board and The Jebsen-Tayler hand function test and the Motor Activity Log (MAL) were conducted before and after the experiment. Results : As a result of measuring the smooth The average score and total scores of for using the Rapael Smart Board to measure the upper extremity function in following each therapeutic session using the Rafale smart pegboard showed that, Subject 3 did not showevidenced a no significant change in the average value, and but Subjects 1 and 2 did showed a significant changes in their average values. All three subjects showed significant changes in the Jepson-Taylor hand function test and in the Motor Activity Log test, as evaluated before and after the intervention. Conclusion : Training The using use of vibration stimulation showed a positive effect on in improving upper limb function and exercise in hemiplegia hemiplegic children with who had little experience on using their hemiplegic side.

Effect of Self Care Training(based on International Classification of Functioning, Disability and Health) on Functional Independence in the Young Children with Spastic Cerebral Palsy (국제 기능 장애 건강분류의 구성요소에 기반을 둔 자기관리 훈련이 경직성 뇌성마비 아동의 기능적 독립성에 미치는 영향)

  • The purpose of this study was to investigate the effect of self-care training based on ICF(International Classification of functioning, Disability and Health) on functional independence in the young children with spastic cerebral palsy. Total of 43 young children(male=25, female=18; age range from 36month to 72month) with spastic cerebral palsy, classified at GMFCS(Gross Motor Function Classification System) levels III-IV. Total of 32sessions of a self-care training (eating, grooming, bathing, toileting) were given 4 times a week for 30minutes from August 1th to September 30th of 2008. Changes in the functional independence after the training obtained by Wee-FIM(Functional Independence Measure for Children). Results were as follows: Functional independence was significantly increased after the training. As a result, a self-care training should be applied as an effective intervention to improve the functional independence in the young children with spastic cerebral palsy.

Effects of Vocalization on Upper Extremity Muscle Activity during Reaching Task in Patients with Hemiplegia (발성이 편마비 환자의 팔 뻗기 시 상지근육의 근활성도에 미치는 효과)

  • This study examined the effects of vocalization on upper extremity muscle activity during reaching task in patients with hemiplegia. Thirteen persons with right brain stroke performed reaching to a cup under four concurrent speech conditions of vocalizing the word "Ah" with hemiplegic side. These four conditions are self-vocalization, external vocalization, imaginary vocalization, and no vocalization. The muscle activity(Biceps brachii, Triceps brachii, Middle deltoid, and Upper trapezius) were measured using MP150. Muscle activity was significantly higher under self-vocalization and external vocalization conditions compared to the muscle activity under imaginary vocalization and no vocalization conditions on triceps brachii muscle.(p<0.05). Triceps brachii muscle was highly correlated with biceps brachii muscle(r=0.777, p<0.05). The results suggest that self-vocalization and external vocalization can be used in facilitating upper extremity movements in patients with stroke. When working with patients with right hemispheric stroke, therapists might explore possibilities of using patient's self-speech to enhance the quality of upper extremity movement performance.
