Pre-surgical and post-surgical change in adult cleft lip and palate patient following Le Fort I advancement osteotomy combined with bone graft was evaluated clinically and cephalometically. We obtained a successful function and esthetic improvement. The bone graft of alveolo-palatal clefts provides a stable bone support to the adjacent teeth of the cleft area, and well union of adjacent bone tissue, the closure of oronasal fistula and improvement of speech problem. Le Fort I osteotomy following the ostectomy of nasal septum for advancement of the maxilla was obtained relative improvement of esthetics and functional occlusion. 1. The orthodontic correction was required before and after surgery. 2. In this case, there was a limited range of anterior advancement of the Premaxillary-segment due to the scar tissue. 3. After 8 months of operation, we could show the new bone deposition on the cleft site in dental radiograph and then the prosthetic treatement to the missing teeth was done.

Objective: CBCT has become popular for orthodontic diagnosis and treatment planning in recent times. The 3D pharyngeal airway space needs to be analysed using a 3D diagnostic tool. The aim of this study was to analyse the pharyngeal airway of different craniofacial morphology using CBCT. Methods: The sample compromised 102 subjects divided into 3 groups (Class I, II, III) and 6 subgroups according to normal or vertical craniofacial patterns. All samples had CBCT (VCT, Vatech, Seoul, Korea) taken for orthodontic treatment. The pharyngeal airway was assessed according to the reference planes: aa plane (the most anterior point on the anterior arch of atlas), $CV_2$ plane, and $CV_3$ plane (most infero-anterior point on the body of the second & third cervical vertebra). The intergroup comparison was performed with one-way ANOVA and duncan test as a second step. Results: The results showed the pharyngeal airway and anteroposterior width of group 2 (Class II) in aa plane, $CV_2$ plane, $CV_3$ plane were significant narrower than in group 3 (Class III). There was no significant difference between vertical and normal craniofacial patterns except for the anteroposterior pharyngeal width of Group 1 (Class I) in aa plane. Conclusions: Subjects with Class II patterns have a significantly narrower pharyngeal airway than those with Class III. However there was no difference in pharyngeal airway between vertical and normal craniofacial morphology.

Many treatment approaches of Cl III malocclusion have been introduced and the choice of treatment should be a function of the individual problem, not of the clinician(personal preference, experience and success rate of the operator). Therefore a function of the individual problem should be analysed exactly Much has been written in the orthodontic literature concerning the nature of Cl III malocclusion. It has been reported by many investigators that a Cl III malocclusion occurs in a variety of skeletal and dental configurations by differences of race and age. Lateral cephalometric radiographs of 125 individuals were studied for the presence and distibution of four horizontal components and one vortical component in a manner similar to McNamara. The results were as follows : 1. Cl III malocclusion is not a single clinical entity. It can result from numerous combinations of skeletal and dental components. 2. Maxillary skeletal retrusion was the most common single charateristic of the Cl III sample. 3. Only a small percentage of the cases in this study exhibited maxillary dentoalveolar protrusion. 4. Only a small percentage of the cases in this study exhibited mandibular dentoalveolar retrusion. 5. Mandible was usually well-positioned, but a wide variation was observed. 6. A large percentage of the cases in this study exhibited excessive vertical development. Thus, it appears that in designing the ideal treatment regime, those approaches which might restrict vertical development and promote maxillary horizontal growth could be more appropriate in many cases.

Obstructive sleep apnea (OSA) is a disorder characterized by repetitive episode of upper airway collapse during sleep. Recent studies showed that not only the anatomic factors but the physiologic factors of the upper airway also have effcts on the occurrence of apnea and that the genioglossus muscle also plays an important role in the maintenance of the upper airway. A variety of therapies were performed to treat OSA, and among them the use of mandibular repositioning appliances showed reasonable results. But there is still a lack of research on the structural and physiological mechanism upon the use of mandibular repositioning appliances. The author selected 26(male 17, female 9) OSA patients that came to the Yonsei University Dental Hospital, Department of Orthodontics, and 20 normal adults (male 10, female 10) and took cephalometric radiographs of them in a supine position before and after the placement of the mandibular repositioning appliance to see the structural changes of the upper airway and compare the therapeutic effects between the two groups. We also studied the waking genioglossus muscle activity in OSA patients and investigated the difference in the electromyogram of the genioglosssus muscle upon the change in body posture and the use of mandibular repositioning appliance. Following results were obtained. 1. Among the cephalometric measurements of the upper airway structure, the length of the soft palate, maximum thickness of the soft Palate and SPAS, MAS, VAL, H-H1, MP-H showed statistically significant differences between the normal and OSA groups, but the IAS and EAS showed no statistically significant differences between the two groups. 2. In both the normal and OSA groups, as the epiglottis moved forward on wearing the mandibular repositioning appliance, the epiglottis level of the upper airway increased and the maximum thickness of the soft palate changed and the hyoid bone also moved forward, but the IAS in both groups showed various results and the effect of the mandibular repositioning appliance on the structure of the upper airway was different in the two groups. 3. Upon changing the position, the electromyogram of the genioglossus muscle showed a increasing tendency but there was no statistically significant differences, and when the mandibular repositioning appliance were worn there was a statistically significant increase in the electromyogram of the genioglossus muscle in both the upright and supine positions. The mandibular repositioning appliances not only have an effect on the anatomical structure of the upper airway but also on the physiology of the upper airway. There are different responses to the use of mandibular repositioning appliance between the normal and OSA groups therefore it could be considered to have the different physiology of the upper airway between the two groups.

Purpose : To compare cephalometric measurement between measuring methods in digital and conventional lateral cephalometric radiograph. Materials and Methods : Twenty digital and conventional lateral cephalometric radiographs were selected. In digital group, cephalometric measurements were performed manually using hardcopies and automatically using $V-Ceph^{TM}$ program on the monitor. In conventional group, the same measurements were performed manually on conventional films, and for automatic measurement conventional films were digitized by scanner. All measurements were performed twice by 4 observers, and 24 cephalometric variables were calculated and the time spent for each measurement was recorded. The differences in measurements data and the time spent for each measurement were compared within each group. Intra-observer and inter-observer comparisons were performed. Results : In both groups, no statistically significant difference between manual and automatic measurements was observed and most of the variables didn't show statistically significant differences between methods. The observer with less experience tended to show statistically significant differences of measurements between methods, and differences from other observers. The differences of measurements between methods in digital group were lesser than those of conventional group with statistical significance in 8 variables out of 24. With automatic method and in digital group, the spent time was shorter. Conclusion : With direct digital radiograph, automatic method using manually idenitified landmarks can be preferable in cephalometric analysis. (Korean J Oral Maxillofac Radiol 2005; 35 : 15-23)
The purpose of this study was to evaluate the position of tongue and hyoid bone in relation to vortical facial patterns in the adult and child. Lateral cephalograms taken in adults(63 cases, 11.7 years in average age) and children(69 cases, 22.6 years in average age) were traced and measured about position and posture of tongue and hyoid bone using the horizontal and vertical reference lines. The angle of mandibular plane to SN Plane was employed to classify the samples into groups of hypodivergent and hyperdivergent. The comparison of the tongue/hyoid bone measurements between hypodivergent group and hyperdivergent group in the adult and child were statistically executed with Student's f-test. The results were as follows, 1. The tongue height was lower in the hyperdivergent group than in hypodivergent group, and higher in children than in adults. 2. The vertical height of hyoid bone was higher in hypodivergent group than in hyperdivergent group and also higher in children than in adults. 3. The anteroposterior position was of no significant difference in relation to age or vortical facial pattern. 4. The inclination of hyoid bone in relation to cranial base was steeper in children than in adults.
The midpalatal suture area has some advantages for supporting miniscrews : it has no specific anatomical structure, it is composed of thick cortical bone, and covered with attached gingiva. So it is suitable area for inserting miniscrews. However, the midpalatal suture area appears thinner when seen in ceph. As a result, Clinicians can misunderstand that inserting miniscrews cause the problem, both the risk of perforation and the decrease of stability. The purpose of this article is measuring the vertical bone thickness of the midpalatal suture area for inserting miniscrews. The total of 25patient (male : 13, female : 12), who are in their twenties, were taken CT. The vertical bone thickness of the midpalatal suture area was measures from the transverse section of CT. As a result, We reached a conclusion from the differences of each area. It is as follows: 1. There is no significant difference between the thickness of male group and that of female group. 2. In coronal section, Bone thickness becomes thinner from the midpalatal suture to Left & Right side, in sagittal section, Bone thickness becomes thinner from incisive foramen to PNS. 3. The area that is within 3mm of left and right from the midpalatal suture area transversely and within 25mm backward from the incisive foramen sagittaly is enough for inserting miniscrews.
The purpose of this study was to investigate the angle formed by the Sella-Nasion(SN) plane and Frankfort-Horizontal(FH) plane and evaluate the correlation and difference of the FH plane to other horizontal reference planes. Through this study we hope to present a basis for selecting a horizontal reference plae which can be implemented in cephalometric studies and in surgical orthodontic treatment planning. 600 subjects were chosen following a clinical examination md lateral cephlometric X-rays were taken. According to cephalometric analysis the subjects were classified into 3 groups , Skeletal Class I malocclusion or normal occlusion group(male 50, female 50), Skeletal Class II malocclusion group(male 50, female 65) and Skeletal Class III malocclusion group(male 50, female 50). The results were as follows. 1. The angle formed by the SN plane and FH plane showed no difference among the malocclusion groups, but there was a significant sex difference. For males the angle measured was $7.47^{\circ}{\pm}2.40^{\circ}$ whereas for females it was $8.93^{\circ}{\pm}2.72^{\circ}$. 2. The angle formed by the SN plane or FH plane and Mandibular plane was higher in females for all malocclusion groups. This angle in the Skeletal Class I malocclusion group was lower than in the other two groups. 3. There was no difference among the sexes or malocclusion groups considering the angle formed by the FH plane and Palatal plane. 4. The genial angle in the Skeletal Class III malocclusion group was higher than in the Skeletal Class I and Class II malocclusion groups in both sexes.
The purpose of this study was to investigate the variations of the skeletal and dentoalveolar dimensions in relation to vertical facial patterns. Lateral cephalogram of 200 cases (100 cases of male and 100 cases of female, average age of which was 23.2 years) were traced and some measurements of skeletal and dentoalveolar dimensions were measured. The ratio of UAFH/LAFH was employed to classify the samples into groups of excess and short lower anterior facial height. And the comparison between two groups were taken statistacally. The following results were obtained. 1. The dentoalveolar height, lower anterior facial height, lower genial angle, and FMA in the excess-lower-anterior-facial-height group were significantly larger than those in short-lower-anterior-facial-height group. 2. The dentoalveolar height, facial height, ramus height, and Jarabak ratio in the male subjects were significantly larger than those in the female subjects. 3. The UAFH/LAFH ratio showed a significant correlation to upper, lower facial height, AUDH, PUDH, ALDH, PLDH, Lower gonial angle, FMA, and $Bj\"{o}rk's$ Sum.

Objective: The purpose of this study was to find changes in the occlusal plane related to different vertical facial patterns and suggest treatment goals and conduct possible treatment mechanisms. Methods: 60 adult patients (28 males, 32 females) who had been diagnosed as Class 1 skeletal malocclusion and treated without extraction were selected. Patients were divided into three groups; short face type (group 1), average face type (group 2) and long face type (group 3), using the data on normal occlusion of Korean adults. Results: The results were achieved by analyzing cephalometric tracings of each group at pre-treatment, end-treatment and post-treatment (about 1 year recall check). The inclination of the occlusion plane tends to gradually increase as the face becomes longer In group 1, COP-X, FOP-X, L6/L1, MP-L6 were significantly decreased, and L1-FOP was significantly increased during the retention period (T3-T2). Group 2 showed no significant change, In group 3, FOP-X was significantly increased during the retention period (T3-T2). During the retention period, FOP-X showed significant change among each group, especially between group 1 and group 3. Conclusion: These results suggest that changes of occlusal plane inclination according to facial vertical pattern need to be considered during the retention period for intrusion, extrusion, and incisor overbite.
