Background and Objectives: The popularity of complementary and alternative medicine(CAM) has grown at an astonishing rate in recent years. Because patients are interested in CAM and want to use it, physicians also should be concerned about CAM and know about the current situation of its usage. In this study, we tried to determine the prevalence of CAM use in the patients with head and neck cancer and correlate with demographics and tumor characteristics. Materials and Methods: A total of 87 patients with head and neck cancer previously treated with surgery at Seoul National University Hospital from March through June, 2004 were enrolled in this study. The patients were completed, questionnaire about the use of CAM and demographic information. Tumor characteristics were obtained from the patient's medical records. Results: Of the 87 patients, 33 patients(37.9%) use at least one CAM. There was no statistically significant correlation between patient's data and prevalence of CAM use. Of those patients using CAM, 16(48.5%) of 33 were using 2 or more therapies. The majority of agents used by patients in this study are mushrooms such as Phellinus and Reishi, followed by dietary therapy. A majority(60.6%) of patients were introduced to alternative medicine by friends or family members. Conclusion: Considerable cancer patients use CAM. Head and neck oncologists should inform themselves about CAM to counsel and educate the patients. Further scientific investigations on promising CAM agents may help advance cancer treatment.

Purpose : To assess the functional change of salivary gland, recovery of salivary destruction, difference of susceptibility to irradiation between the parotid and sub-mandibular gland, correlation between the xerostomia and excretory rate by irradiation in salivary scan using $^{99m}Tc$-pertechnetate in patients with head and neck cancer. Materials and Methods : We evaluated 19 patients with head and neck cancer who had taken $^{99m}Tc$-pertechnetate salivary gland scan before, during and after radiotherapy. We measured the uptake of radioactivity every 5 minutes for 60 minutes after injection of $^{99m}Tc$-pertechnetate(370MBq). We also evaluated the first excretory function stimulated by 5% citric acid at 30 minutes after injection of $^{99m}Tc$-pertechnetate and then the second excretory function stimulated by 10% citric acid at 45 minutes. Results : The mean uptake curves showed a sharp increase at the 1st week of radiotherapy and then showed a gradual decrease afterwards during radiotherapy. And the mean uptake curves after radiotherapy Persistently decreased on follow up scans without being recovered. The excretory rates decreased gradually as radiation dose increased and were not recovered after radiotherapy. Our study was not satisfactory enough to show the difference of susceptibility to irradiation between the parotid and salivary gland and the correlation between the symptom of xerostomia and the change of the excretory rates. Conclusion : The parotid and submandibular gland scans using $^{99m}Tc$-pertechnetate are useful in the assessment of the functional change of the salivary gland by radiotherapy and predicted the functional change of the salivary gland until 1 year-follow up scan.
Radiation therapy of oral and head and neck cancers often involves skin in the therapeutic range, and the use of bolus is frequently used. Dose irregularities provide dose uncertainty in patient application. In this study, the physical properties of patients with gel bolus, poly lactic acid (PLA), and silicon using 3D printing were fabricated. Dose uncertainties arising from the actual radiation dose delivery were measured. As a result, PLA bolus was stable in the Common irregularities. Silicon bolus may be useful for patients with severe irregularities or frequent changes in patient's body shape.

Purpose: The checking method of target and normal structure are used by MVCBCT, KVCBCT, CT On-rail System, Ultrasound in H&N cancer patient. In case of MVCT, the utilization of bone structure is valuable to check around tissue. But the utilization of soft tissue is not enough. The point of this paper is dose variation in movable parotid and changeable volume of H&N cancer patient of CT On-rail System. Materials and Methods: The object of H&N cancer patient is 5 in this hospital. The selected patient are scanned ARTISTE CT Vision (CT On-ral System) a triweekly. After CT scanning, tranfered coordinates are obtained by movable of parotid gland comparison with planning image. Checking for the changeable volume of parotid gland. A Obtained CT image are tranfered to the RTP System. So dose variation are checked by following changed volume. Results: The changes of target coordinate by the parotid gland movement are X: -0.4~0.4 cm, Y: -0.4~0.3 cm, Z: -0.3~0.3 cm. the volume of GTV is decreased to about 7.11%/week and then both parotid gland volume are shrinked about 4.81%/week (Lt), 2.91%/week (Rt). At the same time, each parotid gland are diminished in radiation dose as 3.66%/week (Lt), 2.01%/week. Conclusion: Images from CT on the rail System which are able to aquire the better quality images of soft tissue in Target area than MVCBCT. After replanning and dose redistribution by required images, It could gain not only the correction of the patient set-tup errors but exact dose distribution. Accordingly, the delivery of compensated dose, It makes that we could do Adaptive Targeting Radiotherapy and need Real Time Adaptive Targeting Radiotherapy by reduce beam delivary time.

연구목적 : 비인두암 환자들을 대상으로 방사선치료 시 삼차원입체조형치료기법과 용적세기조절회전치료기법을 비교하고 이하선을 포함한 정상조직 보호에 있어 그 차이점을 알아 보고자 본 연구를 시행하였다. 대상 및 방법 : 비인두암 환자 5명을 대상으로 치료계획용 CT(computed tomography)를 시행 후 삼차원입체조형방사선치료계획 과 용적세기조절회전치료계획을 시행하였다. 이를 바탕으로 얻은 선량분포, conformity index(CI) 그리고 선량체적 히스토그램을 통해 손상위험장기(organ at risk)와 계획용표적체적(planning target volume)을 비교 분석하였다. 결 과 : 분석결과 이하선에 조사되는 평균선량이 용적세기조절회전치료계획에서는 43.9%로 삼차원입체조형치료계획에서의 89.4% 보다 유의하게(p=0.043) 감소하였다. 계획용표적체적 conformity index의 경우 용적세기조절회전치료계획 (CI=1.06)에서 삼차원입체조형치료계획(CI=2.55) 보다 유의하게(p=0.043) 향상된 결과를 보였다. 결 론 : 비인두암 환자에서 용적세기조절회전 치료계획 시 삼차원입체조형치료계획 보다 유의하게 이하선에 평균선량이 줄었고 계획용 표적체적에 대한 conformity도 유의하게 향상되는 결과를 보였다. 본 연구가 적은 수의 환자를 대상으로 하였으나 용적세기조절회전치료기법을 시행 시 구강건조증의 발생을 줄일 수 있을 것으로 기대된다. 향후 더 많은 환자군을 대상으로 한 임상연구가 필요할 것으로 사료된다.

Simultaneous second primary tumors are not uncommon in patients with oral cavity squamous cell carcinoma. Many studies have previously shown that oral cavity squamous cell carcinoma patients with simultaneous second primary tumor generally have a poor prognosis. Additionally, the choice of the optimal therapeutic modality for oral cavity squamous cell carcinoma patients who present with simultaneous second primary tumor remains problematic. We reported a case of simultaneous squamous cell carcinomas in mouth floor and esophagus, that multidisciplinary team performed resection and reconstruction simultaneously.

Purpose: A selection of proper energy in treatment planning is very important because of having different dose distribution in body as photon energy. In generally, the low energy photon has been used in intensity-modulated radiation therapy (IMRT) for head and neck (H&N) cancer. The aim of this study was to evaluate the effect of partially used high energy photon at posterior oblique fields on IMRT plan for H&N cancer. Materials and Methods: The study was carried out on 10 patients (nasopharyngeal cancer 5, tonsilar cancer 5) treated with IMRT in Seoul National University Bundang Hospital. CT images were acquired 3 mm of thickness in the same condition and the treatment plan was performed by Eclipse (Ver.7.1, Varian, Palo Alto, USA). Two plans were generated under same planing objectives, dose volume constraints, and eight fields setting: (1) The low energy plan (LEP) created using 6 MV beam alone, (2) the partially used high energy plan (PHEP) created partially using 15 MV beam at two posterior oblique fields with deeper penetration depths, while 6 MV beam was used at the rest of fields. The plans for LEP and PHEP were compared in terms of coverage, conformity index (CI) and homogeneity index (HI) for planning target volume (PTV). For organs at risk (OARs), $D_{mean}$ and $D_{50%}$ were analyzed on both parotid glands and $D_{max}$, $D_{1%}$ for spinal cord were analyzed. Integral dose (ID) and total monitor unit (MU) were compared as addition parameters. For the comparing dose to normal tissue of posterior neck, the posterior-normal tissue volume (P-NTV) was set on the patients respectively. The $D_{mean}$, $V_{20Gy}$ and $V_{25Gy}$ for P-NTV were evaluated by using dose volume histogram (DVH). Results: The dose distributions were similar with regard to coverage, CI and HI for PTV between the LEP and PHEP. No evident difference was observed in the spinal cord. However, the $D_{mean}$, $D_{50%}$ for both parotid gland were slightly reduced by 0.6%, 0.7% in PHEP. The ID was reduced by 1.1% in PHEP, and total MU for PHEP was 1.8% lower than that for LEP. In the P-NTV, the $D_{mean}$, $V_{20Gy}$ and $V_{25Gy}$ of the PHEP were 1.6%, 1.8% and 2.9% lower than those of LEP. Conclusion: Dose to some OARs and a normal tissue, total monitor unit were reduced in IMRT plan with partially used high energy photon. Although these reduction are unclear how have a clinical benefit to patient, application of the partially used high energy photon could improve the overall plan quality of IMRT for head and neck cancer.
Purpose: The purpose of this study was to retrospectively examine the factors and characteristics of cancer patients who visited the emergency room, as well as to offer some educational materials for to manage acute symptoms. Methods: Data for this study were selected from the period of January to December, 2006. A total of 564 patients were examined using the tool which we developed by ourselves for the study. The collected data were analyzed using the SAS program for frequencies and percentage. Results: As for disease-related characteristics of the subjects, 28.9% of them had gastric and colorectal cancer; 66.9% were in stage 4; 51.6% had been in chemotherapy prior to visiting the emergency room; and 82.5% had their anticancer drug administrated average 1~5 times. As for the characteristics in regard to visit the emergency room, 62.9% were admitted to hospital within 2 weeks of being treated. As for chief complaints for visiting the emergency room, the worst symptom was pain, followed by symptoms such as gastro-intestinal symptoms, respiratory symptoms, high fever, and weakness. As for the disease-related symptoms, the worst symptom that gastric, colorectal, pancreatic, liver and gallbladder cancer patients complained of was pain, high fever for lymphoma patients was respiratory symptoms for lung cancer patients, and gastrointestinal symptoms for head and neck cancer and other patients. Conclusion: Therefore, according to their need and background, an individualized consultation and teaching program should be provided to cancer patients.

Purpose : We compared the set-up accuracy and right-left Shoulder position variation of the manufactured device and other commercial shoulder-retractors in the head and neck radiation treatment. Materials and Methods : Six patients consist of three groups which were used three different Shoulder retractors. We measured position corrections of left and right Shoulder and the couch after the image guidance by using on board imager (OBI) for six head and neck patients who has the extended target to the neck node lower region. Results : The position variation correction of left (right) Shoulder after image guidance were $1.07{\pm}3.99mm$ ($-4.35{\pm}2.09mm$), $-0.37{\pm}5.91mm$ ($1.26{\pm}5.28mm$), $-0.63{\pm}2.44mm$ ($0.25{\pm}1.61mm$) for group A, B and C. The vertical, lateral, longitudinal position and angular corrections of the couch after image guidance were $-2.06{\pm}2.68$, $-1.11{\pm}8.15$, $0.34{\pm}3.78mm$, and $0.51{\pm}0.77$ degree for group A, $-1.18{\pm}1.82$, $-0.94{\pm}2.13$, $-0.67{\pm}1.98mm$, and $0.91{\pm}1.04$ degree for group B and $0.12{\pm}2.18$, $-0.79{\pm}2.64$, $0.79{\pm}2.64$, and $0.00{\pm}0.49$ degree for group C. Conclusion : In this preliminary study, we found the positioning accuracy of the manufactured Shoulder retractor is comparable to other commercial Shoulder retractors. We expect that the reproducibility and accuracy of the patient set-up could be improved by using the home made Shoulder retractor in the head and neck radiation treatment.

비인두암에서 시행되는 방사선 치료 이후에 구강건조증과 피부 변화는 흔하게 볼 수 있는 합병증 중 하나이다. 비인두암의 좋은 예후를 고려할 때, 그러한 치료와 관련된 독성들은 상대적으로 오랫동안 문제를 야기하며 삶의 질 저하를 불러온다. 특히 류마티스 관절염, 루푸스와 같은 결합조직 질환을 가진 환자들에게서 방사선 치료를 시행하였을 때, 빈번하게 심각한 독성이 관찰된다. 본 증례는 성인형 스틸씨 병에서 병발된 비인두암의 치료로 항암방사선 동시치료를 실시한 결과, 비교적 경미한 구강 건조, 점막염, 불면증 등의 합병증이 관찰되었으나, 완전 관해가 획득되었다. 이에 저자들은 과거에 보고된 적 없는 성인형 스틸씨 병에 병발한 비인두암의 치료 경험을 다른 결합조직질환들에서 방사선 치료의 문헌들과 함께 보고하는 바이다.
