Anti-atherogenic Effect of Isoflavone through Hypolipidemic, Anti-oxidative and Anti-inflammatory Actions in C57BL/6 Mice (C57BL/6 Mice에서 이소플라본의 지질강하, 항산화, 항염증효과를 통한 항동맥경화 효과)

  • This study was carried out to investigate the effect of isoflavone on the atherogenic effect in C57BL/6 mice. C57BL/6 female mice, 5 weeks of age, were fed on chow diets for 2 weeks during adjustment period. Mice weighing approximately $17.9{\pm}0.9\;g$ were divided into 4 groups and were fed on the experimental diets containing isoflavone for 8 weeks. Experimental groups were control (atherogenic diet), IF-10 (atherogenic diet with isoflavone 10 mg/100 g diet), IF-40 (atherogenic diet with isoflavone 40 mg/100 g diet) and IF-100 (atherogenic diet with isoflavone 100 mg/100 g diet). Food efficiency ratio was not different among the experimental groups. Plasma triglyceride (TG) concentrations were lower after 4 weeks in isoflavone supplementation groups than in control group, whereas monocyte chemoattractant protein-1 and thiobarbituric acid reactive substances (TBARS) levels of plasma were significantly (p<0.05) decreased in the isoflavone supplementation groups in a dose dependent manner. Both hepatic TG and cholesterol levels were significantly lowered in IF-100 than control. Hepatic glutathione concentrations were higher in the IF-100 group than in the other groups. Hepatic antioxidant enzyme activities including glutathione-reductase, glutathione-peroxidase, catalase, and Mn-superoxide dismutase were significantly higher in the isoflavone supplemen-tation groups in a dose dependent manner. From the above results, it is concluded that isoflavone may reduce the risk of atherosclerosis via hypolipidemic, anti-oxidative and anti-inflammatory effects.

  • Purpose : Homocysteine is a strong and independent risk factor for cardiovascular disease. The deleterious effects of homocysteine included endothelial dysfunction, arterial intimal-medial thickening, wall stiffness and procoagulant activity. However, the precise mechanism responsible for homocysteine release in children with coronary artery disease is still unknown. The purpose of this study was to investigate serum homocysteine and tumor necrosis $factor(TNF)-{\alpha}$ levels and identify whether these levels had any association with the development of coronary artery lesions in Kawasaki disease(KD). Methods : Serum homocysteine and $TNF-{\alpha}$ levels were measured in 24 KD patients(group 1, eight patients with normal coronary artery; group 2, 16 patients with coronary artery lesions) and 21 controls(group 3, 10 afebrile controls; group 4, 11 febrile controls). Blood samples were drawn from each study group before and after intravenous immunoglobulin(IVIG) therapy and in the convalescent stage. Results : The homocysteine levels before IVIG therapy were significantly higher in group 1 than in group 3, and in group 2 than in group 3 and 4. The $TNF-{\alpha}$ levels before IVIG therapy were significantly higher in group 2 than group 3 and 4. Serum homocysteine and $TNF-{\alpha}$ levels were highest in group 2 before IVIG therapy. In the acute KD patients, serum homocysteine levels correlated significantly with $TNF-{\alpha}$ levels. Conclusion : The increased serum homocysteine levels in the acute stage increase the susceptibility to coronary arterial lesions in KD. $TNF-{\alpha}$ may also play an important role in the formation of coronary arterial lesions in KD.

Evaluation of short-term Hypolipidemic Effect and Safety of Simvastatin($Zocor^{(R)}$) in Patients with Hyperlipidemia (고콜레스테롤혈증 환자에서 Simvastatin($Zocor^{(R)}$) 단기 투여후 효과와 안정성 평가)

  • Background : Hyperlipidemia is the one of the major risk factors causing the atherosclerosis of coronary arteries. Treatment of hyperlipidemia with drugs has been confirmed the effects of therapy showing a decreased incidence of coronary artery disease. Simvastatin is a new drug of HMG-CoA reductase inhibitors and the short-term hypolipidemic effects and safety of simvastatin is evaluated in patients with hyperlipidemia. Methods: We studied 63 patients (39 males and 24 females, mean age 58) for 12 weeks whose plasma levels of total cholesterol were higher than 240 mg/dL or higher than 220 mg/dL with ischemic heart disease. Simvastatin was administered 20 mg/day and measured lipid profile at 12 week interval. Result: 1) Simvastatin significantly reduced the level of the plasma total cholesterol(-29.3%), LDL-cholesterol(-36.9%) and triglyceride(-13%)(p<0.05) but the level of HDL-cholesterol was not changed after 12 weeks simvastatin therapy. 2) the clinical symptoms and laboratory examination before and after simvastatin treatment showed no particular abnormal findings in short term follow up. Conclusion: These results suggested that short-term simvastatin therapy in patients with hyperlipidemia seeems to be very effective and safe.

Effect of Saengmaec-san on the Level of Blood Glucose and Serum Components in Streptozocin-Induced Diabetic Rats (생맥산의 식이가 Streptozocin으로 유도된 당뇨 Rat의 혈당과 혈청 성분 함량에 미치는 영향)

  • This study was carried out to investigate the effect of saengmaec-san on the level of blood glucose and serum components in streptozocin (STZ)-induced diabetic rats. The experimental groups were divided into normal group (normal), diabetic control group (Dia-control), 10% saengmaec-san (white ginseng) group (DA), 10% saengmaec-san (fermented white ginseng) group (DB), 10% saengmaec-san (fermented red ginseng) group (DC), 5% saengmaec-san (extruded and fermented white ginseng) group (DDL), 10% saengmaec-san (extruded and fermented white ginseng) group (DDH). The body weight after induction of diabetes was 85.4% in the Dia-control group compared with the normal group. But in the DA group, the body weight showed clear sign of recovery almost normal level after administration of saengmaec-san for two weeks. The food efficiency ratios (FER) were 5.94% in the normal group. But it was significantly decreased in the Dia-control group (0.58%). All the treatment groups showed increase of FER compared with the Dia-control group. The level of blood glucose was significantly increased in the STZ-induced diabetes groups but it was decreased in all the treatment groups after administration for 2 weeks. Serum creatinine level were significantly higher in the STZ-induced diabetes groups and after administration of saengmaeg-san for 2 weeks while the level of serum creatinine was decreased 33.3% in the DB group. After administration of saengmaec-san for two weeks, serum total cholesterol level were significantly lower in all treatment groups than the first day of the total cholesterol level. The level of serum triglyceride was increased in all the treatment groups compared with the first day of triglyceride level. The level of serum HDL-cholesterol, after STZ-induced diabetes, was decreased in all treatment groups but particularly in the DDL and DDH groups increased HDL-cholesterol level compared with the first day of the saengmaec-san administration. Compared with the beginning of experiment, Atherogenic index (AI) were significantly decreased in all treatment groups than the Dia-control group and showed clear sign of recovery almost normal level. These results suggested that the saengmaec-san could be developed as an antidiabetic agent.

Anti-inflammatory Effect of Lactuca sativa L. Extract in Human Umbilical Vein Endothelial Cells and Improvement of Lipid Levels in Mice Fed a High-fat Diet (상추 추출물(Lactuca sativa L.)의 혈관내피세포에서 항염증 작용과 고지방 식이 생쥐에서 혈중 지질농도 개선에 미치는 영향)

  • The objective of this study was to investigate the effects of a lettuce (Lactuca sativa L.) extract on the inflammation of human umbilical vein endothelial cell (HUVEC) and blood lipid improvement in hypercholesterolemic mice fed a high cholesterol diet. The lettuce extract (100% ethanol extract) inhibited the expression of intercellular adhesion molecule-1 and vascular cell adhesion molecule-1 in HUVEC treated with tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$). The lettuce extract suppressed the adhesion of THP-1 to TNF-${\alpha}$-treated HUVEC. The lettuce extract decreased the TNF-${\alpha}$-stimulated production of proinflammatory cytokine interleukin-6, interleukin-8 and chemokine monocyte chemotactic protein 1. In hypercholesterolemic mice, the lettuce extract reduced serum total cholesterol, triglyceride, and low-density lipoprotein-cholesterol level, while the lettuce extract elevated high-density lipoprotein-cholesterol level, resulting in the decrease of atherogenic index and cardiac risk factor level. These results suggested that lettuce extract can be an useful resource to show an anti-inflammatory effect and improve lipid metabolism.

Effect of Polygonum multiflorum Thunberg Extract on Lipid Metabolism in Rats Fed High-Cholesterol Diet (적하수오 추출물이 고콜레스테롤 급여 흰쥐의 지질대사에 미치는 영향)

  • The principal objective of this study was to determine the effects of an ethanol extract of Polygonum multiflorum Thunberg (PMT) on body lipid metabolism in rats fed a high-cholesterol diet for 5 weeks. The rats were divided into 6 groups: a control group (I), a cholesterol-control group (II), a control group treated with 0.5% PMT (III), a control group treated with 1% PMT (IV), a cholesterol-control treated with 0.5% PMT (V), and a cholesterol-control group treated with 1% PMT (VI). Body weight gains and food efficiency ratios were not significantly different among the groups. The levels of serum blood glucose and triglycerides of the treated animals significantly decreased compared to the control and the cholesterol-control groups (p<0.05). There was a clear tendency of decreased LDL-cholesterol level in PMT-treated animals compared to the cholesterol-control group. However, no significant differences were observed in the serum HDL- and LDL-cholesterol levels. In the ratio of HDL-cholesterol to total cholesterol and HDL-cholesterol to LDL-cholesterol concentration, both the 0.5% PMT and 1% PTM extract intake groups had a higher percentage than in the control and cholesterol-control groups (p<0.05). In addition, the atherosclerotic index in serum was significantly lower in the PMT intake group than those in the group control and the cholesterol-control. These results indicated that the PMT extract was effective on the improvement of lipid metabolism in SD rats.

  • GRAS probiotics can be used to modulate intestinal microbiota and to alleviate various gastrointestinal disorders. In several recent studies, researchers have explored the potential expansion and usability of probiotics to reduce the risk factors associated with diseases, including obesity, hypercholesterolemia, arterial hypertension, hyperhomocysteinemia, and oxidative stress. In this review, our aim was to clarify the mechanism underlying interactions between hosts (animal or human) and probiotics and the beneficial effects of probiotics on human health.

Mitral Valve Reconstruction in Mitral Insufficiency : Intermediate-Term Results (승모판 폐쇄부전증에서 승모판 재건술의 중기평가)

  • The advantages of mitral valve reconstruction have been well established and so mitral valve reconstruction is now considered as the procedure of choice to correct mitral valve disease. This is the report of intermediate-term results of 38 cases that performed mitral valve reconstruction for valve insufficiency(the total number of mitral valve reconstruction were 49 cases, but 11 cases that performed mitral valve replacement due to incomplete reconstruction were excluded). Material and Method : From March 1991 to March 2001, 38 patients underwent mitral valve repair due to mitral valve regurgitation with or without stenosis. Mean age was 47.6$\pm$14.7 years(range 15 to 70 years) : 11 were men and 27 were women. The causes of mitral valve regurgitation were degenerative in 14, rheumatic in 21, infective in 2 and the other was congenital. Result : According to the Carpentier's pathologic classification of mitral valve regurgitation, 3 were type 1 , 16 were type II and 19 were type III. Surgical procedures were annuloplasty 15, commissurotomy 19, leaflet resection and annular plication 9, chordae shortening 11, chordae transfer 5, new chordae formation 2, papillary muscle splitting 2 and vegetectomy 2. These procedures were combined in most patients. There were 2 early death and the causes of death were respiratory failure, renal failure and sepsis. There was no late death. Valve replacement was done in 6 patients after repair due to valve insufficiency or stenosis 3 weeks, 1, 3, 51, 69, 84months later respectively. These patients have been followed up from 1 to 116 months(mean 43.0 months). The mean functional class(NYHA) was 2.36 pre-operatively and improved to 1.70. Conclusion : In most cases of mitral valve regurgitation, mitral valve reconstruction when technically feasible is effective operation that can achieve stable functional results and low surgical and late mortality.

  • Monocyte chemoattractant protein 1 (MCP1) plays a key role in monocyte /macrophage infiltration to the sub-endothelial space of the blood vessel wall, which is a critical initial step in atherosclerosis. In this study, we examined $interleukin-1{\bate}$ ($IL-1{\beta}$) induced MCP1 expressions via activation of transcription factor $NF-{\kappa}B$ in primary human aorta smooth muscle cells. We determined the effect of several anti-inflammatory agents on $IL-1{\beta}-induced$ MCP1 expression. The pretreatment of luteolin significantly suppressed $IL-1{\beta}-induced$ MCP1 expressions through blocking activation and translocation of $NF-{\kappa}B$ to the nucleus.

Dietary Behavior Score and Serum Lipid Profiles of Old Men with Atherosclerosis Operated by Angiography (하지 혈관조영술을 시행한 노년기 남성 동맥경화증 환자의 식행동 점수, 영양소 섭취량 및 혈청 지질 상태)

  • This study was conducted to examine the association of dietary behaviors, serum lipid profiles according to the progression of angiographically evaluated atherosclerosis. The subjects were 32 male patients aged 59-80 yrs living in the Daegu area who underwent initial angiography for their lower extremities. We classified the subjects into two groups according to the seriousness of iliac lesions based on angiographic results : Group I (lower lesion group) and Group II (higher lesion group). Dietary habits were evaluated by 10-item questionnaires. Daily food intake of each subject was assessed by the 24-hour recall method. There were no significant differences in serum cholesterol and triglyceride levels between the two groups. The food habit score of Group II was significantly lower than that of Group I (P<0.001). Group II showed significantly lower dietary habit scores in the consumption of fish and bean product (P<0.001), seaweed product (P<0.01), and salt use (P<0.001) than those of Group I. Dietary intake of vitamin C was significantly lower in Group II (P<0.01). Our results indicate that the more serious of atherosclerotic lesions the patients had, the poorer dietary habits they exhibited. Therefore, medical nutrition therapy for atherosclerotic patients should emphasize maintaining a balanced diet by consuming more fishes, beans, and seaweed as well as by reducing salt intake.
